The purpose of this study was to evaluate the general characteristics, complications and level of social adaptation of spinal cord injured patients. The subjects were one hundred forty five members who were inpatients or outpatients from October 1, 2004 to April 30, 2005 in general hospitals and municipal welfare centers for the handicapped located in the metropolitan city of Gwangju. The following results were obtained using Modified Barthel Index (MBI). 1) Gender distribution was 77.9% male and 22.1% female. The mean age at the time of injury was 35.4 and the mean age during the study was 44.2. 2) The degree of paralysis among the subjects was as follows: 49.0% had complete paralysis and 51.0% suffered incomplete paralysis. The most frequently injured lesion among the subjects was cervical (49.0%), followed by thoracic (35.9%), and lumbar (15.2%). 3) The mean MBI score was 63.5. There was statistically significant difference in the MBI score in the relation between complete and incomplete paralysis, the relation between cervical, thoracic, and lumbar injury, and the relation between a recovery period of less than three years and more than three years according to the characteristics of injury (p<.05). 4) There was statistically significant difference in the MBI score of subjects who had complications concerning spasticity, deformity, urinary tract infection, and sexual dysfunction (p<.05). 5) The most serious emotional pain after spinal cord injury resulted from economic issues, which affected 35.2% of the subjects. The group having a shorter recovery period after spinal cord injury complained of psychological matters, the group having a longer recovery period complained about the surrounding environment (lack of convenient facilities), suggesting statistically significant difference (p<.05). 6) The most common activities of the group with injuries more than ten years old included meeting schoolmates and working, while most common activities of the group with injuries less than three years old included attending religious functions and miscellaneous others (watching TV, spending time with family), suggesting statistically significant difference (p<.05).
Objective : In Modern society, patients with facial nerve paralysis are increasing because of many factors - irregular life, cold, overwork and stress etc. We have troubles in presuming the prognosis, though how to diagnosis and examine facial nerve paralysis are many. Methods : A clinical study was done on 89 patients who were diagnosed and treated as facial nerve paralysis(Bell's palsy) from January 2001 to May 2003 at the Dep. of Acupuncture and Moxibustion, college of Oriental Medicine, Woo-suk University. We classified 89 patients as the Sasang(四象) constitution, contributing factor, season, age and existence of diabetes items and analyzed, as we would research differences of changing point-Period from on set of Bell's palsy to the day which the change begins to be seen at the face- and improvement -Period which Bell's palsy is improved from on set to H-B grade II. Results : 1. Among the Sasang constitution, Soeumin($10.67{\pm}3.77days$) were faster than other constitutions, but Soyangin($16.25{\pm}6.75days$) were slower than other constitutions in changing point. Taeumin($4.12{\pm}1.49weeks$) were shorter than other constitutions, but Soyangin($4.88{\pm}2.11weeks$) were longer than other constitutions in improvement. 2. Among contributing factors, overwork and stress group were slower than other contributing factors in changing point($13.95{\pm}6.52days$), and longer than others in improvement($4.67{\pm}1.87weeks$). 3. Changing point and improvement of season, age, and existence of diabetes had the difference of the average according to an each item, but they were not statistically significant. 4. In 89 patients with Bell's palsy, average changing-point was $13.11{\pm}5.99$(days) and average improvement was $4.47{\pm}1.82$(weeks). Their correlation was 0.687 and statistically significance(P<0.01), therefore we could decide that their relation is highly correlation.
Objectives : This study was perfomed to know the effect of functional dyspepsia on the prognosis of peripheral facial paralysis. Methods : We divided 42 patients with Peripheral Facial Paralysis who had admitted at Semyung University Chung-ju Oriental Hospital into two groups, The A group which had Functional Dyspepsia, and The B group which didn't have Functional Dyspepsia. Then we applied acupuncture, physical therapy, herb medicine to the patients. The effects of treatment was evaluated by Yanagihara's unweighed grading system. Results : A group's improvement index which is measured by Yanagihara's unwei ghed grading system was lower than B group's improvement index. Conclusions : Through oriental medicine treatment, the patients recovered significantly. The patients who has functional dyspepsia recovered slowly than the patients who has not functional dyspepsia.
