Reduction in mechanical allodynia in complex regional pain syndrome patients with ultrasound-guided pulsed radiofrequency treatment of the superficial peroneal nerve

  • The superficial peroneal nerve is vulnerable to damage from ankle sprain injuries and fractures as well as surgery to this region. And it is also one of the most commonly involved nerves in complex regional pain syndrome type II in the foot and ankle region. We report two cases of ultrasound-guided pulsed radiofrequency treatment of superficial peroneal nerve for reduction of allodynia in CRPS patients.

Interview Survey Methods for Moxibustion Treatment of Knee Pain, Neck Pain and Back Pain: Subject to Oriental Doctors in Seoul (슬통(膝痛), 경항통(頸項痛), 요통(腰痛)의 뜸 시술(施術) 방법(方法)에 대한 면접조사 -서울시 한의사를 대상으로-)

  • Objectives : The aim is to comprehend the difference of moxibustion used according to applied body parts (knee pain, neck pain, back pain) to oriental doctors running clinic in Seoul. Methods : A sampling of 288 oriental doctors running clinic in Seoul was done and the doctors were asked to fill out 20 questions by interviewing directly from Nov. 23. 2009 to Jan. 9. 2010. Results : The result shows that moxibustion was used for knee pain and back pain more than neck pain. The type of moxibustion mostly used was sticker moxibustion for all three symptoms. Burn marks were left mostly in knee and back pain. Most large volume of moxa was used to back pain. More than half of the oriental doctors used 1-3 acupoints for one procedure and on average 2-3 sessions was done. Patients with knee pain and back pain had more than 4 sessions compared to neck pain. Treatments were mostly done 3 times a week, and as for total treatment period, it took less than 1 week for acute symptom, and it took more than 1 week and less than 5 weeks for chronic symptom. Conclusions : This research shows that the method of moxibustion used differs according to the area of treatment(knee pain, neck pain, back pain) by survey of oriental doctors through direct interviews.

Treatment of 43 Patients with Buerger's Disease (Buerger환자 43명의 치료 경험)

  • Buerger's disease is a nonatherosclerogic occlusive inflammatory disease of medium and small arteries, and veins, of unknown cause. It occurs predominantly in young males who are habitual tabacco users. These patients often complain of painful ulcerations of their digits. The care of this disease is very difficult when the treatment is delayed. Consequently, early treatments are most important to patients with Buerger's disease. This disease can be treated with sympathetic block such as stellate ganglion block for upper extremities and lumbar epidural block, and lumbar sympathetic block for lower extremities. Intravascular regional sympathetic block can be another method of treatment. However, discontinuation of smoking is the most basic and essential treatment for Buerger's disease. We treated 43 Buerger's disease patients with stellate ganglion block and laser therapy. The treatment was not effective for three patients who definitely required amputation.

  • Numerous treatment modalities for acute or subacute herpes zoster and postherpetic neuralgia have been introduced. Therefore, we updated the treatment modalities by conducting a wide review of the medical literature and we created a new treatment algorithm for herpes zoster and postherpetic neuralgia.

The Effects of Mobilization on the Pain and Recovery of Function of Acute Low Back Pain Patients (관절가동기법이 급성요통환자의 통증과 기능회복에 미치는 영향)

  • Purpose: The purpose of this study was to investigate effects of mobilization combined active movement(SNAGS) on the pain and recovery of function of acute low back pain patients. Methods: The subjects were consisted of 135 patients with acute low back pain. All subjects randomly assigned to mobilization group, stretching exercise group and modality treatment group. The mobilization group received mobilization combined active movement(SNAGS) with modality treatment, exercise group received stretching exercise with modality treatment and modality treatment group received modality treatment. Visual Analogue Scale(VAS) was used to measure patient's pain level and Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. Results: The results of this study were summarized as follows : 1. Visual Analogue Scale(VAS) was mobilization group showed significantly decreased more than comparison group(p<.01) and active treatment group showed significantly decreased more than passive treatment group(p<.01). 2. Patient Specific Functional Scale(PSFS) was mobilization group showed significantly increased more than comparison group(p<.01) and active treatment group showed significantly increased more than passive treatment group(p<.01). Conclusion: It maybe suggested that mobilization combined active movement(SNAGS) is beneficial treatment for acute low back pain patient.

Comparison of treatment outcomes in chronic coccygodynia patients treated with ganglion impar blockade versus caudal epidural steroid injection: a prospective randomized comparison study

  • Background: Coccygodynia is one of the chronic, refractory painful musculoskeletal disorders. Interventional procedures are applied to patients unresponsive to initial treatment in coccygodynia. This study aims to compare the treatment outcomes of ganglion impar block (GIB) and caudal epidural steroid injection (CESI) in patients with chronic coccygodynia. Methods: This study was a prospective randomized comparison study conducted between June 2019 and January 2021. Patients diagnosed with chronic coccygodynia were randomly divided into two groups: the GIB group and the CESI group. The severity of pain, presence of neuropathic pain, and quality of life were evaluated using the Numeric Rating Scale, Leeds Assessment of the Neuropathic Symptoms and Signs Scale, and Short Form-12 Health Survey (SF-12), respectively. Results: A total of 34 patients in each group were included in the final analyses. While there was a significant decrease in pain intensity in both groups in the 3-month follow-up, this decrease was more significant in the GIB group at the 3rd week. There was a significant improvement in the SF-12 physical score and the number of patients with neuropathic pain in both groups in the 3rd week, but this improvement was not observed in the 3rd month. Conclusions: Although GIB may provide more pain relief in short term, both GIB and CESI are useful treatment methods in coccygodynia unresponsive to more conservative treatments.

