노드 다변화 및 손실률 최소화를 위한 이동환자 상시 모니터링 시스템 시뮬레이션 개선 연구 (Improving the Simulation of a Mobile Patient Monitoring System for Node Diversification and Loss Minimization)

  • U-Healthcare service is a real-time service using the vital signs which are continuously transferred from monitoring sensors attached to mobile patients under the wireless network environments. It should monitor the health condition of mobile patients everywhere at any time. In this paper, we have improved two features of the three layered mobile patient monitoring system with load balancing ability. First, the simulation process has been improved by allowing the number of related nodes to be changed. Secondly, we have modified S node to which queue is added to reduce the loss rate of collecting data from patients during the delay of S node process. And the data from the patient with high priority can be transferred to the server immediately through the filtering function. Furthermore, we have solved the problem of redundancy in sharing information among S nodes by differentiating process time to each S node. By performing a DEVS Java-based system simulation, we have verified the efficiency of this improved system.

골밀도 검사의 재검사 방지에 따른 업무효율 향상 (Increasing Work Efficiency with Prevention of Reinspection for Bone Mineral Density (BMD) Exams)

  • 최근 대형병원에서 고객서비스의 일환으로 원 스탑 서비스(One Stop Service)를 적용하여 진료에서 진찰, 검사, 결과까지가 하루에 이루어질 수 있도록 변화되어 가고 있다. 따라서 검사를 한 번 방문에 종료될 수 있도록 하기 위해 대부분의 검사를 같은 날 예약하여 시행 하지만 같은 날 여러 검사가 함께 시행됨으로 검사 특성에 따라 재방문하여 검사를 해야 하는 경우가 발생되고 있다. 본 논문은 개선 전 후의 재검사의 건수를 비교 분석하였고 골밀도 검사를 우선순위로 지정하고 OCS의 I-manual Update 및 그 내용의 홍보와 교육을 실시한 결과 재검사 건수를 병동은 58건에서 21건으로, 외래는 51건에서 12건으로 감소시킬 수 있었고 영상의학과 직원 예약에 의한 오류는 98건에서 11건으로 감소시킬 수 있었다. 따라서 재검사의 원인을 정확히 파악하고, 검사 전에 이력을 확인하여 검사 우선순위를 지정하면 재검사를 줄일 수가 있으며 이는 근무자들의 업무 스트레스 감소에 따른 효율성의 증가 및 환자들의 불만 감소로 인한 고객만족도 향상과 나아가 병원 신뢰도 향상에 영향을 줄 것으로 예상된다.

의료기관 사회적 책임(HSR)활동의 IPA 평가 (IPA Assessment of Hospital Social Responsibility Activities)

  • Objectives: We analyzed the importance and performance of hospital social responsibility (HSR) according to the characteristics of hospitals, and presented strategies for HSR activities. Methods: An online HSR questionnaire was sent to hospitals nationwide from October 12 to 26, 2018. The 206 responses received were analyzed in accordance with the IPA to assess the performance and importance of HSR. Results: There was a statistically significant difference between the employees and hospitals regarding the importance and performance of HSR activities. In the area of "sustained maintenance", items related to consumer issues such as "compliance with personal information processing policy", "patient confidentiality", "fair information provision", and "system for patient safety and infection prevention" were derived. In the area of "'high priority for improvement", there were three common items between hospitals and general hospitals: "regular donations and support from local communities", "active cooperation with related institutions", and "compliance with process-related laws and regulations". In the area of "low priority", four items were derived: "support for employee participation in community activities", "efforts to hire local residents", "education and cultural programs for local communities", and "transparent support for political activities". In the area of "sublation of excessive efforts", two items of "employee welfare efforts" and "efforts to improve labor relations", were commonly found in hospitals and general hospitals. Conclusions: It is necessary to improve the management efficiency of hospitals by the systematic allocation of manpower and resources through the establishment of four regional strategies based on the results of IPA analysis.

