The rapid increase in the number of patients with chronic diseases is an important public healthcare issue in many countries, which accelerates many studies on a healthcare system that can, whenever and wherever, extract and process patient data. A patient with a chronic disease conducts self-management in an out-of-hospital environment, particularly in an at-home environment, so it is important to provide integrated and personalized healthcare services for effective care. To help provide effective care for chronic disease patients, we propose a service flow and a new cloud-based personalized healthcare system architecture supporting both at-home and at-hospital environments. The system considers the different characteristics of at-hospital and at-home environments, and it provides various chronic disease care services. A prototype implementation and a predicted cost model are provided to show the effectiveness of the system. The proposed personalized healthcare system can support cost-effective disease care in an at-hospital environment and personalized self-management of chronic disease in an at-home environment.

Background: The purpose of this study was to explore the effectiveness of a spiritual care training package in maternal caregivers of children with cancer. Materials and Methods: This study was a quasi-experimental study with pretest and posttest design consisting of a sample of 42 mothers of children diagnosed as having cancer. Participants were randomly assigned to either an experimental or a control group. The training package consisted of seven group training sessions offered in a children's hospital in Tehran. All mothers completed the Spirituality & Spiritual Care Rating Scale (SSCRS) and the Depression, Anxiety and Stress Scale (DASS-21) at pre and post test and after a three month follow up. Results: There was significant difference between anxiety and spiritual, religious, Personalized care and total scores spiritual care between the intervention and control groups at follow-up (P<0.001).There was no statistically significant difference in stress and depression scores between the intervention and the control groups at follow-up. Conclusions: Findings show that spiritual care training program promotes spirituality, personalized care, religiosity and spiritual care as well as decreasing anxiety in mothers of children with cancer and decreases anxiety. It may be concluded that spiritual care training could be used effectively in reducing distressful spiritual challenges in mothers of children with cancer.
"Personalized medicine," the goal of which is to provide better clinical care by applying patient's own genomic information to their health care is a global challenge for the $21^{st}$ century "genomic era." This is especially true in Korea, where provisions for clinical genetic services are inadequate for the existing demand, let alone future demands. Genomics-based knowledge and tools make it possible to approach each patient as a unique biological individual, which has led to a paradigm-shift in medical practice, giving it more of a predictive focus as compared with current treatment oriented approach. With recent advancements in genomics, many genetic tests, such as susceptibility genetic tests, have been developed for both rare single gene diseases and more common multifactorial diseases. Indeed, genetic tests for presymtomatic individuals and genetic tests for drug response have become widely available, and personalized medicine will face the challenge of assisting patients who use such tests to make appropriate and wise use of genetic risk assessment. A major challenge of genomic medicine lies in understanding and communicating disease risk in order to facilitate and support patients and their families in making informed decisions. Establishment of a health care system with provisions for genetic counseling as an integral part of health care service, in addition to genomic literacy of health care providers, is vital to meet this growing challenge. Realization of the promise of personalized medicine in the era of genomics for improvement of health care is dependent on further development of next generation sequencing technology and affordable sequencing test costs. Also necessary will be policy development concerning the ethical, legal and social issues of genomic medicine and an educated and ready medical community with clinical practice guidelines for genetic counseling and genetic testing.
The heterogeneous nature of lung cancer has become increasingly apparent since introduction of molecular classification. In general, advanced lung cancer is an aggressive malignancy with a poor prognosis. Activating alterations in several potential driver oncogenic genes have been identified, including EGFR, ROS1 and ALK and understanding of their molecular mechanisms underlying development, progression, and survival of lung cancer has led to the design of personalized treatments that have produced superior clinical outcomes in tumours harbouring these mutations. In light of the tsunami of new biomarkers and targeted agents, next generation sequencing testing strategies will be more appropriate in identifying the patients for each therapy and enabling personalized patients care. The challenge now is how best to interpret the results of these genomic tests, in the context of other clinical data, to optimize treatment choices. In genomic era of cancer treatment, the traditional one-size-fits-all paradigm is being replaced with more effective, personalized oncologic care. This review provides an overview of lung cancer genomics and personalized treatment.
Objectives: This study aimed to review the characteristics of personalized medicine and Korean medicine, and the correlation between personalized medicine and Korean medicine. Methods: We investigated various studies in PubMed, Scopus and domestic Korean medicine journals. In addition, we discussed the topic based on literature. Results: Western medicine developed as evidence-based medicine. However, its limitations are being reached, so a new paradigm of medicine is needed. As a result, personalized medicine has appeared. Recently, through the development of human genomics, personalized medicine has been researched on the basis of individual genetic characteristics. Korean medicine has developed with a unique holistic approach and treats not the disease itself but the patient's body. Its characteristic is well expressed through syndrome differentiation and treatment. Syndrome differentiation represents the nature of person-centered medicine and becomes the root of personalized medicine. Conclusions: Compared with genome-based personalized medicine of Western medicine, Korean medicine could be classified as syndrome-based personalized medicine. It would be great to apply this characteristic to clinical practices.
