한국판 일상생활활동중심 작업기반 신경행동평가(A-ONE)의 개발 및 평가 (Cross-cultural Adaptation and Psychometric Evaluation of the Korean Version of the A-ONE)

  • 목적 : 본 연구는 타문화권에서 개발된 일상생활활동중심 작업기반 신경행동평가(A-ONE)를 국내 임상에서 사용할 수 있도록 한국판을 개발하고 한국 환자에게 적용하여 신뢰도와 타당도를 검증하고자 하였다. 연구방법 : 영문판 A-ONE을 한국어로 번역하고 문화적 차이를 검토하였다. 완성된 한국판을 13명의 작업치료학과 교수와 작업치료사에게 국내 적용 가능성을 확인받고, 뇌졸중 환자 42명을 대상으로 신뢰도와 타당성을 검증하였다. 결과 : 총 3개의 문항을 국내 문화에 알맞도록 수정한 결과, 한국판 A-ONE은 Index at the item level(I-CVI)=0.92-1.00을 보이며 평가 문항들이 국내 문화를 잘 대표하고 있는 것으로 나타났다. Functional Independence Measure(FIM)과 상관관계는 의사소통영역을 제외하고 r=0.52-0.77(p>0.01)로 높게 나타난 반면, Lowenstein Occupational Therapy Cognitive Assessment(LOTCA)와는 전반적으로 유의미한 상관관계를 보이지 않았다(p>0.05). 내적일치도는 기능적 독립성이 Cronbach's α=0.58-0.93, 신경행동손상이 α=0.42-0.93을 보였다. 검사-재검사 신뢰도는 기능적 독립성이 Intraclass correlation coefficient(ICC)=0.79-1.00, 신경행동손상이 ICC=0.74-1.00으로 높은 수준을 보였다. 마지막으로, 검사자간 신뢰도는 기능적 독립성이 ICC=0.75-1.00, 신경행동손상이 ICC=0.72-1.00으로 높은 신뢰도를 보였다. 결론 : 한국판 A-ONE은 뇌졸중 환자의 일상생활활동 수행능력과 신경행동손상의 종류 및 손상정도를 평가하기 위해 국내 임상에서 사용될 수 있는 유용한 평가도구이다.

ICR 마우스 모델을 이용한 녹용 추출물의 생화학적 평가 및 급성 경구 독성을 포함한 세포 독성 효과 (Biochemical Assessment of Deer Velvet Antler Extract and its Cytotoxic Effect including Acute Oral Toxicity using an ICR Mice Model)

  • 녹용은 수많은 연구에서 면역력 강화를 포함한 영양 및 의학적 가치를 입증하였으며 전통적인 약으로 널리 사용되고 있다. 본 연구는 녹용 추출물 (sample 1: 생녹용 추출물, sample 2: 건녹용 추출물, sample 3: 동결 건조 추추물)의 일반성분과 우론산, 황산화 글리코사미노글리칸, 시알릭산, 콜라겐을 포함한 유효성분을 조사하고, 액체 크로마토그래피quadrupole-time-of-light mass spectrometry (UPLC/QTOFMS)를 사용하여 녹용 추출물의 화학 성분을 분석하는데 목적이 있다. 또한, HT22 해마 세포, BV2 미세아교세포, RAW264.7 대식세포 및 HaCaT 케라틴 세포를 사용하여 MTT 분석을 통해 녹용 추출물의 세포 독성 효과 평가와 암컷과 수컷 ICR 마우스에 녹용 추출물을 각각 (0, 500, 1000, 2000 mg/kg) 경구투여 하여 급성 독성평가를 실시하였다. 투여후에는 OECD 가이드라인에 따라 마우스의 일반독성, 생존율, 체중 변화, 사망률, 임상 징후 및 부검 결과를 관찰하였다. 결과적으로 녹용 추출물은 HaCaT 케라틴 세포에서 세포 독성 효과가 없었으며, 건녹용 추출물에서는 HT22 해마 세포에서 500 ㎍/mL, RAW264.7 대식세포의 경우 1000 ㎍/mL 에서, 동결건조추출물에서는 RAW264.7 세포와 BV2 미세아교세포의 경우 500 ㎍/mL 및 1000 ㎍/mL 농도에서 세포 독성을 가지고 있음을 보였다. 그러나 마우스를 이용한 급성 독성 평가에서는 녹용 추출물 시료를 처리한 모든 마우스에서 사망률, 임상 징후 및 부검 결과 특이사항이 없었으며 이는 LD50이 2000 mg/kg 이상으로 사료된다. 그러나 인간에 대한 안전성에 대한 충분한 증거를 확보하기 위해서는 동물과 사람에 대한 추가적인 연구가 필요하다.

