Mitral Valve Repair for Mitral Regurgitation (승모판막폐쇄부전에 대한 승모판막재건술)

  • From February 1996 to May 1997, 18 patients underwent mitral valve repair for mitral regurgitation. There were 9 male and 9 female patients aged from 19 to 68 years(mean, 53). Thirteen patients were in New York Heart Association(NYHA) class III and IV. The cause of mitral regurgitation was degenerative in 12 patients, rheumatic in 5 patients and infective in 1 patient. Fifteen patients were in Carpentier's functional classification II, 2 patients in Carpentier's class III and 1 patient in Carpentier's class I. Surgical procedures included prosthetic ring annuloplasty(16 cases), rectangular resection of posterior leaflet(15 cases), chordal shortening(5 cases), triangular resection of anterior leaflet(2 cases), commissurotomy(2 cases), partial transposition of posterior leaflet(1 case). These procedures were combined in most patients. There was no operative death. These patients have been followed from 1 to 15 months, mean of 6.7 months. There was one late death resulted from low cardiac output following mitral valve replacement. The function of the repaired valve in other 17 patients has remained satisfactory during the observed interval. We consider that mitral valve repair is highly satisfactory in patients with mitral regurgitation.

Integrating UAV Remote Sensing with GIS for Predicting Rice Grain Protein

  • Purpose: Unmanned air vehicle (UAV) remote sensing was applied to test various vegetation indices and make prediction models of protein content of rice for monitoring grain quality and proper management practice. Methods: Image acquisition was carried out by using NIR (Green, Red, NIR), RGB and RE (Blue, Green, Red-edge) camera mounted on UAV. Sampling was done synchronously at the geo-referenced points and GPS locations were recorded. Paddy samples were air-dried to 15% moisture content, and then dehulled and milled to 92% milling yield and measured the protein content by near-infrared spectroscopy. Results: Artificial neural network showed the better performance with $R^2$ (coefficient of determination) of 0.740, NSE (Nash-Sutcliffe model efficiency coefficient) of 0.733 and RMSE (root mean square error) of 0.187% considering all 54 samples than the models developed by PR (polynomial regression), SLR (simple linear regression), and PLSR (partial least square regression). PLSR calibration models showed almost similar result with PR as 0.663 ($R^2$) and 0.169% (RMSE) for cloud-free samples and 0.491 ($R^2$) and 0.217% (RMSE) for cloud-shadowed samples. However, the validation models performed poorly. This study revealed that there is a highly significant correlation between NDVI (normalized difference vegetation index) and protein content in rice. For the cloud-free samples, the SLR models showed $R^2=0.553$ and RMSE = 0.210%, and for cloud-shadowed samples showed 0.479 as $R^2$ and 0.225% as RMSE respectively. Conclusion: There is a significant correlation between spectral bands and grain protein content. Artificial neural networks have the strong advantages to fit the nonlinear problem when a sigmoid activation function is used in the hidden layer. Quantitatively, the neural network model obtained a higher precision result with a mean absolute relative error (MARE) of 2.18% and root mean square error (RMSE) of 0.187%.

  • Human neutrophil elastases (HNElastase, EC 3.4.21.37), a causative factor of inflammatory diseases, are regulated by plasma proteinase inhibitors, alpha-proteinase inhibitor and ${\alpha}_2-macroglobulin$. Under certain pathological conditions, however, released enzymes or abnormal function of inhibitors may cause various inflammatory disease. NSAIDs have been clinically applied for treatment of inflammatory diseases. Inhibition of cyclooxygenase is a known mechanism of action of NSAIDs in the treatment of inflammatory disease. In in vitro experiments, HNElastase was inhibited by naproxen, phenylbutazone, and oxyphenbutazone, but ibuprofen, ketoprofen, aspirin, salicylic acid, and tolmetin did not inhibit elastase. HNElastase was also inhibited by chelating agents, EDTA & EGTA, and tetracyclines. Removal of divalent metal ions by EDTA caused inhibition of elastase, and reconstitution of the metal ions recovered the enzyme activity to a certain level. Frequencies and contours in the Raman spectra of various conditions of human neutrophil elastase undergo drastic changes upon partial removal and/or reconstitution of calcium and zinc ions. The metal ion content dependent activities and change of the contour of the Raman spectrogram suggest us that the mechanism of action of a chelator or chelator-like agents on neutrophil elastase may be related to the conformational change at/or near the active site, especially -C=O radical or -COOH radical.

