Patients' and parents' concerns and decisions about orthodontic treatment

  • Objective: Patients' and parents' expectations are important in orthodontic treatment decision making. The literature generally demonstrates the perceived benefits of orthodontic treatment, but patients' and their parents' concerns about orthodontic treatment have not been investigated comprehensively. The aim of this study was to identify patients' and parents' concerns about orthodontic treatment and compare them according to sex, age, and treatment demand level. Methods: One hundred and eighty-nine children and their parents were interviewed about concerns related to orthodontic treatment. Patients and parents were asked about orthodontic treatment decisions. Answers were recorded as "yes," "no," or "don't know." Chi-squared and Fisher's exact tests were used to compare concerns between age groups, sexes, and treatment demand levels. Kappa statistics were used to assess agreement between patients and their parents. Results: Concerns about orthodontic treatment were gathered under 10 items as follows: "feeling pain," "the appearance of braces," "being teased," "avoiding smiling," "speech problems," "dietary changes," "problems with transportation," "economic problems," "long treatment duration," and "missing school." There was no statistically significant difference in concerns between the sexes or age groups. Some concern items and treatment demand were inversely related in patients. Conclusions: The results of this study demonstrate patients' and parents' concerns about orthodontic treatment. Differences between the concerns of patients with different treatment demands imply that children might reject orthodontic treatment because of their concerns. Appropriate consultation of patients addressing their concerns may help reduce anxiety and improve the acceptance of treatment.

구강안면영역에서의 경피성 신경자극과 전기침자극요법이 전류인지역치에 미치는 영향 (The Effects of Transcutaneous Electrical Nerve Stimulation and Electroacupuncture Stimulation Therapy on the Current Perception Threshold of Orofacial Region)

  • 구강안면동통 환자의 치료법으로 널리 쓰이는 전기요법은 연조직과 신경계 구조물에 대한 치료 시 중요한 역할을 담당하고 있다. 저자는 현재 구강안면동통의 치료법으로 널리 쓰이고 있는 전기요법들 중 경피성 신경자극(Transcutaneous electric nerve stimulation)과 전기침자극요법(Electroacupuncture stimulation therapy)이 각각의 신경섬유에 미치는 효과를 평가하고자 정상 성인 남녀 29명에게 경피성 신경자극 및 전기침자극을 시행하고 시행 전 및 시행 후 삼차신경 영역의 3가지 종류($A{\beta}$, $A{\delta}$, C fiber)의 신경섬유의 전류인지역치(CPT) 변화를 측정하여 그 차이점을 분석하였으며 이를 대조군과 비교하였다. 경피성 신경자극 및 전기침자극 모두에서 대조군에 비해 삼차신경 영역의 모든 신경섬유에 걸쳐 고른 전류인지역치의 증가를 나타내었으며, 경피성 신경자극과 전기침자극 후의 전류인지역치 변화량은 서로 유의할만한 차이를 나타내지 않았다. 이는 경피성 신경자극 과 전기침자극 모두 3가지 종류($A{\beta}$, $A{\delta}$, C fiber)의 감각신경섬유의 전류인지역치에 영향을 미치며, 구강안면동통의 감소에 효과적으로 사용될 수 있으리라 생각된다.

  • Motion at the sacroiliac joints(SIJ) has been reported in the medical literature since the mid-19th century. However, for many years authorities vie~d that no movement occurred at the sacroiliac joints and therefore were not clinically significant. Recent contributions from both the basic and clinical sciences caused a change in perception of the role of the sacroiloiac joint in low back pain(LBP). Movement within the sacroiliac joint is now generally recognized, although it is only a small amount. Controversy continues as to the type of motion available and the axes of motion, and continues as to the ability of a clinician to identify a significant sacroiliac dysfunction. Dysfunction of the pelvic girdle is complex and not easily understood. It is common to find serveral dysfunctions within the same pelvic girdle. Each needs to be individually diagnosed and appropriately treated. The diagnostic and therapeutic system designed by Philip E. Greenmam, D. O. allows the operator to deal with any combination of physical findings that are found within the pelvic girdle Restoration of pelvic girdle function within the walking cycle is a major therapeutic goal, particularly from the biomechanical postural-structural model.

