Comparison of treatment options in meniere's disease

  • Meniere's disease (MD) is a disease that affects the inner ear. It is formed as a result of endolymphatic hydrops. Hearing loss and vertigo are important in the diagnosis of MD. There is fluctuating and progressive hearing loss. Vertigo attacks cause severe dizziness in the patient. There are many treatment options in MD. These are hearing aid, diet, medication and surgery. In this study, we will discuss the advantages and disadvantages of the different treatment options. Treatment options have been compared to find out what the appropriate treatment is. Another concern is the importance of surgery in MD. This study is combination of qualitative and quantitative studies. Much focus will be on vertigo, and appropriate treatment options of MD will be mentioned also the importance of surgery. The main question in this study is the necessity of surgery. Surgical procedures are the most doubtful treatment option because of their indications and contraindications. In this study, it has been noticed that surgical operation should be delayed as much as possible. Priority is to try other treatment options. Surgery can be considered as a last resort. When we look at the operations performed, operations are mostly done in advanced Meniere cases.

  • We focus on the solutions to prevent fatal risk to patient's life caused by transfusing blood which is wrong type or has exceeded norm temperature. Also, this study gives priority to the verification of medical safety of using blood, of which management is applied advanced sensor tag technology adopted RFID(Radio Frequency Identification) temperature sensor and USN(Ubiquitous Sensor Network) for temperature management of blood. Therefore, this study can contribute to protect of health of patients who take blood transfusion through construction of basis of new process of blood management.

A Study On The Experiences of Turnover among Hospital Nurses (임상간호사의 이직경험)

  • The purpose of this research was to understand the structure of the experiences of turnover among hospital nurses. The research question was 'What is the structure of the experiences of turnover among hospital nurses?' The sample consisted of 16 hospital nurses who experienced one or more turnover. The unstructured interview were carried out from July 15. through August 31., 1997. Intervewed by audio-recording and analyzed by Van Kaam's method. There were 361 descriptive expressions and priority classifications. The result summerized as 107 common elements, 38 syntheses of hypothetical definitions and 6 identifications of the structural definition. The structural definitions and hypothetical definitiones were as follows ; 1. There were different views on the turnover intention. Hard task, dissatisfaction of initial expectancy, uncomfortable relationship with doctors, inappriate reward, uncomfortable relationship with workers of other parts, uncomfortable relationship with patients, unreasonable promotion, uncomfortable relationship with co-worker, helplessness, poor environment of working conditions. 2. Motivating factor of turnover can be varied : self development, problem of social support, boring task, problem of relation-ship with the senior, unfair promotion, night duty. 3. Social supportive factors were co-worker support, senior support, self control, family support, time support. 4. There were different views on the job satisfaction. Task, relationship with patient, recognition, professionalism, promotion, working environment, relationship with co-worker, reward.

Opinion on the legalization of the actual duties of clinical dental hygienists (임상치과위생사의 실제적 업무의 법제화에 대한 견해)

  • purpose : The purpose of the study is to investigate opinion on the legalization of the actual work of clinical dental hygienists. Methods : A self-reported questionnaire was completed by 171 dental hygienists in Busan and Gyeongnam from December 1, 2017 to January 31, 2018. Structured questionnaires were uesd for analysis. The collected data was analyzed using IBM SPSS. Statistic 20.0. Results : The first, assist of dental treatment, Second, dental health care education and consulting duties, and third priority preventive duties were found to be the most important duties in the dental office. Currently, it was investigated perform in a lot of workplace. Preventive treatment tasks include scaling, teeth polishing, applying fluoride and assist of dental treatment include preparation for medical care, preparation for implant surgery, preparation for oral surgery, implant surgery assistant and patient care training, and preparation for periodontal surgery. Conclusion : As a result, the dental hygienist performing a lot of dental assist tasks, oral health education counseling and preventive duties. Nonetheless, clinical dental hygienists are outlaws due to the disparity between practical and legal duties. Therefore, it is urgent to legislate actual duties of dental hygienist so that experts can fully exercise the competency of the public in promoting dental health. Relevant governments and related organizations should take reasonable measures to solve this problem.