Open Source를 이용한 MicroPACS의 구성과 활용 (Application of MicroPACS Using the Open Source)

  • 목적 : Small-scalled PACS, Pc-based PACS로 표현되는 MicroPACS 시스템 구축에 대한 관심도가 급격하게 증가하고 있는 추세이다. MicroPACS 시스템은 PACS를 작은 규모에서 사용할 수 있도록 구성해놓은 것이고, 이 시스템을 구성하기 위해서는 DICOM viewer나 연결프로그램 등이 필요하다. 이것은 공개소스프로그램(Open Source Program)을 통해서 어느 누구나 쉽게 무료로 다운로드를 받을 수 있게 되어있다. 본 논문은 Open source program으로 MicroPACS를 직접 구성해보았고, 저장매체로서의 활용가치를 측정하기위하여 성능, 안정성 측면에서 기존의 광 저장매체(CD, DVDRAM)와 비교 분석하였다. 실험재료 및 방법 : 1. 소형 PACS를 구축하기 위해서 먼저 다음 기준에 맞는 DICOM Server Software를 검색한다. (1) 윈도우체제에서 사용가능할 것. (2) Free ware일 것. (3) PET/CT scanner와 호환되어야 할 것. (4) 사용하기 쉬워야 할 것. (5) 저장의 한계가 없어야 할 것. 2. (1) MicroPACS의 성능을 평가하기 위해 환자 1명의 Data ($^{18}F$-FDG Torso PET/CT)를 현재 Back-up장치로 쓰이는 광 저장매체(CD, DVD-RAM)와 MicroPACS에 저장하는데 소요되는 시간(Back up time)과 workstation으로 복구되기까지의 시간(Retrieval time)을 비교해 보았다. (2) PET/CT 검사를 시행했던 환자 1명의 병록번호와 검사 시행날짜를 핵의학과 직원 7명을 대상으로 알려주고 Data를 찾는데 소요되는 시간을 MicroPACS와 광 저장매체(CD, DVD-RAM)에서 각각 측정하여 비교하였다. 3. 기존의 백업장치로 쓰였던 CD들 중에서 2004년부터 2006년까지 500장을 무작위로 뽑아서 loading을 하였고 그중에서 얼마만큼의 에러가 발생하였는지를 측정하여 MicroPACS의 안정성을 비교평가하였다. 결과 : 1. Server와 DICOM viewer 기능을 갖춘 11개의 open source software 중에서 Conquest DICOM Server를 선택하였다. 2. (1) Backup과 Retrieval 시간 비교(단위 : 분)는 다음과 같다; DVD-RAM(5.13,2.26)/Conquest DICOM Server (1.49,1.19) by GE DSTE (p<0.001), CD (6.12,3.61)/Conquest (0.82,2.23) by GE DLS (p<0.001), CD (5.88,3.25)/Conquest (1.05,2.06) by SIEMENS. (2) CD ($156{\pm}46$초), DVD-RAM ($115{\pm}21$초) and Conquest DICOM Server ($13{\pm}6$초). 3. 1년간 MicroPACS에서의 데이터손실은 없었으며(0%), 500장의 CD 중에서 14개(2.8%)가 Loading하는데 실패하였다. 결론 : 현재 많은 병원에서 도입되고 있는 Full PACS를 open source software를 통하여 소규모의 PACS로 재현해 보았고, 그 결과 가능하다는 결론이 나왔다. 데이터 저장의 유용성을 평가한 결과에서 MicroPACS를 이용하는 것이 기존의 광저장매체를 사용하는 것보다 효율적이고 작업속도가 향상 된다는 것을 확인할 수 있다.