The Therapeutic Effect of Neoadjuvant Chemotherapy in Locally Advanced Oral Cavity Cancer (국소 진행성 구강암에서 선행 항암 화학 요법의 효과)

  • Objective: The role of chemotherapy in locally advanced head and neck cancer has been established in nasopharynx and larynx as definitive therapy and organ preserving therapy, respectively. Oral cavity cancers are relatively uncommon and local recurrence is the main cause of treatment failure. We planned this retrospective study to evaluate the role of neoadjuvant chemotherapy in locally advanced oral cavity cancer patients. Materials and Methods: From 1988 March to 2001 February, locally advanced, previously untreated oral cavity cancer patients who received neoadjuvant chemotherapy were examined. Chemotherapy had been done in the following patients: Histologically proven squamous cell or poorly differentiated carcinoma, stage 3 or 4, and performance state 0-2 patients. Chemotherapy regimen consisted of cisplatin and infusional 5-fluorouracil. Response was evaluated after 2 cycles and in case of no response, definitive local therapy was done; otherwise 3 cycles was done before local treatment. Results: 48 patients were treated and 47 patients were evaluable for responses. Complete response rate was 6.4%(3/47) and partial response 80.0%(38/47), scoring overall response rate of 87.2%. Median time to progression was 27.0 months (95% CI : 0-58months) and overall 5 year survival was 54.8%. 5-year disease-free survival in the patients in remission after local treatment was 51.9%. In multivariate analysis, contributing factor to the survival were response to neoadjuvant chemotherapy and local treatment modalities. Extensive surgery was done in 10 patients and 25 patents (52.1%) was followed up with preserved function. With median follow-up of 57.0 months, 19 recurrences were detected, most of which were local or regional type. Conclusion: Neoadjuvant chemotherapy followed by local treatment in oral cavity cancer showed high response rate and was thought to be effective therapeutic approach especially in view of organ preservation.

Numerical Study on H2 Preferential Diffusion Effect in Downstream Interactions between Premixed H2-air and CO-air Flames (상호작용 하는 H2-공기/CO-공기 예혼합화염에 미치는 H2 선호 확산 영향에 대한 수치적 연구)

  • The effects of preferential diffusion of hydrogen in interacting counterflow $H_2$-air and CO-air premixed flames were investigated numerically. The global strain rate was varied in the range $30-5917s^{-1}$, where the upper bound of this range corresponds to the flame-stretch limit. Preferential diffusion of hydrogen was studied by comparing flame structures for a mixed average diffusivity with those where the diffusivities of H, $H_2$ and $N_2$ were assumed to be equal. Flame stability diagrams are presented, which show the mapping of the limits of the concentrations of $H_2$ and CO as a function of the strain rate. The main oxidation route for CO is $CO+O_2{\rightarrow}CO_2+O$, which is characterized by relatively slow chemical kinetics; however, a much faster route, namely $CO+OH{\rightarrow}CO_2+H$, can be significant, provided that hydrogen from the $H_2$-air flame is penetrated and then participates in the CO-oxidation. This modifies the flame characteristics in the downstream interaction between the $H_2$-air and CO-air flames, and can cause the interaction characteristics at the rich and lean extinction boundaries not to depend on the Lewis number of the deficient reactant, but rather to depend on chemical interaction between the two flames. Such anomalous behaviors include a partial opening of the upper lean extinction boundary in the interaction between a lean $H_2$-air flame and a lean CO-air flame, as well as the formation of two islands of flame sustainability in a partially premixed configuration with a rich $H_2$-air flame and a lean CO-air flame. At large strain rates, there are two islands where the flame can survive, depending on the nature of the interaction between the two flames. Furthermore, the preferential diffusion of hydrogen extends both the lean and the rich extinction boundaries.

