유치와 후속 영구치 근원심 폭경의 상관관계에 관한 연구 (A STUDY ON THE CORRELATIONS BETWEEN MESIODISTAL CROWN DIAMETERS OF THE DECIDUOUS AND SUCCESSIONAL PERMANENT TEETH)

  • The primary objective of this study is to estimate of the mesiodistal crown diameters of the unerupted permanent successors derived from the mesiodistal crown diameters of the deciduous teeth in Korean population. The subjects were 54 individuals (twenty nine boys and twenty five girls) with normal occlusion aged 6 to 13 years. The mesiodistal crown diameters of the deciduous and the successional permanent teeth were measured from the longitudinal dental cast models using the sliding calipers (Mitutoyo Co.). From the study, the results are as follows; 1. Sex differences of mesiodistal won diameters were less in the deciduous teeth, but male were more than that of female in the successional permanent teeth. 2. The mesiodistal crown diameters of the deciduous central incisors, lateral incisors, canines were smaller than that of the successional permanent teeth and the deciduous 1st molars, and 2nd molars were more larger than that of the successional permanent teeth. 3. Size differences between sum of the mesiodistal crown diameters of central incisors and lateral incisors in the decidous teeth and the successional permanent teeth were $7.20{\pm}1.79mm$ in upper, $5.38{\pm}1.64mm$ in lower and that of canine,1st molar and 2nd molar in the deciduous teeth and the successional permanent teeth were $0.56{\pm}1.19mm$ in upper, $2.22{\pm}1.19mm$ in lower. 4. In male, the correlation coefficients between the upper deciduous central incisor and the successional permanent tooth (r = 0.57) and in female, the correlation coefficients between the upper deciduous 1st molar and the successional permanent tooth (r=0.67) appeared the highest. 5. The regression constants were determined to estimate the mesiodistal crown diameters of the unerupted successional permanent teeth.

유치의 외상에 의한 계승 영구치의 발육 장애 (DEVELOPMENTAL DISTURBANCES OF SUCCESSIVE PERMANENT TEETH BY TRAUMA TO THE PRIMARY TEETH)

  • 유치에 가해진 외력은 하방으로 전달되어, 계승 영구치의 손상이라는 합병증이 야기시킬 수 있기 때문에 유치에 손상이 가해진 경우의 치료계획은 영구치의 안전성에 초점을 두어야만 한다. 유치의 외상에 의한 계승 영구치 발육 장애의 정도는, 외상 당시 소아의 연령 및 치아의 발육 상태, 손상의 종류와 정도 및 외상 후부터 치료 전까지의 경과 시간 등에 따라서 달라지며, 그 정도는 미약한 석회화 부전에서 성장 정지에 이르기까지 다양하게 나타난다. 본 증례는 33개월, 만 5세, 26개월의 소아에서 각각 유치의 함입, 완전탈구, 아탈구 등의 외상성 손상이 발생한 후, 후속영구치의 발육 장애가 관찰된 경우로 비슷한 연령대임에도 불구하고, 아탈구와 같은 경미한 외상이 발생한 경우에는 법랑질저형성증과 같은 경미한 수준의 발육 장애가 나타났지만, 합입이 일어난 경우는 치근의 만곡이 관찰되었다. 또한 영구치의 교환 시기에 가까운 나이인 5세경이라 하더라도 완전탈구가 일어난 경우에는 치근의 형성부전 등 보다 심각한 합병증이 관찰되어, 유치의 손상양상과 외상을 당한 시기의 환아의 나이에 따라 계승 영구치에 미치는 영향이 달라진다는 사실을 확인 할 수있었다. 따라서 유치의 변위를 초래하는 모든 종류의 외상이 발생한 경우, 술자는 외상의 유형 및 영구치의 발육상태를 고려하여 적절한 치료 계획을 세워나가는 것이 중요하리라 사료된다.

