The Surgical Treatment of Chronic Constrictive Pericarditis -A Report of 35 Cases- (만성교약성 심낭염의 외과적 요법 -35예 보고-)

  • An analysis of &5 cases of constrictive pericarditis treated surgically in this department of Seoul National University Hospital between the years 1958 and 1974 has been presented. 1.Of the patients with constrictive pericarditis,29 were males and 6 were females. Their ages averaged 23.8 years. 2. All patients who had pericardiectomy showed excellent postoperative results, and no operative mortality was noted in this group of patients but 3 hospital death [9.1%] occurred. 3. Besides shortening of circulation time elevated venous pressure was restored to normal range after pericardiectomy from a preoperative average 25.8cm $H_2O$ to a postoperative average 14.8cm $H_2O$. 4. Electrocardiographic changes consisted chiefly of low voltage complexes, P-wave and T-wave changes and characterized by vertical or semivertical heart position. The ECG was returned to normal or near normal after Pericardiectomy. 5. There were seven postoperative complications in this series. 6. In ten cases [32.3%], the pathology revealed evidence of tuberculosis in the pericardium or the myocardium, and others were described as chronic, proliferative, fibrous pericarditis or hyalinization of the pericardium.

  • Objective & Method : By investigating physiological function of tri-energizer, symptoms, and organ coordination, we obtained following conclusion. Tri-energizer is the membrane structure that surrounds the five visceral organs and six hollow organs, and filled with fluid. Tri-energizer acts as the passage for the flow of qi energy. Result & Conclusion : Therefore, dysfunction of the tri-energizer is caused by abnormal evaporation and metabolism. Upper-energizer regulates cardiopulmonary function, middle-energizer regulates spleen and stomach functions, and lower-energizer regulates liver, kidneys, small and large intestines, and bladder functions. Such a functional specialization is possible by receiving the source of qi through the wall wrapping around the internal organs. Tri-energizer represents the exterior and interior relationship by acting as the membrane structure supporting the five visceral organs and six hollow organs and at the same time, as the pericardium surrounding the heart.

  • Aortic valve repair with the use of tailored autologous pericardial extension to the native cusp was performed in one patient with rheumatic aortic valve incompetence. The patient was a 10-year-old girl with Grade II aortic regurgitation and tiny postoperative recannalization of the patent ductus arteriosus. The left aortic coronary cusp appeared to be a little thickened and a cicatrical shortening of the distance between the free edge of the cusp and its annular attachment. A semilunar shaped patch of autologous pericardium, treated with glutaraldehyde solution[6 minutes in 0.6% solution] was sutured along the free edge of the left coronry cusp. Postoperative recovery was uneventful. Echocardiography 8 months later showed Grade I aortic regurgitation. She is now conducting as usual life.

A Large Dumb Bell Shaped Desmoid Tumor in Chest Wall: A Case Report (흉벽에 발생한 거대한 유건종 1례)

  • Desmoid tumor is found most often in the anterior abdominal wall of parous women. Although it may originate in virtually any musculotendinous structure, those of the chest wall are rare. We experienced a case of large dumb bell shaped desmoid tumor originated in intercostal muscle and invaded anterior chest wall, pericardium, pleura and the lung. The patient was healthy in appearance except a painful swelling on the anterior chest wall. Roentgenographic studies demonstrated a huge homogenous mass in the right anterior chest cavity. He was treated with resection of the tumor including .anterior chest wall, a portion of the pericardium, middle lobe, and part of upper & lower lobes of the right lung because of tumor invasion. The tumor composed with two parts, one [$5{\times}4{\times}3$cm in size] is over the rib cage and another [$10{\times}15{\times}10$cm in size] is in the right chest cavity. Postoperative course was uneventful and there was no evidence of recurrence until last visit, 5 months after surgery.

Peritoneopericardial diaphragmatic hernia in a immature dog (미성숙견에서 선천성 Peritoneopericardial diaphragmatic hernia 발생례)

  • A 5-month-old, 5kg, intact male mixed dog was admitted to the Veterinary Medical Teaching Hospital of the College of Veterinary Medicine Cornell University because of severe systolic murmur. On physical examination, the dog appeared to be thin and slightly small for his age. Radiographic studies with barium sulfate clearly showed the presence of loops of intestines in the pericardium. Based on these findings peritoneopericardial diaphragmatic hernia was diagnosed. Many abnormalities were found in dog's heart : ventricular septal defect, pulmonic stenosis with moderate pulmonic valve insufficiency, and mild mitral and tricuspid valve insufficiency. Puppy's longterm prognosis was fair. His intestines were removed from his pericardium and the defect in his diaphragm was closed. The surgery went smoothly and puppy recovered very nicely from anesthesia.

