Triple Pelvic Osteotomy and Autograft to the Gap of Ischium for the Treatment of Pelvic Canal Narrowing in a Cat

  • Pelvic canal narrowing secondary to pelvic fractures can lead to episodes of recurrent constipation in cats. Triple pelvic osteotomy is considered as a surgical treatment method; however, there is potential for future recurrence of pelvic canal narrowing. This report describes a surgical method using a pelvic symphyseal autograft to keep the distraction of the ischial osteotomy gap to prevent the recurrence of pelvic canal narrowing. A triple pelvic osteotomy was planned to expand the narrow pelvic canal. The cranial ramus of the pubis was cut, and ischiatic and iliac osteotomies were performed. After expanding the ilium, the malunion pelvic symphysis was cut approximately 1 cm and then autografted to the gap of the ischiatic osteotomy line to keep the distraction. The patient showed clinical improvement postoperatively without recurrent pelvic canal narrowing related to triple pelvic osteotomy. However, constipation recurred on post-operative month-5. It was managed conservatively, and subtotal colectomy was performed eventually nine months post-operatively. There were no complications for five months of follow-up.

골반강 협착증으로 인해 발생한 변비를 가진 고양이에서의 골반강 확장술을 이용한 치료 증례 (Pelvic Symphyseal Distraction Osteotomy for Constipation Management Secondary to Pelvic Stenosis)

  • 7살 중성화된 수컷 한국집 고양이가 6개월동안 지속된 변비를 주증으로 본원에 내원하였다. 직장검사에서 골반강의 우측 외측부분의 좁아짐을 보였고, 방사선검사에서는 골반골절의 부정유합으로 인한 골반강의 협착을 확인할 수 있었다. 골반강 직경 비율은 0.68이었다. 직장검사와 방사선검사를 기반으로 골반강 협착으로 인한 변비로 진단하였다. 그래서 골반결합을 절골하여 골반강 확장술을 시행하였고 이를 용이하게 하기 위하여 장골 절골술도 함께 시행하였다. 확장된 골반강을 유지하기 위하여 골반결합 사이에 폴리메칠메타크릴레이트를 성형하여 구조물로 삽입하였다. 절골 한 장골은 뼈판과 나사를 이용하여 다시 고정시켜주었다. 방사선 검사에서 술 후 골반강 직경 비율은 0.91로 증가했음을 알 수 있었다. 술 후 처치로는 항생제, 진통소염제, 락툴로오스경구제 투여와 함께 수액요법을 시행하였다. 환자는 골반강 확장술 후 일주일부터 정상적인 배변을 보였고, 그 이후 6개월동안 재발은 없었다. 이 증례를 바탕으로 골반강 확장술은 골반골절의 부정유합으로 인한 골반강 협착에 따라 발생한 변비에서 유용한 치료법으로 사료된다.

Minimally Invasive Treatment for Sacroiliac Dislocation in Dogs

  • Sacroiliac dislocation is the separation of the iliac wing from the pelvic bone and needs to be repaired by surgery. Corrective surgical methods include open reduction and minimally invasive techniques. In the present study, we used a minimally invasive surgical technique in seven dogs with sacroiliac dislocation. Five cases had unilateral sacroiliac joint luxation and two cases had bilateral sacroiliac joint luxation; all were referred to hospital after being hit by an automobile. All cases were treated with a fluoroscope-assisted, minimally invasive technique. Patients were evaluated by measuring surgery time, postoperative ambulatory time, and calculating pelvic canal diameter ratios. Surgery time was measured from initial incision to completion of skin closure. Mean surgery time was 30.6 minutes in unilateral sacroiliac joint luxation and 68 minutes in bilateral sacroiliac joint luxation. Mean preoperative pelvic canal diameter ratio was 1.22 (${\pm}0.27$), immediate postoperative pelvic canal diameter ratio was 1.26 (${\pm}0.10$), and at 2 weeks after surgery, the pelvic canal diameter ratio was 1.37 (${\pm}0.22$). All cases were ambulatory within 1 week and mean postoperative ambulatory time was 5 days. Based on the results, the use of a minimally invasive technique for correction of sacroiliac dislocation can decrease surgical time, lessen operative and postoperative burdens on patients, and provide owners with a good prognosis.

심한 변비를 보이는 고양이에서 반골반절제술을 이용한 외과적 치료 증례 (Hemipelvectomy in a Cat with Obstipation)

  • 암컷, 9개월, 2.2 kg의 단모종 고양이가 2주간의 심한 변비 증상을 주증으로 내원하였다. 보호자 보고에서 증상 발생 4개월 전 골반 골절이 있었음을 확인 하였다. 신체 검사에서 이급후중 및 오른쪽 고관절의 운동 제한을 확인 하였고, 직장 검사에서 양측 관골구의 내측 돌출과 치골의 배측 변위를 동반한 골반 강 협착을 확인할 수 있었다. 방사선 검사에서 확장 된 대장과 골반 강의 비정상적 구조를 동반한 골반 강 협착을 확인 하였다. 배변 유연제와 관장을 이용한 보존적 치료를 실시하였으나 심한 변비 증상이 개선 되지 않았다. 장골, 좌골, 치골, 관골구의 부분 절골술을 실시 하였다. 수술 후 방사선 검사와 직장검사에서 확장 된 골반 강을 확인 할 수 있었다. 수술 후 배변 유연제(5 ml, PO, q12h; Duphalac$^{(R)}$)를 2주간 투약 하였고 그 후 2주간은 2.5 ml 용량으로 투여 하였다. 수술 후 따뜻한 물을 이용한 관장을 2회 실시 하였다. 수술 후 14일 째 검사에서 배변과 보행에 문제가 없음을 확인 하였고 5개월 째 검사에서 심한 배변과 오른쪽 뒷다리 파행 증상이 관찰 되지 않았다.