Cognition and Attitude of Hospital CEOs toward Healthcare Quality Improvement Activity (의료 질 향상 활동에 대한 병원장의 인식 및 태도)

  • Background : The purpose of this study was to investigate the understanding and the attitude of Korean hospital CEOs toward the healthcare quality improvement. Methods : A mailed questionnaire survey to the CEOs of hospitals with 400 beds or more was conducted between September 15 and October 30, 2000. Of the 108 hospitals eligible for the study, 58 participated, yielding a response rate of 54 percent. Result : The hospital CEOs have expressed that their hospital management was arduous job, and they had been pressured by increasing competitions among healthcare providers. They indicated that the low fees of health insurance made their hospital management difficult. The results also indicated that there was general consensus that the improvement of service quality was important in encouraging their organizations, but the investment of manpower and equipment ranked higher than the improvement of service quality. The majority of the CEOs have good understanding about quality improvement activities. However the facts that in general QI must be focused at the process of services and customer satisfaction, meanwhile quality improvement activities are helpful for the organizational productivity embarrassed them. The hospital CEOs responded that there were successful changes in terms of quality of care, patient satisfaction, and process efficiency after QI activities, but no increase in patient number and profit. Lack of understanding to QI activities and limited budget seem to attribute unsatisfactory outcomes. Conclusion : The majority of Korean hospital CEOs have a good understanding and attitude about QI activities. As mentioned in the result, despite of several limitations, several facts regarding the CEOs of hospital in Korean can be elucidated. (1) The general cognition of the QI project is relatively high, and it is accepted with positive concern, (2) the priority of the QI project, however, is not set higher than other projects and (3) the specific concepts of the actual QI project such as customer (patient)-focused work driving, the recognition of the work accomplishment, and the importance of rewards have not sufficiently understood.

Study on the Network System between of the Outpatient and Central Treatment Department of Long Term Care Hospitals (요양병원 외래 및 중앙진료부의 의료영역 간 연결관계에 관한 연구)

  • Purpose: As our population ages and becomes an elderly society the number of elderly care hospitals is rapidly increasing. Because physical functions and spatial perception in the elderly decrease with age, these hospitals require more systematic and intelligent space designs. The design of these spaces are even more complex because they must accommodate medical programs to treat various different diseases and ailments and also because there are many first time patients and irregular short term patients that seek out outpatient treatment services. Also by analyzing the spatial configuration systems and systematic relationships between each of the functional spaces of the outpatient treatment service departments for hospitals specialized in care for the elderly by focusing on the hallway and corridor systems of these hospitals, the according characteristics and trends were examined. Methods: Based on preceding research, the types of hallway and corridor systems of these hospitals were categorized into five types, including gallery corridors, middle corridors, hall-type, mixed type and cyclic type corridors, and into six types according to function including by medical diagnosis, patient registration, examination, administration and convenience and shared common space to derive any interconnecting relationships between the corridor systems. Also by comprehensively examining the types and combined utilization of the corridor types and the integration and the intelligibility of the space syntax, any trends within the corridor system were derived. The elderly care hospitals examined in this research study were twelve hospitals that opened after the year 2000 in Korea with more that 150 sick beds with areas larger than $1000m^2$ and with all outpatient medical service related rooms located entirely on a single floor of the hospital. Results: The following results could be confirmed based on this research study. 1) The spaces where medical diagnosis and examination occurred were adjacent, and the movement lines for first time patients and re-visiting patients were taken into consideration by separating the treatment space. 2) This research study confirmed that the larger the size of the hospital was, there were more detailed categorizations of treatment services and that there was a tendency for treatment areas to be separated and independent from examination areas. 3) There was a tendency for integration and intelligibility to decrease the more complex and diverse the combination of hall types designed into the corridor systems of these hospitals was. cyclic type corridors dramatically decreased the intelligibility of the corridor systems of these hospitals. 4) The priority rank of these spaces were confirmed to be highest in the order of registration, diagnosis, examination, treatment, administration and shared common spaces. However it was confirmed for the local integration that the diagnosis scope had the highest priority rank. Implications: There were exceptional cases confirmed where the number of unit spaces did not have an absolute effect on integration and intelligibility. These results can be interpreted to mean that this can be overcome through efficient architectural planning.

호스피스 대상 환자들에 대한 목회자들의 인지도

  • This paper has surveyed above all what the patients call for physically, psychologically, and socially and researched how the ministers have acknowledge on cancers, their patients and hospice by means of enquete. To the difficulty the pastors should give their hands. This study researched what thoughts and behaviors the ministers actually had by the method of questionnaire. It was done from October 20, 1997 through December 10, 1997 on seven thousand of questionnaire paper of which 149 given back on the line of post or firsthandly were analyzed. The questioness consisted of 141 men(94.6%), 8 women(5.4) and 104 pastor on appentice(69.8%). There were 58 questionees(38.9%) who were hospitalized of their own diseases, and 121 questionees(81.3%) who had cancer patients hospitalized among their church people, relatives, or friends. Most of all the questionees(79.9%) had not any experience, such as they took some instruction about cancer patients. 72.5% of all the questionees had not ever served patients around them. The followings are the contents of the enquete: the questionees' view to chronic patients and death, where they put the priority in the case of that they look after patients who are under terrible pain ahead of death, what they think of such a situation as a patient has no technical possibility to be recuperated, why they think pastors do not like to visit chronics, which therapy they choose, whether they mainly control their pain or they do their best in order to heal their diseases, how much the questioned ministers know hospice, where and how they get the information on it, how much important role they play in the hospice team, whether the volunteers for hospice have not worked on account of pastors' misunderstanding to hospice service, whether the ministers want to take part in hospice service with their church people or not, and so forth. Suggestion: First, the education and P.R. about hospice are requested for the ministers. Second, the ministers must participate in hospice activities actively. Therefore the ministers and the churches must firsthandly take part in hospice activities beyond the education only and the raising of the recognition to them.

