  • Acupuncture has been used as a clinical treatment in Oriental medicine for various diseases. In the present study was carried out to investigate the effects of acupuncture and electrical stimulation on the change neuronal nitric oxide synthase(nNOS) immunoreactive cells in the periaqueductal gray(PAG) area of the male SD rats. Enhanced expression of nNOS was detected in the dorsolateral-PAG(DL-PAG) area of rat with stress by fixed body, and acupuncture and needle electrode electrical stimulation groups at Hapgok like acupoint decreased the stress-induced enhancement in the expression of nNOS. The present results demonstrate that acupuncture and needle electrode electrical stimulation is effective in the modulation of expression of nNOS in the DL-PAG area under stress conditions.

  • Background: Resting-state functional connectivity (rs-FC) may aid in understanding the link between painmodulating brain regions and the descending pain modulatory system (DPMS) in fibromyalgia (FM). This study investigated whether the differences in rs-FC of the primary somatosensory cortex in responders and non-responders to the conditioned pain modulation test (CPM-test) are related to pain, sleep quality, central sensitization, and the impact of FM on quality of life. Methods: This cross-sectional study included 33 females with FM. rs-FC was assessed by functional magnetic resonance imaging. Change in the numerical pain scale during the CPM-test assessed the DPMS function. Subjects were classified either as non-responders (i.e., DPMS dysfunction, n = 13) or responders (n = 20) to CPM-test. A generalized linear model (GLM) and a receiver operating characteristic (ROC) curve analysis were performed to check the accuracy of the rs-FC to differentiate each group. Results: Non-responders showed a decreased rs-FC between the left somatosensory cortex (S1) and the periaqueductal gray (PAG) (P < 0.001). The GLM analysis revealed that the S1-PAG rs-FC in the left-brain hemisphere was positively correlated with a central sensitization symptom and negatively correlated with sleep quality and pain scores. ROC curve analysis showed that left S1-PAG rs-FC offers a sensitivity and specificity of 85% or higher (area under the curve, 0.78, 95% confidence interval, 0.63-0.94) to discriminate who does/does not respond to the CPM-test. Conclusions: These results support using the rs-FC patterns in the left S1-PAG as a marker for predicting CPM-test response, which may aid in treatment individualization in FM patients.

  • Rodents exposed to a 15-min pretest swim in the forced swimming test (FST) exhibit prolonged immobility in a subsequent 5-min test swim, and antidepressant treatment before the test swim reduces immobility. At present, neuronal circuits recruited by antidepressant before the test swim remain unclear, and also less is known about whether antidepressants with different mechanisms of action could influence neural circuits differentially. To reveal the neural circuits associated with antidepressant effect in the FST, we injected desipramine or citalopram 0.5 h, 19 h, and 23 h after the pretest swim and observed changes in c-Fos expression in rats before the test swim, namely 24 h after the pretest swim. Desipramine treatment alone in the absence of pretest swim was without effect, whereas citalopram treatment alone significantly increased the number of c-Fos-like immunoreactive cells in the central nucleus of the amygdala and bed nucleus of the stria terminalis, where this pattern of increase appears to be maintained after the pretest swim. Both desipramine and citalopram treatment after the pretest swim significantly increased the number of c-Fos-like immunoreactive cells in the ventral lateral septum and ventrolateral periaqueductal gray before the test swim. These results suggest that citalopram may affect c-Fos expression in the central nucleus of the amygdala and bed nucleus of the stria terminalis distinctively and raise the possibility that upregulation of c-Fos in the ventral lateral septum and ventrolateral periaqueductal gray before the test swim may be one of the probable common mechanisms underlying antidepressant effect in the FST.

  • Sarcodon aspratus is the mushroom of Telephoracea which was been classified into Alphllophorales. The aqueous extract of Sarcodon aspratus in known to have anti-tumor activity, immune modulatory effect, and anti-oxidative action. The descending pain control system consists of three major components: the periaqueductal gray (PAG) of the midbrain, the rostroventral medulla including the nucleus raphe magnus, and the spinal dorsal horn. Glutamate is the primary excitatory neurotransmitter in the brain. Glutamate ionotropic receptors are classified as N-methyl-D-aspartate (NMDA) receptor, ${\alpha}$-amino-3-hydroxy-5-methyl-4-isoxazolepropionate (AMPA) receptor, and kainate receptor. In the present study, the modulation of Sarcodon aspratus on the ion currents activated by glutamate, NMDA, AMPA, and kainate in the acutely dissociated PAG neurons was investigated by nystatin-perforated patch-clamp technique under boltage-clamp condition. Sarcodon aspratus increased glutamate- and NMDA-induced ion currents were not increased by Sarcodon aspratus. The present results show that Sarcodon aspratus may activate the descending pain control system in rat PAG neurons through NMDA receptor.

  • Objectives : The aim of this study was to investigate the effect of various electroacupuncture stimulation on neuronal nitric oxide synthase(nNOS) in cerebral cortex, brain stem, cerebellum of spontaneously hypertensive rats. Methods : We evaluated the changes of nNOS-positive neurons using a immunohistochemical method. The staining intensity of nNOS positive neurons was assessed in a quantitative fashion using a microdensitometrical method based on optical density by means of an image analyzer. Results : The average optical density of nNOS-positive neurons of 100 Hz (bipolar square wave 0.2 ms duration and 100 Hz frequency) electroacupuncture treatment group significantly decreased in most cortical areas comparison between the manual acupuncture and 2 Hz (bipolar square wave 0.2 ms duration and 2 Hz frequency) electroacupuncture groups. In the brain stem, the optical density of nNOS-positive neuron at superficial gray layer of the superior colliculus area, dorsolateral periaqueductal gray area and paralemniscal nucleus were same as cerebral cortex. Conclusion : We conclude that the morphological evidence for nNOS-positive neurons may be have regional change in cerebral cortex brain stem and cerebellum according to various electroacupuncture stimulations.

  • Pain is a complex symptom consisting of a sensation underlying potenial disease and associated emotional state. Acute pain is a reflex biological response to injury, in contrast, chronic pain consists of pain of a mininum of 6 months duration and associates with physical, emotional past experience, economic resources of the patient, family and society. Moreover, chronic pain is characterized by physiological affective and behavioral responses that are quite different than those of acute pain. The different type of stimuli exciting pain receptor are mechanical, thermal and chemical stimli and chronic pain are concerned with three of all stimli. The major three components of pain central(Analgesia) system in the brain and spinal cord are 'periaqueductal gray area of the mesencephalon', 'the raphe magnus nucleus' and 'pain inhibitory complex located in the dorsal horns of the spinal cord'. But unfortunately, the central biochemical mechanisms of chronic pain are not clearly defined. To proper management of chronic pain, comprehensive urderstanding as a psychosomatic aspect and multidisciplinary therapeuti-team approach must be emphasized.