치면열구 전쇄재의 미세누출에 관한 자가방사법적 연구 (AN AUTORADIOGRAPHIC STUDY OF THE MICROLEAKAGE AROUND A PIT AND FIGURE SEALANT)

  • The purpose of this study was to observe the microleakage of a pit and fissure sealant at the tooth-restoration interface. One hundred and sixty-seven extracted permanent and deciduous teeth were used and some simple mechanical pretreatment were done before etching. The degree of microleakage was assessed by $Ca^{45}$ after 24 hours, and 2 months. The results were as follows: 1. Among total 167 teeth, 53 teeth (32%) were demonstrated the microleakage. 2. After 2 months the leakage was increased from 22 percent to 35 percent in permanent teeth, and from 32 percent to 43 percent in primary teeth. 3. The group treated by fluoride paste was demonstrated the largest leakage, 50 percent in primary teeth and 33 percent in permanent teeth. There are more leakage in primary teeth (38%) than permanent teeth (28%).

영구치 전치의 근원심경과 치관 길이에 관한 연구 (A Study on the Mesio-Distal Crown Diameters and Crown Length of the Permanent Anterior Teeth)

  • The mesiodistal crown diameters and crown length of the permanent anterior teeth have been studied and analyzed about the mean size, S.D, S.E etcs from stone models of 100 Korean male and 74 female. From the study, the following conclusions were made : 1. Statistical differences of left and right teeth were not found in the mesio distal crown diameters and crown length of the permanent anterior teeth. 2. The mean values of mesiodistal crown diameters of permanent anterior teeth were slightly larger in male than in females and statistical differences of males and females were found in maxillary right central incisor, mandibular left canine, mandibular right lateral incisor and mandibular right canine. 3. The mean values of crown length of permanent anterior teeth were slightly larger in male than in female and statistical differences of males and females were found in mandibular left canine, mandibular right canine. 4. Mesiodistal crown diameters and crown length of maxillary left lateral incisors and right lateral incisors of female are larger than that of male.

  • In the Pediatric Dentalc profession the occurrence rate intrusion of young permanent teeth is rather low, and the treatment method varies from one another. Intrusion of the teeth bring complications to the pulp and periodontal ligament, and thus the possibility of endodontic treatment increases. For the formation of calcific barriers, calcium hydroxide is used in young permanent teeth and this makes the final canal filling possible. In this case report, we will show two cases of young permanent teeth intrusion and the treatment methods to help assess the recognition of treatment for young permanent teeth intrusion.

유치와 영구치의 외상에 관한 연구 (A study on injuries in deciduous teeth and permanent teeth)

  • Objectives : The writer aimed to examine the dynamic aspect on damage and to be conducive to the swift cure according to right treatment to educate prevention of injury through surveying on the number of the damaged teeth given injury, tooth kind, type in damage, place and cause for being damaged, frequency by month and by time level, and location of the damaged tooth. Methods : Targeting 343 teeth of receiving injury in 201 patients who visited the department of pediatric dentistry, the department of preservation, and the oral and maxillofacial surgery in a dental hospital in Cheonan city from April 2007 to April 2009, by having the dental injury as the main reason, the following conclusions were obtained. Results : 1. Deciduous teeth were indicated to be the highest in imperfect luxation with 60.7%, and were indicated to be in order of crown fracture with 25.5%, root fracture with 12.4%, and perfect luxation with 1.4%. The permanent teeth were indicated to be the highest in crown with 58.1%, and were indicated to be in order of imperfect luxation with 27.3%, root fracture with 13.1%, and perfect luxation with 1.5%(p<0.001). 2. In case of deciduous teeth for a place with damage, the home was indicated to be the highest with 31.7%. The permanent teeth were indicated to be the highest(p<0.001) in the road and stairs with 40.4%. As to a cause for damage, both deciduous and permanent teeth were indicated to be the highest(p<0.001) in a cause for falling with 53.1% and 30.8%, respectively. 3. As for a treatment method, in case of deciduous teeth, the close observation was indicated to be the highest with 46.9%. In permanent teeth, the resin restoration was indicated to be the highest(p<0.001) with 22.2%. Dental injuries in deciduous teeth and permanent teeth are showing diverse aspects. Conclusions : It is important to arrange guidelines on cure of the damaged teeth by injury through continuing a dynamic research on these aspects. Also, the damage in deciduous teeth may have direct and indirect influence upon growth in successional permanent teeth. Thus, through careful treatment, the injury needs to be minimized. It is considered to be likely necessary for a right coping method when injury occurs, and above all, for enough prior education so that injury cannot occur.

