Robotic-assisted gait training applied with guidance force for balance and gait performance in persons with subacute hemiparetic stroke

  • Objective: Robot assisted gait training is implemented as part of therapy for the recovery of gait patterns in recent clinical fields, and the scope of implications are continuously increasing. However clear therapy protocols of robot assisted gait training are insufficent. The purpose of this study was to investigate the effects of robot-assisted gait training applied with guidance force on balance and gait performance in persons with hemiparetic stroke. Design: Two group pre-test post-test design. Methods: Nineteen persons were diagnosed with hemiparesis following stroke participated in this study. The participants were randomly assigned to the unilateral guidance group or bilateral guidance group to conduct robot-assisted gait training. All participants underwent robot-assisted gait training for twelve sessions (30 min/d, 3 d/wk for 4 weeks). They were assessed with gait parameters (gait velocity, cadence, step length, stance phase, and swing phase) using Optogait. This study also measured the dynamic gait index (DGI), the Berg balance scale (BBS) score, and timed up and go (TUG). Results: After training, BBS scores were was significantly increased in the bilateral training group than in the unilateral guidance group (p<0.05). Spatiotemporal parameters were significantly changed in the bilateral training group (gait speed, swing phase ratio, and stance phase ratio) compared to the unilateral training group (p<0.05). Conclusions: The results of this study suggest that robot-assisted gait training show feasibility in facilitating improvements in balance and gait performance for subacute hemiparetic stroke patients.

장애물 보행 시 노화에 따른 신체질량중심의 변화 (Age Effects on Center of Mass during Obstacle Crossing)

  • PURPOSE: The purpose of this study was to compare three dimensional displacement and peak velocity of the center of mass (COM) during obstacle crossing in young and older adults. METHODS: 10 young adults (6 males/4 females, $24.6{\pm}1.9$ years, age range: 22.0-26.9) and 10 older adults (1 male/9 females, $76.9{\pm}5.1$ years, age range: 65.2-81.2) participated in the study. Both groups crossed an obstacle, which is 10% of leg length, and COM was measured using motion analysis system. Independent t-test was used to find significant differences between two groups. RESULTS: The older adults showed significantly greater and faster COM displacement and peak velocity in mediolateral (M-L) direction as compared with young adults (p<.01 and p<.001 respectively). However, the young adults showed significantly greater and faster COM displacement and peak velocity in anteroposterior (A-P) direction as compared with older adults (p<.05 and p<.001 respectively). Furthermore, the young adults showed faster peak velocity of COM in vertical direction as compared with older adults (p<.001). However, no significant difference was found in the COM displacement in vertical direction between two groups. CONCLUSION: Greater and faster COM displacement and peak velocity in M-L direction in older adults were due to compensatory adjustment for appropriate contact on base of support of swing limb. Thus, the motion of the COM in M-L direction may be a crucial factor to identify risk of falls in older adults.

골프 드라이버 스윙 시 Early Extension에 따른 임팩트 변화 (Change of Impact by the Early Extension in during a Golf Driver Swing)

  • The purpose of this study is to validate that change of impact by the Early Extension in during a golf driver swing. 13 golf players who were diagnosed with symptoms of Early Extension participated in a proactive corrective training programs that took place 3 times a week for a 4 month period. Data was collected by recording 5 pre and 5 post training driver swings and analyzing the data to calculate the change in velocity and its effect in the shot used the TRACK MAN. After the training, the changes of early extension were -0.21 cm in backswing section E2(take away), -0.64 cm in E3(halfway backswing), and -0.94 cm in E4(backswing top). The downswing section changes were -1.34 cm in event E5(halfway downswing), -1.74 cm in E6(impact). Impact force increased and thus club speed increased by 6.32 km/h, ball speed increased by 10.94 km/h, max height decreased by -6.22 m, carry increased by 10.85 m, carry side(left deviation) decreased by 4.84 m, flight time by increased by 0.4 sec, and total length increased by 17.96 m while landing angle decreased by -7.74 deg.

앞십자인대 재건술 후 엉덩관절 벌림근 강화운동이 근력과 보행에 미치는 영향 (The Effects of Strengthening Exercise of Hip Abductors on Muscle Strength and Ambulation in Patient with ACL Reconstruction)

  • Purpose: The purpose of this study was to investigate the effect of strengthening exercises of hip abductors on muscle strength and ambulation for patients with ACL reconstruction. Methods: The subjects were randomly assigned to the intervention group (general exercise plus strengthening of hip abductors) or the control group (general exercise without the strengthening of hip abductors). Both groups participated in a six-week exercise protocol after the surgery for ACL reconstruction. The knee strength test (quadriceps, hamstring, hip abductor) and gait analysis were performed in pretest and post-test. Results: Muscle strength was measured using the Biodex system III model (Biodex Medical System, Inc, NY, US). The results showed no significant difference in knee strength (quadriceps, hamstring) between the two groups (p>0.05), however, there was a significant difference in hip abductors (p<0.05). The gait analysis was measured with Gaitrite system (CIR System Inc, US). Results of comparison between groups showed a significant increase in the step length of both groups. (p<0.05), and there was a significant difference in the affected leg of the intervention group(p<0.05). Conclusion: Exercise of hip abductors for patients with ACL reconstruction provides significant benefits with respect to improvement of muscle strength in hip abductors, increasing the stride. This result indicates that a training program designed for ACL reconstruction patients should include strengthening of the hip abductor. We think that further study is needed to determine the relationship of hip muscles and knee joint for patients with ACL reconstruction.

