Large Variation in Clinical Practice amongst Pediatricians in Treating Children with Recurrent Abdominal Pain

  • Purpose: To evaluate intra- and inter-observer variability and guideline adherence amongst pediatricians in treating children aged between 4 and 18 years referred with recurrent abdominal pain (RAP) without red flags. Methods: The first part of the study is a retrospective single-center cohort study. The diagnostic work-ups of eight pediatricians were compared to the national guidelines. Intra- and inter-observer variability were examined by Cramer's V test. Intra-observer variability was defined as the amount of variation within a pediatrician and inter-observer variability as the amount of variation between pediatricians in the application of diagnostic work-up in children with RAP. Prospectively, the same pediatricians were requested to provide a report on their management strategy with a fictitious case to prove similarities in retrospective diagnostic work-up. Results: A total of 10 patients per pediatrician were analyzed. Retrospectively, a (very) weak association between pediatricians' diagnostic work-ups was found (0.22), which implies high inter-observer variability. The association between intra-observer diagnostic was moderate (range, 0.35-0.46). The Cramer's V of 0.60 in diagnostic work-up between pediatricians in the fictitious case implied the presence of a moderately strong association and lower inter-observer variability than in the retrospective study. Adherence to the guideline was 66.8%. Conclusion: We found a high intra- and inter-observer variability and moderate guideline adherence in daily clinical practice amongst pediatricians in treating children with RAP in a teaching hospital.

울산 지역 소아청소년과 및 이비인후과에서의 항염증제 처방 형태 분석 (Outpatient Prescription Pattern of Anti-inflammatory Drugs by Pediatricians and ENT Physicians in Ulsan City)

  • The prescription sheets for outpatients from July 2008 to June 2009 from 7 community pharmacies in Ulsan City were surveyed for the anti-inflammatory drug (AID) prescription pattern. The AID prescription rate of pediatricians and ENT physicians were 30.0% and 34.8%, respectively. The oral steroidal anti-inflammatory drugs (SAIDs) were prescribed as much as 3.9% by pediatricians and 10.3% by ENT physicians. The chiefly prescribed oral SAID was prednisolone in pediatric clinics and methylprednisolone in ENT clinics. Meanwhile the prescription rate of oral non-steroidal anti-inflammatory drug (NSAID) was 22.5% by pediatricians and 21.4% in ENT physicians. The most favorable NSAIDs were propionate derivatives in both clinics. In case of externally-applied SAIDs, the prescription rate of pediatricians was 3.6% and that of ENT physicians was 2.8%. Among them, nasal spray, inhalant and gargle formulations for upper respiratory infection (URI) treatment occupied 35.8% of externally-applied SAIDs in pediatric clinics and 59.7% in ENT clinics. Further, it was observed that ENT physicians favored much stronger SAIDs in Group III of ATC classification (75.4% of externally-applied SAIDs) than pediatricians (49.2%). In the survey of AID combination rate, pediatric clinics showed much lower rate (1.4% of total AID prescriptions) than ENT clinics (7.5%). Among them, the combination rate of oral SAID and oral NSAID by ENT physicians (52.2% of total AID combinations) was much higher than pediatricians (36.6%), which might be over-prescription of AID agents. In conclusion, the AID prescription rate as well as AID combination rate, especially in SAID prescriptions, was much higher in ENT than pediatric clinics, which implies the higher confidency on AID drugs of ENT physicians even though the severity of patient's symptom could be considered.

Management of Acute Gastroenteritis in Children: A Survey among Members of the Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition

