Purpose: Closed reduction and nasal packings for 3-7 days is usual procedures in managing nasal bone fracture. Most patients experience several discomfort owing to lack of nasal breathing. There are many reports concerning how to reduce patient's discomforts or complications. But it is obvious that the duration of nasal packing is 3-7 days. The aim of this study is evaluate the effect of half day nasal packing in results of nasal bone fracture. Methods: The study was performed on 97 nasal bone fracture patients who had simple nasal bone fractures from January to June 2012. The incidence, cause, patient's discomfort and satisfaction with half day nasal packing are analyzed according to patient's medical records and questionnaire at each nasal packing removal, postoperative 4 weeks. Results: Young male patients, especially the teenagers and the twenties were the common age group, and physical violence was most common cause of injury. A total of 78 out of 97 patients were male. In 92 cases, closed reduction were applied. Approximately, 87% of the patients were satisfied with the outcomes. Conclusion: Half day nasal packing is considered as an effective method to minimize patient's discomfort owing to prolonged absence of nasal breathing with maintenance of stability.
Objectives : The purpose of this case is to report the improvement of a patient with lymphedema due to axillary lymph node dissection after acupuncture therapy and herbal medicine. Methods : We treated the patient with Dong-Qi acupuncture, herbal medication(木通大安湯), lymph absorption massage, compresses, abdominal breathing. We measured the circumference of the patient's elbow and wrist every day at 6pm. Results : Since being hospitalized, the circumference of the patient' elbow and wrist has been decreased and VAS score has gotten lower gradually. Conclusions : We considered Dong-Qi acupuncture, herbal medication had a useful effect on lymphedema due to axillary lymph node dissection with lymph absorption massage, compresses, abdominal breathing.
The review of the literature determines that large osteomas of the mandible are relatively rare. We present a case of a 60-year-old man with painless swelling of the left submandibular area and mild difficulty in breathing. The patient complained that the mass have been progressing slowly for at least 20 years. Radiographic studies showed a giant osteoma that attached to the lingual surface of the left mandibular angle and extended from submandibular space to infratemporal space. After the excision of the tumor the patient made a full recovery.
This study was to exhibit the effective emergency care method for the drowning and non-drowning who are reached two-thousand peoples every year in our country. For investigate the effective emergency care, this study was discussed as follows ; Pathophysiology of the water submersion, Fresh-water & sea-water drowning, Factors affecting survival, and Prehospital management. The conclusions from this study were summarized as follows; 1. Remove the patient from the water. If you suspect neck or spinal injuries, Always support the head and neck level with the back and, begin rescue breathing. 2. Maintain the airway and support ventilation in the water use the jaw-thrust technique to avoid farther injury to the neck or spine. We might encounter more resistance to ventilations than you expect because of water in the airway. Once you have determined that there are no foreign objects in the airway, apply ventilations with more force; adjust ventilations until you see the patient's chest rise and fall but not until you see gastric distention. Do not attempt to remove water from the patient's lungs or stomach. 3. If there is no pulse, begin CPR. 4. Administer high-flow supplemental oxygen; suction as needed. 5. Once the patient is breathing and has a pulse, assess for hemorrhage; control any serious bleeding that you find. 6. Cover the patient to conserve body heat, Handle the patient very gently, and, Transport the patient as quickly as possible to Emergency Department, Continuing resuscitative measures during transport. If the patient have the hypothermia, follow hypothermia management.
Abdominal circumference changes due to breathing by the respiratory muscle activity such as diaphragm, which would partially represent the lung volume variation. The present study introduced conductive rubber molded in a cord shape incorporated with a patient's pants. The conductive rubber cord operated as a displacement transducer to measure the lung or abdominal volume changes. Signal extraction circuitry was developed to obtain the volume and its derivative(or the flow) signals followed by wireless transmission based on the Zigbee communication protocol in a size of $65mm{\times}105mm$ easily put in pocket. Breathing frequency was accurately evaluated and breath pattern analysis seemed feasible, since respiratory behaviours such as maximal inspiration and cough were well identified. Remote wireless receiver module also enabled to monitor both volume and flow signals during resting breathing on a PC terminal.
For the environmental safety of the operating room and patient healthcare, the closed type rebreathing system is widely adopted. In order to reduce the anesthesia gas during surgery, the mixing ratio of anesthesia gas with breathing air should be precisely controlled. Generally, the breathing air passes through the vaporizer to mix the anesthesia gas, but there is a difficulty in controlling the mixing ratio precisely. In this paper, the stand-alone style vaporizer is designed and the operating characteristics are investigated. The vaporizer measures the temperature and pressure in the vaporizing chamber and chamber temperature is precisely controlled by proportional controlled heater. Exact quantity of anesthesia media is feeded by PID controlled peristaltic pump and vaporized gas is mixed with breathing air flow by PWM controlled solenoid valve. The experimental result shows that the vaporizer has an excellent command following performances that it can be applied to the low flow anesthesia system.
