Periodontal prosthesis on medically compromised patient with few remaining teeth: hybrid telescopic double crown with friction pin method (의과적 문제가 있고 소수 잔존치를 가지는 환자에서의 치주보철 임상증례: 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관법)

  • Successful results of treatments using double crown prostheses for the partially edentulous patients who have a few remaining teeth have been reported in several journals. A double crown removable partial denture can be an alternative treatment for the patients with a poor periodontal condition of remaining teeth. Since a double crown removable partial denture can be applied without the risk of surgical operation to the medically compromised patients with a poor periodontal condition which is inadequate for dental implants, it has psychological and economical advantages. In this case, there were sufficient remaining teeth to be restored with fixed prostheses in maxilla, while there were a few remaining teeth with a very poor periodontal condition so that it was almost impossible to restore with a clasp removable partial denture using these remaining teeth in mandible. In addition, the patient had the medical history of surgical operation due to osteomyelitis in the mandibular anterior areas a year ago, thus difficult to conduct an implant placement. The main objective of this report is to introduce our case because a double crown partial denture using a few mandibular remaining teeth showed satisfactory results in functional and esthetical aspects during more than two years follow-up period in this unfavorable condition.

Effects of mouth rinsing with foam vitamins and its intake on reduction in oral microorganisms (발포비타민을 이용한 구강세정 및 섭취가 구강내 치주질환 유발 세균 감소에 미치는 효과 비교)

  • Objectives: This study was conducted to examine the subjects' oral health status and changes in the oral environment after mouthwash with effervescent Vitamin C and its intake and to understand the impact on changes in the number of bacteria causing periodontal diseases in the oral cavity based on the oral health status and oral environmental condition. Methods: After obtaining approval from the Institutional Review Board of D-University, 45 people (24 in an experimental group and 21 in a control group) who participated in the oral health program were selected, among visitors of the Clinical Dental Hygienics Laboratory of D. University in B. Results: Based on the subjects' periodontal health status, the number of bacteria causing periodontal diseases in the oral cavity decreased in both experimental and control groups before and after the experiment. However, in the experimental group, it was significant when the Bleeding on Probing(BOP) level was below average, and Calculus index(CAL) was above average, in Aggregatibacter actinomycetemcomitans and the red complex. It was also significant when CAL was above average in the orange complex in the control group. Regarding bacterial changes in periodontal diseases based on the subjects' oral environmental condition, there was a significant change in the Adenosine Tri Phosphate (ATP : intraoral contaminants test) test in the oral cavity in A. actinomycetemcomitans and the red complex in the experimental group and in the O'Leary index in the control group. Further, there was a significant change in the orange complex in the ATP test in the experimental group. Conclusions: A comprehensive analysis of the results revealed that the oral health care program for dental hygiene decreased the number of bacteria causing periodontal diseases in the oral cavity in both experimental and control groups.

Effects of chitosan on the characteristics of periodontal ligament, calvaria cells and gingival fibroblasts (Chitosan이 치주인대, 두개관 및 치은섬유아세포의 성상에 미치는 영향)

  • Chitosan, with a chemical structure similar to hyaluronic acid, has been implicated as a wound healing agent. The purpose of this research was to evaluate the effects of chitosan on the characteristics of periodontal ligament cells, calvaria cells and gingival fibroblasts and to define the effects of chitosan on bone formation in vitro. In control group, the cells were cultured alone with Dulbecco's Modified Eagle's Medium contained with 10% Fetal bovine serum, 100unit/ml penicillin, $100{\mu}g/ml$ streptomycin, $0.5{\mu}g/ml$ amphotericin-B. In experimental group, chitosan($40{\mu}g/ml$) is added into the above culture condition. And then each group was characterized by examining the cell proliferation at 1,3,5,7,9,12,15 day, the amount of total protein synthesis, alkaline phosphatase activity at 3, 7 day and the ability to produce mineralized nodules of rat calvaria cell at 11 day. The results were as follows : 1. At early time both periodontal ligament cells and calvaria cells in chitosan-treated group proliferated more rapidly than in non-treated control group, but chitosan-treated group of periodontal ligament cells at 9 days and calvaria cells at 12days showed lower growth rate than control group. Gingival fibroblast in chitosan-treated group had lower growth rate than in control group but the difference was not statistically significant (P< 0.01).2. Both periodontal ligament cells and calvaria cells in chitosan-treated group showed much protein synthesis than in control group at 3 days, but showed fewer than in control group at 7 days. Amount of total protein synthesis of gingival fibroblast didn't have statistically significant difference among the two groups(P< 0.01). 3. At 3 and 7 days, alkaline phosphatase activity of periodontal ligament cells and calvaria cells was increased in chitosan-treated group, but at 7 days there was not statistically significant difference among the two groups of calvaria cells (P< 0.01). Alkaline phosphatase activity of gingival fibroblast didn't have statistically significant difference among the two groups(P<0.01). 4. Mineralized nodules in chitosan-treated group of rat calvaria cells were more than in control group. In summery, chitosan had an effect on the proliferation, protein systhesis, alkaline phosphatase activity of periodontal ligament cells and calvaria cells, and facilitated the formation of bone. It is thought that these effects can be used clinically in periodontal regeneration therapy.