  • Purpose: No national survey has yet described the guidelines followed by Korean pediatricians to treat acute gastroenteritis (AGE). An online survey was performed to investigate the management of AGE followed by members of The Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition, and the results were compared between pediatric gastroenterologists (PG) and general pediatricians (GP). Methods: Questionnaires were sent to pediatricians between June 2 and 4, 2018 regarding the type of hospital, indications for admission, antiemetic and antidiarrheal drugs and antibiotics prescribed, and dietary changes advised. Results: Among the 400 pediatricians approached, 141 pediatricians (35.3%) responded to the survey. PG comprised 39% of the respondents and 72.7% worked at a tertiary hospital. Both PG and GP considered diarrhea or vomiting to be the primary symptom. The most common indication for hospitalization was severe dehydration (98.8%). Most pediatricians managed dehydration with intravenous fluid infusions (PG 98.2%, GP 92.9%). Antiemetics were prescribed by 87.3% of PG and 96.6% of GP. Probiotics to manage diarrhea were prescribed by 89.1% of PG and 100.0% of GP. Antibiotics were used in children with blood in diarrheal stool or high fever. Dietary changes were more commonly recommended by GP (59.3%) than by PG (27.3%) (p<0.05). Tests to identify etiological agents were performed primarily in hospitalized children. Conclusion: This survey assessing the management of pediatric AGE showed that the indications for admission and rehydration were similar between GP and PG. Drug prescriptions for diarrhea and dietary changes were slightly commonly recommended by GP than by PG.

Pediatricians' perception of factors concerning the clinical application of blockchain technology to pediatric health care: a questionnaire survey

  • Background: Interest in digital medical information has increased because it allows doctors to easily access a patient's medical records and provide appropriate medical care. Blockchain technology ensures data safety, reliability, integrity, and transparency by distributing medical data to all users over a peer-to-peer network. This study attempted to assess pediatricians' thoughts and attitudes toward introducing blockchain technology into the medical field. Methods: This study used a questionnaire survey to examine the thoughts and attitudes of 30- to 60-year-old pediatricians regarding the introduction of blockchain technology into the medical field. Responses to each item were recorded on a scale ranging from 1 (never agree) to 7 (completely agree). Results: The scores for the intentions and expectations of using blockchain technology were 4.0 to 4.6. Pediatricians from tertiary hospitals responded more positively (4.5-4.9) to the idea of using blockchain technology for hospital work relative to the general population (4.3-4.7). However, pediatricians working in primary and secondary hospitals had a slightly negative view of the application of blockchain technology to hospital work (p=0.018). Conclusion: When introducing the medical records of related pediatric and adolescent patients using blockchain technology in the future, it would be better to conduct a pilot project that prioritizes pediatricians in tertiary hospitals. The cost, policy, and market participants' perceptions are essential factors to consider when introducing technology in the medical field.

대구지역 소아청소년과 의사들의 요로감염 관리 실태분석 (Management of Urinary Tract Infections in Children: A Survey of Pediatricians in Daegu City)

  • 목적: 소아의 요로감염은 적절히 관리 되지 않는 경우에 여러 문제를 일으킬 수 있어 다양한 관리 지침이 계속 개발되고 있고, 근래에는 근거 중심의 지침들이 만들어져서 진료에 적용되고 있다. 하지만 소아과 의사들의 요로감염 관리 실태에 대한 조사는 매우 드문 실정이다. 이 연구에서는 소아과 의사들의 요로감염에 대한 인식을 알아보고, 적절하게 관리가 이루어지는지를 평가하고 개선점을 찾고자 하였다. 방법: 대구시 지역의 78명의 소아청소년과 전문의들을 대상으로 요로감염의 진단, 영상검사 및 치료와 예방을 위한 환자교육을 어떻게 하는지를 설문조사로 알아보았다. 결과: 대부분의 응답자들은 발열의 원인을 찾을 수 없는 경우에 요로감염을 감별하기 위하여 소변검사를 하였다. 환자가 소변을 가릴 수 없는 경우 상당수의 응답자가 무균채뇨백을 사용하여 채뇨를 하였으며, 소변배양 검사를 하지 않는 경우도 많았다. 가장 많이 시행하는 영상검사는 초음파였다. 항생제는 입원한 경우에만 주사로 투여하고 외래 통원 시는 경구투여를 하였다. 요로감염의 중요성, 재발과 예방 등에 대한 환자 교육을 선택적으로 하고 있었다. 방광 요관역류의 관리 지침을 가지고 있는 경우는 28.6%로서 많지 않았다. 결론: 요로감염을 진단하기 위한 시도는 적절하게 이루어지고 있었으나, 무균채뇨백을 사용하여 소변배양을 하는 경우가 많아 위양성진단율이 높아져서 과잉치료가 우려되었다. 또한 영상검사를 하지 않는 경우가 많아 동반된 요로 기형의 진단이 조기에 되지 않을 수 있다는 것도 우려되었다. 근거-중심의 요로감염 관리 지침에 따른 보다 적절한 관리를 하기 위한 노력이 필요하다.