이동 환자 상시 모니터링 시스템의 시스템 명세 기법 기반 설계와 검증 (System Specification-based Design and Verification of Mobile Patient Monitoring System)

  • 유-헬스케어(U-Healthcare)에서 이동 환자 상태에 대한 모니터링 기술은 필수적이다. 모니터링 기술에서는 이동 환자의 생체 정보가 동적 위치 정보와 함께 정해진 목적지 서버로 전송되어야 하는데 모니터링할 환자의 수가 증가하며 각 환자의 이동성이 불규칙함에 따라 적절한 부하균형 단계가 중간에 필요해진다. 토르소(Torus) 구조를 기반으로 한 이동 환자 상시 모니터링 시스템에서는 이를 반영하여 먼저 환자 생체 정보를 중간노드로 전송한 후 부하균형 기반의 우선순위 정책에 따라 목적지 서버로 연이어 전송된다. 본 논문에서는 계층적이며 객체지향적인 정형화 명세를 기반으로 하는 시스템 명세 기법을 사용하여 이러한 이동 환자 상시 모니터링 시스템의 전체 구조와 구성 노드들의 결합 관계를 정의한 후 각 요소의 기능을 유사코드로 설계함으로써 본 시스템이 효과적인 환자 정보 수집과 전송, 분배 및 판단에 효율적임을 시스템 명세 기법을 통하여 검증한다.

인삼 복용 후 발생한 비출혈 및 심계항진을 주소로 한 소양인 환자 1례 (A Case Study of a Soyangin Patient with Palpitation and Epistaxis that Occurred after Taking Panax Ginseng)

  • Objectives The purpose of this case study was to report improvement of palpitation, epistaxis, nausea, and anorexia after treatment with Dojeokgangki-tang gamibang in a middle-aged woman who has been taking Panax Ginseng continually. Methods Upon beginning her Dojeokgangki-tang gamibang regimen the patient was asked to self-evaluate the degree of discomfort in her symptoms using a using a number scale from 0 (no symptoms) to 10 (most severe). Evaluation of palpitation and chest discomfort was made a high priority. Results The palpitation the patient experienced, and self-evaluated as an "8" on the number scale at the time of her admission to the hospital, had disappeared (become "0") by the fourth day of hospitalization. The numerous symptoms which accompanied her palpitations (chest discomfort, nausea, anorexia, general weakness, weight loss, and depression) also improved accordingly. Conclusions Health functional food can pose serious health risks such as sudden bleeding and palpitations in individuals with pre-existing health problems. One must exercise extreme caution in deciding to take such supplements. As seen in the case of the middle-aged woman, the quality of life of patients who had experienced adverse reactions to such supplements can be treated through the administration of Dojeokgangki-tang gamibang.

입원기간에 따른 의료서비스 품질요인이 고객만족에 미치는 영향에 관한 연구 (The Impact of Health Service Quality Factors on Patient's Satisfaction According to Duration of Hospitalization)

  • The purpose of this study is to identify which factors of medical service quality were considered important by patients based on survey on eight hospitals located in Seoul. We analyzed the difference in impact of health service quality factors between the two groups with short and long inpatient days. In addition, we analyzed which service factors affected the customer satisfaction more differently across the inpatient groups. We found that patient satisfaction was not different between the two groups with short and long inpatient days. But the two groups showed different priority orders of medical service factors that affect patient satisfaction. In the group with long inpatient days, the order of medical service factors are cleanness, speciality, process. In the group with short stay, important factors are speciality, cleanness and convenience of facilities. These findings imply that hospitals should set up different marketing strategies across the different inpatients groups.

  • Emergency room process is very important in the whole hospital processes because it is first diagnosis for patient. Above all, triage is important activity which quickly diagnose the status of emergency patient and sets the priority for treatment. This paper analyzes the treatment process pattern by triage type. The results show that the treatment process after triage such as residence time, diagnosis and checkup type, and joint treatment are dependent on triage types. We can use these analysis results for improving the current triage system and developing the new triage system considering a domestic emergency medical service environment.