한정된 O-D조사자료를 이용한 주 전체의 트럭교통예측방법 개발 (DEVELOPMENT OF STATEWIDE TRUCK TRAFFIC FORECASTING METHOD BY USING LIMITED O-D SURVEY DATA)

  • The objective of this research is to test the feasibility of developing a statewide truck traffic forecasting methodology for Wisconsin by using Origin-Destination surveys, traffic counts, classification counts, and other data that are routinely collected by the Wisconsin Department of Transportation (WisDOT). Development of a feasible model will permit estimation of future truck traffic for every major link in the network. This will provide the basis for improved estimation of future pavement deterioration. Pavement damage rises exponentially as axle weight increases, and trucks are responsible for most of the traffic-induced damage to pavement. Consequently, forecasts of truck traffic are critical to pavement management systems. The pavement Management Decision Supporting System (PMDSS) prepared by WisDOT in May 1990 combines pavement inventory and performance data with a knowledge base consisting of rules for evaluation, problem identification and rehabilitation recommendation. Without a r.easonable truck traffic forecasting methodology, PMDSS is not able to project pavement performance trends in order to make assessment and recommendations in the future years. However, none of WisDOT's existing forecasting methodologies has been designed specifically for predicting truck movements on a statewide highway network. For this research, the Origin-Destination survey data avaiiable from WisDOT, including two stateline areas, one county, and five cities, are analyzed and the zone-to'||'&'||'not;zone truck trip tables are developed. The resulting Origin-Destination Trip Length Frequency (00 TLF) distributions by trip type are applied to the Gravity Model (GM) for comparison with comparable TLFs from the GM. The gravity model is calibrated to obtain friction factor curves for the three trip types, Internal-Internal (I-I), Internal-External (I-E), and External-External (E-E). ~oth "macro-scale" calibration and "micro-scale" calibration are performed. The comparison of the statewide GM TLF with the 00 TLF for the macro-scale calibration does not provide suitable results because the available 00 survey data do not represent an unbiased sample of statewide truck trips. For the "micro-scale" calibration, "partial" GM trip tables that correspond to the 00 survey trip tables are extracted from the full statewide GM trip table. These "partial" GM trip tables are then merged and a partial GM TLF is created. The GM friction factor curves are adjusted until the partial GM TLF matches the 00 TLF. Three friction factor curves, one for each trip type, resulting from the micro-scale calibration produce a reasonable GM truck trip model. A key methodological issue for GM. calibration involves the use of multiple friction factor curves versus a single friction factor curve for each trip type in order to estimate truck trips with reasonable accuracy. A single friction factor curve for each of the three trip types was found to reproduce the 00 TLFs from the calibration data base. Given the very limited trip generation data available for this research, additional refinement of the gravity model using multiple mction factor curves for each trip type was not warranted. In the traditional urban transportation planning studies, the zonal trip productions and attractions and region-wide OD TLFs are available. However, for this research, the information available for the development .of the GM model is limited to Ground Counts (GC) and a limited set ofOD TLFs. The GM is calibrated using the limited OD data, but the OD data are not adequate to obtain good estimates of truck trip productions and attractions .. Consequently, zonal productions and attractions are estimated using zonal population as a first approximation. Then, Selected Link based (SELINK) analyses are used to adjust the productions and attractions and possibly recalibrate the GM. The SELINK adjustment process involves identifying the origins and destinations of all truck trips that are assigned to a specified "selected link" as the result of a standard traffic assignment. A link adjustment factor is computed as the ratio of the actual volume for the link (ground count) to the total assigned volume. This link adjustment factor is then applied to all of the origin and destination zones of the trips using that "selected link". Selected link based analyses are conducted by using both 16 selected links and 32 selected links. The result of SELINK analysis by u~ing 32 selected links provides the least %RMSE in the screenline volume analysis. In addition, the stability of the GM truck estimating model is preserved by using 32 selected links with three SELINK adjustments, that is, the GM remains calibrated despite substantial changes in the input productions and attractions. The coverage of zones provided by 32 selected links is satisfactory. Increasing the number of repetitions beyond four is not reasonable because the stability of GM model in reproducing the OD TLF reaches its limits. The total volume of truck traffic captured by 32 selected links is 107% of total trip productions. But more importantly, ~ELINK adjustment factors for all of the zones can be computed. Evaluation of the travel demand model resulting from the SELINK adjustments is conducted by using screenline volume analysis, functional class and route specific volume analysis, area specific volume analysis, production and attraction analysis, and Vehicle Miles of Travel (VMT) analysis. Screenline volume analysis by using four screenlines with 28 check points are used for evaluation of the adequacy of the overall model. The total trucks crossing the screenlines are compared to the ground count totals. L V/GC ratios of 0.958 by using 32 selected links and 1.001 by using 16 selected links are obtained. The %RM:SE for the four screenlines is inversely proportional to the average ground count totals by screenline .. The magnitude of %RM:SE for the four screenlines resulting from the fourth and last GM run by using 32 and 16 selected links is 22% and 31 % respectively. These results are similar to the overall %RMSE achieved for the 32 and 16 selected links themselves of 19% and 33% respectively. This implies that the SELINICanalysis results are reasonable for all sections of the state.Functional class and route specific volume analysis is possible by using the available 154 classification count check points. The truck traffic crossing the Interstate highways (ISH) with 37 check points, the US highways (USH) with 50 check points, and the State highways (STH) with 67 check points is compared to the actual ground count totals. The magnitude of the overall link volume to ground count ratio by route does not provide any specific pattern of over or underestimate. However, the %R11SE for the ISH shows the least value while that for the STH shows the largest value. This pattern is consistent with the screenline analysis and the overall relationship between %RMSE and ground count volume groups. Area specific volume analysis provides another broad statewide measure of the performance of the overall model. The truck traffic in the North area with 26 check points, the West area with 36 check points, the East area with 29 check points, and the South area with 64 check points are compared to the actual ground count totals. The four areas show similar results. No specific patterns in the L V/GC ratio by area are found. In addition, the %RMSE is computed for each of the four areas. The %RMSEs for the North, West, East, and South areas are 92%, 49%, 27%, and 35% respectively, whereas, the average ground counts are 481, 1383, 1532, and 3154 respectively. As for the screenline and volume range analyses, the %RMSE is inversely related to average link volume. 'The SELINK adjustments of productions and attractions resulted in a very substantial reduction in the total in-state zonal productions and attractions. The initial in-state zonal trip generation model can now be revised with a new trip production's trip rate (total adjusted productions/total population) and a new trip attraction's trip rate. Revised zonal production and attraction adjustment factors can then be developed that only reflect the impact of the SELINK adjustments that cause mcreases or , decreases from the revised zonal estimate of productions and attractions. Analysis of the revised production adjustment factors is conducted by plotting the factors on the state map. The east area of the state including the counties of Brown, Outagamie, Shawano, Wmnebago, Fond du Lac, Marathon shows comparatively large values of the revised adjustment factors. Overall, both small and large values of the revised adjustment factors are scattered around Wisconsin. This suggests that more independent variables beyond just 226; population are needed for the development of the heavy truck trip generation model. More independent variables including zonal employment data (office employees and manufacturing employees) by industry type, zonal private trucks 226; owned and zonal income data which are not available currently should be considered. A plot of frequency distribution of the in-state zones as a function of the revised production and attraction adjustment factors shows the overall " adjustment resulting from the SELINK analysis process. Overall, the revised SELINK adjustments show that the productions for many zones are reduced by, a factor of 0.5 to 0.8 while the productions for ~ relatively few zones are increased by factors from 1.1 to 4 with most of the factors in the 3.0 range. No obvious explanation for the frequency distribution could be found. The revised SELINK adjustments overall appear to be reasonable. The heavy truck VMT analysis is conducted by comparing the 1990 heavy truck VMT that is forecasted by the GM truck forecasting model, 2.975 billions, with the WisDOT computed data. This gives an estimate that is 18.3% less than the WisDOT computation of 3.642 billions of VMT. The WisDOT estimates are based on the sampling the link volumes for USH, 8TH, and CTH. This implies potential error in sampling the average link volume. The WisDOT estimate of heavy truck VMT cannot be tabulated by the three trip types, I-I, I-E ('||'&'||'pound;-I), and E-E. In contrast, the GM forecasting model shows that the proportion ofE-E VMT out of total VMT is 21.24%. In addition, tabulation of heavy truck VMT by route functional class shows that the proportion of truck traffic traversing the freeways and expressways is 76.5%. Only 14.1% of total freeway truck traffic is I-I trips, while 80% of total collector truck traffic is I-I trips. This implies that freeways are traversed mainly by I-E and E-E truck traffic while collectors are used mainly by I-I truck traffic. Other tabulations such as average heavy truck speed by trip type, average travel distance by trip type and the VMT distribution by trip type, route functional class and travel speed are useful information for highway planners to understand the characteristics of statewide heavy truck trip patternS. Heavy truck volumes for the target year 2010 are forecasted by using the GM truck forecasting model. Four scenarios are used. Fo~ better forecasting, ground count- based segment adjustment factors are developed and applied. ISH 90 '||'&'||' 94 and USH 41 are used as example routes. The forecasting results by using the ground count-based segment adjustment factors are satisfactory for long range planning purposes, but additional ground counts would be useful for USH 41. Sensitivity analysis provides estimates of the impacts of the alternative growth rates including information about changes in the trip types using key routes. The network'||'&'||'not;based GMcan easily model scenarios with different rates of growth in rural versus . . urban areas, small versus large cities, and in-state zones versus external stations. cities, and in-state zones versus external stations.

