Characteristics and Health Status of Outdoor Workers Exposed to High Temperature (우리나라 고온 노출 야외작업자의 특성과 건강수준)

  • Purpose: This study aims to identify general, workplace, and health-related characteristics of outdoor workers exposed to high temperatures, and to compare the risk of disease according to outdoor high temperature exposure. Methods: This secondary analysis study used the 5th Korean Working Conditions Survey (2017) to identify 4,915 outdoor workers exposed to high temperatures. Results: Outdoor workers exposed to high temperatures were mostly male, elderly, less educated, and daily contract workers. Most of them were engaged in agriculture, forestry and fishing, and construction industries. About 40~50% of them complained of musculoskeletal pain and overall fatigue. The results showed that high temperature exposure increased the risk of illness (hearing problem, skin problem, backache, muscular pains in upper and lower limbs, headache/eyestrain, injuries, depression, and overall fatigue) among workers. Conclusion: High temperature exposure might increase the risk of illness among workers. The results of this study demonstrated that the outdoor workers should be protected from high temperatures.

After Hemorrhoidectomy Korean Medicine Treatment with Whidam's SuGi Therapy: A Case Report (치핵근치술 후 휘담식 수기요법을 병행한 한방관리 증례 1)

  • Objective : The purpose of this study is to report about a 50's female patient after Hemorrhoidectomy, whose symptoms were relieved after Korean medicine treatment with Whidam's SuGi Therapy. Methods : The patient in this study received Korean medicine treatment such as herbal medication, acupuncture treatment, cupping therapy and specially Whidam's SuGi therapy. Results : After the treatment, the symptoms of pains were relieved. Vas fell from 9 to 2 and overall, the body condition has improved. Conclusions : This study showed that Whidam's SuGi therapy added with Korean Medicine treatment can be an effective choice for after Hemorrhoidectomy.

Development of a scale to Measure the Self Concept of Cesarean Section Mothers. (제왕절개술 산모의 자아개념 측정 도구 개발)

  • Recently, the rate of cesarean section in Korea has been increasing. The results of several previous studies in foreign countries on the emotional responses of cesarean section mothers showed that they might experience difficulties in the mother- infant interaction due to fatigue, lack of early mother - infant interaction, disappointments, anger, feelings of loss of control, and other factors. Human behavior is said to be determined by one's self concept, and self concept is influenced by both internal and external environmental factors. A scale to measure the self concept of cesarean section mothers was needed in order to identify those who might have difficulties in the mother- infant interactions in future. The purposes of this study were to develop a measuring scale, and to test its reliability and validity. The process of this study was as follows. A structured interview was done with 50 cesarean section and vaginal delivery mothers to find their state of emotional reaction after giving birth to their babies. Based on the results of the interviews, a 50 items Likert scale was developed. The self concept of 268 cesarean section and vaginal delivery mothers who were hospitalized at six hospital in seoul were measured, during the period between Feb. 1 and April 30. Reviewing the discriminating power of each item by means of crosstabulation, ten items were selected for the final scale. The reliability and validity of this ten item scale were tested by Cronbach's alpha and t-test, using spss pc+package. The results of this study and recommendation are as follows. 1. The ten selected items were as follows. I feel pains in my breast. (-) I have a good appetite now. (+) I feel pains in my flank. (-) I feel fine now. (+) My body seems to have returned to its prepregnant state. (+) Thinking of the delivery process, I feel sorry. (-) I want to hold my baby in my arms. (+) I want to keep my own life, even if I became a mother. (-) I want to delegate the care of the baby to my mother / mother in law. (-) I think baby is my alter ego. (+) 2. The reliability of this scale was tested by Cronbach's alpha, and the coefficient of this scale was .8066. 3. The construct validity of this scale was tested by means of known group methods. The value of self concept for cesarean section mother was significantly lower than for vaginal delivery mothers(t=-5.51, df=266, p=0.007). 4. The criterion validity of this scale was tested indirectly. Though this scale could discriminate the differences in the self concept between cesarean section and vaginal delivery mothers, the five items on the personal self concept scale didn's show any differences between cesarean section and vaginal delivery mothers. Therefore, the study indicates that those who shows lower values in the personal self concept measurement, that is, lower than 12.03 points, could be regaled as “risk mothers” 5. Further studies using this scale to clarify the influencing factors on negative self concept are strongly recommended.

Improving Effects of Fructooligosaccharide and Isomaltooligosaccharide Contained in Sponge Cakes on the Constipation of Female College Students (스폰지 케이크에 함유된 프락토올리고당과 이소말토올리고당이 여대생의 변비개선에 미치는 효과)

