The purpose of this study was to investigate the compliance and need for diet therapy among diabetics and their caregivers. A total of 625 respondents participated in this survey. Appropriate management for long-term blood glucose control and difficult-to-manage cases appeared to be diet therapy, physical activity, medical treatment and folk remedies, in that order. The most important educational avenues for diet therapy were books, magazines and TV, Most respondents were educated in diet therapy within a year after diagnosis and had practiced diet therapy for over four years. The most common diet therapy Practices were food selection and Quantity control. Of the specific problems in practicing diet therapy, time and labor, taste, and meal planning appeared to be equally difficult factors. The prevailing diet therapy skills were controlling food amounts at breakfast and supper, and selecting food for lunch and refreshments. Respondents practicing both skills were about 20% of the participants; this shows the need to improve the current situation. The nutrients most frequently considered in diet therapy practice were simple sugars (sugar and sweet goods) , total fats and cholesterol. Since the perception of the role of dietary fiber among respondents was very low, nutrition education about the importance of dietary fiber and food sources of dietary fiber was needed. Diabetics seemed to be concerned about diabetic menus. Three difficult problems in planning diabetic menus were daily menu planning, patient's preferences, and the calculation of calories. As a means of nutrition counseling, they preferred interviews. The higher the educational status and the lower the age, the higher the preference was for internet or PC counseling. Therefore, it is necessary to develop nutrition educational programs so that diabetics can obtain practical knowledge of diet therapy. Furthermore, it is also necessary to develop additional means of informing diabetics about menu planning, while still considering Korean dietary behavior.

Purpose: The aim of this study was to evaluate the trend of non-compliance with treatment (NCT) among gastric cancer patients in the Korean population. Materials and Methods: Using data from the Korea Central Cancer Registry from 1999 to 2015, patients who did not receive any treatment for gastric cancer within 4 months after diagnosis were defined as the NCT group. The annual incidence rate, distributions according to age group and stage, and 5-year relative survival of the patients exhibiting NCT were analyzed. Results: The number of NCT patients was 5,871 (30.6%) in 1999 and continuously decreased to 4,434 (15.3%) in 2015. Between 2006 and 2015, the proportions of NCT patients decreased from 72.9% to 55.0% among those 80 years old or older and from 9.2% to 5.4% among patients younger than 40 years. In patients with distant metastases, this proportion decreased from 35.5% to 32.7%, and this proportion also decreased from 17.6% to 8.2% among those with localized disease. The 5-year relative survival rates of NCT patients between 2011 and 2015 were significantly lower than those of the treated patients in each stage (60.2% vs. 99.7%, 13.8% vs. 67.1%, and 2.0% vs. 8.3% among those with localized, regional, and distant disease, respectively). Conclusions: The proportion of NCT gastric cancer patients has decreased during the last 16 years. However, considerable numbers of elderly patients are still NCT. There must be a strategy to decrease NCT and improve the nationwide survival rate of patients with gastric cancer.
Purpose: This study was conducted to examine the effect of education on infection control for multidrug resistant organism (MDRO). Methods: One group pre-post time series design was used. Infection control education for MDRO infection was provided to nurses working in the Neonatal Intensive Care Unit (NICU). Knowledge and recognition were evaluated before and after education. Hand hygiene compliance, MDRO isolation rate and central line-associated bloodstream infection (CLABSI) rate were used as outcome variables. Data from 45 nurses was used for analysis. Results: General knowledge about MDRO increased (p=.011). Responses to questions about image of MDRO and MDRO management tended to change in a positive direction (p=.046). Hand hygiene compliance was 100% at pre-test, 79.5% during education period and 98.4% at post-education period. MDRO isolation rate was 6.83 per 1,000 patient days at pre-test, 10.24 during education period and 6.68 at post-education period. CLABSI rate was 3.76 per 1,000 central line days at pre-test, 6.84 during education period and 4.71 at post-education period. Conclusion: Findings indicate that the education program is effective in improving knowledge about MDRO in NICU nurses. However, more reliable indicators should be used to determine long-term effects.
Purpose: The aims of this study were to (1) summarize the HF self-management interventions and (2) identify gaps and priorities for further investigation. Methods: A computer search of the literature over the past decade yielded 9 HF self-management studies. Six studies used non-experimental design; only 3 (33%) studies used quasi-experimental design which were included in this review. Results: The three pivotal HF self-management interventions studies were important because they highlight the positive effects of self-management compliance and other quality of life outcomes as well as discussing various issues. The informational booklet and telephone follow-up were the most common modalities in these interventions. The periodic telephone counseling offered in 2 studies effectively facilitated patient self-management compliance. There were some methodological flaws such as small sample sizes (range 8~21 in each group) and lack of experimental designs, long-term follow-up, and random group assignment. Additionally, the use of valid, reliable outcome measures is necessary to compare the effects of the interventions worldwide. Conclusion: More clinical evidence on HF self-management is needed using a larger sample size and the efficacy needs to be tested for various outcomes, including morbidity, mortality, and health care cost.