Single-flap versus double-flap approach for periodontal pocket reduction in supraosseous defects: a comparative study

  • Purpose: The single-flap approach (SFA) is a minimally invasive technique with limited mucoperiosteal flap elevation to gain access to the buccal/palatal aspects, thus limiting post-surgical complications. The purpose of the present study was to gain insights into the impact of the SFA over the double-flap approach (DFA) on periodontal flap treatment outcomes and patient compliance in terms of discomfort and time taken for surgical procedures. Methods: Twenty patients with persistent probing pocket depths of ≥5 mm were scheduled for the SFA (test site) and for the DFA (control site). All the clinical periodontal parameters were recorded at baseline, 3 months, and 6 months. Radiographic bone level (cone-beam computed tomography) was evaluated at baseline and 6 months. Patients' postoperative pain perception and wound healing were also assessed. Results: The SFA showed a significant reduction in periodontal pocket depth, gain in clinical attachment level (CAL), and gain in bone level when compared with the DFA. The SFA substantially improved wound healing and induced less postoperative pain than the DFA. Conclusions: The SFA resulted in substantial improvement in the composite outcome measures, as shown by a reduction in pocket depth with minimal gingival recession, gain in CAL, early wound healing, less postoperative discomfort, and better patient-centered outcomes.

Effects of Psychological Stress and Living Environments on Perceived Hand Deformities: a Community-Based Cohort Study

  • Objective: Despite known associations between negative body image and health declines in chronic pain patients, few studies have examined longitudinal associations between psychological stress and perceived hand deformities. This study examined whether psychological distress was associated with hand deformities 4 years later and if rural and urban adults differed in the association. Design: A community-based cohort study. Methods: Community dwelling adults (mean age=51.97, 52.3% women) in a rural (n=2968) and urban area (n=2784) provided demographic data at baseline and, in the 4-year follow-up, responded to questionnaires about psychological distress. Perceived hand deformities were assessed at the 8-year follow-up. Linear regression was conducted to examine the effects of psychological distress on hand deformities and moderation by residential areas. Results: The perceived hand deformities were more likely among those with severe psychological stress, hand osteoarthritis, or any chronic disease condition (p<0.01) but less likely among those with younger age, higher education, or income (p<0.01). The regression results showed that psychological distress predicted more perceived hand deformities 4 years later even after adjusting for demographic and health covariates (p<0.01). The residential areas did not significantly moderate the association between distress and hand deformities. Conclusions: This study suggected that psychological distress may trigger later perception of hand deformities in both urband and rural adults. The findings indicate that stress management interventions that are customizable to regional contexts may be effective at preventing negative body image related health problems of community-dewelling adults.

A Study on the General Public's Perceptions of Dental Fear Using Unstructured Big Data

  • Background: This study used text mining techniques to determine public perceptions of dental fear, extracted keywords related to dental fear, identified the connection between the keywords, and categorized and visualized perceptions related to dental fear. Methods: Keywords in texts posted on Internet portal sites (NAVER and Google) between 1 January, 2000, and 31 December, 2022, were collected. The four stages of analysis were used to explore the keywords: frequency analysis, term frequency-inverse document frequency (TF-IDF), centrality analysis and co-occurrence analysis, and convergent correlations. Results: In the top ten keywords based on frequency analysis, the most frequently used keyword was 'treatment,' followed by 'fear,' 'dental implant,' 'conscious sedation,' 'pain,' 'dental fear,' 'comfort,' 'taking medication,' 'experience,' and 'tooth.' In the TF-IDF analysis, the top three keywords were dental implant, conscious sedation, and dental fear. The co-occurrence analysis was used to explore keywords that appear together and showed that 'fear and treatment' and 'treatment and pain' appeared the most frequently. Conclusion: Texts collected via unstructured big data were analyzed to identify general perceptions related to dental fear, and this study is valuable as a source data for understanding public perceptions of dental fear by grouping associated keywords. The results of this study will be helpful to understand dental fear and used as factors affecting oral health in the future.

Repeatability of the Measurement of Electrical Taste Detection Thresholds in Healthy Young Females

  • Purpose: The aim of this study was to assess the repeatability of the electrical measurement of taste detection on different dates and in different sessions in healthy young females. Methods: The sites of electrical stimulation were the tip of the tongue, the posterolateral border of the tongue and the soft palate on the right side unilaterally. The measurements were repeated over three consecutive days, three sessions per day and three times for each session in seventeen healthy females. The repeatability of the measurement was assessed by the intraclass correlation coefficient (ICC). Results: In the dB unit, the ICC of the tip of the tongue and the soft palate was good (61.03 and 66.03, respectively); however, the lateral border of the tongue was a little lower (58.07). In the ㎂ unit, all three test sites had poor ICC. Variability was more significantly associated with the subject factor than with other factors such as trials, sessions and days in dB and ㎂ units. Conclusions: Electrogustometry, which measures electrical taste detection thresholds in the dB unit, is repeatable and acceptable for clinical use in assessing taste function in healthy young females.

