이하선 종양으로 오인된 모기질종 2예 (Pilomatrixoma Mimicking Parotid Gland Tumor - Report of 2 Cases -)

  • Pilomatrixoma is an uncommon tumor arising from hair follicles. Commonly occur in children and most frequently in the head and neck region. It can be mistaken for parotid gland tumor, dermoid cyst, or epidermoid cyst, especially when large lesions develop over the periauricular area, difficulty discerning them from lesion developing within the superficial lobe of the parotid gland may occur. We experienced two cases of pilomatrixomas mimicking parotid gland tumor. Although their histologic appearance is characteristic, they may be clinically misdiagnosed. However, combining clinical information with the distinct histologic features should lead to the correct diagnosis.

부이하선에 생긴 원발성 혈관육종 1예의 장기추적 (Primary Angiosarcoma of Accessory Parotid Gland : A Case Report of Long Term Follow-up)

  • Angiosarcoma is a rare and highly malignant neoplasm which develops from the endothelium of blood vessels. A few cases of primary angiosarcoma of the parotid gland have been reported. However, there is no report of primary angiosarcoma of the accessory parotid gland. In this case, we report a primary angiosarcoma of the accessory parotid gland in a 45-year-old man with growing cheek mass. Ultrasonography revealed a $2.0{\times}2.6cm$ sized homogeneous hypoechoic mass and computed tomography showed a contrast enhanced homogeneous mass. Fine needle aspiration biopsy suggested a benign tumor. The mass was completely excised with a minimal vertical incision. The histopathology showed anastomosing vascular channels lined by atypical endothelial cells and many branching vessels with staghorn appearance with positive immunohistochemical staining for CD34, a highly specific endothelial marker. The patient underwent postoperative radiotherapy and was followed for 8 years without recurrence and metastasis.

이하선에 발생한 지방종 (LIPOMA OF THE PAROTID GLAND)

  • 이하선에 발생한 지방종은 과거에 적절한 진단방법의 부재와 낮은 발생빈도로 인하여 종종 양성혼합종이나 와르틴종양으로 진단되어 종물을 포함하는 이하선을 절제하여 왔다. 하지만 진단방법의 발달로 전산화단층촬영이나 자기공명영상을 통하여 술전에 지방종의 진단이 가능해져 다른종양들과 감별되므로 이하선의 실질을 보존하며 종물을 제거하는 것이 용이하게 되었다. 본증례에서는 이하선 천엽에 발생한 지방종을 전산화단층촬영을 통하여 진단하고 이하선절제술을 시행하지 않고 이하선의 실질의 손상을 최소화하며 종양을 제거하였다. 술후 3개월 후에 방사선 동위원소를 이용한 이하선의 기능검사를 통하여 이하선의 정상적 기능을 확인하였다.

  • Dermoid cysts are developmental anomalies that represent the simplest form of teratoma. They are the result of the sequestration of the skin along the lines of embryonic closure. These cysts of the head and neck are uncommon and account for 7% of all dermoid cysts. They are predominantly found in the orbit, floor of mouth, and nose. As a dermoid cyst of the parotid gland is extremely rare, it is often misdiagnosed preoperatively. By way of imaging modalities such as computed tomography, MRI and ultrasongraphy along with FNAB, it can be differentiated from many other cystic lesions of the parotid gland. We report a case of dermoid cyst of the parotid gland which masqueraded as lipoma before complete surgical excision.

이하선의 단일섬유종(Solitary Fibrous Tumor) 1예 (A Case of Solitary Fibrous Tumor of the Parotid Gland)

  • Solitary fibrous tumors are commonly arise in the pleura and less commonly in extrapleural sites. In head and neck area, solitary fibrous tumors can occur in nose, paranasal sinus, soft palate, epiglottis, thyroid, parotid and submandibular gland. To our knowledge, this is the 5th case of solitary fibrous tumor arising from the parotid gland in English literature. We report a case of solitary fibrous tumor occurred in the superficial lobe of right parotid gland which was successfully treated by superficial parotidectomy with preservation of the facial nerve.

Branchial cleft cyst in the parotid gland in a human immunodeficiency virus-negative patient

  • In branchial lymphoepithelial cyst (BLEC), which is also known as branchial cleft cyst, the remnants of a branchial arch develop into a cyst, causing swelling. The first case of BLEC in the parotid gland was reported by Hildebrant in 1895. Since then, BLEC in the parotid gland has continued to be reported, but in rare cases. A 45-year-old man presented to our hospital with a swelling of the left cheek of approximately 6 months' duration. The patient underwent a superficial parotidectomy and was pathologically diagnosed with BLEC. Of note, this was the first case of non-human immunodeficiency virus (HIV)-related BLEC of the parotid gland in South Korea. BLEC is a benign condition, but its treatment depends on the presence of HIV infection. In HIV-negative patients, BLEC does not require a further work-up to evaluate metastasis. Our case report describes the diagnosis and treatment of BLEC in a patient without HIV.