Effect of Armeniacae Semen Extracts on Human Gingival Fibroblasts and Periodontal Ligament Cells under the High Glucose Conditions (행인 추출물이 고포도당 상태의 치은섬유아세포 및 치주인대세포에 미치는 영향)

  • The purpose of this study was performed to evaluate the effect of Armeniacae Semen extracts on human gingival fibroblasts and periodontal ligament cells in vitro. A experiment was done to evaluate the effect of Armeniacae Semen extracts in high glucose media. $400mg/d{\ell}$ glucose was added to the culture media of all groups. In control group, the cells($4.5{\times}10^4cells/ml$) were cultured with Dulbecco's Modified Eagle's Medium contained with 10% fetal bovine serum. In experimental groups, Armeniacae Semen extracts was added to the above culture media at the final concentrations of $1{\mu}g/m{\ell}$(Test group 1) and $l0{\mu}g/m{\ell}$(Test group 2). Then each group was tested for the rate of cell proliferation at 1, 2, 5 days, protein levels at 2, 5 days, and alkaline phosphatase activity at 2, 5 days. The results were as follows ; 1. Under the high glucose condition 1)As centration of Armeniacae Semen extracts increased, the rate of cell proliferation decreased significantly in test group 2 at 5 days in human gingival fibroblasts, but increased significantly in test group 2 at 5 days in human periodontal ligament cells(P<0.05). 2)In human gingival fibroblasts, test group 2 showed significantly decreased protein levels as compared to control group at 5 days. In periodontal ligament cells, test group 1 and 2 showed not significantly increased protein levels as compared to control group at 2, 5 days(P<0.05). 3)Alkaline phosphatase activity of human periodontal ligament cells increased as concentration of Armeniacae Semen extracts increased. The test group 1and 2 showed significant increase as compared to control group at 5 days(P<0.05). From the above results, Armeniacae Semen extracts appeared to enhance cellular activities including the rate of cell proliferation, protein levels and alkaline phosphatase activity of selectively human periodontal ligament cells in high glucose media. This study suggests that Armeniacae Semen extracts seem to be able to subside the inflammation of periodontal tissue and regenerate the destructed periodontal tissue.

The distribution of Xive implant patients and the type of implant site and survival rate in mandible (하악에 식립된 Xive implant 환자의 분포 및 식립부 유형과 생존율)

  • This study is an analysis of distribution of patients who installed Xive implant in Yonsei University Hospital and types of implant site for about 2 years recall check and cumulative survival rate. 41 implant were used in this study. It shows the conclusion below. 1. Patients at the age of 40s and 50s were 60% of all implant cases and average number of implant was 2.4(man), and 1.9(woman). All cases were operated on mandible, 3 implants on anterior region and 38 implants on posterior region. 2. The major cause of tooth loss is dental caries(48.8%), followed by periodontal disease. 3. Most distribution of bone qaulity for mandibular implant site was type II(65.8%) and bone quantity was type B(75.6%). 4. The majority of implants were those of 11, 13mm in length(95%) and regular diameter in width (64%). 5. The 41(19 persons) Xive implants that were placed in the mandibular anterior and posterior region were all survival and showed a 100% 2 year cumulative survival rate. The results provided us with basic data on patient type, implant distribution, bone condition, and survival rate. We wish that our results coupled with other research data helps assist in the further study for better implant success rates, etc.