Knowledge and Attitudes about Human Papillomaviruses and Immunization among Turkish Pediatricians

  • Background: Human papillomavirus (HPV) is one of the most common sexually transmitted infectious agents, and the effectiveness of vaccine delivery programs will depend largely upon whether providers recommend vaccines. The objectives of this study were to examine pediatrician characteristics, knowledge, and attitudes associated with HPV and HPV immunization. Materials and Methods: Attendees of the national pediatric meeting in 2011, were asked to complete a questionnaire that, aside from demographic information, elicited level of agreement with statements regarding HPV, its related diseases, and HPV vaccination. It also documented attitudes and beliefs about HPV vaccination. Results: Of the 480 attendees, 226 (47%) filled in the questionnaire. The level of pediatrician HPV-related knowledge varied. The majority (78%) were aware that HPV infection is the most common sexually transmitted infection, while 51% were unaware that a condom is ineffective protection against HPV infection. Between 60-80% of respondents were aware of the effectiveness of HPV vaccination for women. On the other hand, only 10% were aware of reasons why men should be vaccinated against HPV. The majority (75%) of Turkish pediatricians were likely to recommend HPV vaccination to their daughter, if they had one. Seventy percent of pediatricians agreed that the HPV vaccination should be added to the National Immunization Program (NIP) in Turkey. However, the respondents documented concerns about the cost of the vaccination. Conclusions: Increasing pediatricians' knowledge and awareness of HPV and HPV vaccination may assist with the implementation of an effective NIP.

  • The ultimate goal of the term alteration to pediatrics is that not only infants and children but also adlescents are subjet to pediatrics. At 27, June, 2007 the term pediatrics was altered to pediatrics (children and aoldescent medicine). From now on the roles of pediatricians for adolescents are needed and adolescent medicine should be reorganized in Korea. Although the numbers of children are decreased in the times of low birth rate, the efforts of pediatricians try to enlarge the range of treatment for adolescents are need.

Prevalence, Risk Factors, and Pediatrician Awareness of Infant Dyschezia in Indonesia

  • Purpose: This study aimed to evaluate the prevalence and risk factors of infant dyschezia as well as pediatrician awareness regarding this disease in Indonesia. Methods: This is a two-part cross-sectional study, which was divided into study A and B. Study A: Parents whose infants were under 9 months old and attended well-baby clinics were recruited at two randomly selected primary health centers. Parents also provided information on the infant's previous medical history, and socio-demographic and family details. The Rome IV criteria was translated and validated to be used for diagnosis of infant dyschezia. Study B: Randomly selected pediatricians were surveyed by using a questionnaire to evaluate their knowledge regarding infant dyschezia. Results: The prevalence of infant dyschezia based on the result of this study was 11.8%. Three risk factors had a significant relationship with infant dyschezia i.e., the number of children in the family (odds ratio [OR], 5.619; 95% confidence interval [CI], 2.194-14.390; p<0.001), complementary food diet (OR, 4.238; 95% CI, 1.902-9.443; p<0.001), and social-emotional disturbance (OR, 5.670; 95% CI, 2.550-12.609; p<0.001). The percentage of pediatricians correctly diagnosed infant dyschezia was 71.5%. Most pediatricians agreed that they did not perform any diagnostic testing (79.7%) and only provided education in cases of infant dyschezia (58.5%). Conclusion: The prevalence of infant dyschezia identified in our study was higher than that in other neighboring Asian countries, with the highest prevalence observed in infants 7-9 months old. Being an only child, receiving complementary food diet, and sociao-emotional disturbances were significant risk factors of infant dyschezia.

