副鼻洞炎에 關한 臨床的 硏究 (A Clinical study of Paranasal Sinusitis)

  • Paranasal sinusitis, especially chronic is one of the most common diseases in the field of otolaryngology. It is similar to Bee Yeun(鼻淵) in oriental medicine. Most cases of sinusitis are due to anatomical abnormalities within ostiomeatal unit or disturbed mucocilliary flow. The ostiomeatal unit is the first place of contact with bacteria and allergens during aspiration, and it can be obstructed easily by minute mucosal swelling due to anatomical narrowness. Therefore the treatment of paranasal sinusitis is not easy and often leads to recurrences in spite of long term treatment or surgical therapy. We studied 83 patients who had visited our hospital with complaints of nasal symptoms; they had been diagnosed as having paranasal sinusitis through an endoscopy or CT scan in another hospital and were diagnosed as the paranasal sinusitis through a PNS series. The results were as follows: 1. Age and sex distribution: The most common occurence was found between 6-10 and 1-5 years old. The Males Were 52($62.7\%$) and Females were 31($37.3\%$). 2. By residence 58 cases lived in apts: 25 lived in houses. 3. The longest duration of disease varied from a lower of 13 to a higher of 36 months in 22 cases and from 7 to 12 months in 21 cases. 4. The most common complication & past history with otolaryngologic or allergic disease were adenoid or tonsil hypertrophy & tonsillectomy and adenoidectomy (21 cases). In decreasing order the others were atopic dematitis, otitis media with effusion and allergic rhinitis. 5. Distribution of paranasal sinus disease was most common in both maxillary sinuses in 52 cases. 12 cases showed a normal PNS X-ray series but these had been diagnosed as paranasal sinusitis with an endoscopy or CT scan in another hospital. 6. Common sinusitis - related symptoms were from highest incidence to lowest nasal obstruction, postnasal drip rhinorrhea, frontal headache, cough with sputum. 7. The most administered of prescription was Gamibangpoongtongsungsan and Sunbangpaedoksan extract. 8. In 26 cases the subjects showed significant improvement symptoms in PNS X-ray series : In 21 case showed partial improvement symptoms.

부비동염에 관한 전산화단층방사선학적 연구 (Computerized Tomographic Study on the Paranasal Sinusitis)

  • Objectives : The purpose of this study is to evaluate the computed tomographic (CT) images of the paranasal sinusitis(PNS). Materials and Methods : The author examined the extent and recurring patterns of the paranasal sinusitis and some important anatomic landmarks. The author analyzed PNS images retrospectively in 500 patients who visited Chonbuk National University Hospital between January 1996 and December 1997. Results : The most frequently affected sinus was maxillary sinus (82.9%), followed by anterior ethmoid sinus(67.9%), posterior ethmoid sinus(48.9%), frontal sinus(42.0%) and sphenoid sinus(41.4%). The characteristic features of CT images of the sinusitis were sinus opacification(22.4%), mucoperiosteal thickening(34.3%), and polyposis(2.0%). Sinonasal inflammatory diseases were categorized into 5 patterns according to Babber s classification. They were 1) infundibular(13.0%), 2) ostiomeatal unit(67.4%), 3) sphenoethmoidal recess (13.0%), 4) sinonasal polyposis (9.6%) and 5) unclassifiable patterns(18.0%). The incidences of contact between sinus and optic nerve were as follows ; the incidences of contact with posterior ethmoid sinus, sphenoid sinus. both posterior sinuses were 11.4%. 66.8%, 6.3%. respectively. The incidences of contact between sphenoid sinus and maxillary nerve, vidian nerve, internal carotid artery were 74.5%. 79.2%. 45.1%. respectively. The incidences of pneumatization of the posterior ethmoid sinus were as follows ; normal 70.6% and overriding type 29.4%. The incidences of sphenoid sinus pneumatization were as follows; normal 56.9% , rudimentary 12.5%, pterygoid recess 22.7%, anterior clinoid recess 2.7%, and both pterygoid and anterior clinoid recess type 5.2%. Conclusions : The inflammatory sinonasal diseases were classified into five patterns using the CT of PNS, which was proven to be an excellent imaging modality providing detailed information about mucosal abnormality, pathologic patterns, and the proximity of the important structures to the posterior paranasal sinuses. This result will aid in the interpretation of CT of PNS functionally and systemically.

