Retrospective Study on Effect of Chuna Therapy, Also Known as Supine JS Cervical Therapy, Combined with Korean Medicine Treatment on Neck Pain and Headache Caused by Whiplash Injury (편타성 손상에 의한 경항통 및 두통 환자의 추나요법 병행 한방복합치료의 효과: 후향적 관찰연구 - 앙와위 경추 JS 신연 교정기법을 중심으로)

  • Objectives To observe the pain reduction effect of Korean medical treatment combined with supine JS cervical therapy on neck pain and headache caused by whiplash injury. Methods The medical records of 30 patients suffering from neck pain and headache caused by traffic accidents from August 1, 2020 to October 30, 2020 were studied. 15 patients receiving supine JS cervical therapy combined with Korean medical treatment were assigned to group A and another 15 patients receiving Korean medical treatment but without Chuna therapy were assigned to group B. The following patient data were analyzed retrospectively - age, sex, and number of admission days, neck pain and headache visual analogue scale (VAS), and neck disability index (NDI). Statistical analysis was conducted using IBM SPSS 25.0. for Windows. A p-value≤0.05 was considered to be statistically significant. Results Both neck pain and headache VAS score in group A and B showed a statistically significant decrease. The changes in neck pain and headache VAS score in group A was statistically significant higher compared to those of group B. NDI in group A showed a statistically significant decrease, but NDI in group B did not decreased statistically significant. Conclusions Korean medicine treatment effectively reduces neck pain and headache. Especially, the pain reduction effect is better in patients receiving Korean medicine treatment combined with supine JS cervical therapy than in those receiving Korean medicine treatment without supine JS cervical therapy.

The Role of Radiologic Study in Diagnostic Work-up of Headache Patients (두통환자에 대한 방사선학적 검사의 진단적 가치)

  • Objective : The goal of this study is to identify the significant radiologic abnormalities in patients complaining headache and to determine predictive factors for clinically significant radiological abnormalities. Method : The study population was 410 patients having underwent CT or MRI study among 1000 patients complaining headache in outpatient basis between 1996-1999. All of these patients answered self-administered questionaire about their headaches. We reviewed the patient's charts and the questionaires and examined the radiologic study results. Result : Of the 410 patients referred for CT or MRI study, male : female ratio was 1 : 1.97. Twenty-five patients(6.1%) revealed clinically significant organic lesions. Mean age was 46.1 in radiologic abnormal group and 48.4 in normal group. Short symptom duration(p<0.01), motor weakness(p<0.05), vomiting(p<0.05), cranial nerve palsy(p<0.05), and trauma history(p<0.05) were factors indicated higher incidence of radiologic abnormality. But, patients age, and severity of headache were not associated with clinically significant radiologic lesion. The ratio of radiologic abnormality was 0.8% in patients not having any risk factor. Conclusion : The results indicate that radiologic study should be done in headache patients having the risk factors such as short symptom duration, motor weakness, vomiting, cranial nerve palsy, trauma history. For patients without any such a risk factor, the radiologic study doesn't seem mandatory.

  • Background: The aim of this study was to report the effect of acupotomy for patients with headache. Methods: Four patients with headache were treated with acupotomy. The improvement of symptoms was evaluated by numeric rating scale(NRS), Headache impact test-6(HIT-6) and headache attack frequency. Results: The numeric rating scale score and headache attack frequency decreased throughout the treatment period. No side effects were observed during treatment. Conclusion: Acupotomy had shown a positive clinical effect in the treatment of a headache in this case series. Further studies are required for its application on various headache.