  • In the bronchial stenosis due to benign causes, bronchoplastic procedure has been considered as one of the best surgical treatment, because of preserving normal lung tissue below the affected bronchi. We have treated 2 patients (tracheal leiomyoma, bronchial stenosis due to chronic inflammatory cicatrization) that suffered from benign tracheal and bronchial stenosis by bronchoplastic procedure using autologous costal cartilage covered with pericardium. Patients showed good patency of bronchoplastic bronchi in bronchoscopic examination that was performed at 6 months afte the operation.

  • Objectives : To research origin of the shape of vital gate(命門) in Yixuerunmen makes us understand more exactly what the author, Li Chan(李梴) thought vital gate shaped. Methods : It's shape described in his book was compared with pictures portraying it in Hua Tuo Xuan Men Nei Zhao Tu(華陀玄門內照圖), one of references of his book. Results : He reasonably modified its passing track, while explaining it according to the paintings in Hua Tuo Xuan Men Nei Zhao Tu. Vital gate, as he thought, was not an real organ like the other five viscera, but a cord or a tube similar to blood vessels. He believed its cord had long connections from pericardium to terminal of urethra, which went through pericardium upward, right kidney downward, right around terminal rectum down-frontward, and urethra in parallel outward. Conclusions : He had consistent understandings for vital gate to penetrate several different viewpoints, as based on pictures in Hua Tuo Xuan Men Nei Zhao Tu.

  • Huge chondrosarcoma is a rare form of primary malignant tumor of the chest wall. We operated on a 60 year old female patient who had a huge anterior chest wall mass with local invasion into the pericardium and satellite tumors on the visceral pericardium of the heart. En-bloc resection of the huge tumor including both upper 3 ribs, both clavicles, manubrium of the sternum, pleura, and pericarium, was followed by complex chest wall reconstruction using a Gore-tex soft tissue patch and latissimus dorsi musculocutaneous free flap.

  • A pericardial recess is frequently seen in patients undergoing chest computed tomography (CT). It is important to be aware of the normal anatomy of the pericardium as it is often mistaken for normal variants and/or disease. Therefore, we will describe the anatomy and location of the pericardial recess and the specific findings in various diseases associated with the pericardial recess.

Anti-calcification of Bovine Pericardium for Bioprosthetic Heart Valves after Surface Modification with Hyaluronic Acid Derivatives

  • Surface modification of glutaraldehyde fixed bovine pericardium (GFBP) was success­fully carried out with hyaluronic acid (HA) derivatives. At first, HA was chemically modified with adipic dihydrazide (ADH) to introduce hydrazide functional group into the carboxyl group of HA backbone. Then, GFBP was surface modified by grafting HA-ADH to the free aldehyde groups on the tissue and the subsequent HA-ADH hydrogel coating. HA-ADH hydrogels could be prepared through selective crosslinking at low pH between hydrazide groups of HA-ADH and crosslinkers containing succinimmidyl moieties with minimized protein denaturation. When HA­ADH hydrogels were prepared at low pH of 4.8 in the presence of erythropoietin (EPO) as a model protein, EPO release was continued up to $85\%$ of total amount of loaded EPO for 4 days. To the contrary, only $30\%$ of EPO was released from HA-ADH hydrogels prepared at pH=7.4, which might be due to the denaturation of EPO during the crosslinking reaction. Because the carboxyl groups on the glucuronic acid residues are recognition sites for HA degradation by hyaluronidase, the HA-ADH hydrogels degraded more slowly than HA hydrogels prepared by the crosslinking reaction of divinyl sulfone with hydroxyl groups of HA. Following a two-week subcutaneous implantation in osteopontin-null mice, clinically significant levels of calcification were observed for the positive controls without any surface modification. However, the calcification of surface modified GFBP with HA-ADH and HA-ADH hydrogels was drastically reduced by more than $85\%$ of the positive controls. The anti-calcification effect of HA surface modification was also confirmed by microscopic analysis of explanted tissue after staining with Alizarin Red S for calcium, which followed the trend as observed with calcium quantification.