Introduction: Guillain-Barre syndrome(GBS) is defined as a recognizable clinical entity that is characterized by rapidly evolving symmetric limb weakness, a loss of tendon reflexes, absent or mild sensory signs, and variable autonomic dysfunctions. Recently there is an opinion that Acute Inflammatory Demyelinating Polyradiculoneuropathy'(AIDP) is more effective than 'GBS' for the symptoms. These symptoms are applicable to rheumatic arthritis(痺), flaccid paralysis of limb(?) caused by wind, cold and dampness(風, 寒, 濕) from an oriental medical view point. On this, we reported one case that diagnosed as 'complex of rheumatic arthritis(痺) and flaccid paralysis of limb(?)' at our oriental medical hospital. Result: we considered these symptoms as 'complex of rheumatic arthritis(痺), flaccid paralysis of limb(?)'. In accordance with the result, we treated the patient with Chung-Sang Tong-Jung On-Ha(淸上通中溫下). We concluded that external factors such as wind, cold and dampness(風, 寒, 濕) and internal factors like vital energy's disharmony(生氣不調和) affected the patient.
Objectives : This study was to compare the incipient grade and improvement rate between each opposite gender and part on peripheral facial nerve paralysis. Methods : We investigated 64 cases of patient with peripheral facial nerve paralysis and divided into four groups with each gender and affected part and compared their incipient grade and rate of improvement. All groups were evaluated by Yanagihara's unweighted grading system before treatment and in everyday and after treatment. Results & Conclusion : 1. There's a difference of incipient grade between each opposite affected lesion in male. But there was not significant statistics. 2. There's no difference of incipient grade between each opposite affected lesion in female. 3. There's a difference of incipient grade between each opposite gender, affected their right side. But there was not significant statistics. Left side affecting palsy is more severe than right in male, and reversed results in female. But there were not significant statistics. There's no differences between each affected lesion in female. 4. There's no differences of improvement rate between each affected lesion in female. 5. The group, affected right side had better rate of improvement than another in male. But it's not significant statistics. 6. Male group had better rate of improvement than female in both gender, affected its left lesion. But it's not significant statistics.
Objectives : The aim of this report was to investigate the effects of Korean medical treatment on facial nerve paralysis with sensorineural hearing loss and tinnitus caused by traumatic temporal bone fracture. Methods : We treated a patient with acupuncture, herbal medicine and physiotherapy. The effect of these treatments was evaluated by House-Brackmann facial grading scale, Yanagihara's unweighted grading system and by Digital Infrared Thermographic Image. Results : After 21 days of Korean medical treatment, House-Brackmann facial grading scale changed from III to II and Yanagihara's unweighted grading score increased from 14 to 27. Digital Infrared Thermographic Image also improved. Conclusions : These results suggest that Korean medical treatments were effective in treating facial nerve paralysis with sensorineural hearing loss and tinnitus caused by traumatic temporal bone fracture. We hope that a more efficient application of this treatment will be the result of clinical data accumulated in future studies.
Objectives : Vocal cord paralysis(VCP) is a complex disorder which may result from numerous causes. We reviewed and analyzed the trend of clinical characteristics and causes of VCP in Korean patient. Method : A total 197 patients with VCP who visited St.Mary's hospital from March, 2000 to August, 2006 were reviewed retrospectively. They were analyzed according to sex, age, cause of VCP, position of paralyzed vocal fold, treatment methods. Results : The male and female ratio was 1.6 : 1. The unilateral paralyzed vocal fold was fixed at paramedian position in 84% of the cases. The left vocal fold was paralyzed about 2 1/2 times as much as the right vocal fold. Among the causes of VCP 30.9% of the cases were due to postoperative paralysis, and most of those were developed after lung, mediastinal surgery. laryngeal EMG was performed in 47 patients for determines the prognosis and treatment method. In the unilateral VCP, 90 patients were treated with injection laryngoplasty, 21 patients were performed thyroplasty type I. Conclusion : The causes of VCP include various diseases, so, detection of the primary disease is very important, because many fatal diseases are included among the primary diseases, and late detection can cause serious problems. VCP is not only a disease entity in itself, but can be seen as a sign of an underlying disease.