Rome IV Clinical Criteria and Management of Functional Constipation: Indonesian Health Care Professionals' Perspective

  • Purpose: The updated ROME IV criteria for functional constipation (FC) in children were published in 2016. However, information on the use of these criteria is scarce. This study aimed to report the frequency of the use of the ROME IV criteria by Indonesian pediatricians and general practitioners (GPs) in FC management in infants and toddlers. Methods: An anonymous cross-sectional online survey was conducted between November 2021 and March 2022. Results: A total of 248 respondents (183 pediatricians and 65 GPs) from 24 Indonesian provinces completed the survey. Most respondents reported an estimated prevalence of FC to be less than 5% both in infants and toddlers. On average, only 64.6% of respondents frequently used the ROME IV criteria. Pediatricians used the ROME IV criteria more often than GPs did (p<0.001). The most frequently used criteria were painful or hard bowel movements (75.0%) and ≤2 defecations/week (71.4%). Lactulose as a laxative was the preferred treatment choice, followed by changing the standard formula to a specific nutritional formula. Most of the respondents carried out parenteral reassurance and education. Normal growth, as a marker of good digestion and absorption function, and normal stool consistency and frequency were the most reported indicators of gut health. Conclusion: The ROME IV criteria for functional constipation are not extensively used by pediatricians and GPs in Indonesia. Laxatives and specific nutritional formulas were the most used management approaches in infants and toddlers. Medical education, especially for general practitioners, should be updated.

전라북도 소아과 개원의의 급성 위장염 환자에 대한 인식 조사 (Pediatrician Perspectives on the Evaluation and Treatment of Acute Gastrointestinal Infections, Jeonbuk, South Korea, 2002)

  • 목 적 : 전라북도 소아과 개원의의 급성 위장염에 대한 이해와 환자의 진단과 치료에 대한 경험과 인식을 알아보고 최근 개발된 로타 바이러스 백신에 대한 인식을 알아보고자 하였다. 방 법 : 2002년 4월부터 6월까지 전라북도에서 소아과 의원을 개원하고 있는 소아과 전문의를 대상으로 미리 작성된 설문지를 이용하여 우편배달을 통해 조사하였다. 배포된 설문지 수는 총 82매였고 이 중 63명의 소아과 전문의가 응답하여 회수율은 76.8%였다. 급성 위장염 환자의 비율, 선호하는 치료제 및 방법, 항생제 사용빈도 및 종류, 항생제 사용을 고려하는 임상소견, 급성 위장염 환자에서 시행해 보고 싶은 검사, 로타 바이러스 백신에 대한 관심도와 사용 계획에 대해 조사하였다. 결 과 : 급성 위장염 환자는 연중 발생하였으며 겨울과 봄에 약간 많았다. 주로 사용하는 치료제로는 유산균 제제(84.1%), 경구용 수액제(79.4%), 효소제(58.8%)를 선호하였다. 대부분 의사들이 전체 위장염 환자의 20% 이하에서 항생제를 사용하였고, 주로 사용하는 항생제는 amoxicillin(61.3%)과 trimethoprim/sulfamethoxazole(39.7%)이였으며, 항생제 사용을 고려하는 임상증상은 혈변이나 점액변, 고열 등의 증상이었다. 급성위장염 환아에서 중요하다고 생각하거나 가능하면 해보고 싶은 검사는 대변 검사(로타 바이러스 항원, 적혈구, 백혈구), 복부 방사선 검사, 혈청 전해질 검사 등이었다. 대부분의 의사들이 로타 바이러스 백신에 대해 긍정적인 관심을 가지고 백신이 도입될 경우 사용하겠다고 하였으며, 백신사용을 결정할 때 중요하게 생각하는 점으로는 예방효과와 부작용이었다. 결 론 : 전라북도 소아과 개원의들의 급성 위장염 환자에 대한 인식은 대체로 긍정적인 것으로 판단되었으며, 앞으로 보다 적극적인 검사방법의 개발과 이용이 중요할 것으로 생각된다.
