Purpose: Pain caused by osteoarthritis is primarily nociceptive pain; however, it is considered that a component of this pain is due to neuropathic pain (NP). We investigated the effects of total hip arthroplasty (THA) in patients with NP diagnosed by the PainDETECT questionnaire. Materials and Methods: One hundred sixty-three hips (161 patients) were evaluated. All patients were asked to complete the PainDETECT questionnaire based on their experience with NP, and clinical scores were evaluated using the Japanese Orthopaedic Association (JOA) Hip Score before and after THA. Results: The patients of 24.5% reported NP before THA; 5.5% reported NP 2 months after THA. Prior to THA, there was no significant correlation between the PainDETECT score and the radiographic severity; however, there was a significant correlation between the PainDETECT score and JOA score. NP at 2 months after THA was not significantly correlated with pain scores at 1 week after THA; however, a significant correlation was observed between the preoperative pain score and NP at 2 months after THA. Conclusion: THA was useful for relieving nociceptive pain and for relieving NP in patients with hip osteoarthritis. Preoperative pain was a risk factor for NP after THA. Controlling preoperative pain may be effective for reducing postoperative NP.
Background Postoperative pain is one of the most common concerns of patients undergoing hair transplantation surgery. Because most patients are satisfied with the cosmetic improvement after transplantation, amelioration of postoperative pain would help to increase patient accessibility to hair restorative surgery and greatly impact patient satisfaction with the final cosmetic results. This study was performed to investigate postoperative pain after hair transplantation. Methods In total, 241 patients (202 who underwent follicular unit transplantation [FUT] and 39 who underwent follicular unit extraction [FUE]) were eligible for the study. Postoperative pain was evaluated on postoperative days 1, 2, 3, 4, 5, and 7 using the Wong-Baker Faces Pain Scale. The patients' medical records were retrospectively reviewed for information on the harvesting method, number of transplanted grafts, size of donor design, and laxity, elasticity, and glidability of the scalp in relation to postoperative pain. Results Postoperative pain after hair transplantation, assessed with the Wong-Baker Faces Pain Scale, seemed to provide very subjective results. None of the variables were correlated with postoperative pain in the FUT group. Such pain, however, tended to disappear by postoperative day 3. Patients in the FUE group experienced significantly less severe pain than those in the FUT group. Conclusions Postoperative pain was significantly less severe in patients whose donor hair was harvested by the FUE than FUT method. Postoperative pain had almost disappeared by postoperative day 3 in the FUT group, whereas only minimal pain was present even on postoperative day 1 in the FUE group.
Cutaneous stimulation and distraction are independent nursing interventions used in various painful conditions, which is explained by gate control theory. This study was aimed at identifying the effect of cutaneous stimulation, distraction and combination of cutaneous stimulation and distraction on the reduction of intravenous injection pain levels of chemotherapy patients. Repeated measurement post test research design was used for one group. Fifty-three cancer patients who received intravenous chemotherapy regulary in outpatient injection rooms of D medical center and Y medical center in Taegu were studied from June 23, 1997 to July 12, 1997. First the intravenous injection pain level of the control period was measured. Second, the intravenous injection pain level of the experimental period using cutaneous stimulation was measured. Third, the intravenous injection pain level of the experimental period using distraction was messured. Fourth, the intravenous injection pain level of the experimental period using a combination of cutaneous stimulation and distraction was measured. The instruments used for this study were a visual analogue pain scale as subjective pain measurement and an objective pain behavior checklist. Analysis of data was done by use of repeated measure ANOVA, bonferni, t-test, and F-test. The results of this study were summerized as follows : 1) The first hypothesis that the subjective pain score of intraveneous injection pain in the experimental period with cutaneous stimulation will be lower than in the control period was rejected. 2) The second hypothesis that the objective pain behavior score of intravenous injection pain in the experimental period with cutaneous stimulation will be lower than in the control period was accepted(F=24.23, p=0.0001, Bornferni p<.05). 3) The third hypothesis that the subjective pain score of intraveneous injection pain in the experimental period with distraction will be lower than in the control period was rejected. 4) The fourth hypothesis that the objective pain score of intravenous injection pain in the experimental period with distraction will be lower than in the control period was accepted(F=24.23, p=0.0001, Bornferni p<.05). 5) The fifth hypothesis that the subjective pain score of intravenous injection pain in the experimental period with combination of cutaneous stimulation and distraction will be lower than in the control period was accepted(F=3.04, p=0.031, Bonferni p<.05). 6) The sixth hypothesis that the objective pain score of intravenous injection pain in the experimental period with combination of cutaneous stimulation and distraction will be lower than in the control period was accepted(F=24.23, p=0.0001, Bonferni p<.05).