이하선에서 발생한 타액선관내 유두종 1예 (A Case of Intraductal Papilloma arising in the Parotid Gland)

  • Intraductal papilloma is an extremely rare benign tumor especially when it arises from major salivary gland. Recently, we experienced a case of an intraductal papilloma arising from the parotid gland in a 51-year-old woman. The radiologic finding showed lobulated enhancing and cystic solid mass in the left parotid superficial lobe. Superficial parotidectomy was performed. Pathological findings showed papillary proliferations with fibrovascular cores lined with bland cuboidal or columnar epithelial cells, and immunohistochemical stain results were consistent with intraductal papilloma. We report a case of intraductal papilloma in parotid gland with a review of literature.

원발성 이하선 비호지킨 림프종: 증례보고 (Primary Parotid Non-Hodgkin's Lymphoma: A Case Report)

  • Purpose: Primary malignant lymphomas of the salivary glands are uncommon, representing only 1.7% to 3.1% of all salivary neoplasms and 0.6% to 5% of all tumors and tumor-like lesions of the parotid gland. Lymphomas of the parotid glands are usually manifestations of a systemic disease process but primary lymphomas of the parotid glands are rare. Most of these lesions are classified as extranodal non-Hodgkin's lymphoma. We report the clinicopathological features of primary malignant lymphoma of the parotid gland based on an analysis of our cases. Methods: The subject was a 48-year-old male patient with a malignant lymphoma originating in the parotid gland, which had been slowly increasing in size over previous 6 months. The diagnosis was established by MRI and a superficial lobectomy. After diagnosis, the patient was referred to an oncologist for staging and medical treatment. Results: The stage was IIIA. The patient was treated with chemotherapy following surgery with rituximab and CHOP (Cyclophosphamide, Adriamycin, Vincristine, Prednisolone). The tumor was controlled successfully by chemotherapy. The patient was followed up for 1 year with no relapse. Conclusion: A case of primary non-Hodgkin's lymphoma of the parotid gland was treated with a superficial parotidectomy and chemotherapy. The disease was well controlled after a 1 year follow-up.

이하선의 Warthin씨 종양 (Warthin's Tumor of the Parotid Gland)

  • Warthin's tumor is a benign and slow growing tumor found exclusively in the parotid gland or the periparotid lymph nodes. It mostly affects males between the age of forty to seventy years and is closely related with smoking history. Between January 1981 and June 1996, 42 patients underwent surgical excision of Warthin's tumor of the parotid gland; which made up 10.6% of all parotid gland surgeries(398 cases) during the same period. Their ages ranged from 36 to 75 years with a mean age of 56 years. There were 33 male and 9 female patients with a 4.3 : 1 male to female ratio. The majority of the tumors were situated in the parotid tail whereas one was in the deep lobe. Bilateral simultaneous involvements of the parotid gland were found in 4 patients(9.5%) ; therefore total of 46 parotid glands were involved. Four(8.7%) of the 46 parotid glands had multifocal tumors ranginging from two to three lumps. Tumor sizes varied from 1.5 to 6.0cm with mean diameter of 3.lcm. Of the 42 patients, 26(61.9%) were diagnosed preoperatively or peroperatively by means of CT scans, ultrasound, 99m-Tc. scan, fine needle aspiration cytology or intraoperative frozen section biopsy. Of the 46 tumors, 30 underwent a superficial(n=29) or total(n=1) parotidectomy and for 16 cases with tumors suspected preoperatively or peroperatively of being single Warthin's tumor, only enucleation was performed. No cases of recurrence were identified during the follow up period regardless of type of operation performed, however the postoperative complication rate was much higher in the parotidectomy group(33.3%) than in the enucleation only group(12.5%). We feel that an enucleation procedure may be appropriate for the patients with single Warthin's tumor.

Parotid gland sparing effect by computed tomography-based modified lower field margin in whole brain radiotherapy

  • Purpose: Parotid gland can be considered as a risk organ in whole brain radiotherapy (WBRT). The purpose of this study is to evaluate the parotid gland sparing effect of computed tomography (CT)-based WBRT compared to 2-dimensional plan with conventional field margin. Materials and Methods: From January 2008 to April 2011, 53 patients underwent WBRT using CT-based simulation. Bilateral two-field arrangement was used and the prescribed dose was 30 Gy in 10 fractions. We compared the parotid dose between 2 radiotherapy plans using different lower field margins: conventional field to the lower level of the atlas (CF) and modified field fitted to the brain tissue (MF). Results: Averages of mean parotid dose of the 2 protocols with CF and MF were 17.4 Gy and 8.7 Gy, respectively (p < 0.001). Mean parotid dose of both glands ${\geq}20$ Gy were observed in 15 (28.3%) for CF and in 0 (0.0%) for MF. The whole brain percentage volumes receiving >98% of prescribed dose were 99.7% for CF and 99.5% for MF. Conclusion: Compared to WBRT with CF, CT-based lower field margin modification is a simple and effective technique for sparing the parotid gland, while providing similar dose coverage of the whole brain.