Evaluation of minimally invasive surgical reduction of sacroiliac luxation in toy breed dogs: a cadaver study

  • Background: Minimally invasive surgery (MIS) technique of sacroiliac luxation in toy breed dogs using cannulated screws has not been described. Objectives: The purpose of this study was to evaluate the effectiveness of pelvic canal recovery, the reproducibility of successful surgery outcomes, and the acceptable difficulty of the procedure in MIS of sacroiliac luxation in toy breed dogs. Methods: MIS using 2.3-mm cannulated screws was demonstrated in 12 toy breed dog cadavers with sacroiliac luxation artificially induced. Pre and postoperative radiographs were used to evaluate the pelvic canal diameter ratio (PCDR), hemipelvic canal width ratio (HCWR), and reduction rate. Dorsoventral angle (DVA) and craniocaudal angle (CCA) of the inserted screw were obtained postoperative computed tomographic scan. Results: The statistically significant difference between the mean pre and postoperative PCDR was found (1.10 ± 0.12 and 1.26 ± 0.11, respectively; p = 0.002), and the mean HCWR close to 1.0 meaning symmetric pelvis also was obtained (0.97 ± 0.07). The mean DVA and CCA were 2.26° ± 1.33° and 2.60° ± 1.86°, respectively. Conclusions: MIS of sacroiliac luxation using 2.3-mm cannulated screws is applicable to toy breed dogs with acceptable difficulty.

First report of Ernogrammus zhirmunskii Markevich & Kharin, 2011 (Perciformes: Stichaeidae) in Dokdo, Korea

  • The first specimen (54.8 mm standard length) of Ernogrammus zhirmunskii Markevich & Kharin, 2011, belonging to the family Stichaeidae, was recorded in Dokdo, East Sea, Korea on July 26, 2021. This species was characterized by a single ventral lateral-line canal from the posterior margin of the pelvic-fin base extending to the anus and one or two rigid spines on the posterior part of the anal fin. This species is similar to Ernogrammus hexagrammus and Ernogrammus walkeri but differs in the number of ventral lateral-line canal present, with E. zhirmunskii consisting of one (unpaired) ventral lateral-line canal compared to other two Ernogrammus species, which have a pair of parallel ventral lateral-line canal. For further analysis of species identification, a partial gene sequence from the mitochondrial DNA cytochrome oxidase subunit I (554 bp) of E. zhirmunskii was obtained for the first time. This study documents the first record of E. zhirmunskii in Korean waters and proposes the new Korean name of 'Il-gob-julbe-do-la-chi' for the species.

Sacral Insufficiency Fractures : How to Classify?

  • Objective : The diagnosis of insufficiency fractures of the sacrum in an elder population increases annually. Fractures show very different morphology. We aimed to classify sacral insufficiency fractures according to the position of cortical break and possible need for intervention. Methods : Between January 1, 2008 and December 31, 2014, all patients with a proven fracture of the sacrum following a low-energy or an even unnoticed trauma were prospectively registered : 117 females and 13 males. All patients had a computer tomography of the pelvic ring, two patients had a magnetic resonance imaging additionally : localization and involvement of the fracture lines into the sacroiliac joint, neural foramina or the spinal canal were identified. Results : Patients were aged between 46 and 98 years (mean, 79.8 years). Seventy-seven patients had an unilateral fracture of the sacral ala, 41 bilateral ala fractures and 12 patients showed a fracture of the sacral corpus : a total of 171 fractures were analyzed. The first group A included fractures of the sacral ala which were assessed to have no or less mechanical importance (n=53) : fractures with no cortical disruption ("bone bruise") (A1; n=2), cortical deformation of the anterior cortical bone (A2; n=4), and fracture of the anterolateral rim of ala (A3; n=47). Complete fractures of the sacral ala (B; n=106) : parallel to the sacroiliac joint (B1; n=63), into the sacroiliac joint (B2; n=19), and involvement of the sacral foramina respectively the spinal canal (B3; n=24). Central fractures involving the sacral corpus (C; n=12) : fracture limited to the corpus or finishing into one ala (C1; n=3), unidirectional including the neural foramina or the spinal canal or both (C2; n=2), and horizontal fractures of the corpus with bilateral sagittal completion (C3; n=8). Sixty-eight fractures proceeded into the sacroiliac joint, 34 fractures showed an injury of foramina or canal. Conclusion : The new classification allowes the differentiation of fractures of less mechanical importance and a risk assessment for possible polymethyl methacrylate leaks during sacroplasty in the direction of the neurological structures. In addition, identification of instable fractures in need for laminectomy and surgical stabilization is possible.

Radiographic Features of Traumatically Caused Retroflexion of Urinary Bladder in Two Dogs

  • The urinary bladder retroflexion, flipped over backwards into the pelvic canal, by trauma has been rarely reported in dogs. This paper describes clinical and radiological features of urinary bladder retroflexion in two dogs with a history of traffic accident. The main clinical signs were hind limb lameness, severe pain and dysuria. Radiography and ultrasonography were performed to evaluate the patient's damages. No remarkable findings were observed except pelvic fracture on survey radiograph. On cystourethrography, caudal displacement and retroflexion of the bladders were identified. Cystopexy was performed and the bladders were repositioned. After surgery one dog with severe azotemia and bilateral hydroureteronephrosis was deteriorated noticeably and euthanized by request of the owner, while the other dog was recovered no recurrence of the problem by 12 months post-surgery. It should be considered that contrast study and serial assessment in urinary tract be important to demonstrate the evidence of bladder retroflexion and evaluate the prognosis in dogs with severe trauma.