유합치와 쌍생치에 관한 방사선학적 연구 (A RADIOGRAPHIC STUDY OF FUSED AND GERMINATED TOOTH)

  • The incidence and several characteristic features of fused and geminated teeth were studied radiographically, with full mouth periapical radiogram and pantomogram, in 4201 patients of mixed dentition and 5358 patients of permanent dentition. The obtained results were as follows: 1. The prevalence was revealed to 2.86%, 0.32%, 0.33%, and 0.06% in deciduous fused tooth, permanent fused tooth, deciduous geminated tooth and permanent geminated tooth respectively, and these anomalies were occured in female more than male. 2. Fused teeth were observed predominantly in lower anterior teeth area, especially in lateral incisor and canine region, and many cases of deciduous geminated tooth were observed in upper central incisor region. 3. Congenital missing rates of succedaneous tooth in deciduous fused teeth were 57.1 %, 85.7%, 71.0%, 69.0% in upper right and left central-lateral incisor regions, lower right and left lateral incisor-canine regions, respectively. 4. Prevalence of dental caries was 42.3%, 18.8% and 5.6% in deciduous fused, deciduous geminated and permanent fused tooth, respectively. 5. In classifying of fused and geminated teeth into 9 types, by following appearance such as number of crown, root, pulp chamber and pulp canal of those teeth, it was more favorable that Type Ⅰ(2 crown, 2 root, 2 pulp chamber, 2 pulp canal) in deciduous fused tooth and Type Ⅸ (I crown, 1 root, 1 pulp chamber, 1 pulp canal) in permanent fused tooth, deciduous and permanent geminated tooth.

어린이의 외상성 치아손상에 관한 연구 (A Study on the Traumatic Teeth Damage of Children)

  • 현대사회에서는 생활환경의 변화와 교통사고의 증가에 의해 해마다 소아의 외상의 증가하고 있는데 구강조직에 대한 외상성 손상을 완전히 치단하는 데에는 한계가 있어 외상성 치아손상에 관한 실태를 조사 분석함으로써 외상처치와 예방에 필요한 기초자료를 제시하고자 강릉대학교 치과병원 소아치과에 2000년 10월 4일부터 2004년 2월 27일까지 외상을 주소로 내원한 113명의 환자를 대상으로 조사 분석한 결과 다음과 같은 결론을 얻었다. 1. 연구대상자의 성별에서는 2.05:1의 비율로 남자의 경우가 외상빈도가 더 높게 나타났다. 평균 연령은 남자 5.27세, 여자 5.27세로 차이가 없었고, 전체 연령중 2세에서 외상환자가 제일 많았다(21.2%) 2. 외상부위별 내원기간을 조사한 결과 유치는 당일(34.4%)에 내원한 경우가 가장 많았고, 영구치는 1일 후(38.8%)에 내원한 경우가 가장 많았다. 3. 외상부위별 치아손상의 원인을 분석한 결과 넘어진 경우가 유치 59.4%, 영구치 55.1%, 물체에 부딪친 경우 유치 26.6%, 영구치 26.5%로 나타나 대부분 넘어지거나 부딪쳐서 외상이 발생한 것으로 나타났다. 4. 성별에 따른 치아손상이 발생한 장소는 가정인 경우가 남자 42.1%, 여자 35.1%로 남녀모두 가정에서 외상이 가장 많이 발생하였다. 외상부위에 따른 치아손상 발생한 장소로는 유치는 가정(59.4%)에서 가장 많이 발생하였고, 영구치는 유치원 및 학교(28.6%)에서 가장 많이 발생하였다. 5. 외상부위에 따라 치아손상을 받은 치아의 수를 분석한 결과 유치는 2개(56.3%), 1개(31.3%), 3개와 4개 이상 각각(6.3%), 순으로 나타났으며, 영구치는 2개(46.9%), 1개(28.6%) 4개 이상(16.3%), 3개(8.2%) 순으로 영구치인 경우 4개 이상인 경우도 비교적 높게 나타났다. 6. 성별에 따른 치아손상이 발생한 부위로는 남자 유치 56.6%, 영구치 43.4%, 여자 유치 56.8%, 영구치 43.2%로 남녀 모두 유치에서 외상이 가장 많이 발생하였다. 7. 유치와 영구치의 치아손상부위는 상악 전치부가 거의 대부분을 차지하고, 유치(75%)와 영구치(63.8%) 모두 상악 중절치에서 외상율이 높게 나타났다. 8. 전체적인 외상성 치아손상에 따른 외상종류의 조사결과는 동요 30.2%, 치관파절 28.0%, 함입 14.7%, 진탕 8.9%, 완전 탈구 7.1%, 정출 2.2%, 변위 1.8% 순으로 나타났다. 유치와 영구치별로 외상성 치아손상의 종류를 분류하였을 때 유치는 동요(35.8%)가 가장 많이 나타났고, 파절에서 치관파절은 영구치(44.8%)에서 많이 발생하였다. 9. 외상부위에 따른 외상성 치아손상처치는 처치없이 경과관찰을 한 경우가 유치는 43.2%, 영구치 38.9%로 나타났으며, 치료한 경우 유치는 치수절제술(22.0%), 영구치는 임시적산부식레진수복(19.5%)을 가장 많이 사용하였다.