Purpose: This study aimed to investigate intensive care unit (ICU) nurses' actual and perceived glove uses for preventing healthcare associated infection. Methods: We observed nurses' donning glove by occasions and adherence to guideline for glove uses in four ICU in a single hospital. Total of 378 cases were observed from August 16 through October 6, 2020. Sixty one nurses of 66 nurses observed responded to a self-reported questionnaire about perceived glove use and knowledge of glove use. Data analysis was performed using descriptive statistics. Results: The number of observed episodes for glove use was 277. Although the highest numbers of occasions of wearing gloves was contact precaution, the compliance rate was 72.1%. The rates of donning gloves were low in the insertion and removal of peripheral venous catheters, blood glucose testing and blood sampling, which were at risk for exposure to blood. We observed misuse of wearing gloves even when they were not required. Results showed that the majority of non-compliance with glove use were a failure of performing hand hygiene before and after glove use and a failure of changing gloves between procedures on the same patient. The participant's knowledge of glove use was high. Conclusion: Based on the results, it is necessary to provide ICU nurses with education and reinforcement of proper glove uses for infection control.
Purpose: The purpose of the study was to examine the level and correlation of compliance and barriers to self-care behavior in patients with type 2 diabetes. Methods: The participants for this study were 122 patients with type 2 diabetes enrolled in the out-patient clinic of C national general hospital. Data collection was done by self-report questionnaires. Results: The mean score of the patients with type 2 diabetes for self-care behavior was higher than average level. Level of barriers to exercise was also higher than average, and higher than other barriers of diet and medication; There were low but significant correlations between self-care behavior and barriers to diet, exercise and medication; Variance in barriers to exercise, using alcohol, regular hospital visits, medical treatment and good foot care explained 29.6% of the variance in self-care behavior. Conclusion: According to the study, the self-care behavior of medication and barriers to exercise were the highest. Barriers to exercise highly influenced self-care behavior. Barriers to diet and exercise were significantly correlated the self-care behavior. Further study or interventions are necessary to decrease barriers in patients with type 2 diabetes.

Objectives : This report intended to estimate effects taken by using Dong-Qi Acupuncture Therapy on a patient with neurogenic bladder induced by transverse myelitis(712, L1 level). Methods : From 6th May, 2005 to 30th June, 2005. The patient received Dong-Qi Acupuncture Therapy. Results : 1. Since being hospitalized, the patient's voiding has improved from intermittent catheterization to independent urination state. And the volume of residual urine has decreased from 150cc-350cc to 0cc-l0cc. 2. The urodynamic findings showed that first sensation, maximal detrusor pressure, compliance and maximal bladder capacity changed from 456ml to 195mL, from 29cmH2O to 31cmH2O, from none to 25.31mL/cmH2O, from under 650mL to 590mL, respectively. Conclusion : We considered Dong-Qi Acupuncture Therapy had a useful effect on Neurogenic Bladder.
Since noncommunicable diseases (NCDs) are generally controllable rather than curable, more emphasis is placed on prevention than on treatment. For the early detection of diseases, primary care physicians (PCPs), as well as general practitioners and family physicians, should interpret screening results accurately and provide screenees with appropriate information about prevention and treatment, including potential harms. The concept of quaternary prevention (QP), which was introduced by Jamoulle and Roland in 1995, has been applied to screening results. This article summarizes situations that PCPs encounter during screening tests according to the concept of QP, and suggests measures to face such situations. It is suggested that screening tests be customized to fit individual characteristics instead of being performed based on general guidelines. Since screening tests should not be carried out in some circumstances, further studies based on the concept of prevention levels proposed by Jamoulle and Roland are required for the development of strategies to prevent NCDs, including cancers. Thus, applying the concept of QP helps PCPs gain better insights into screening tests aimed at preventing NCDs and also helps improve the doctor-patient relationship by helping screenees understand medical uncertainties.
Generalized severely worn dentition causes occlusal disharmony, esthetic problems, and temporomandibular joint disorders. In order to solve these problems, it is necessary to make a precise analysis of vertical dimension and treatment plans considering it. This case report demonstrates the complete mouth rehabilitation of a 58-year-old male patient with a lot of worn teeth by increasing vertical dimension. Provisional restorations were cemented and after 4 months of evaluation for patient's compliance, permanent prostheses were fabricated. With these treatments, functionally and esthetically satisfactory results were obtained.
Purpose: Outbreaks resulting from medication injections have recently been on the rise in Korea despite various established guidelines. The objective of this study was to assess the degree to which healthcare professionals are aware of safe injection practice guidelines and to account for the adherence to and the deviation from safe injection guidelines formulated by healthcare providers. Methods: In November 2016, a cross-sectional anonymous questionnaire covering general characteristics of injections, patient safety culture, awareness of safe injection practices, and adherence to and barriers to safe injection guidelines was issued to healthcare providers who administer medication injections or manage and supervise these injections (N=550). Multivariate logistic regression analysis via enter method was performed to define the influencing factors of adherence of safe injection practices. Results: On average, respondents adhere to 17 of the 24 guidelines. Multivariate logistic regression found that those who were more likely to adhere to safe injection guidelines either underwent a patient safety training experience within the last year, provided care in a setting characterized by a highly developed patient safety culture, or were employed as physicians or nurses, as opposed to some other type of care provider. Barriers to safe injection guidelines were attributable to; thoughts of waste to discard leftover medicine, provisions that made adherence cumbersome, a weak culture of compliance, and insufficient amounts of injectable medicine, products, and education. Conclusions: The results of this study indicate that controllable factors like training experience of healthcare providers and patient safety culture were positively associated with adherence to safe injection practices. It was suggested that the training of healthcare providers on safe injection practices be a continuous process to promote patient safety. Additionally, there should be an increased focus on developing and implementing policies to improve patient safety culture from a prevention rather than post-management perspective.