부비동과 비의 미분화 암종 - 1예 보고 - (Sinonasal Undifferentiated Carcinoma - A Case Report -)

  • Sinonasal undifferentiated carcinoma(SNUC) is a distinct, relatively rare neoplasm arising in the nasal cavity and paranasal sinuses composed of undifferentiated epithelial cells and clinically characterized by a fulminant course. We report a case of SNUC in a 56-year old man who have had bilateral neck masses since one month ago before coming to our hospital. The paranasal computed tomography showed soft mass density in the left maxillary sinus and the nasal cavity with bone destruction in the anterior medial and the inferior maxillary sinus wall. This mass was extruded into the left orbital wall. Biopsy of the nasal mass and fine needle aspiration(FNA) of the neck mass were done. FNA revealed medium-sized neoplastic cells forming clusters or individually dispersed. Nuclei were round to oval, slightly to moderately pleomorphic, and hyperchromatic. Chromatin was finely granular, but occasionally was coarsely granular. Nucleoli varied from large to inconspicuous and the cytoplasm was scanty.

성인에서 우연히 발견되는 부비동혼탁화의 유병률 (Prevalence of incidental paranasal sinus opacification in an adult dental population)

  • Purpose : The purpose of this study was to determine the prevalence of sinus opacification among adult dental patients. Materials and Methods : Five hundred and sixty-four Cone Beam Computed Tomography (CBCT) scans of dental patients over the age of 18 were reviewed for sinus opacification. Opacification was graded as clear, mild, moderate or severe. Patients with any sinus-related signs or symptoms were excluded. Results : Sinus opacification in one or more sinuses was found in 59.2% of the patients. The sinus opacification was mild in 49.8%, moderate in 8.3%, and severe in 1.1%. The maxillary (37.7%) and ethmoid (37.4%) sinuses were most frequently affected. The prevalence was higher in the older age group and showed a male predomination (p<0.05). Conclusion : Sinus opacification in asymptomatic adults is very common and emphasizes the importance of clinical correlation before deciding on the final diagnosis and treatment.

  • 비강 및 부비동에 발생하는 반전성유두종은 매우 드문 양성종양으로 1854년에 Ward에 의해 기술되어 여러 학자들에 의해 증례 보고되어 왔으며 수술 후 재발이 잘 되고 드물게 악성으로 변하는 경향이 있으며 계속적인 추적관찰이 필요한 질환이다. 이에 저자들은 약 8년전 개인의원에서 비용으로 진단받아 비용절제술을 받은 바 있는 환자와 약 2년전 역시 같은 진단으로 수술을 받았던 환자에서 조직검사 결과 반전성유두종으로 판명된 2례 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

사골동에 발생한 골종 2례 (Two Cases of Osteoma of the Ethmoid Sinus)

  • Osteomas are uncommon benign turners, occasionally located in the paranasal sinuses, of which the frontal and ethmoid sinus is the most frequent site. The etiology of the osteoma is still unknown but three possible theories have been proposed; embryological, traumatic and infective. The most common symptoms of osteoma are facial pain and headache, but most of them are asymptomatic and discovered incidentally on routine sinus radiography. Recently, the authors experienced two cases of large osteoma of the ethmoid sinus that were removed successfully by external ethmoidectomy.

