소아 빈혈의 감별진단 및 치료 (Differential Diagnosis and Treatment of Anemia in Children)

  • Anemia can be defined as a reduction in blood hemoglobin concentration or red cell mass relative to age matched normal values. Clinical presentation may range from obviously pale and lethargy to an incidental finding during screening of an otherwise well appearing child. The differential diagnosis of anemia in each instance is broad with numerous possible etiologies. A careful history and physical examination as well as complete blood count, peripheral blood smear and additional laboratory tests are necessary in defining underlying cause of the anemia and guide in further treatment plans. In addition, Iron deficiency anemia and anemia of inflammation are common causes of mild to moderate anemia in children, but most pediatricians have some confusions to differentiate these two entities.

  • Recently, accumulated clinical experience and advanced neuroradiological techniques have led to a better understanding of pediatric cerebrovascular disease (CVD), which was once considered rare. Approximately 10% of pediatric neurosurgical patients have CVD; therefore, it is no longer uncommon to pediatricians and pediatric neurosurgeons. Furthermore, children with CVD tend to recover better than adults after stroke because the immature brain is more plastic than the mature one. Therefore, active treatment often proves more rewarding in pediatric medicine than in adult medicine.

소아 전문 한의사를 대상으로 실시한 하태독법의 인식 및 사용실태 조사 (A Study Regarding Current Usage of Ha-Taedok Method in Korean Pediatrician)

  • Objectives This study is to know the current usage of Ha-Taedok Method in Korean pediatrician. Methods 69 questionnaires were statistically analyzed by using PASW Statistics 18. Results 55 Korean Medicine doctors knew Ha-Taedok Method, but only 2 doctors were using Ha-Taedok Method in their practice. The doctors tend not to use Ha-Taedok Method because it was hard to get permission from the patient's caregiver. 11 Korean Medicine doctors used Ha-Taedok Method to their own children though. The pediatricians felt satisfied from the effect of Ha-Taedok Method especially because it can prevent from disease. They reported no side effect and adverse effect. Coptidis Rhizoma is a preferred herb to use along with Ha-Taedok Method. For a better result from Ha-Taedok Method, it is recommended to start within 3 days old of patient. Conclusions Ha-Taedok Method may be effective as preventive method.

소아기 수면 장애 (Sleep disorders in childhood)

  • Sleep disorders are very common among pediatric patients. Its prevalence is between 10% and 45% in preschool- and school-aged children. However parents commonly do not concern about their children's sleeping habits and for many pediatricians, there is not part of the routine office visit about a childs sleep. Sleep disorders were classified by International Classification of Sleep Disorder (ICSD) as dyssomnias, parasomnias, sleep disorders associated with mental, neurologic, or other medical disorders, and proposed sleep disorders. There are lots of differences in the causes, manifestations, and managements of sleep disorders between children and adult. The sleep disorders in childhood may manifest themselves as bedtime resistance, refusal to go to bed at a parentally described time, sleep-onset delay, inability to fall asleep within a reasonable time, prolonged nighttime awakening, and inability to return to sleep without assistance after waking during the night, and so have wide-ranging influences on children's behavior, mood, school performance, and family life. It's very important for pediatrician to concern about the sleep disturbances in childhood and so the problems of sleep in children should be early detected and managed.

Steroid-induced Psychosis in Adolescent Patient with Crohn's Disease

  • Crohn's disease is a chronic inflammatory disease that involves the upper and lower gastrointestinal tracts, including the oral cavity. For the conventional treatment of inflammatory bowel diseases, steroids are usually used as induction therapy, and azathioprine is used as maintenance therapy. Steroids are associated with various side effects. Under rare circumstances, they can even cause psychotic symptoms, and reports on steroid-induced psychosis in the pediatric population are few. Here, we report the first case of steroid-induced psychosis in a 16-year-old female with Crohn's disease administered with steroids as induction therapy in South Korea. She showed psychotic symptoms, including auditory hallucination, loosening of association, and idea of reference, after 2 weeks of administering prednisolone 60 mg/day. One week after steroid discontinuation, the patient gradually recovered and was treated with quetiapine. Therefore, pediatricians should be aware of serious psychiatric adverse effects when administering corticosteroids for various indications.