  • After performing the auricular acupuncture therapy for the functional headache, we came to gain the following therapy effects. 1. In the whole of the patients of the functional headache, housewives of forties were most of all. 2. In the classification of mordern medicine on the patients of the functional headache, muscular contractional headache was most of all and in the classification of oriental differentiation of symtoms and signs(辨證), headache due to deficiency of blood(血虛頭痛) was most of all. 3. In the regional auricular acupuncture therapy for the functonal headache, Taiyang of auricular point(太陽點) had an effect on the lateral headache around the area of Taiyang of auricular point(太陽點) though the actions of sedation, analgesia, Neck of auricular point(頸點) had an effect on the muscular contractional headache of the occipital region, and Stomach of auricular point(胃點) had an effect on the headache of stagnation of phlegm(痰濁頭痛) of anterior region with a chronic digestive disease. 4. The average treatment times of the auricular acupuncture therapy in the patients of the functional headache was 4.08 times, if we have them take auricular acupuncture therapy about 4 times it could be thought to take excellent effect of treatment. 5. The effect of auricular acupuncture therapy for functional headache was turned out to be effective in the following orders ; lateral region(Group A), occipital region(Group B), anterior region(group C). Through the clinical consequences of therapy mentioned above, we came to confirm regular therapeuric effects and hereafter, it is considered that a clinical comparative study on oriental cure and auricular acupuncture therapy for headache has to be more progressed than now.

  • Objectives: Since we found patients with dyspepsia as their major symptom had high rate of headache, would like to find out a rational diagnosis ground by using EAV correlation between headache and gastrointestinal disorder Methods: We did survey and EAV inspection on 128 patients came by gastrointestinal disorder who quoted 4 and 5 degree for their headache symptoms) Results & Conclusion The results of this study were as follows: 1. From dyspepsia, we saw higher rate by order of epigastric discomfort, bloating, stomach trouble, bleching, etc. 2. By EAV analysis, found functional deterioration by order of stomach, nerval degeneration vessel, gall bladder/bile duct, liver. 3. By EAV analysis, functional deterioration of stomach, gall bladder/bile duct, liver represent dyspepsia and nerval degeneration vessel represent headache. We found the correlation between dyspepsia and headache by observation at a time.

A Clinical Study of Tension Headache Patients using Depression & Anxiety Scales (우울(憂鬱), 불안(不安) 척도(尺度)를 이용(利用)한 긴장성(緊張性) 두통(頭痛) 환자(患者)의 임상고찰(臨床考察))

  • Objective : The aims of this study were to show the clinical state and to investigate depression and anxiety by BDI and STAI scale in tension headache patients. Method : The patient group was consisted of the 20 patients with Tension Headache who were treated in Department of Neuropsychiatry, Daejeon University Oriental Hospital from 3 March 2003 to 30 September 2003. The control group was consisted of the 20 patients on physical therapy with cerebaral vascular disorder in the Daejeon University Cheonan Oriental Hospital. The patient group was investigated by various characteristics, and compared with the control group by BDI and STAI scores. Result : 1. The ratio of female was higher, the 40 aged were higher frequence, in distribution of the period of the clinical history, short term within 1 week was the most.. 2. Tension headache was most frequent at whole portion, neck stiffness, shoulder pain, general body weakness were mainly coexited, and the prescription invigorating spleen supplementing qi and tonifying qi and blood. 3. The BDI score was higher in patient group, but there was no significant difference 4. The STAI score was higher in patient group, but there was no significant difference.

Alexithymia in Patients with Tension-Type Headache (긴장성 두통 환자에서의 감정표현 불능증)

  • Objective: Various psychological factors influence the occurrence of tension-type headaches. The aim of this study is to compare the level of alexithymia between tension-type headache patients and normal controls. Methods: Sixty-six subjects with tension-type headaches and 59 controls were studied. The Beck Depression Inventory and Toronto Alexithymia Scale (TAS-20K) were administered to the tension-type headache group and TAS-20K to the normal control group. Results: Compared with normal controls, the tension-type headache group had significantly higher alexithymia scores. There was also significant association between the level of alexithymia and the severity of the depression in tension-type headache patients. Conclusions: These findings suggest that patients with tension-type headaches have difficulty in expressing their emotions. And in patients with tension-type headaches, the more alexithymic they are, the more depressive.