Periodontal status in patients with osteoporosis (골다공증 환자의 치주조직 상태)

  • The purpose of this study was to evaluate the relationship between osteoporosis and periodontal condition in postmenopausal women. Twenty-eight patients who have been treated at Chonnam national university hospital with osteoporosis(osteoporotic group, mean $age62.2{\pm}6.6$) and 21 normal postmenopausal women with periodontitis(control group, mean $age60.4{\pm}3.7$) were examined. Bone mineral density(BMD) of lumbar spine(L2-L4) was measured by dual energy X-ray absorptiomemtry(DEXA). Percentage to peaked bone mass in osteoporotic group was 70.9% which had lower BMD than in control group(p<0.05). The number of present teeth were 23.6 in osteoporotic group and 23.1 in control group. Percentage of bleeding on probing was 41% in osteoporotic group and 37% in control group. probing depth and attachment level were shown 3.18 mm, 3.63 mm in osteoporotic group and 2.85mm, 3.11mm in control group, respectively. Probing depth and attachment level were significantly greater in osteoporotic group than in control group(p<0.05). The significant negative correlation was found between BMD level and periodontal attachment level(p<0.001, ${\gamma}=-0.56$). These results suggest that osteoporosis may be associated with periodontal breakdown.

CLINICAL STUDY ON THE EFFECT OF WIND INSTRUMENT PLAYING ON PERIODONTAL CONDITIONS (관악기 연주자의 치주상태에 관한 연구)

  • This study investigates the effect of wind instrument playing on the periodontal condition by comparing and analyzing wind instrument players and non-wind instrument players by various clinical paramenters, including periodontal index, bleeding index, simplified oral hygiene index, and non-functional tooth index. The subjects consist of 65 wind instrument players and 31 non-players. The players were divided into 4 groups ; Those who use cup-shaped mouthpieces as Group A, single reed mouthpieces as Group B, double reed mouthieces as Group C, and hole mouthpieces as Group D. Non-players are designated as Group E. The results are as follows ; 1. Periodontal Index Group A recorded $1.2{\pm}0.7$, Group B $1.1{\pm}0.7$, Group C $1.3{\pm}0.5$, Group D $2.1{\pm}0.5$, and Group E $0.9{\pm}0.6$, with no statistically significant differences among the groups. 2. Bleeding Index Group A shwed 0.23, Group B 0.25, Group C 3.0, Group D 0.38, and Group E 0.23, with no statistically significant differences among the groups. 3. Non-functional Tooth Index Group A recorded 2.0, Group B 1.0, Group C 3.0, Group D 0, Group E 0.7, with no statistically significant differences among the groups. 4. Simplified Oral Hygiene Index Buccal sides of maxillary 1st molars, lingual sides of mandibular 1st molars, labial sides of maxillary anterior teeth, and lingual sides of mandibular anterior teeth were investigated, and there was no statistically significant differences among the groups.

EFFECT OF AMINOACETONITRILE ON THE DEVELOPING RAT PERIODONTAL MEMBRANE (Aminoacetonitrile이 백서발육치근막에 미치는 영향에 관한 실험적 연구)