Background and Objectives : Artecoll(R) is an injectable soft tissue filler, which is a suspension of polymethylmethacrylate microspheres in $3.5\%$ bovine collagen solution. The authors aimed to determine the clinical of Artecoll of Artecoll(R) as an injection material into the vocal fold to correct the glottal insufficiency caused by unilateral vocal cord paralysis. Materials and Methods : Forty-one consecutive patients with unilateral vocal cord paralysis received percutaneous Artecoll injections under local anesthesia. Acoustic, aerodynamic and stroboscopic analyses were prospectively provided before, 1 week and 3 months after injection. Perceptual GRBAS grading by speech language pathologists and subjective ratings of the hoarseness and aspiration by the patients were also obtained. Results : Aerodynamic parameter(maximal phonation time) were significantly improved after the injection (p<0.05). Acoustic parameters (jitter and shimmer) were improved at the 3rd month follow-up. GRBAS uading and patients own subjective scaling of hoarseness and aspiration also showed significant improvement (p<0.05). Early or delayed significant side effects were not observed. Conclusion : Vocal fold injection with Artecoll is a convenient, safe and useful method of temporarily correcting the glottal insufficiency. Further long-term follow-up studies will answer the usefulness and safety of the Artecoll injection laryngoplasty.
Objectives : Videostrobokymography(VSK) has been recently developed and reported by Sung et at. We aimed to analyze vibratory patterns and objective parameters in various benign vocal fold lesions using VSK, and examine the efficacy of VSK in clinical application. Materials and Methods : Using VSK, we analyzed the vibration pattern of normal vocal fold and various benign lesions, such as nodules, polyps, cysts, Reinke's edema and unilateral vocal fold paralysis. We also calculated objective parameters, open quotient and asymmetric index, and compared them with mean values of parameters in normal controls. Results : In nodules, polyps, and cysts, the open quotient on the site of the lesion was similar to the mean value in normal controls, however, on the other part of the vocal folds it was much larger than normal mean value. In Reinke's edema, irregular and asymmetric vibration was observed. The posterior portion of the vocal folds showed larger open quotients than the anterior portion. In the unilateral vocal fold paralysis, irregular vocal folds vibration and incomplete closure of the vocal folds were documented. Much larger asymmetric indices were calculated in the unilateral vocal fold paralysis than in normal controls and other lesions. The asymmetric index could be a good quantitative parameter of vibration from a patient with vocal fold paralysis. Conclusion : This study demonstrated that VSK could generate clear quantitative documentations of fine vibrations of vocal folds in many different benign lesions. VSK has a potential as an effective tool for quantitative analysis of vibratory patterns of the vocal folds iii clinical settings.
Background and Objectives : Patients with pathologic voice often concern about recovery of voice after surgery. In our investigation, we give controlled values of three parameters of voice synthesis program of Dr. Speech Science. such as jitter, shimmer, and NNE(normalized noise energy) which characterize someone's voice from others and deviced a method to synthesize the predicted voice after performing operation. Subjects and Method : Values of vocal jitter, vocal shimmer, and glottal noise were measured with voices of 10 vocal cord Paralysis and 10 vocal Polyp Patients 1 week Prior to and 1 month after the surgery. With Dr. Speech science voice synthesis program we synthesized 'ae' vowel which is closely identical to preoperative and post-operative voice of the patients by controlling the values of jitter, shimmer, and glottal noise. then we analyzed the synthesized voices and compared with pre and post-operative voice. Results : 1) After inputting the preoperative and corrected values of jitter, shimmer, and glottal noise into the voice synthesis Program, voices identical to vocal Polyp Patients' Pre- and Postoperative voices withiin statistical significance were synthesized 2) After elimination of synergistic effects between three paramenter, we were able to synthesize voice identical to vocal paralysis patients' preoperative voices. 3) After inputting only slightly increased jitter, shimmer into the synthesis program, we were able to synthesize voice identical to vocal cord paralysis patients' postoperative voices. Conclusion : Voices synthesized with Dr. Speech science program were identical to patients' actual pre and postoperative voice, and clinicians will be able to give the patients more information and thus increased patients cooperability can be expected.