Traumatic Brain Injury-Induced Mixed Chemosensory Disorder: a Case Study on Taste and Smell Dysfunction

  • We present a case report of a 52-year-old male patient who suffered head trauma in a car accident and subsequently experienced taste and smell disorders. Following the accident, the patient reported difficulty detecting salty and sour tastes and diminished olfactory perception. Neurosurgical evaluation revealed subarachnoid and subdural hemorrhages, while otolaryngology investigations revealed hyposmia-a decreased sense of smell. Upon referral to the Department of Oral Medicine, a comprehensive assessment revealed a general bilateral reduction in taste sensation, particularly ageusia for salty taste. Electric taste-detection thresholds significantly exceeded the normal ranges. Integrating our findings from neurosurgery, otolaryngology, and oral medicine resulted in a diagnosis of mixed chemosensory disorder attributed to head trauma. This case highlights the intricate interplay of alterations in taste and smell following head injury, emphasizing the significance of multidisciplinary evaluations in diagnosing mixed chemosensory disorders resulting from traumatic brain injury.

입원중인 암환자에 대한 통증관리의 적절성평가 - 한 3차 의료기관 내과 전공의를 대상으로 - (Evaluation of the Adequacy of Pain Management in the Admitted Cancer Patients)

  • 목적 : 암환자에서 가장 흔히 겪게 되고, 심각한 증세는 통증이다. 진행성 암인 경우 $60{\sim}90%$의 환자들이 통증으로 고상하게 되며, WHO통계에 따르면 전세계적으로 4백만 정도의 암환자들이 통증으로 고생하고 있다. 통증관리원칙에 따르면 환자 $70{\sim}90%$이상에서 적절한 통증조절이 이루어질 수 있음에도 불구하고 이들 중 $60{\sim}70%$의 암환자들이 적절한 통증치료를 받지 못하고 있다. 본 연구는 1) 암환자의 통증 빈도 2) 환자와 담당전공의에서 통증인지의 일치 여부 3) 암환자의 통증치료에서 진통제 투약의 적절성 등을 조사함으로써, 암환자 통증에 대한 이해와 통증관리에 대한 지침을 마련하는데 기초자료를 제공하고자 하였다. 대상 및 방법 : 서울대학교병원 내과병동에 입원한 암환자들의 통증 빈도, 담당전공의와 환자의 통증인지 일치도, 통증 관리의 적절성 등을 평가하기 위해서 환자와 담당전공의를 대상으로 설문조사를 시행하고, 진료기록을 평가하였다. 결과 : 7일 이상 내과병동에 입원해 있는 암환자 59명중 43명(62.8%)이 통증을 호소하였다. 전공의들은 담당 암환자들의 통증을 실제보다 과소평가하고 있었으며, 진통제 투약 원칙이 지켜지지 않은 경우도 상당수 있었고, 환자의 통증이 심해질수록 통증에 대한 치료가 더 불충분한 경향이 있었다. 결론 : 암환자에 있어서 통증은 아직까지 낮게 평가되고 있으며 통증 치료는 여전히 불충분하다, 이와 같은 점을 고려할 때, 통증의 평가 및 치료에 대한 더 많은 교육이 필요할 것으로 생각된다.

Intraosseous anesthesia in symptomatic irreversible pulpitis: Impact of bone thickness on perception and duration of pain

  • Background: Intraosseous anesthesia (IO) allows the anesthetic solution to be injected directly into the cancellous bone. The anesthetic solution immediately reaches the periapical region, and thus the axonal area of the nerve, where it can temporarily disable the sodium pump. The effect is felt almost without any time delay, and only a small amount of anesthetic solution is required. Methods: This study aims to investigate the efficacy of IO using the AnestoⓇ device after infiltration anesthesia (IA) and/or inferior alveolar nerve block anesthesia (IANB) failed to work in symptomatic irreversible pulpitis (hot tooth). The 33 patients included in the study were treated additionally with 1.7 ml articaine hydrochloride with 1:100,000 epinephrine hydrochloride (UltracainⓇ D-S, Sanofi-Aventis, Frankfurt, Germany) IO. Results: The electrical pulp test showed that 95.76% of the volunteers reacted positively to the combination of IANB or IA with the IO. In women, the additive IO was effective at 97.22%. In men, the IO led to pain elimination in 94.00% of cases. The duration of the IO was less than a quarter of an hour (13.03 min). The IO worked longer in women than in men (13.61 min vs. 12.33 min). Overall, more than every third tooth that needed trepanation was located in the posterior area of the mandible (36.4%). Treatment of hot teeth in this area was associated with an increased pulse rate and increased residual pain. There was a moderate correlation (Spearman-Rho [IRI] = 0.280) between the Visual Analog Scale (VAS) score and bone density, and a significant correlation (IRI = 0.612) between subjective residual pain and bone width. The IO resulted in a moderate, transient increase in the pulse rate by approximately 20 bpm. This is similar to the temporary increase in heart rate after conventional anesthesia techniques in non-preloaded patients and can be considered clinically irrelevant. Conclusion: IO with the AnestoⓇ device as an extension and deepening of local pain elimination is recommended for the treatment of hot teeth.
