본 연구의 목적은 치과적 외상이 있는 소아청소년 입원환자의 치과 협진 의뢰에 대한 본원의 현황을 분석하고 발전 방향을 모색하는 것이다. 이에 2011년 3월부터 2017년 2월까지 6년간 원주세브란스기독병원 의과에 입원한 소아청소년 환자의 치과 협진 의뢰에 대한 후향적 분석을 하였다. 조사 항목으로 협진 의뢰 주소와 치과적 진단명, 협진 의뢰과, 치과적 외상 후 협진 의뢰 시기, 입원 기간과 치과적 외상 후 협진 의뢰 시기의 관계, 외상으로 의과에 입원하는 것과 치과적 외상 발생의 관계, 치과적 외상의 원인과 발생 나이, 치과적 외상에 대한 치료, 치과적 외상 환자의 퇴원 후 치과 외래 재내원 등이 있었다. 협진 의뢰 주소와 치과적 진단명에서 치과적 외상이 차지하는 비율이 가장 높았고, 협진 의뢰과는 외과 계열의 진료과가 가장 높은 비율을 보였다. 입원 기간과 치과적 외상 후 협진 의뢰 시기는 양의 상관 관계를 보였다. 외상을 이유로 의과에 입원하는 것과 치과적 외상 발생은 통계적으로 유의하게 연관이 있었고, 교통 사고로 인해 의과에 입원한 환자의 경우 100% 치과적 외상이 동반되었다. 입원실에서 발생한 치과적 외상 환자의 나이는 유의하게 적었다. 치과적 외상과 입원 중 치료, 치과적 외상과 퇴원 후 치과 외래 재내원은 각각 통계적으로 유의한 연관이 없었다. 교통 사고에 의해 의과에 입원한 소아청소년 환자에 대해서 치과적 외상이 동반될 가능성이 크고, 치과 협진 의뢰 시기가 지연될 수 있다. 어린 소아 환자의 경우 입원실에서 치과적 외상의 발생 가능성이 크다. 또한, 치과적 외상 환자의 입원 중 치료와 퇴원 후 치과 외래 재내원이 잘 이루어지지 않고 있다. 이와 같은 결과들에 대해 협진 의뢰 체계의 개선, 입원 병동 내 전담 인력 배치와 치과 시설 및 장비 구비, 의과 주치의에 대한 교육, 주치의 간의 원활한 의사소통 등이 대안이 될 수 있다. 본 연구는 의과와 치과의 교육 및 연구 자료로 활용되어 치과적 외상이 있는 소아청소년 입원환자의 치과 협진 의뢰에 관한 발전에 기여할 것으로 기대된다.
Most patients and parents and guardians display frequent anxiety due to radiation exposure during outpatient, ward, and pediatric general radiographic examinations. This is a behavior that perceives only the harmfulness of radiation. For the recognition of medical radiation, we conduct surveys on outpatients, inpatients, and pediatric parents and guardians to identify their awareness, and then use the radiation dose promotional materials After providing accurate information on the use of radiation, the outpatient, inpatient, and pediatric parents and guardians were asked to explain the change in awareness. The questionnaire items were classified into five categories: repetitive radiation awareness for diagnosis, awareness of exposure dose, availability of exposure information, awareness of radiation risk, and awareness of health problems caused by radiation. There was a statistically significant difference in the items of recognition result of medical radiation, although there was a slight difference in the individual items in the pre and post-recognition results of providing information about the radiologists of the protector and the outpatient(p<0.05). Therefore, through the installation of these promotional materials, we will improve our awareness of medical radiation safety during general radiography surveillance in the Department of Radiology to provide better quality medical information and medical services, thereby contributing to strengthening the competitiveness of the hospital.
Purpose: The aim of this study was to evaluate the efficiency of the Humpty Dumpty Falls Scale as one of the falls risk assessment tools, and also to evaluate risk factors as predictors of falls in pediatric patient populations. Methods: In a retrospective, case-control design with data from the electronic medical records of 13 pediatric patients who fell and 1,941 who did not fall before matching and 429 who did not fall after matching by gender, age, diagnosis, and length of stay. Results: All the variables showed no significant differences after matching. At the cutoff score of 13, sensitivity, specificity, negative and positive predictive values were 92.3%, 37.1%, 99.9%, and 0.01%, respectively. The area under the Receiver Operating Characteristics was 0.597. The results from the logistic regression showed that the pediatric inpatient population who had higher risk scores was significantly associated with falls. The odds ratios ranged from 1.31 to 4.71 with 90% confidence interval. Conclusion: The saturation impairments criterion as one of the diagnostic parameter was negatively associated with falls, but the relative risk score was higher than the other criteria. Therefore, it seems that the diagnostic parameter seems to be required to verify results through large sample studies.