  • Specific Language Impairment is one of the serious disorders that interferes with spontaneous communication skills in children. Children suffering from this disorder may have reading, speaking, or listening impairments, and such type of disorders are also termed Autism Speech Disorder (ASD) in medical terminology. The aim of the article is to define specific language impairment in children and the problems it can cause. The different methods adopted by speech pathologists to diagnose language impairment. Finally implementing machine learning models to automate the process and help speech pathologists and pediatricians/ in diagnosing the specific language impairment.

  • Pediatric chronic kidney disease (CKD) is a chronic illness that affects the overall quality of life of patients during childhood. This article highlights the psychological and social burden of CKD in patients and their families. Patients with CKD and their families require comprehensive treatment for psychosocial problems. Therefore, it is crucial for pediatricians to screen for these issues and refer patients and their families for therapy. Tools that are short, easy to administer, and easy to score, such as the Pediatric Quality of Life Inventory or the Childhood Depression Inventory, can be utilized during routine clinical appointments. Reducing the negative impact of CKD on the family will improve the well-being and coping skills of patients and their families.

예방접종에 대한 부모의 인식에 관한 조사 (A survey of parental knowledge of vaccination)

  • 목 적 : 예방접종은 감염성 질환 예방에 매우 중요한 수단이나 지금까지 예방접종에 대한 부모들의 의식에 대한 연구가 거의 없었다. 본 연구의 목적은 예방접종 실태 및 부모들의 예방주사에 관한 인식에 대하여 설문 조사를 통하여 알아보고 잘못된 인식의 개선과 올바른 정보를 제공하고자 하며, 예방접종 시 소아과 전문의의 역할에 대하여 의견을 제시하고자 하였다. 방 법 : 대상군으로는 2004년 3월부터 6월까지 순천향대학교 3개 병원과 가톨릭대학교 2개 병원을 내원한 환자 보호자 총 466명을 대상으로 29문항의 설문조사를 실시하여 분석하였다. SPSS 프로그램 version 11.0(SPSS Inc., Chicago, IL, USA)을 이용하였으며, 통계처리는 Chi-square법을 시행하여 P값이 0.05 미만일 때 통계적으로 유의한 것으로 판정하였다. 결 과 : 예방접종 장소는 소아과 개인의원 49.4%, 보건소 27.7%, 종합병원/대학병원 소아과 15.0%, 타과 개인의원 4.9%였으며, 부모의 수입이나 학력에 따른 차이는 없었다. 예방접종날짜를 잘 기록해 주는 경우가 80.0%이었다. 예방접종을 소아과 전문의에게 시행받는 이유는 타과와 차이가 없지만 믿음이 가기 때문이다고 한 경우가 38.8%였으며, 소아과 전문의에게 시행 받는 것과 타과에서 시행받는 것이 차이가 없다고 생각하는 경우가 15.0%였고, 앞으로 예방접종을 시행할 장소로 소아과 개인의원이 53.0%였다. 예방접종 예진표를 쓰기를 원하는 경우는 62.4%이었다. 예방접종에 관한 정보는 예방접종 수첩을 통해서 얻는다고 응답한 경우가 57.9%로 가장 많았고, 수막염 예방주사를 맞으면 모든 수막염이 예방되는 것으로 안 경우가 52.6%이었다. 예방접종을 제 때에 하지 못한 이유로 접종 날짜를 잘 몰라서인 경우가 9.9%였다. 동시접종할 수 있는 백신을 의사들의 개인적인 판단에 따라 서로 다른 접종 간격을 두고 접종하였다(77.3%). B형 간염 백신은 접종 후 41.6%에서 항체 검사를 시행하였다. 같은 예방주사인 경우가 접종 비용이 비싼 예방주사가 있을 경우에는 의사와 상의하여 결정하겠다가 73.8%였고, 새로운 예방주사에 대한 임상 연구에 대해서는 51.7%가 무조건 참여 의사가 없다고 응답하였으며, 부모의 수입이나 학력에 따른 차이는 없었다. 결 론 : 본 연구 결과 예방접종에 대하여 잘못 인식하고 있는 부모들이 많을 것이란 예측이 가능하였다. 이에 따라 소아과 전문의는 예방접종에 대한 올바른 정보를 제공하기 위한 적극적인 노력이 필요할 것이다. 그리고 예방접종과 관련된 진료에 있어 예방접종 예진표의 적극적인 사용, 예방접종시 성장과 발달 및 이유식에 대한 상담 등 타과와의 차별성이 아직 미흡하여 이에 대한 개선책이 고려되어야 할 것이다.