무릎관절 골관절염 환자의 보행기 보행에서 생역학적 특성 (Biomechanical Properties of the Anterior Walker Dependent Gait of Patients with Knee Osteoarthritis)

  • Purpose: Osteoarthritis occurs in many different joints of the body, causing pain, stiffness, and decreased function. The knee is the most frequently affected joint of the lower limb. The aim of this study was to investigate the differences of biomechanics between independent gait and anterior walker dependent gait of patients with osteoarthritis of the knee. Methods: Lower limb joint kinematics and kinetics were evaluated in 15 patients with knee osteoarthritis when walking independently and when walking with an anterior walker. Participants were evaluated in a gait laboratory, with self-selected gait speed and natural arm swing. Results: When walking with a dependent anterior walker, participants walked significantly faster (p<0.01), using a longer stride length (p<0.01), compared to independent gait. When walking with a dependent anterior walker, participants exhibited significantly greater knee flexion/extension motion (p<0.01) and lower knee flexion moment (p<0.05) compared to independent gait. When walking with a dependent anterior walker, participants showed significantly greater peak ankle motion (p<0.01), ankle dorsiflexion/plantarflexion moments (p<0.01), and ankle power generation (p<0.05) compared to independent gait. Conclusion: These biomechanical properties of gait, observed when participants walked with a dependent anterior walker, may be a compensatory response to impaired knee function to allow sufficient power generation for propulsion. Therefore, rehabilitative strategies for patients with osteoarthritis of the knee are needed in order to improve not only knee function but also hip and ankle function.

불안정한 지지면의 척추안정화 운동이 척추측만증 환자의 체간 자세와 균형에 미치는 영향 (Effect of Unstale Surface Lumbar Stabilization Exercise on Trunk Posture and Balance Ability in Patients With Scoliosis)

  • Purpose : This study was somatosensory less in patients with idiopathic scoliosis somatosensory input to the lumbar stabilization exercises carried out to determine the most effective treatment method to be stable and unstable in terms of supporting the lumbar stabilization exercises the patient's torso length and postural sway by comparing the distance from a standing position and looked for differences in effect on the balance. Methods : The subjects of the study were 18 patients who showed the symptom of scoliosis. The study classified the patients into two experimental groups, one using an unstable surface and one a fixed surface, and the patients were required to do a lumbar stabilization exercise a total of 12 times for 60 minutes per session, three times a week for four weeks. The study carried out a paired comparison t-test so as to compare differences between measurement values in each experimental group before and after the exercise. Results : Superior iliac spine on the left, there was a significant reduction in the group doing the lumbar stabilization exercise on an unstable surface (p<0.05). Regarding change in sway distance to the left and right directions in the group doing the lumbar stabilization exercise on the unstable surface, there was a significant decrease in both the condition of closed eyes or open eyes (p<0.05). As for change in sway distance in forward-and-backward direction, there was a significant reduction in the condition of either closed eyes or open eyes (p<0.05). Conclusion : The lumbar stabilization exercise on an unstable surface improved the trunk posture of patients with scoliosis symmetrically, and the static balance ability in a standing posture was discovered to be improved. In the future, the lumbar stabilization exercise on an unstable surface may be used as a posture correction and balance increase exercise for patients with scoliosis.

20대 정상 성인의 남.여 보행분석 연구 (A Study on Gait Analysis of Normal 20' Adult Male and Female)

  • The aim of this study is to present the basic reference data of age and specipic gait parameters for Korean Adult. The basic gait parameters were extracted from 20 Adult, 10 men and 10 women, 21 to 24 years of age using VICON 512 Motion Analyzer. The temporal gait parameters and kinematic parameters of Korean Adult similar to other western reference data. The results were as follows: 1) The mean Cadence of the male to the female were 113.5 steps/min to 117.6 steps/min. 2) The mean Walking Speed of the male to the female were 1.30 m/s, to 1.27 m/s. 3) The mean Stride Length of the male to the female were 1.35 m, to 1.30 m. 4) The mean maximal angles of pint on the pelvic tilt motion for different male to the female were $9.80^{\circ}$ to $7.75^{\circ}$. (p<0.05) 5) The mean maximal angles of pint on the hip flexion motion for different male to the female were $22.62^{\circ}$ to $27.65^{\circ}$. (p<0.05) 6) The mean maximal angles of pint on the knee flexion motion for different male to the female were $55.78^{\circ}$ to $55.49^{\circ}$. (p>0.05) 7) The mean maximal angles of pint on the ankle dorsiflexion motion for different male to the female were $12.47^{\circ}$ to $13.39^{\circ}$. (p>0.05).

