Sagittal spinal balance means standing postural balance at sagittal plane. Postural imbalance with displacement of the patient's center of gravity can cause chronic back pain and ambulatory difficulty. The sagittal spinal balance is determined based on the deviation of the C7 plumb line, originating at the middle of the C7 vertebral body, from the posterior superior endplate of S1. The line is called as sagittal vertical axis (SVA). In the traditional Korean medicine, the meridian sinews, which are the most superficial pathways of the meridian system, associated with movement, muscle balance and defense. They too are separate from the main meridians, though they intersect the main meridians. Some creative and pioneer researchers in Korea thought that the anatomy trains, which suggested by Myers is a concept familiar to the meridian sinews. A reciprocal relationship between the superficial back line and the superficial front line used to be compared to the rigging of a sailboat. Therefore, We suggest that spine may be compared to a mast of the sailboat and that the sagittal spinal balance can be maintained with systemic reciprocal interacts between the front line muscles and the back.
Magnetic Resonance Image represents three-dimensional diagnostic imaging technique using both nuclear magnetic resonance phenomenon and computer. Compared with computed tomography (CT), MRI have advantages harmless to patient's body, three-dimensional image with high resolution and disadvantages long data acquisition time because of long T1 relaxation time, relatively low signal to noise ratio, high cost of setting, also. As physiologic motion of tissue results in motion ghost in MRI, high 2.0Tesla make improve low signal to noise ratio. This study have aim to improve image quality with controling motion ghost of tissue. Supposing a moving pixel in constant frequency, one pixel make two ghosts which are same size and different anti-phase. So, this study will show adjust parameter on locational control of motion ghost. Author made moving phantom replaced by respiratory movement of human, researched change of motion frequency, FOV by location shift, and them decided optimal FOV (field of view). The results are as follows: 1. The frequency content of the motion determines how far the image always appear in phase-encoding direction, the morphology of the ghost image is characteristic of the direction of the motion and its amplitude. 2. Double FOV of fixed signal object for locational control of motion ghost is recommended. Decreasement of spatial resolution by increasing FOV can compensate on increasing of matrix in spite of scan time increasement.
Purpose: The purpose of this study was to investigate the effect of early ankle exercise with functional electrical stimulation(FES) on spasticity, strength and active range of motion of ankle in patients with stroke. Methods: This study included 21 patients with stroke, who were performed early ankle exercise combined FES. The exercise program comprised 5 sessions of 30 minutes per week for 4 weeks. The spasticity, strength and active range of motion of ankle were evaluated before and after training. The spasticity was measured by modified ashworth scale(MAS), strength of ankle was measured by hand-held dynamometer and active range of motion of ankle dorsi-flexion was measured by goniometer. All data were analyzed using SPSS 18.0. Results: Significant differences were observed the subjects for strength of ankle and active range of motion. The results of the study were as follow: strength of ankle was significantly increased subjects(p<.001) and active range of motion was significantly increased subjects(p<.001). Conclusion: Ankle is very important part of body in stroke patients. early ankle exercise with FES is effective for improvement of strength of ankle and active range of motion in patients with stroke. ealry ankle exercise with FES about stroke patient is very useful and effective. It is effective in clinical practice.
Purpose : The purpose of this study was to record, analyze and describe in detail, content of PNF intervention for recovery of function after stroke using SPIRIT intervention recording tools. Methods : A convenience sample of 21 physical therapist from a rehabilitation hospital recorded the intervention for 69 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the intervention. Results : A convenience sample of 21 physical therapist from a rehabilitation recorded the interventions for 64 patients with stroke who treated during 30 minutes. Descriptive statistics assessed for describe the interventions accurately and investigate the difference according to time since stroke. Conclusion : Facilitation techniques were the most frequently used intervention(n=1226,35.6%), practice(n=967, 28.0%) and exercise(n-670, 19.4%) in clinical practice of physical therapist. In contrast, Teaching careers and health care professionals to do or assist category is not frequently used intervention(n=5, 0.1%). Therefore, We need to be careful again with category of teaching careers and health care professionals.