Number of existing permanent teeth is associated with chronic kidney disease in the elderly Korean population

  • Background/Aims: The aim of this study was to assess the association between the number of existing permanent teeth and chronic kidney disease (CKD) in a representative sample of the elderly Korean population. Methods: A total of 2,519 subjects who participated in the Korean National Health and Nutrition Examination Survey were cross-sectionally examined. The number of existing permanent teeth was evaluated by clinical oral examination. CKD was defined based on definition and classification by Kidney Disease: Improving Global Outcomes (KDIGO) 2012 guidelines. Multivariable logistic regression analyses were performed controlling for age, gender, income, education, tooth-brushing frequency, periodontitis, state of dentition, smoking, alcohol consumption, hypertension, obesity, diabetes mellitus, and hypercholesterolemia. Subgroup analyses by age and gender were also performed. Results: The number of teeth was significantly associated with CKD after controlling for all potential confounders (adjusted odds ratio [AOR], 1.67; 95% confidence interval [CI], 1.04 to 2.70 for lower number of teeth; AOR, 1.59; 95% CI, 1.14 to 2.23 for moderate number of teeth). In the subgroup analyses, the association was highlighted in females aged 75 years over (AOR, 2.55; 95% CI, 1.05 to 6.20 for lower number of teeth; AOR, 1.95; 95% CI, 1.01 to 3.80 for moderate number of teeth). Conclusions: Our findings suggest that the number of existing permanent teeth may be associated with CKD among Korean elderly.

유치와 영구치 상아질의 전처리 시간에 따른 표면 미세구조 변화와 글라스 아이오너머 수복재의 결합력에 관한 연구 (A STUDY ON THE EFFECT OF DENTIN WITH VARIOUS PRETREATMENT APPLICATION TIME ON THE DENTIN SURFACE STRUCTURE AND THE BOND STRENGTH OF GLASS IONOMER RESTORATIVE MATERIALS)

  • The main purpose of this study was to evaluate the effect of dentin with various pretreatment application time on the dentin surface structure and the bond strength of glass ionomer restorative matrials between primary and permanent tooth. There are difference of composition, structure and pretreated effect of dentin conditioning between the primary and permanent teeth. Inspite of thease fact, we used the same application time of permanent tooth. This study intends to determine the most effective application time using SEM analysis of the surface change by dentin conditioning. Forty five intact primary and permanent teeth were splited mesiodistally with seperating diamond disk. Each eight specimens were prepared for shear bond strength test. (randomely devided eight group as follows:) group I conditioning for five seconds group II conditioning for ten seconds group III conditioning for fifteen seconds group IV conditioning for twenty seconds Additional specimens were prepared to SEM analysis. The result are as follows: 1. Primary teeth are higher shear bond strength in group I and group II. 2. There was no statistically significant difference between primary and permanent teeth in group III and group IV. 3. In SEM analysis, smear layer removal by dentin pretreatment of primary teeth was faster than permanent teeth. Primary teeth may require shorter pretreatment time of dentin for G-I restoration than that of permanent teeth.