의원의 특성에 따른 상병진단군의 분포에 대한 연구 (Chracteristics of Primary Health Practice and Diagnosis-Cluster Pattern in Health Insurance)

  • This study is designed to find out some intra-clinic factors affecting the content of practice provided by primary care physicians in Korea, and proposed factors in this study are characteristcs of each private clinc --- physician-related variables(age, sex, specialty), bfed-related variables for inpatient care, laboratory-related variables for precise diagnosis. We have tried to estimate the difference of disease entities cared by each primary care physician according to above factors by analyzin gdisease data claimed during one month(April, 1992) to National Federation of Medical Insurance. The diagnosis codes by ICD-9 in the research disease data were reclassified to 'diagnosis clusters' by virtue of clinical similarities for effective analyses. We have converted frequent-tsing ICD-9 codes to 86 diagnosis clusters, which incorporated 97.4 percents of all ambulatory visits to private clinics. This result means proposed diagnosis-cluster method is effective tool for analysis of the content of ambulatory medical care carried out by primary care physicians. Comparisons and analyses of multiple diagnosis-clusters made on the basis of presented factors were done and the results were as follows; - Major factors affecting the difference between diagnosis-cluster pattern by each variables were phyusician's age, sex, specialty and bed counts of each private clinic for inpatient care and the size of laboratories of each clinic. - Middle aged(30th to 40th) group physicians are providing more comprehensive care than 20th or above 50th aged groups. Male physicians are more adequate for comprehensive care than female physicians, because woman-doctors are providing narrow-spectrum care. The content of practice of obstetricians and gynecologists shows much difference from primary medical practice, and they cannot be included in primary care physician, this study suggested. Pediatricians are also providing short-spectum acre, and nearly all visits to pediatricians were incorporated only 2-3 diagnosis-clusters. General surgeons' practices are very similar to general practioners' or family physicians' practices, the means they are providing primary care rather than special surgical care. And small number of beds(under 5 beds) and only basic(2-3 sorts of)diagnostic apparatuses are sufficient for primary physicians' clinic to carry out primary care. In conclusion, to reinforce primary care department in Korea, there must be support with health policy to expand office-based primary care practice-- with small number of beds for inpatient care and only basic laboratories-- provided by general practitioner of family physician.

Awareness and Attitude Relating to the Human Papilloma Virus and its Vaccines Among Pediatrics, Obstetrics and Gynecology Specialists in Turkey

  • Background: To determine the level of knowledge on human papillomavirus (HPV) infection and vaccination, and the attitude towards HPV vaccination in pediatricians, obstetricians and gynecologists (OBG). Materials and Methods: Participants were administered a 40-question survey, investigating the demographic properties, the knowledge on the HPV infection-vaccination and attitudes towards vaccination. Results: The study enrolled a total of 228 participants (131 pediatricians and 97 OBGs). At a rate of 99.6%, the participants agreed with the fact that the HPV infection was the most common sexually transmitted disease and 33.8% of the participants had the opinion that the HPV vaccination should be administered only in women. The lowest level of HPV vaccine recommendation was among the pediatrics specialists (59.4%, p=0.012). When asked whether they would have their daughters receive HPV vaccination, 79.5% of the participants answered favorably; this rate was 36.7% for the sons. At a rate of 59.5% of the participants thought that the HPV vaccine needed to be included in the national vaccine schedule. Most of the participants (91.6%) had the idea that reduction of the vaccine costs would increase the vaccination frequency. Conclusions: We observed that the consideration of the costs and the prejudices relating to the inefficacy of vaccination as well as the inadequate level of knowledge were involved in the physicians' resistance to HPV vaccination. We believe that the healthcare professionals should be informed adequately to overcome false beliefs, thereby ensuring success of the HPV vaccine upon inclusion in the national vaccine schedule in the future.