A Preliminary Comparison of Efficacy of Needle-Embedding Therapy with Acupuncture for Chronic Tension-type Headache Patients (만성 긴장성 두통환자의 매선요법과 침의 효과에 대한 예비적 비교 연구)

  • Objectives : The purpose of this study was to compare the efficacy of Needle-Embedding Therapy with Acupuncture for Chronic Tension-type Headache (CTTH) patients. Methods : The study recruited Chronic Tension-type Headache patients. A total of twenty-six patients were divided into 2 groups. 13 subjects were placed into a needle-embedding therapy group and 13 subjects into an acupuncture therapy group by a randomization table. The needle-embedding therapy group was treated with needle-embedding at Fengchi (GB20), while the acupuncture therapy was acupunctured at Fengchi (GB20). Then, a comparative analysis was conducted by comparing the results with those measured by a Blood pressor, Heart rate variability (HRV), Henry Ford Headache Disablity Inventory (HDI) Internet Headache Impact Test (HIT), and Visual Analogue Scale (VAS). Results & Conclusions : As a result of evaluation by using the self-rating headache index (VAS, HDI, HIT), pain score declined in both groups. But they were not difference between the two groups.

Effects of Scalp Myofascial Technique and Manual Therapy on Pain and Quality of Life in Tension Type Headache Patients : Six Month Follow-up Results (두피근막이완기법과 도수치료가 긴장성두통 환자의 통증과 삶의 질에 미치는 영향 : 6개월 추적연구)

  • Purpose : We aimed to validate a new manual therapy to treat tension type headache(TTH) by applying myofascial release techniques to the scalp and to examine the changes in the quality of life and the headache characteristics after treatment and at the 6-month follow-up. Methods : Thirty patients were recruited in this study and were assigned to two groups through simple random sampling. Fifteen patients were assigned to the manual therapy group (MT) and 15 patients to the scalp myofascial release technique (SMT) group. However, five patients from the MT group and one from the SMT group were excluded. Therefore, 24 patients with TTH (10 males, 14 females) participated in the study. Patients underwent either MT or SMT. The procedures were performed by a physical therapist twice per week for 4 weeks. The quality of life [using the brief pain inventory (BPI) and the headache impact test (HIT)], and the frequency, duration, and intensity of the headache [on a visual analog scale (VAS)] were assessed before and after the treatment, and at the follow-up. Results : After 4 weeks of SMT, the frequency (p<.001), duration (p<.05), and intensity (p<.001) of the headache and the quality of life (HIT; p <.001, BPI; p<.001) significantly improved in the patients with TTH. The improvement in these parameters remained significant even after 6 months of follow-up. Similarly, After 4 weeks of MT, the frequency (p<.05), duration (p<.05), and intensity (p<.01) of the headache, and the quality of life (HIT; p<.05, BPI; p<.001) significantly improved in the patients with TTH. The improvement in these parameters remained significant even after 6 months of follow-up. There was no significant difference in these parameters between the two groups. Conclusion : It has been suggested that MT using the SMT can be used as a non-invasive treatment to treat the frequency, duration, and intensity of the TTH, and to improve the quality of life.

Evaluation of Craniocervical Posture in the Patients with Chronic Tensional Headache (만성 긴장성 두통환자에 있어서 두경부 자세의 평가)

  • The purpose of this study was to evaluate the effect of craniocervical posture on craniomandibular disorders with chronic headache. The author measured craniocervical posture on frontal and sagittal plane with photographs for 26 headache patients, 23 TMD patients, and 27 nonpatients. Range of cervical spine motion was also measured. The bilateral electromyograms of masseter and anterior temporalis muscles were recorded at rest and during maximum clenching. The results were as follows : On the lateral view photos, eye-tragus-C7 line angle was larger and the tragus-C7-horizontal line angle was smaller in the patient groups than in the nonpatient group (p<0.05). On the frontal view photos, mouth corner line angle was larger in the headache patient group than in the nonpatient group and TMD patient group (p<0.05) Interclavicular angle was smaller in the headache patient group and TMD patient grop than in the nonpatient (p<0.01) The right and left differences of SAIC-plane distance and finger tip-plane distance were significantly larger in headache patient group than TMD patient group and nonpatient group (p<0.01, p<0.001). Cervical motion range was smaller in the TMD patient group and headache patient group than in the nonpatient group (p<-.001, p<0.05, p<0.05). The resting EMG activities of right masseter muscle were higher in the headache patient group than in the nonpatient group (p<0.05). However, the EMG activities of masseter and anterior temporalis muscles during maximal clenching were lower in the patient group than in the nonpatient grop (p<0.01). The asymmetry index of resting EMG of masseter muscles was higher in the headache patient group than nonpatient group (p<0.05).