Purpose: This study was conducted to obtain data for the development of an effective fall risk assessment tool for pediatric inpatients through a systematic review and meta-analysis of the diagnostic test accuracy of existing scales. Methods: A literature search using Medline, Science Direct, CINAHL, EMBASE, and the Cochrane Library was performed between March 1 and 31, 2018. Of 890 identified papers, 10 were selected for review. Nine were used in the meta-analysis. Stata version 14.0 was used to create forest plots of sensitivity and specificity. A summary receiver operating characteristic curve was used to compare all diagnostic test accuracies. Results: Four studies used the Humpty Dumpty Falls Scale. The most common items included the patient's diagnoses, use of sedative medications, and mobility. The pooled sensitivity and specificity of the nine studies were .79 and .36, respectively. Conclusion: Considering the low specificity of the pediatric fall risk assessment scales currently available, there is a need to subdivide scoring categories and to minimize items that are evaluated using nurses' subjective judgment alone. Fall risk assessment scales should be incorporated into the electronic medical record system and an automated scoring system should be developed.
The purpose of this study was to analyze the differences in the outcome for CABG according to whether hospitals provided heart related surgeries. The 2011 National Inpatient Sample (NIS) and inpatient quality indicator principles from the Healthcare Research and Quality (AHRQ) were used for analysis. Hospitals were divided into three groups according to the surgeries they provided. The length of stay and in-hospital deaths were adjusted for the differences in risks. ANOVA was performed to examine the differences for the risk-adjusted in-hospital mortality rate and risk-adjusted length of stay among the three groups. The analysis results showed that hospitals providing CABG, PTCA, and PHS had lower risk-adjusted in-hospital mortality rates or similar risk-adjusted lengths of stay compared to those of hospitals providing only CABG. However, the three groups did not have statistically significant differences in outcome indicators. Another study will be needed with a larger sample.
This study was intended to contribute towards the development of proper drug use system for pediatric patients by investigationg problems related to their medication and identifying drugs that need to be developed into low dosage tab-lets or syrups for pediatric use based on our analysis on the prescriptions for pediatric inpatients from 22 hospitals in South Korea on a day of Feb. 2003. The usage rates in the proportion of less than 0.5 and 1 per unit of oral solid formulation were $29.9{\%}$ and $54.3{\%}$ in hospitals with 1000 beds or more, 36.5 and $60.6{\%}$ in hospitals with 500 to less than 1000 beds, $60.8{\%}$ and $81.6{\%}$ in hospitals with less than 500 beds. Of the 63 oral solid formulation products that were used two or more times in the proportion oi less than 0.5 units, 34 products ($54.0{\%}$) were used as such despite the fact that syrups and lower dosage tablets or capsules were available in the market, and 24 products ($38.1{\%}$) so even when syrup formulations were available. Therefore, it would be desirable that pharmacist communities in charge of dispensing identify the most frequently crushed drugs or those that require special attention in choosing dispensing powders or solutions and develop dispensing guidelines that can be adopted by pharmacists in practice. Moreover government-led policies are needed to encourage development and manufacture of the formulations for pediatrics and to correct unsound prescription and dispensing practices such as using crushed forms of certain oral solid formulations although alternative formulations are available in the market.