  • This study examines the importance of location and transportation in the operation of small- and medium-sized hospitals in the rapidly changing clinical environment. Approximately 350 persons were surveyed for approximately 40 days from July-August 2017, and responses were analyzed using SPSS and AMOS Statistical Package for 335 parts excluding 8 parts of 343.The main research results were as follows: First, a hypothesis was adopted that the attractiveness of building materials will positively affect customer satisfaction. Second, the hypothesis that transport infrastructure has a significant impact on customer satisfaction was rejected. Third, the hypothesis that the building appeal will positively affect the site was supported. Fourth, the hypothesis that transport infrastructure will have a significant impact on location marketability was supported. Fifth, the hypothesis that location marketability will significantly impact customer satisfaction was supported. Sixth, the hypothesis that the relationship between the site and customer satisfaction with the relationship between the client and the client is explained is partly attributable to the fact that there is a partial function. The assumption that the relationship between traffic infrastructure and customer satisfaction was confirmed. The hypothesis that the relationship between traffic infrastructure and customer satisfaction was supported. This research focuses on small- and medium-sized hospitals located in Seoul, which does not include the recognition of patients' awareness of the location and transportation of small- and medium-sized hospitals. Moreover, this study has the limitation in analyzing patient satisfaction, rather than the objective data of the financial management performance of the hospital. The results of this study could provide the basis for selecting the future locations of health care facilities.

The Clinical Study of the Supracondylar Fracture of the Femur (대퇴골 과상부 골절 및 치료에 대한 임상적 고찰)

  • Recently, the Supracondyle fracture of the femur is so severe and comminuted that it is difficult to treat open reduction and rigid fixation sufficiently due to violent traffic accident and external forces. The associated injuries are accompanied variously. So complications of the supracondyle fractures including joint stiffness, delayed union, infection are serious. However development of internal implant and operation skill result in relatively satisfactory function than conservative management. The authors reviewed 20 cases of the supracondyle fracture of the femur treated at Yeungnam University Hospital from 1989, Dec. To 1992, Jan. The results were as follows : 1. The male was common(70%). The 2nd-4th decade was most(87%). 2. The most common cause of injuries was traffic accident. The rate of open fracture was 35%, most of it was traffic accident. 3. According to the ASIF group classification. 4 cases were type A, 4 in type B, 12 in type C (10 in type $C_2$&$C_3$). 4. Accompanied injuries included multiple fractures in 9 cases and femoral artery injury in 1 case. 5. Among the 20 cases, conservative treatment was done in 5 cases and operative treatment was done in 15 cases. 6. Functional results (by Schatzker criteria) were as follows. The satisfactory results were achieved in 40% of cases in conservative treatment and 67% in operative treatment. 7. The average time of partial weight bearing was 15 weeks. 8. Complications were knee stiffness in 6 cases and 4 in delayed union.

Integrated Approaches to Berth Productivity Improvements in Port Development and Operation and Logistics : A Conceptual Perspective (항만개발.운영과 로지스틱스에 있어서 부두생산성 향상에 대한 종합적 접근법 : 개념적 관점)