Recently, the beauty industry has implemented personalized services based on skin big data. To increase competitiveness in the beauty industry, systematic data measurement and evaluation indicators are necessary to select data and obtain necessary knowledge. In response, this study sought to stably enhance the accuracy of skin diagnosis based on satisfaction and reliability. To this end, the research was conducted through focus group interviews (FGI), a case study of brands, and analysis of prior research results. In particular, as a result of analyzing keywords that classify skin types by brand, common survey items for skin types were oiliness and using moisturizer, cosmetic use and vascular of skin, external stimulus and blemish & freckles, facial wrinkle, outside activities, self-consciousness, and smoke. In additioin to the common questions of the preceding study and the brand survey items, the questions concerning complex skin types, seasonal change, facial color, wrinkles and elasticity were added, and the questions were presented in a total of 40 items. In particular, it suggested that skin measurement could be conducted by focusing on elasticity and pigmentation items by reflecting the results of prior research and the opinions of FGI, and based on this, a customized service with high satisfaction and high accuracy of beauty care based on this could be proposed. We hope that this study will facilitate active self-care by providing more satisfying skin stones and personalized cosmetics proposals, thus laying the foundation for the further development of the cosmetics industry.
Purpose: The study aimed to design and develop an automated personalized self-care (APSC) program for patients with type 2 diabetes mellitus. The secondary aim was to present a clinical protocol as a mixed-method research to test the program effects. Methods: The APSC program was developed in the order of analysis, design, implementation, and evaluation according to the software development life cycle, and was guided by the self-regulatory theory. The content validity, heuristics, and usability of the program were verified by experts and patients with type 2 diabetes mellitus. Results: The APSC program was developed based on goal setting, education, monitoring, and feedback components corresponding to the phases of forethought, performance/volitional control, and self-reflection of self-regulatory theory. Using the mobile application, the participants are able to learn from educational materials, monitor their health behaviors, receive weekly-automated personalized goals and feedback messages, and use an automated conversation system to solve the problems related to self-care. The ongoing two-year study utilizes a mixed method design, with 180 patients having type 2 diabetes mellitus randomized to receive either the intervention or usual care. The participants will be reviewed for self-care self-efficacy, health behaviors, and health outcomes at 6, 12, 18, and 24 months. Participants in the intervention group will be interviewed about their experiences. Conclusion: The APSC program can serve as an effective tool for facilitating diabetes health behaviors by improving patients' self-care self-efficacy and self-regulation for self-care. However, the clinical effectiveness of this program requires further investigation.
Involvement of a multidisciplinary team in cancer care may have added benefits over the existing system of patient management. A paradigm shift in the current patient management would allow more focus on nutritional support, in addition to clinical care. Malnutrition, a common problem in cancer patients, needs special attention from the early days of cancer care to improve quality of life and treatment outcomes. Patient management teams with trained oncology dietitians may provide quality personalized nutritional care to cancer patients.
Background: Life-long anticoagulant therapy is mandatory for patients who undergo heart valve replacement with implantation of a mechanical prosthesis. The aim of this study was to investigate the effects of a nurse-led patient educational program concerning oral anticoagulant therapy intake after heart valve replacement surgery on patients' knowledge of important parameters of anticoagulant administration. Methods: In this single-center study, 200 patients who underwent surgical implantation of a mechanical prosthesis were divided into 2 groups. The control group received the basic education concerning oral anticoagulants, while the intervention group received a personalized educational program. Results: Personalized education was correlated with a better regulation of therapeutic international normalized ratio (INR) levels and adequate knowledge among patients. Therapeutic levels of INR were achieved in 45% of the patients during the first month, 71% in the third month, and 89% in the sixth month after discharge in the intervention group, compared to 25%, 47%, and 76% in the control group, respectively. Patients' satisfaction with the information was higher in the intervention group than in the control group. The percentage of satisfaction reached 80% for the intervention group versus 37% for the patients of the control group. Conclusion: The implementation of the nurse-led educational programs was associated with improved clinical results and increased adherence to oral anticoagulant treatment.
코로나로 인해 건강관리에 대한 관심이 증가하고 있는 요즘, 여러 사람이 함께 이용하는 헬스장이나 공용시설을 이용하는데 어려움이 늘어남에 따라 홈 트레이닝을 하는 이들이 늘어나고 있다. 이에 본 연구에서는 홈 트레이닝 사용자들에게 좀 더 정확하고 의미 있는 운동 추천을 제공하기 위해 개인 성향 정보를 활용한 개인화된 운동 추천 알고리즘을 제안한다. 이를 위해 식습관 정보, 육체적 조건 등 개인을 나타낼 수 있는 개인 성향 정보를 사용해 k-최근접 이웃 알고리즘으로 데이터를 비만의 기준에 따라 분류하였다. 또한, 운동 데이터 셋을 운동의 레벨에 따라 등급을 구별하였으며 각 데이터 셋의 이웃 정보를 바탕으로 모델 기반 협업 필터링 방법 중 차원 축소모델인 특이값 분해 알고리즘(SVD)을 통해 사용자들에게 개인화된 운동 추천을 제공한다. 따라서 메모리 기반 협업 필터링 추천 기법의 데이터 희소성과 확장성의 문제를 해결할 수 있고, 실험을 통해 본 연구에서 제안하는 알고리즘의 정확도와 성능을 검증한다.