Brain disorder disability is assessed when organic brain lesion such as cerebral palsy, traumatic brain injury, or stroke causes physical impairment which limits daily activites substantially according to its level and range of paralysis or the presence of involuntary movement. According to the disabled person welfare law in Korea, grade I brain disorder is assessed when one is in continuous irreversible state of coma without the ability to prolong one's life without other's help. Self-injurious behavior is defined as the non-suicidal intentional or unintentional injury to one's own body which can occur repetitively temporarily or chronically. People with decreased consciousness often exhibit increased self-injurious behavior which is most commonly associated with tongue or lip biting. This report documents a successful self -injurious lip biting treatment of a brain lesion patient within a short time by applying a removable acrylic resin appliance including posterior bite block.
Tremor is a rhythmic, involuntary muscular contraction characterized by oscillations of a part of the body and is the most common movement disorder. Clinically, tremor isclassified into postural tremor, resting tremor, action tremor, or other kinds of tremor, and treated according to the causes. The appropriate treatment of tremor depends on accurate diagnosis of its cause. We have recently examined a number of tremor-patients diagnosed as cerebellar infarction. Based on traditional Korean medicine curative way, we diagnosed this patient as deficiency of Qi and blood(氣血兩虛). On this account, suitable treatment had been provided. Our results show the clinical symptoms had improved through treatment with herbal medicine (Palmultang-gamibang) and acupuncture, so we report it for better treatment.

Objective: There are currently no disease-modifying medications or definite long-term sustainable interventions for patients with Parkinson's disease (PD), indicating an unmet treatment need. Our goal was to create a long-term sustainable intervention for PD patients that can be used in Korean medicine clinics. Methods: The Meridian Activation Remedy System (MARS) was created to stimulate a patient's 12 meridians and sinew channels using a combination of acupoint stimulation and exercise. The acupoints and motions used in MARS were selected through literature studies and expert advice. The methodologies were refined using observational and case studies. With slow and fast movements, the MARS intervention was intended to activate both slow- and fast-twitch muscle fibers. Intradermal acupuncture and motion that shift the center of gravity were employed to enhance the patient's balance and proprioception. In addition, the intervention included alternating movement exercises to address the complex cognitive decline commonly occurring in PD patients. Results: The following acupoints were chosen for the MARS intervention: bilateral Hegu (LI4), Houxi (SI3), Waiguan (TE5), Neiguan (PC6), Zhongchong (PC9), Yuji (LU10), Zusanli (ST36), Yanglingquan (GB34), Taichong (LR3), Kunlun (BL60), and Taixi (KI3). We also developed actions that can stimulate the body's 12 meridians. Conclusion: We developed the MARS intervention, which combines acupuncture and exercise, to address the unmet therapeutic needs of PD patients. We hope that with additional research, the MARS intervention can be set as an effective therapeutic program for PD patients.

There are not a few cases in Oriental medicine where a different prescription should be given to some of the patients whose symptoms are similar to each other. In other words, there might happen a misdiagnosis due to an extremely delicate difference in symptoms of the same diseases, causing a patient's condition to get worse or to be even on the brink of death. In such a context, the records in "WooJam JabJeo(愚岑雜著)" are worthwhile to do in-depth research on. Jang-Taegyung described his first-hand experience in major medical treatment very vivaciously during his ages between 25 and 42. Particularly, most of the prescriptions recorded in this book include not only his empirical prescriptions on the patients who life was almost on the brink of death but it also so plentifully contained the process of overcoming the side effect caused by other clinics' misdiagnosis; thus, that this book is drawing attention in that it could be indispensable materials not only in the research on medical history but also for clinical treatment. Particularly, as regards two cases of insanity, this book mentions the reason for using totally different prescription on one case from the other case even though the two cases had a similar symptom, through which we can acquire somewhat concrete experience in curing scenes though indirectly during the period of the Joseon Dynasty. We were able to get the result from the analysis of the two cases of insanity as follows: 1. WooJam, in treating the two cases of insanity due to the severe exacerbation of yang energy, managed to treat the one case by inducing a bowel movement and the other case by inducing urination. Such a different treatment seems to be greatly attributable to the constitutional factor of the two patients. Such an Oriental-medicine-based method of diagnosis and prescription as WooJam's is rarely found in Western medicine-i.e., that's why more thorough research on Oriental medicine is deeply required. 2. In case of the second patient, the patient arrived at insanity due to another clinic's treatment on perspiration on the patient with severe mouth thirst. This medical story once again highlights the importance of a diagnosis in today's Oriental medicine and at the same time it's a good example showing how big the side effect caused by the abuse of medicinal herbs is. The second patient's body fluids ran dry and finally his yang energy got exacerbated all the more due to the treatment by perspiration.