Retrospective Study on Effect of Chuna Therapy, Also Known as Supine JS Cervical Therapy, Combined with Korean Medicine Treatment on Neck Pain and Headache Caused by Whiplash Injury (편타성 손상에 의한 경항통 및 두통 환자의 추나요법 병행 한방복합치료의 효과: 후향적 관찰연구 - 앙와위 경추 JS 신연 교정기법을 중심으로)

  • Objectives To observe the pain reduction effect of Korean medical treatment combined with supine JS cervical therapy on neck pain and headache caused by whiplash injury. Methods The medical records of 30 patients suffering from neck pain and headache caused by traffic accidents from August 1, 2020 to October 30, 2020 were studied. 15 patients receiving supine JS cervical therapy combined with Korean medical treatment were assigned to group A and another 15 patients receiving Korean medical treatment but without Chuna therapy were assigned to group B. The following patient data were analyzed retrospectively - age, sex, and number of admission days, neck pain and headache visual analogue scale (VAS), and neck disability index (NDI). Statistical analysis was conducted using IBM SPSS 25.0. for Windows. A p-value≤0.05 was considered to be statistically significant. Results Both neck pain and headache VAS score in group A and B showed a statistically significant decrease. The changes in neck pain and headache VAS score in group A was statistically significant higher compared to those of group B. NDI in group A showed a statistically significant decrease, but NDI in group B did not decreased statistically significant. Conclusions Korean medicine treatment effectively reduces neck pain and headache. Especially, the pain reduction effect is better in patients receiving Korean medicine treatment combined with supine JS cervical therapy than in those receiving Korean medicine treatment without supine JS cervical therapy.

Combination Treatment of Korean Medicine with Chungpochukeo-tang for Acute Pelvic Pain by Ruptured Ovarian Cyst : 2 Cases Report (난소낭종 파열 후 발생한 급성 골반통에 대해 청포축어탕 가감방을 포함하는 한의치료 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on acute pelvic pain followed by ruptured ovarian cyst. Methods: The patients who diagnosed with ruptured ovarian cyst complained of acute pelvic pain, lower abdominal pain, abdomen distention, constipation, lower back pain and dysuria. They received combination treatment of Korean medicine during hospitalization. The treatment included Chungpochukeo-tang, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through Numeric Rating Scale (NRS). Results: After the treatment, the clincial symptoms such as acute pelvic pain, lower abdominal pain, abdomen distention, constipation, lower back pain and dysuria were improved. Conclusions: This case report shows that the combination treatment of Korean medicine with Chungpochukeo-tang may be effective for treating acute pelvic pain by ruptured ovarian cyst.

  • Authors have experienced the treatment for the uppermost frequent cases, herpes zoster, frozen shoulder and low back pain among those who visited our pain clinic. 1. Herpes zoster The Patients who received the treatment within 4 weeks of the onset of the disease, not only healed herpes without leaving post herpetic neuralgia but also crust formation was fast. 2. Frozen shoulder Over 90% of the patients who received suprascapular nerve block accompanied with trigger points electric stimulation was very efficient procedure, futhermore, authors felt keenly the necessity each patients co-operation and individual endurance for this treatment. 3. Low back pain An epidural steroid administration to the patients who did not have any organic disturbances was effective treatment. If this non-effective, search other cause of the disease or operation was advisable one.

Study on clinical effects of indirect moxibustion at Geogol (LI16) on the shoulder pain. (거골(巨骨)(LI16)혈(穴)에 간접구(間接灸)를 시술한 견비통(肩臂痛) 환자 11례(例)에 대한 증례보고)

  • Objective : This study is designed to find out the effects of indirect moxibustion at Geogol (LI16) on the shoulder pain. Methods : The patients were hospitalized Donghwa oriental medical clinic from 19th Sep. 2006 to 4th Oct. 2006, and treated with indirect moxibution at Geogol (LI16) on the shoulder pain. Results : 1. The effects of treatment by duration was follow : Below 1 month duration was improved highly. 2. The effects of treatment by condition of patients was follow : After treatment, the percentage of above "Good" level was 81%. 3. The effects of treatment by pain region of Meridian part. was follow : pain region of LI(手陽明大腸經) Meridian part was improved highly. Conclusion : According to the results, indirect moxibustion at Geogol (LI16) may be good effects on the shoulder pain.