호흡은 환자의 상태를 판단하는 중요한 생체 신호 중 하나이다. 특히 진정 마취 시 환자의 무호흡 및 저호흡은 지속적인 모니터링 없이는 탐지하기가 어렵기 때문에 환자의 호흡 상태를 정확하고 간편하게 판단할 수 있는 지속적인 호흡 모니터링 방법이 필요하다. 현재 호흡 상태의 모니터링을 위한 다양한 장치들이 사용되고 있으나 임상 사용단계에서 응답 시간이 느릴 뿐 아니라 사용에 불편한 단점을 안고 있다. 본 연구에서는 이러한 문제점을 해결하고자 PVDF(polyvinylidene fluoride) 필름을 이용한 부착형 센서와 회로를 설계하고 제작하였으며, 진정 마취 시 호흡 신호를 감지하여 이상 호흡 징후를 조기에 발견할 수 있는 알고리즘을 포함하는 모니터링 시스템을 개발하였다. 본 연구의 결과는 진정 마취 시 뿐만 아니라 수면 관련 호흡 상태의 원격진료를 통한 다양한 의료산업 분야에 적용될 수 있을 것이다.
Objectives: This case report intended to examine how herbal medicine treatment for breathingrelated sleep disorders applied with polysomnography affected the structure of sleep. Methods: From January to November 2019, we retrospectively reviewed the progress records and polysomnography reports of patients who visited Korean medical hospitals in Daejeon and Cheonan. Results: In case 1, the patient complained of daytime sleepiness and snoring and was diagnosed with obstructive sleep apnea disorder and treated with Ukgan-san-gami. There were significant changes in the polysomnographic parameters, such as sleep efficiency, waking after sleep onset, sleep latency, the total arousal index, and the apnea-hypopnea index after herbal treatment. In case 2, the patient complained of frequent waking after sleep onset, daytime sleepiness, and snoring, and was diagnosed with obstructive sleep apnea disorder and treated with Yeoldahanso-tang-gami. There were significant changes in the polysomnographic parameters, such as the total arousal index and the apnea-hypopnea index after herbal treatment. In case 3, the patient complained of daytime sleepiness and was diagnosed with obstructive sleep apnea disorder and treated with Bangpungtongsung-san. There was no significant change in the polysomnographic parameters after herbal treatment. Conclusions: It is meaningful to note that objectively identifying the outcome of treatment for breathing-related sleep disorders is necessary in the clinical practice of Korean medicine.
호흡 연동 방사선 치료 시 사용되는 음성 유도 및 음성-영상 유도의 두 가지 호흡생체자기제어 방식이 호흡 재현성 및 선량에 미치는 영향을 평가하였다. 본 연구에서는 Medical College of Virginia (MCV)에서 획득한 19명의 폐암 환자에 대한 호흡 데이터를 이용하였다. 호흡 데이터는 총 5주간 1주 간격으로 자유 호흡, 음성 유도, 음성-영상 유도의 세 가지 형태로 획득하였으며 선량 평가는 MATLAB을 이용하였다. 그 결과, 자유 호흡의 경우에는 반복되는 치료에서 호흡재현성이 감소하여 CTV 선량이 약 30.0% 감소하는 것을 알 수 있었으며, 음성-영상 유도 방식을 이용할 경우 5주 후 CTV 선량이 20.0% 개선됨을 알 수 있었다. 이는 환자가 영상을 통하여 자가호흡조절 능력이 향상되기 때문으로 판단된다. 또한 음성 유도만 사용할 경우에도 호흡재현성을 유지하는 데는 효과적임을 확인하였다.
Purpose: The purpose of this study was to identify the effects of the heated-humidified breathing circuit applied on the body temperature, shivering, and thermal comfort of general anesthesia patients. Methods: The participants were patients who received general anesthesia at University Hospital K located in City B, with 25 patients in the experimental group and 25 patients in the control group. The period of this study was from Mar 19 to Apr 26, 2019. The experimental equipment included a heated-humidified breathing circuit, which connects the intubation tube with the anesthesia machine for mechanical ventilation after airway intubation in general anesthesia patients. Results: The body temperature, shivering, and thermal comfort after surgery were significantly different between the two groups (p<.001). However, the body temperature during surgery was slightly different between the experimental group and the control group. Conclusion: A heated-humidified breathing circuit may be actively used in a warming method to prevent the hypothermia of general anesthesia patients.