  • The improvements of berth productivity is closely related to berth/port systems and its relationship extends to the overall pattern of port development/logistics, transport and trade. Since ports in general and berths in specific function as a bridge transferring seaborne cargoes from seaside to landside and vice versa, berth productivity improvements should be considered in terms of the total system in which the cargoes are transported from origin to destination via berths/ports. In that respect, the objective of this study is to improve berth productivity which is a vital factor in the strategic planning in port development and operation and logistics. According to a literature survey on the subject, the contents of the studies have been limited to either a macro-perspective or a micro-perspective which is only a partial solution to the problem. Hence, this limitation requires an integrated approach to find and ultimate solution, which should be considered not separately, but as a whole. The contents and scope of this study cover the improvements of berth productivity in conjunction with strategic planning in port development. efficient operation and logistics from two different viewpoints, macro- and micro-perspective. After that, a combination of these two perspectives is applied to the integrated approach due to lacking a comprehensive perspective from either macro-approach or micro-approach. In reality, a port itself is a complex and dynamic organization. Therefore, a comprehensive insight needs an integrated approach for the long-term national economic development. In this approach, the utmost importance is how to find the bottlenecks and to solve the problems effectively from the beginning stage in port development. In conclusion, derived from the macro- and micro-perspective. the attempt to establish an integrated approach is the merit of this study, which is required as a comprehensive measure since none of the two can be a solution independently. In terms of reinforcing this study, it needs further empirical research due to the restriction of the theoretical method of this paper. Consequently, the reinforced research will be able to enhance the integrated approach on the basis of the conceptual perspective.

Clinical Cases Analysis of Forearm Free Fasciocutaneous Flaps on Oral Cavity Defect Area (구강 결손부에 적용된 요골 유리전완 피부피판 적용례 분석)

  • The radial forearm free flap (RFFF) has become a workhorse flap as a means of reconstructing surgical defects in the head and neck region. We have transferred 12 RFFFs with fasciocutaneous type on oral cavity defects in 12 patients after cancer resection and submucous fibrotic lesion ablation from 2005 to 2007 at Department of oral and maxillofacial surgery, Pusan National University Hospital. We reviewed retrospectively patients' charts and followed up the patients. Clinical analysis on the cases with RFFFs focusing on flap morbidity, indications and available vessels was done. The results of study are follows: 1. RFFF could be applied for all kind of defects after resection of tongue, floor of mouth, buccal mucosa, denuded bone of palate, maxilla, and mandible. 2. All free flaps could be used for primary reconstruction. The survival rate of 12 RFFFs was 92%. Partial marginal loss of the flaps was shown as 3 cases among 12 cases. Large size-vessels like superior thyroid artery, facial artery, internal jugular vein were favorable for microvascular anastomosis. 3. Parenteral nutrition instead of nasal L-tube also can be favorable for postoperative a week for better healing of the flap if the patients couldn't be tolerable with nasal tubing. 4. Donor sites with thigh skin graft were repaired with wrist band for 2 weeks. The complications included scarring, abnormal sensation on hand, and reduced grip strength in few patients, but those didn't induce major side effects. 5. Most RFFFs were well healed even if mortality rate of cancer patients was shown as 50% (5/10 persons). The mortality of patients was not correlated with morbidity of the flaps. We could identify the usefulness of RFFF for restoration of oral function, esthetics if the flap design, tissue transfer indications, and well controlled operation are proceeded.

Anterior Cruciate Ligament Augmentation Using Autogenous Semitendinosus Tendon (자가 반건양건을 이용한 전방 십자 인대 보강술)

  • Purpose: This study presents clinical results of anterior cruciate ligament(ACL) augmentation using autogenous two-strand semitendinosus tendon in the treatment of partial ACL tears with preservation of residual portion. Materials and Methods: From January 2004 to June 2006, twenty two patients who had an ACL injury underwent ACL augmentation using autogenous two-strand semitendinosus tendon were enrolled in this study. We evaluated the clinical results with regard to clinical findings(range of motion, Lachman test, pivot shift test), standard knee scales(Lysholm, Modified Feagin Scoring System), and KT-1000 arthrometer testing. Results: At minimum one year postoperatively, there was no limitation of range of motion. Lachman and pivot shift tests were negative in all knees. On instrumented anterior laxity test by KT-1000 arthrometer, mean side to side difference was improved from 4.6mm preoperatively to 1.7mm postoperatively. Average Lysholm score was improved from 70 to 92. Ninety one percent of cases were rated as good or excellent in Modified Feagin Scoring System. Conclusion: Augmentation using autogenous two-strand semitendinosus tendon with preservation of residual portion of the ACL seems to be an acceptable method for restoring knee stability and proprioceptive function.
