남성의 안드로겐성 탈모증에 대한 한의학적 치료 효과 (Oriental Medicine Therapy in the Treatment of Men with Androgenetic Alopecia)

  • Background : Recently the demand was increasing on the data describing treatment rates and the outcome of Androgenetic Alopecia using Oriental Medicine. Thus this study was designed to observe androgenetic Alopecia treatment using Oriental Medicine including acupuncture and herb because there has been no such statistics. Objectives : Our purpose was determine whether oriental medicine treatment leads to clinical improvement in men with androgenetic alopecia. Methods : Eighty two Men, 17 to 53 years of age, with male pattern hair loss according to a modified Norwood/Hamilton classification scale(II to VI) were enrolled in this study. Patients were treated with multidisciplinary Korean oriental medicine treatment (herbal acupuncture, acupuncture, herb, chuna therapy) at twice a week for 6 months to 12 months. Efficacy was evaluated by patient and investigator assessment, and global photographic review. Results : Oriental medicine treatment improved scalp hair by all evaluation. Patient self assessment demonstrated that Oriental medicine treatment slowed hair loss, increased hair growth, and improvement appearance of hair. These improvements were corroborated by investigator assessments and assessments of photographs. Conclusion : Oriental medicine treatment has significant effects on the androgenetic alopecia. Longer follow up studies are needed to examine whether its effects last long enough for the patients with androgenetic alopecia.

퇴행성슬관절염에 대한 봉약침과 온침효과 비교 (A Comparative study of Warm needling and Bee Venom Pharmacopuncture on Osteoarthritis of the Knee - a Randomized Controlled Trial -)

  • Objective This study was performed to investigate whether Bee Venom Pharmacopuncture(BVP) could be a more effective modality than Warm Needling(WN) in relieving pain and symptoms of knee osteoarthritis(OA). Design Prospective, randomized and controlled clinical trial. Setting Single center trial in Korea Patients 49 volunteers with knee OA participated in the study. All the participants were screened through an inclusion and exclusion criteria. 33 participants were completed the clinical trial. Intervention The subjects were randomly assigned to one of two groups. One group received BVP(n=18), while the other group received WN(n=15). Sixteen sessions of BVP or WN were given at the pain region of the problematic knee for 8 weeks. Primary outcome measure is the Korean translation of Western Ontario and McMaster Universities Osteoarthritis Index scores(Korean WOMAC, KWOMAC). Secondary outcome measure is the physical health scores based on the 36-Item Short-Form Health Survey(SF-36) and Patient Global Assessment(PGA). KWOMAC and SF-36 were measured third (baseline, 4 and 8 weeks). PGA was measured twice(4 and 8 weeks). Results BVP group showed significant decrease compared to WN group in pain, function and total scores of KWOMAC according to the Mann-Whitney U-test. In the PGA, BVP group, compared to WN group, showed a significant increase. Conclusions BVP was more effective in relieving pain of knee OA than WN. These findings suggest that BVP is a promising alternative for treating knee OA.

입원 환자 영양검색 지표 개발 (Development of Nutrition Screening Index for Hospitalized Patients)

  • Several studies about hospital malnutrition have been reported that about more than 40% of hospitalized patients are having nutritional risk factors and hospital malnutrition presents a high prevalence. People in a more severe nutritional status ended up with a longer length of hospital stay and higher hospital cost. Nutrition screening tools identify individuals who are malnourished or at risk of becoming malnourished and who may benefit from nutritional support. For the early detection and treatment of malnourished hospital patients , few valid screening instruments fur Koreans exist. Therefore, the aim of this study was to develop a simple, reliable and valid malnutrition screening tool that could be used at hospital admission to identify adult patients at risk of malnutrition using medical electrical record data. Two hundred and one patients of the university affiliated medical center were assessed on nutritional status and classified as well nourished, moderately or severely malnourished by a Patient-Generated subjective global assessment (PG-SGA) being chosen as the 'gold standard' for defining malnutrition. The combination of nutrition screening questions with the highest sensitivity and specificity at prediction PG-SGA was termed the nutrition screening index (NSI). Odd ratio, and binary logistic regression were used to predict the best nutritional status predictors. Based on regression coefficient score, albumin less than 3.5 g/dl, body mass index (BMI) less than $18.5kg/m^2$, total lymphocyte count less than 900 and age over 65 were determined as the best set of NSI. By using best nutritional predictors receiver operating characteristic curve with the area under the curve, sensitivity and 1-specificity were analyzed to determine the best optimal cut-off point to decide normal or abnormal in nutritional status. Therefore simple and beneficial NSI was developed for identifying patients with severe malnutrition. Using NSI, nutritional information of the severe malnutrition patient should be shared with physicians and they should be cared for by clinical dietitians to improve their nutritional status.