  • It has been studied that aminoacetonitrile was associated with the inhibition of collagen fiber, argyrophilic fiber and oxytalan fiber synthesis. This experiment was performed, by the basic knowledge of above mentioned study, to study on the biological effect of aminoacetonitrile to the developing periodontal ligament in Sprague Dawley rat. twenty two of female rats weighing about 200gm were gestated. In 7 days after gestation, the experimental rats were injected aminoacetonitrile 7 times intraperitoneally. After parturition, delivered fJtuses were divided into 4 groups and each group was sacrificed to 1 day, 7 days, 14 days, and 21 days after delivery, schematically. All the fetuses were observed on their periodontal ligament by histological and histo chemical methods. To study on the components of periodontal ligament fiber in these experimental study van Gieson, Masson's trichrom, argyrophilic fiber, oxytalan fiber, methyl green pyronin and periodic acid-Shiff staining were performed. Results were as follows; 1) Retardation of functional orientation in periodontal ligament collagen fiber was observed in 1 day fetuses hut this appearance was diminished gradually and recovered in normal condition in 7 days fetuses. 2) Distribution of argyrophilic fiber in 1 day fetuses was oriented delicately and loosely but volume of this fiber was gradually thickened and distributed densely. 3) Oxytalan fiber was oriented dendritic ally and contradictorily in 14 days fetuses but their orientation was changed into oblique form in middle portion of roof and their numbers were increased gradually. 4) Pyronin-philic stain of fibroblast was gradually deepened in 7 days fetuses and this finding also suggested to the depreciation of collagen synthesis in this specimen. 5) PAS positive line was observed continuousely at the portion of cervical to the middle root surface.

  • Number of fixtures supporting prosthesis for rehabilitation of partial edentulism in distal area is an important factor in distal area to the bone tissue response around dental implant. Optimal number and optimal positioning of dental implant has leaded to the stable condition of bone tissue and successful long-term treatment outcome. This clinical and radiographic study was performed to document and evaluate the short-term result of occlusal rehabilitation by means of implant-supported fixed prostheses (ISPs) especially for partial edentulism in distal area in patients treated for advanced periodontal disease and to verify the number of fixture affecting the bone tissue response. A total of 30 consecutive patients referred because of advanced periodontal disease were included. Before the implant therapy was initiated, periodontal treatment was performed and the outcome evaluated during at least a 6-month period. An individual maintenance care program was designed for each patient. All 75 implants were placed using a 2-stage surgical approach. The patients were divided into 2 groups, in one of which two fixtures were placed and in the other of which three fixtures were placed with tripodal geometry. Following installation of the ISPs, all patients underwent a baseline examination including evaluation of i) oral hygiene, and ii) periodontal/ peri-implant conditions, and iii) radiographs. These examinations were repeated annually during the 1 or 2-year observation period. The results were as follows: 1.No single implant was lost during the observation follow-up period. 1.The percentage of plaque harboring surfaces and bleeding units upon probing were found to be low (<10%), and no soft tissue complications were recorded. 1.Two-fixture group showed bone destruction ranged from 0.0mm to 1.5mm and the mean was 0.31mm. Three-fixture group showed more bone destruction of 0.51mm. There was no statistically difference between two groups. These results suggested that the factor for success is not the number of fixture but the strict maintenance of peri-implant tissue health and initial stability of fixture.

The expression of a nitric oxide derivative, tissue inhibitors of metalloproteinase-3, and tissue inhibitors of metalloproteinase-4 in chronic periodontitis with type 2 diabetes mellitus

  • Purpose: The purpose of this study was to analyze the expression of inducible nitric oxide synthases (iNOS), tissue inhibitors of metalloproteinase $(TIMP)_{-3}$, and $TIMP_{-4}$ in the gingival tissues of periodontal patients with or without type 2 diabetes mellitus (DM). Methods: Depending on the patient's systemic condition and clinical criteria of the gingiva, each gingival sample was classified into one of three groups. Sixteen clinically, systemically healthy patients (group 1), 16 periodontal patients (group 2), and 16 periodontal patients with DM (group 3) were included. Tissue samples in each group were collected, prepared, and analyzed by western blotting. Quantification of the relative amount of $TIMP_{-3}$, $TIMP_{-4}$, and iNOS was performed. Results: The expression levels of iNOS and $TIMP_{-3}$ both increased in group 1, group 2, and group 3 in increasing order, and were significantly higher in both group 2 and group 3 as compared to group 1 (P<0.05). The expression levels of $TIMP_{-4}$ increased in the same order, but significantly increased in group 2 as compared to group 1, in group 3 as compared to group 1, and group 3 as compared to group 2 (P<0.05). Conclusions: This study demonstrated that iNOS, $TIMP_{-3}$, and $TIMP_{-4}$ might be involved in the progression of periodontal inflammation associated with type 2 DM. It is thought that further study of these factors can be applied practically for the diagnosis and control of periodontitis in diabetics.