  • The aim of this study was to evaluate the effects of 40% sucrose replacement with oligosaccharides (fructo- orisomalto-) in sponge cakes. The 18 female college students were divided into two groups; fructooligosaccharide sponge cake group (FOS) and isomaltooligosaccharide sponge cake group (IMOS). In each group, the subjects received 50 g/d of sponge cake which contained about 8 g oligosaccharides for 2 weeks. The results were as follows: (1) Frequency of stool evacuation tended to increase after 1 week and significantly increased after 2 weeks in both groups. (2) Shape and color of stool did not change in FOS group but the IMOS group showed significant improvement. (3) The stool amounts per defecation were not significantly different in both groups. (4) The time of defecation decreased 50% after 1 week in FOS group and tended to decrease gradually in IMOS group. (5) Consistency of stool was significantly decreased after 1 week in both groups and continuously decreased until 2 weeks. (6) Effort for evacuation and comfort feeling after evacuation were significantly improved only in IMOS group after 2 weeks. (7) Gastrointestinal complaints concerning diarrhea, abdominal pains, nausea, flatulence, loss of appetite and weight and bloating were apparent. The symptoms such as abdominal pains, flatulence, and bloating in FOS group lasted for 2 weeks. However, the complaints appeared only during two or three days in IMOS group. In conclusion, our results suggest that isomaltooligosaccharide was more effective than fructooligosaccharide for preventing constipation because isomaltooligosaccharide worked safely in gastrointestinal lumen.

A STUDY ON THE IDENTIFICATION OF Porphyromonas endodontalis BY PCR USING SPECIES SPECIFIC PRIMERS FOR THE 16S rDNA (16S rDNA sequence에 대한 종특이성 primer를 이용한 중합효소연쇄반응증폭에 의한 Porphyromonas endodontalis의 동정에 관한 연구)

  • P. endodontalis which was known to be associated with the infected root canals and periapical lesions is very difficult to detect by culture methods or traditional methods. Detection of bacteria using polymerase chain reaction(PCR) for 16S ribosomal DNA(rDNA) is fast, simple, and accurate with relatively small amount of target cells. 16S rDNA consist of conserved regions those are same to all species, and variable regions which represent species specificity. The 16S rDNA sequences of P. endodontalis and P. gingivalis were aligned and two highly variable regions were selected as a pair of species specific oligonucleotide primers for P. endodontalis. And then the pair of primers for PCR amplification was synthesized to identify P. endodontalis. The sequences of the species specific primers for the 16S rDNA of P. endodontalis were as follows ; sense primer[endo1]: 5'-CTATATTCTTCTTTCTCCGCATGGAGGAGG-3' antisense primer[endo2]: 5'-GCATACCTTCGGTCTCCTCTAGCATAT-3' In this study, for the identification of P. endodontalis without culture from the mixed clinical samples, PCR was done with species specific primers for the 16S rDNA sequences of P. endodontalis. The results were as follows : 1. The species specificity of the primers for the 16S rDNA of P. endodntalis was determined by the PCR methods. About 490bp amplicon which was specific only for P. endodntalis was produced with P. endodontalis. No amplicon was produced by PCR with other strains similar to P. endodontalis. 2. The synthesized species specific primers reacted with conventionally identified P. endodontalis which we have in conservative dentistry laboratory. 3. The identification of P. endodontalis using PCR technique with samples collected from infected root canals or periapical lesions was more sensitive than that of culture methods. 4. Seven samples revealed including P. endodontalis by PCR technique. Five of them were related with pains, two of them with sinus tract, three of them with foul odor, and three of them with purulent drainage. P. endodontalis was shown to have great relation with pains.

Prognosis of Recurred TMD Patients According to Conservative Treatment (재발된 측두하악장애 환자의 보존적 처치에 따른 예후)

  • The purpose of this study is to compare the differences between first visits and the recurred time of TMD patients about the number of visits, the treatment plans, the symptoms and the results after the conservative treatments. This could lead us to assess the prognoses of TMDs treated conservatively. We investigated 54 patients who have visited the Department of Oral Medicine in PNUH from 1991 to 2001, diagnosed as TMDs and treated conservatively with medications, physical therapies and splints. The treatments were terminated since the subjects have shown much improvements and resumed when TMDs recurred from 1992 to 2002. We researched the diagnoses, symptoms, the number of visits, the treatment plans and the results comparing the time before and after the treatments were carried out and following are the results. 1. Both the primary and the recurred groups improved after having been treated conservatively. 2. Both the primary and the recurred groups showed no differences in pains and MCOs when they first visited though the noises and LOMs turned out to be more serious in primary group. 3. Both the primary and the recurred groups had no differences in pains, noises, LOMs, MCOs when the treatments were over. 4. The results of treatments were not affected by treatment plans, sex, diagnoses in both primary and recurred groups. 5. Most of the patents tended to visit less than 10 times in recurred patients.

General Consciousness on Medical Ethics of Korean Clinicians (임상의(臨床醫)들의 의료(醫療) 윤리관(倫理觀))

  • This is carried out to study on the general consciousness for medical ethics of Korean local and hospital clinicians from March to May, 1999. The subjects are 288 respondents out of 1,500 those who selected by systemic random sampling method from the list of physicians published on 1997. An adequate questionnaire with 21 questions made by author was distributed through mail for those. The characteristics of respondents are as follows. Women are 15%. For like this questionnaire, those who were graduated from. The national universities were more actively concerned than those graduated from the private universities. Christian was 35.4%, Catholic was 28.5% and buddhist was 14.9% and 17.4% had no religion. Now a day in generally, it is clearly confusing status between an ideal thinking and actuality in medical ethical aspects in Korea. They want more developed medical delivery system. The most of physicians are improving on four mind-sets, the mind of love, pity, joy and abandonment as much as they are cognizing that the pains of patients are themselves pains. But they do not want to be the ministry. On the problems of the easy to die, it seems that they have somewhat different opinions according to their religions, respectively;- christians are more responsible than others to keep to the terminal point of life of patients.