고유수용성 신경근 촉진법의 하지패턴이 퇴행성 슬관절염 환자의 근기능에 미치는 영향 (Effect on Muscle Questionnaire of Knee Osteoarthritis with Lower Extremity Patterns of the Proprioceptive Neuromuscular Facilitation)

  • Purpose : this study was to investigate the influence on lower extremity pattern of PNF to muscle questionnaire ability of patients with knee OA. Methods : The subjects were consisted of 60 women patients with knee OA. And all subjects were randomly devide to two group which was modality group and PNF group. Each group had a treatment for 30 minutes per day and three times a week during 8 weeks period. VAS was used to measure pain, patient specific functional scale(PSFS) was used to measure patient's functional disability level, global perceived effect scale(GPES) was used to measure recovery or worse of patient's condition, muscle assessment questionnaire (MAQ) was used to measure patient's pain level. Results : 1. VAS was showed that PNF group had more significantly decreased than modality group(p<.05). 2. PSFS was showed that PNF group had more significantly increased than modality group(p<.05). 3. GPES was showed that PNF group had more significantly decreased than modality group(p<.05). 4. MAQ was showed that PNF group had more significantly decreased than modality group(p<.05). Conclusion : There are strong significant between PNF group and modality group.

양무지 안추 신전법을 이용한 악관절장애로 인한 개구장애 환자 치험 3례 (Case Report of Temporomandibular Joint Disorder Patient by "Yangmuji Anchu Traction Technique")

  • Objectives : This study was carried out to investigate the effects of "Yangmuji Anchu Traction Technique" on Temporomandibular Joint Disorder Patient. Methods : The therapies were Chuna("Yangmuji Anchu Traction Technique"), acupuncture treatment, Carthami-Semen Herbal Acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring from a lower front teeth to upper front teeth in open mouth, measuring a gap of center line on front teeth in open mouth. And by measuring Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements : the length measuring from a lower front teeth to upper front teeth in open mouth and the gap of center line on front teeth in open mouth was improved in all cases. Also Visual Analog Scale(VAS) was improved in all cases. Conclusions : These results suggested that "Yangmuji Anchu Traction Technique" effected for Temporomandibular Joint Disorder Patient.

태음병 황달증으로 진단한 소음인 황달 환자 치험 1례 (A Case Study of a Soeumin Patient with Jaundice Diagnosed as Greater-Yin Accompanied Jaundice Pattern)

  • Objectives This case study is about a Soeumin patient with jaundice identified as Greater-Yin accompanied Jaundice pattern. In this study, we report significant improvements of anorexia, dyspepsia, fatigue and weight loss after treatment with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture.Methods The patient was identified as Greater-Yin accompanied Jaundice pattern and treated with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture. The Global Assessment Scale (GAS) was used to assess the improvements of anorexia, dyspepsia, fatigue and pelvic pain after the treatment. And the liver function test was performed to measure the overall function of the liver.Result and Conclusion The symptoms of anorexia, dyspepsia, fatigue and pelvic pain were improved from GAS 100 to GAS 0-10 for four months. And The liver function test showed improved the response to treatment. The levels of AST (aspartate aminotransferase)/ALT (alanine aminotransferase) decreased from 43/42 to 29/14 IU/L. The level of total bilirubin was decreased from 2.7 to 1.9 mg/dL. Furthermore total weight gain was 8 kg from starting weight of 36.5 kg for four months. In conclusion, this study shows that Sasang constitutional medicine can be effective and safe treatment for the Soeumin patient with jaundice.