부비동염은 흔하고 일반적으로 소아에서는 양성질환인 경우가 많다. 경험적으로 사용하고 있는 항생제로 인해 소아의 부비동염의 두 개강 내 합병증은 드물지만, 발생할 경우 생명에 위협적일 수 있다. 두 개강 내 합병증은 정맥동 혈전, 안와 합병증, 뇌수막염, 뇌수막하 농양 등이 있다. 이러한 합병증은 소아에서 심각한 예후를 보일 수 있다. 따라서 뇌수막하 농양을 초기에 정확하게 진단하여 치료를 시작하는 것이 중요하다. 저자들은, 급성 부비동염 치료 중이던 15세 남아가 3일간의 발열, 두통을 동반한 편마비 증상으로 내원하여 뇌 전산화 단층촬영에서 뇌수막하 농양으로 진단되어 항생제 투여 및 신경학적 수술로 치료된 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
심신장애자, 심한 불안과 공포가 있는 경우, 너무 어려서 의사소통이 되지 않는 환자 등, 일반적인 행동조절방법이 가능하지 않은 경우에 전신마취를 고려하게 된다. 이런 환자들은 대부분 소아 치과에서 치료하게 되므로 소아치과의사의 역할이 중요하다. 전신마취하에 시행되는 치과치료를 고려할 때, 환자의 전신적 건강상태, 환자의 현재 구강 형태, 필요한 치과치료의 종류, 책임감 있는 보호자의 유무에 따라서 입원없이 행해지는 외래환자수술을 고려할 수 있다. 전신마취하에 치과치료를 시행하고자 할 때, 치과의사나 보호자의 편의를 위해서 사용되어져서는 안되며, 적절한 환자 선택하에 시행된다면, 보통의 방법으로 치과치료를 받기 어려운 장애아동에게 유리한 치료가 될 수 있다.

A total of 450 stool samples were collected from inpatient and outpatient clinics of Pediatric Department, Minia University Hospital, Minia District, Egypt. Two groups of patients were studied, including 200 immunosuppressed and 250 immunocompetent children. Stool samples were subjected to wet saline and iodine mounts. A concentration technique (formol-ether sedimentation method) was carried out for stool samples diagnosed negative by wet saline and iodine mounts. Samples were stained by 2 different methods; acid fast stain (modified Ziehl-Neelsen stain) and Giemsa stain. Total 188 cases (94%) were diagnosed positive for parasitic infections among immunosuppressed children, whereas 150 cases (60%) were positive in immunocompetent children ($P$<0.0001). The most common protozoan infection in immunosuppressed group was $Cryptosporidium$$parvum$ (60.2%), followed by $Blastocystis$$hominis$ (12.1%), $Isospora$$belli$ (9.7%), and $Cyclospora$$caytenensis$ (7.8%). On the other hand, $Entamoeba$$histolytica$ (24.6%) and $Giardia$$lamblia$ (17.6%) were more common than other protozoans in immunocompetent children.
목 적 : 소아 관찰병실(pediatric observation unit: POU)의 운영내역을 분석하여 국내에서도 그 적용이 가능한지를 평가하기 위함이다. 방 법 : 2006년 3월부터 2007년 2월까지 1년간 가톨릭대학교 성모자애병원 소아과 POU에 입원한 환아들을 대상으로 하여 의무기록을 분석하였고 평균재원일수와 병상회전율을 평가하기 위하여 연구기간 1년 전 자료와 비교하였다. 결 과 : 총 1,076명이 POU에 입원하였으며 환아들의 중앙 연령은 2.4세이었고 중앙 재원시간은 14시간 00분이었다. 질환별로는 장염(42.7%)이 가장 많았고 그 외 급성인후염(19.1%), 모세기관지염(7.8%), 폐렴(5.5%), 열성경련(5.2%) 순이었다. 전체 환아 중 7.5%(81/1,076명)는 일반입원으로 전환되어 입원이 연장되었다. 전환율이 낮은 질환들은 변비와 장염, 중이염과 인후염을 포함한 상기도 감염성 질환, 경련성 질환, 후두염이었고 전환율이 높은 질환들은 폐렴, 발열이 조절되지 않은 열성경련, 천식이었다. POU 시행 1년전 자료와 비교할 때, 전체 입원환자의 평균 재원일수는 4.69일에서 3.75일로 감소하였고 병상회전율은 1병상 당 78.8명에서 98.2명으로 증가하였다. 결 론 : POU는 국내에서도 효율적으로 운영될 수 있음을 확인하였다. 향후 POU는 외래와 응급실 진료의 제한점을 보완하며 불필요한 입원기간을 줄일 수 있는 소아과 영역의 새로운 진료 형태로 자리잡을 수 있을 것이라고 기대한다.