  • Toothache is a common complaint in the dental office. Most toothaches have their origin in the pulpal tissues or periodontal structures1). These odontogenic pains are managed well and predictably by dental therapies. One of the most frequent encounters and most confusing phenomena with which the dental diagnostician must deal is the problem of referred pain. The most important step toward proper management of a toothache is to consider that the pain may not be of dental origin. And Patients with orofacial pain, especially those in chronic pain, present a diagnostic and management challenge for the practitioners. There are many structures in the head and neck that can produce heterotopic pains felt in the teeth and other structures. Once referred pain is suspected, the true source of the pain must be located in order to render effective therapy. With increased interest in temporomandibular disorders and orofacial pain, many studies of accurate diagnosis and differential diagnosis about orofacial pain have been established. The purpose of this paper is to present a case for pansinusitis which produced pain referral in teeth and mimicked the symptoms of migraine.

The Study of Vascular Dynamics for the Effect of a Compress Pack on Pain Relief using Magnetic Resonance Angiography (자기공명영상을 이용한 찜질 자극에 의한 통증 완화 효과에 대한 혈류 영상학적 고찰)

  • This study was to investigate the effects of the hot compress pack on alleviating local muscular discomfort, stiffness in limbs as well as the chronic pains such as migraine in terms of hemodynamics. In this study, the hot compress band was put on the neck and the local physiological change on the stimulation site and the cranial blood circulation change were examined. We recruited healthy volunteers (n=8, mean age: 32.13 (4.61)), who participated in the magnetic resonance imaging (MRI) study. Local skin color and temperature were measured for the local effect of the hot compress band and the changes of intra-cranial and extra-cranial blood vessels were examined with MR angiography (MRA) images. The skin temperature increased from $36.4^{\circ}C$ at the rest condition to $36.7^{\circ}C$ and $37.1^{\circ}C$ after 15 min and 30 min stimulation, respectively. The change of the extra-cranial blood vessels between pre-stimulation and post-stimulation of 30 min was significantly increased (+38.8%), while the change of the intra-cranial blood vessels was negligible. In this study, we demonstrated that the hot compress band on the neck yielded the increase of local skin temperature on the stimulation site and it made an effect on the extracranial circulation. In conclusion, the stimulation with a hot compress could facilitate the blood circulation, causing to relieve the muscular discomfort, stiffness in limbs as well as the chronic pains such as migraine.

Mediating Roles of Perceived Money Importance and Gratitude in the Effects of Social Support on Pleasure in Spending for Other-gift (사회적 지원과 돈의 중요성 및 고마움, 그리고 선물구매에서 지불의 기쁨)

  • Purpose - A lot of preceeding studies have focused on the pains that might be felt in spending money, that is an important role in getting psychological safety. Money and social resources can be all for consumers, and they are likely to form a substituting relationship with one another. Being motivated by the idea that spending money for others can come with joy if one's objective is accomplished by social support, this paper aimed to explore the mediating role of the perceived money importance and gratitude in the relationship between social support and pleasure in spending for other-gift. Research Design, Data, and Methodology - In this empirical study, the experimental group is expected to receive social support while the control group is composed of those who are usually indulged in reminiscences of their sweethearts. From the college students, 160 experiment participants were selected and 80 participants of them were assigned to control group as well as to experimental group respectively at random. Empirical study for each of the two groups was performed respectively by means of questionnaire survey. Experimental group data and control group data were combined together to be used for testing hypotheses. Linear structural equation model in Amos was used to verify the hypotheses, and Bootstrap was also used to examine whether there were the mediating roles of the perceived money importance and gratitude or not. Results - From the empirical study, following conclusions could be drawn: First, social support of others makes one perceive the importance of money less; Second, social support of others makes one perceive gratitude to others; Third, less perceived importance of money and gratitude to others can make one feel pleasure in spending for other-gift; and Fourth, less perceived importance of money and gratitude to others can partially mediate the effect of social support from others on the pleasure in spending for other-gift. Conclusions - The outcomes of this study might offer theoretic and managerial implications as follow: Even though many hitherto studies have asserted that spending money usually comes with pains, this study discovered that social support might reduce perceived the importance of money but make others feel gratitude and, thus, one would feel joy in spending money to buy gift for others, and made a contribution to the progress of the theory of pleasure in spending for other-gift. This paper also made contributions toward the development of emotion marketing theory by showing that the effect of social support on the pleasure in spending for other-gift could be partially mediated by the perceived the importance of money and gratitude to others. Based on the above conclusions, it may be affirmed that marketers should help consumers perceive the importance of money less, and help feel gratitude to others by pointing up the support of others to consumers in an attempt to accelerate spending for other-gift.