도침을 이용한 안면부 위축성 흉터의 치험례 (A Clinical Report on Facial Atrophic Scars treatment with Acupotomy)

  • Objectives: The purpose of this study is to evaluate the clinical effectiveness of acupotomy in the treatment of various facial atrophic scars except acne scars. Methods: Four patients with facial atrophic scars were treated by superficial dermal undermining with acupotomy. They were treated six times, and evaluated by comparing before and after photos, PGA(Physician's Global assessment), SESES(The Stony Brook Scar Evaluation Score) and patient satisfaction. Results and Conclusions: After observing case photos, PGA, SBSES, patient's satisfaction, it can be considered that acupotomy is an effective treatment modality for facial atrophic scars and it has no significant side effects.

Therapeutic Evaluation of a Topical Unani Formulation, Tila-i Muhāsā in Buthūr Labaniyya (Acne Vulgaris): A Randomized, Controlled Clinical Study

  • Introduction: Buthūr Labaniyya (Acne vulgaris) is a multifactorial disorder of the pilosebaceous units characterized by non-inflammatory and inflammatory clinical lesions. Several Unani medications have been used for centuries to treat acne. Objectives: Evaluation of safety and efficacy of Tila-i Muhāsā in patients with acne vulgaris Materials and Methods: This clinical study was conducted in patients with acne vulgaris. Patients applied Tila-i Muhāsā or 5% Benzoyl Peroxide (BPO) once daily for 6 weeks. Assessment of efficacy was carried out by Global Acne Grading System (GAGS) and Patient Global Assessment (PGA) scores. In addition, overall severity of acne was evaluated on a 5-point Cook's acne grading scale using photographic standards. Assessment of safety was performed through adverse drug reactions, local dermal tolerability, vital signs, and routine laboratory investigations. Results: A total of 60 patients (30 in each group) completed 6 weeks of treatment. The mean percentage reduction in GAGS score at 6 weeks from baseline in Unani group (66.97%) and BPO group (59.09%) was statistically significant (P<0.0001). After 6 weeks of therapy, the mean percentage reduction in PGA score compared to baseline in Unani group (57.44%) and BPO group (50.23%) was statistically significant (P<0.0001). No serious adverse events were reported in both groups; however, mild adverse events occurred more frequently in BPO group (30%) compared to Unani group (10%). Conclusion: Tila-i Muhāsā was found to be effective and safe in the treatment of acne vulgaris. However, further clinical studies with larger sample size and longer duration of therapy need to be conducted.

Acupuncture Treatment for Shoulder Impingement Syndrome: A Review of Randomized Controlled Trials

  • To evaluate the effectiveness of acupuncture treatment for shoulder impingement syndrome (SIS) a literature review was conducted of randomized controlled trials (RCTs) where acupuncture was used as an intervention for patients diagnosed with SIS. Relevant clinical studies (N = 181) were retrieved from several databases based on the inclusion/exclusion criteria, and the interventions and results were analyzed. Six RCTs were selected to review based on the inclusion and exclusion criteria. In all 6 studies, the acupuncture treatment group showed significant positive changes in indicators evaluating pain, disability, and quality of life. A significant decrease in the evaluation indicators (Shoulder Pain and Disability Index, numeric rating scale, Visual Analogue Scale, Constant Murley Shoulder Assessment Score, patient's global assessment, and doctor's global assessment) and a significant increase in the questionnaire scores (UCLA, AL-score, EuroQol 5 Dimension Self-Report Questionnaire, and disabilities of the arm, shoulder, and hand) were observed. In addition, 1 study showed similar improvements in pain and quality of life measures in the acupuncture group and corticosteroid injection group. No major side effects were reported. Acupuncture may be an effective and safe treatment for SIS however, further RCTs are required.

추나요법을 이용한 악관절장애로 인한 전신증상을 동반한 하악관절편위환자 치험 1례 (Case Report of Temporomandibular Joint Disorder Patient (Functional Lateral Mandibular Displacement) with Many Clinical Symptoms Treated by Chuna & General Oriental Method)

  • Objectives : This study was carried out to investigate the effects of Chuna & General Oriental Method on Temporomandibular Joint Disorder Patient(Functional Lateral Mandibular Displacement) with Many Clinical Symptoms. Methods : The therapies were Chuna, acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring the length from the end of frenulum labii superioris to the frenulum labii inferioris, Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements: the length from the end of frenulum labii superioris to the frenulum labii inferioris changed from 10-7mm to 0-2mm, the VAS was changed from 10 to 0-2 and many clinial symptoms improved. Conclusions : These results suggusted that Chuna & General Oriental Method effected for Temporomandibular joint disorders caused many clinical symptoms.