Patient Expectations of Visiting Department of Oral Medicine for Burning Mouth Syndrome: Relationship between Expectations and Clinical and Psychological Characteristics

  • Purpose: The aims of this study were to investigate the expectation of patients with burning mouth syndrome (BMS) and assess the relationship between patient expectations and clinical and psychological characteristics. Methods: Eligibility was retrospectively assessed on 93 patients with BMS. A total of five questionnaires on patients' expectation for a visit, pain, sleep quality (Pittsburgh Sleep Quality Index), cognition (Pain Catastrophizing Scale) and psychological distress (Symptom Checklist-90 revised) were measured. Results: Sixty-five patients were included in this study. The top 3 priorities of expectation for a treatment visit to the Department of Oral Medicine were as following; "I want my pain to be free"; "I want to understand why it hurts"; "I want to communicate better with doctors about pain". Patients with priority of pain relief showed poor sleep quality than patients who are more interested in the disease related information. Conclusions: To improve treatment outcomes of BMS, clinicians should improve their own understanding of patients who are suffering from BMS. Inquiring about the patient expectations may be one of them, and it would contribute to the enhancement of patients' overall well-being.

안락사에 대한 의료계열 학생들의 인식도 (The awareness of medical department students about euthanasia)

  • Purpose: This study examined medical college students' perception of euthanasia and analyzed whether there were significant differences in their perceptions based on their grade, religion, and economic situation. Methods: A questionnaire comprising items relating to students' knowledge about euthanasia, their economic situation relationships with patients, and the extent to which euthanasia would be allowed depending on the patient's condition. Results: Results by age and school year revealed there were no statistically significant differences concerning permissible levels of euthanasia. However, differences were found in the permissible levels of euthanasia according to changes in the patients' condition. Conclusion: It can be interpreted that students' personal values and religion had a great effect on the perception of euthanasia than the process of acquiring medical knowledge. The religion or values they have in your life affect your perception of euthanasia more than the process of acquiring and learning medical knowledge. The suffering of patients is the most important priority and it supports euthanasia on the grounds of relieving psychological distress for patients' families and the right of every patient to die gracefully. Nevertheless, considering the possibility of patient survival and euthanasia being used for murder, it is necessary to legislate the use of euthanasia and have the appropriate qualifications for its administration on patients.

응급의료 통신체계의 발전방안 연구 (A Study on Developmental Policies of The Emergency Medical Dispatch System in Korea)

  • The purposes of this study which was conducted by studying the literature on Emergency Medical Dispatch System are to provide some developmental policies of quality management, pre-arrival instructions, priority dispatch protocols, training program for the dispatchers(Emergency Medical Dispatchers or EMDs) in Korea and to promote understanding emergency medical dispatching. The conclusions from this summarized as follows; (1) It is confirmed that there has been little study on the Emergency Medical Dispatch System in Korea, because for the first time, the real Emergency Medical Services were introduced into Korea in 1994, and the importance of the Emergency Medical Dispatch System has not been realized. (2) Only some squads are using a set of dispatch protocols, others aren't. (3) In spite of trying to introduce a new set of dispatch protocols, it isn't the priority dispatch system using a complete set of dispatch protocols which has key questions, pre-arrival instructions, mode & configuration based on patient assessment. (4) The EMS is unable to promote the service capacity by using quality management, because there is no medical control on the emergency medical dispatching and the EMDs. (5) There are no medical directors in the communications center who are in charge of the medical control to detect problems derived from the EMS and to solve them. (6) There are no systematic training program for the EMDs who are taking charge of dispatching. (7) Having a deep relation to the elements of the EMS, the emergency medical dispatching is subject to restriction of those elements.