온.오프라인 채널에서 지각된 품질이 서비스의 개인가치에 미치는 영향에 관한 연구 -인지욕구의 조정효과를 중심으로- (A Study on Perceived Quality affecting the Service Personal Value in the On-off line Channel - Focusing on the moderate effect of the need for cognition -)

  • 본 연구는 서비스 시장에서의 지각된 품질과 개인가치간의 인과적 관계 및 고객의 인지욕구에 따른 온 오프라인상의 조절효과에 대해 실증분석하였으며 이를 통해 개인가치에 대한 서비스 전략과 마케팅 관리의 중요성을 제시하고 있다. 서비스 시장에서 서비스 제공자와 구매자간의 장기적 거래관계의 중요성이 크게 부각됨에 따라 관계구축 및 강화에 매우 중요한 역할을 하는 개인가치에 관한 연구는 학계뿐만 아니라 실무적으로도 고객관계관리의 관점에서 시사하는 바가 크다고 할 수 있다. 실증분석을 위해 대형마트(할인점)와 인터넷 쇼핑몰을 이용하는 고객을 대상으로 설문을 통해 데이터를 수집하였으며 온 오프라인의 비교분석을 통한 차이검증을 위한 인과적 구성모델에 대해 구조방정식 모델분석을 통해 가설검증하였다. 구성모델에 대한 분석결과 물리적 환경, 상호작용 품질, 그리고 결과품질로 구성된 지각된 품질은 안정적 삶, 사회적 인식, 사회적 통합으로 구성된 서비스 개인가치에 통계적으로 매우 유의한 정(+)의 영향을 미치는 것으로 나타났으며 집단간 차이효과분석을 통해서도 온 오프라인에 따른 조정효과는 온라인에서보다는 오프라인에서 더 유의한 것으로 나타났다. 그리고 온라인상에서의 서비스에 대한 인지욕구가 높을 때보다는 오프라인상에서의 서비스에 대한 인지욕구가 높을 때 개인가치에 더 유의한 영향을 미치는 것으로 나타났다. 마지막으로 본 연구의 구성모델에 대한 적합도 역시 수용할만한 수준인 것으로 나타났다.