  • 비강 및 부비동의 점막에서 발생하는 양성상피종양인 반전성 유두종은 비교적 드물며 1854년 Ward에 의해 처음으로 기술된 이래 여러 학자들에 의해 본증이 논의되어 왔으나 한국에서는 그 보고례가 매우 드물었다. 병리조직학적으로 양성인 이 종양은 비강이나 골 및 주위조직의 파괴성 병변이 빈번하고 수술 후 재발이 잘되고 드물게 악성으로 변하는 성질을 나타내므로 임상적으로 악성으로 알려져 왔다. 일단 반전성 유두종으로 확진되게 되면 가능한 한 광범위한 절제가 필요하며 술 후에도 계속적이 추적조사가 필요하다고 하겠다. 최근 저자들은 비폐색, 종류감을 주소로하여 내원한 48세된 남자환자에서 우측비강, 상악동에서 발생하여 국소적으로 악성화 변화를 일으킨 반전성 유두종 1례를 비내 및 Caldwell_LUC식 방법으로 절제후 Bleomycin 정맥주사, 5_FU 국소분무요법 및 방사선요법($Co^{60}$)을 병용하여 좋은 결과를 얻었기에 이를 문헌적 고찰과 함께 보고하는 바이다.

Study of frontal and ethmoid sinus of sinonasal complex along with olfactory fossa: anatomical considerations for endoscopic sinus surgery

  • The Functional endoscopic sinus surgery through transnasal approach is a common modality of treatment for disorders of the nasal cavity, paranasal air sinuses as well as cranial cavity. The olfactory fossa (OF) is located along the superior aspect of cribriform plate which varies in shape and depth. This variable measurement of the depth of OF is mostly responsible for greater risk of intracranial infiltration during endoscopic procedures in and around the nasal cavity. The morphology of frontal and ethmoid sinus (ES) vary from simple to complex. This cadaveric study is planned to improve the ability of the otolaryngologist, radiologist to understand the possible morphological variations and plan steps of less invasive "precision surgery" to have a safe and complication free procedures. A total of 37 human head regions were included in the study. For classification of OF, Modified Kero's classification was used. The size, shape and cells of frontal and ES were noted. We found, type II (60.8%) OF was more common followed by type I (29.7%) than type III (9.5%). The shape of frontal sinus was comma shaped (55.4%) followed by oval (18.9%) than irregular (16.2%). Most common two cells type of ES was seen in 50.0% of both anterior and posterior ES. Out of 74 ES, 8.1% of Onodi cells and 14.9% of agger nasi cells were seen.

Maxillary sinus volumetric changes in jet aircraft pilots: A multislice computed tomography pilot study

  • Purpose: This study evaluated maxillary sinus volume changes in military jet aircraft pilot candidates before and after the training program, in comparison with a control group, considering the effects of pressurization, altitude, and total flight hours, through multislice computed tomography. Materials and Methods: Fifteen fighter pilots were evaluated before initiating the training program and after the final approval. The control group consisted of 41 young adults who had not flown during their military career. The volumes of each maxillary sinus were measured individually before and at the end of the training program. Results: When comparing the initial and final volumes in the pilots, a statistically significant increase was observed both in the left and right maxillary sinuses. When evaluating the average total volume of the maxillary sinuses(i.e., the average volume of the right and left maxillary sinuses together), a significant increase in the volume of the maxillary sinuses was observed in the pilot group when compared to the control group. Conclusion: The maxillary sinus volumes in aircraft pilot candidates increased after the 8-month training program. This may be explained by changes in the gravitational force, the expansion of gas, and positive pressure from oxygen masks. This unprecedented investigation among pilots might lead to other investigations considering paranasal sinus alterations in this singular population.

Cavernous sinus thrombosis caused by a dental infection: a case report

  • Cavernous sinus thrombosis not only presents with constitutional symptoms including fever, pain and swelling but also with specific findings such as proptosis, chemosis, periorbital swelling, and cranial nerve palsies. It is known to occur secondary to the spread of paranasal sinus infections in the nose, ethmoidal and sphenoidal sinuses. However, paranasal sinus infection of dental origin is rare. The following is a case of cavernous sinus thrombosis due to the spread of an abscess in the buccal and pterygomandibular spaces via buccal mucosal laceration.