Immediate implant placement in areas of aesthetic priority (상악 전치부에서 단일 치아 발거 후 즉시 임플란트 식립)

  • Implants placed immediately after tooth extraction have been shown to be a successfully predictable treatment modality. Several clinical papers suggest that placing implants immediately after tooth extraction may provide some advantages: reduction of the number of surgical procedures or patient visits, preservation of the dimensions of alveolar ridge, and shortening of the interval between the removal of the tooth and the insertion of the implant supported restoration. In this case report, three patients received single immediate implant placements to replace a maxillary anterior tooth at the time of extraction. As the three cases were somewhat different, treatment protocols had to be modified as follows: Case 1. Immediate implant placement with healing abutment connection. Case 2. Immediate implant placement with immediate provisionalization. Case 3. Immediate implant placement with Guided Bone Regeneration(GBR). Every implant of these cases was placed in proper position buccolingually, mesiodistally and apicocoronally, The procedures following implantation such as immediate provisionalization and GBR were free of problem. Healing of each case was uneventful. In all cases, treatment outcomes were mostly satisfactory and the results maintained during follow-up periods. However, one case (Case 3) showed some papilla loss due to failure in delicate soft tissue handling during surgery. This papilla loss was compromised by prosthetic means. In conclusion, immediate implant placement in the fresh extraction socket can be a valid and successful option of treatment in aesthetic area. Moreover, this treatment protocol seems to maintain the preexisting architecture of soft and hard tissues in most cases.

Survey on Oral Health Awareness by Pneumoconiosis Patients of Care Hospitals (요양병원 진폐환자의 구강건강인식 조사)

  • This study was conducted to examine the oral health awareness, needs of dental treatment and the oral and maxillofacial pains targeting pneumoconiosis patients of care hospitals for the purpose of providing basic data for preparing a plan to improve the oral health of patients plagued with pneumoconiosis. As the subjects of survey, total 120 patients who have agreed to the research among the patients of three care hospitals, were enrolled between March and July in 2015. As the result of the study on the factors that influence the oral health perception, it was verified that the persons of an average monthly income of 2 million won or more, were shown to have significantly better oral health awareness than the subjects of monthly earning of less than 2 million won (p<0.05), thus showing that the oral health awareness by the people of monthly income of more than 2 million won, was 0.77 times higher compared to the others. Therefore, in order to improve the oral health awareness of pneumoconiosis patients, the economic stability of the patients should be taken into account as a priority when examining the oral health policy decisions and it was recommended that the survey and research on the oral health of patients with pneumoconiosis can be done in amore systematic manner.

Proposal on the Process and Prognosis of Popular Diseases (질병의 경과와 예후 판별에 대한 제언)

  • Objectives : This study was designed to make beneficial proposal for clinical application on some of the most common disorders treated by Oriental medicine by analyzing treatment process and prognosis. Methods : Number of peculiar attributes pertaining to a specific disorder were analyzed and based on those attributes, patterns associated with process and prognosis were interpreted in reference with classical literatures. Results : 1. Factors which can influence the progression and prognosis include time of onset, intensity of symptoms, course of passage, effects of risk factors, condition of the patient's righteous qi(正氣), accuracy of differential diagnosis made by the practitioner, accuracy of treatment methods, and other unexpected external influences. 2. Correlation between the condition of disorders and treatment progression is closely associated with proper treatment procedures and performances. The time of onset and intensity play critical roles in the treatment process and prognosis and showed pattern tendency with mutual interactions. 3. When there is complication of various disorders, it is ideal to give priority to more urgent illness and take care of moderate illness later. If there isn't any correlation between disorders, treat them in the order of acute to chronic disorders. The approach is reversed when disorders are related, treating in the order of most chronic to most acute. 4. In a case of complication of various disorders, depending on the disorder being acute or chronic, intensity, and accuracy of treatments, either a domino effect or gradual fade out of symptoms were witnessed. 5. The concept of "Five Evils Theory" according to Nan Jing(Difficult Classic) is essential in grasping disease progression due to interrelationships between zangfu organs. Conclusions : Predicting of disease process and prognosis for vast array of disorders treated by Oriental medicine is a very difficult task, yet evaluating the disorder's peculiar properties and influential factors resulted in few principles which can be effectively applied into clinical applications.