편마비환자의 균형기능 향상에 관한 문헌적 고찰 (Improvements in Balance of Patients with Hemiplegia A Literature Review)

  • Background: Hemiplegic patients usually have difficulty maintaining balance. Balance training is a major component of there habilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of PLS(Posterior Leaf Spring), FES(Functional Electrical stimulation), treadmill training, and neurodevelopmental treatment on the improvement of balance in patients with hemiplegia. Methods: We looked into published studies from Dankook University’s electronic library databases of RISS4U, KMbase, NCBI, and MEDLIS concerning the effectiveness of any form of intervention leading to improvement of balance. All types of studies relevant to the topic that were published in English during the time period of 1986 to 2005 were included. Results: 1. There were significant differences in standing balance, dynamic activity balance, and gait speed between barefoot subjects and subjects who wore SPAFO and HPAFO(p<.05). 2. The changes in ROM and FRT related to sex, age, height, and weight part of the diagnosis, as well as experience relapse, was of meaningless value. Changes in ROM related to the duration of pain and experiences of falling down were also meaningless. However, FRT showed significant static differences(p<.05). 3. The body-weight-support treadmill training scoring of standing balance, step length, and a timed 10m walking test showed definite improvement. 4. The proprioceptive control approach improved dynamic balance in patients with hemiplegia. Conclusion: Consequently, further study is needed to verify methods when physical therapists are researching balance ability in hemiplegic patient.

The effect of treadmill gait training with patellar taping on gait abilities in chronic stroke patients

  • Objective: The purpose of this study is to investigate the effect of treadmill gait training with patellar taping on gait abilities in chronic stroke patients. Design: Randomized controlled trial. Methods: Thirty chronic stroke patients who have been diagnosed at least six months or before were recruited in Gyeongin Rehabilitation Center Hospital, located in Incheon. Patients who were enrolled in this study were randomized to experimental group (n=15) or control group (n=15). Treadmill with patella taping training group patients were applied with patellar taping when they were being trained on a treadmill. Control group patients were being trained on a treadmill without any kind of taping. Gait parameters were measured with a GAITRite$^{(R)}$ system which evaluated gait performances. Gait trainings were done for 30 min/day, 5 days/week, for 4 weeks. Results: After treadmill training, treadmill with patella taping training group showed a significant improvement in gait abilities, including velocity, cadence, paretic and non-paretic step length, and double support period (p<0.05). However, in general treadmill group, there were no significant differences in gait parameters except velocity and cadence. There was a significant difference in gait performance in the experimental group compared with the control group, except for the gait symmetry ratio (p<0.05). Conclusions: According to this result of this study, it seems that application of patellar taping in treadmill gait training for chronic stroke patients significantly improved gait abilities of these patients. Also, we can conclude that patella taping is thought to be useful in real clinical settings where there are many chronic patients who are in need of improvement in their gait abilities.

C. M. I 반응에 의한 간호원의 심신 건강 평가 (Psychiatric Evaluation of Nurses by C. M. I.)

  • It is a well accepted fact that mental and physical health of nurses has a direct bearing or influence in their practice in the nursing profession. Recently, with this view in mind, the study of the mental and physical health of nurses has attracted the attention of many especially those in research hospitals. According to Soon Hyang Cha(1974) a nurse in clinical practice or service has a daily energy consumption of 39% and San Cho Chun (1974) asserts that Nurses'duties are more demanding on her emotionally than on her nursing technical skills. Many more studies has been made to this effect, here and abroad and similarly stressed the importance of mental and physical health for nurses. This study was made in an attempt to analyze the trend or tendency of the mental and physical health of nurses by employing the Cornell Medical Index (CMI) method. The data has been collected from May 1977 to November 1977, 200 nurses from 8 University hospitals and 200 nurses from general hospitals in Seoul, Theses who participated were selected at random. The data were tabulated and comparison made. The results were as follows : 1. Among nurses, the analysis based on the length of experience, it is apparent that, the longer or more experience one ha s, the more complaints they have. 2 The longer the nurses are engaged in nursing practice, the more they have problems in their digestive, musculoskeletal, respiratory and nervous systems and are more prone to diseases, and get tired easier than those of shorter experience. But on the other hand it could be seen that younger nurses or nurses with less experience in the profession are more melancholic and prone to stress than those with longer experience.