Background: We assessed the relationship between patient age and remifentanil dosing rate in patients managed under general anesthesia with spontaneous breathing using low-dose remifentanil in sevoflurane. Methods: The participants were patients with an American Society of Anesthesiologists Physical Status of 1 or 2 maintained under general anesthesia with low-dose remifentanil in 1.5-2.0% sevoflurane. The infusion rate of remifentanil was adjusted so that the spontaneous respiratory rate was half the rate prior to the induction of anesthesia, and γH (㎍/kg/min) was defined as the infusion rate of remifentanil under stable conditions where the respiratory rate was half the rate prior to the induction of anesthesia for ≥ 15 minutes. The relationship between γH and patient age was analyzed statistically by Spearman's correlation analysis. Results: During dental treatment under general anesthesia using low-dose remifentanil in sevoflurane, a significant correlation was detected between γH and patient age. The regression line of y = - 0.00079 x + 0.066 (y-axis; γH, x-axis; patient's age) was provided. The values of γH provide 0.064 ㎍/kg/min at 2 years and 0.0186 ㎍/kg/min at 60 years. Therefore, as age increases, the dosing rate exhibits a declining trend. Furthermore, in the dosing rate of remifentanil when the patient's respiratory rate was reduced by half from the preanesthetic respiratory rate, the dosing rate provided was around 0.88 mL/h in all ages if the remifentanil was diluted as 0.1 mg/mL. EtCO2 showed 51.0 ± 5.7 mmHg, and SpO2 was controlled within the normal range by this method. In addition, all dental treatments were performed without major problems, such as awakening and body movement during general anesthesia, and the post-anesthetic recovery process was stable. Conclusion: General anesthesia with spontaneous breathing provides various advantages, and the present method is appropriate for minimally invasive procedures.
In motor learning, the relative frequency of external feedback is the proportion of external feedback presentations divided by the total number of practice trials. In earlier studies, increasing the percentage of body weight loading on the affected leg of hemiplegic patients, external feedback was continuously produced as the patient attempted to perform a movement. This feedback was produced to enhance the learning effect. However, recent studies in nondisabled populations have suggested that compared with 100% relative frequency conditions, practice with lower relative frequencies is more effective. My study compared the effect of 100% relative frequency conditions with 67% relative frequency conditions to determine what effect they exerted on motor learning for increasing the percentage of body weight loading on the affected lower limbs of patients with hemiplegia. Twenty-four hemiplegic patients were randomly assigned to one of two experimental groups. Each group practiced weight transfer motor learning on a machine. During practice, visual feedback was offered to all subjects. The experiment was carried out with full visual feedback for patients in group one but only 67% visual feedback for patients in group two. The percentage of loading on the affected leg was recorded four times: before learning (baseline value), immediately after learning, 30 minutes after learning, 24 hours after learning. The results were as follows: 1. In the 100% visual feedback group, the percentage of loading on the affected leg increased significantly in all three testing modes over the baseline value. 2. In the 67% visual feedback group, the percentage of loading on the affected leg increased significantly in all three measurements. 3. Immediately after learning, the learning effect was not significantly different between the two groups, but was significantly greater after both the 30 minutes delay and the 24 hours period. These results suggest that the 33% reduction in the provision of visual feedback may enhance the learning effect of increasing the percentage of body weight loading on the affected leg in patients with hemiplegia.