PURPOSE: This study examines the therapeutic effect of functional adjustment procedure therapy (FAPT), by comparatively analyzing behavioral disorders due to shoulder pain and subjective pain intensity subsequent to therapy, in patients with shoulder pain. METHODS: The intervention was performed on 48 employee patients with shoulder pain, Patients were administered 16 sessions of FAPT for 8 weeks, twice a week for 30 minutes per session. This study applied the neck pain and disability scale (NPDS) as a measurement tool, and used the questionnaire to measure subjective pain intensity to investigate the difference before and after administering FAPT to employee patients with shoulder pain. RESULTS: Statistically significant difference was obtained between the mean difference in shoulder pain disorder, before and after FAPT. Assessing gender and age differences in the reduction effect of shoulder pain, showed significant difference in 8 of the 10 factors examined. Finally, examining the difference in the effect of reducing subjective shoulder pain by gender and age, we observed that gender-based disability reduction was significant for all 12 factors examined. CONCLUSION: Taken together, the results of this study, validate the therapeutic efficacy of FAPT for patients with shoulder pain. We believe that this data will provide basic information to understand the health conditions and psychological variables of patients with shoulder pain.
Background: This study aims to examine the effects of sling exercise and common exercise on the pain level of various body parts. Methods: This study divided the subjects into a sling-exercise group and a common exercise group to for each group across the measurement times after practicing exercises twice per week for 8 weeks (total 16times). To compare the means of the experimental group and the control group on each variable, the ANOVA for repeated measure was used and if there was any significant difference across the measurement times, post-hoc comparisons were conducted for the difference. The T-tests for group difference according to exercise type on each of the variables showed the results as follows. Results: The results of this study suggest that there were significant difference in pain in shoulder pain, but no significant difference could be found in the Thoracic pain & Knee joint pain. There was no significant difference to distinguish between the sling exercise and the common exercise. Conclusions: Currently the sling exercise is not spread out widely in Korea, so that the recognition and studies of the sling exercise is still lacking. Hence, this study can hopefully facilitate popularity of the sling exercise and systematic prospect studies on it in the near future.
Purpose : This study was performed to evaluate the effects of magnetic therapy (MT) on pain threshold, blood flow, and balance in patients with knee osteoarthritis. Methods : A single-blind, randomized controlled trial (RCT) was conducted with 30 patient with knee osteoarthritis. They were randomly allocated 2 groups; magnetic therapy group (MTG; n=15) and placebo magnetic therapy group (PG; n=15). The MTG group received 30 minutes magnetic therapy and 20 minute conservative physical therapy (Hotpack, ICT), magnetic therapy was conducted in magnetic therapy device (OM-100, NUGA, Korea). In the placebo magnetic group received 30 minutes placebo magnetic therapy and 20 minute conservative physical therapy. Each group performed 50 minutes a day 3 times a week for 8 weeks. The primary outcome pressure pain threshold test, blood flow, balance ability were measured by a pressure threshold meter (Commander algometer, JTECH medical, USA), laser dofler image (Moor LDI2-IR, Moor instruments, USA), balance measurement system (BioRescue, Marseille, France). The measurement were performed before and after the 8 weeks intervention period. Results : Both groups demonstrated significant improvement of outcome in pain threshold, blood flow, and balance ability during intervention period. magnetic therapy group revealed significant differences in pain threshold, blood flow, and balance as compared to the placebo magnetic therapy group groups (p<.05). Our results showed that magnetic therapy was more effective than placebo therapy on pain threshold, blood flow, and balance in patients with knee osteoarthritis. Conclusion : Our findings indicate that magnetic therapy can improve pain threshold, blood flow, and Balance, highlight the benefits of magnetic therapy. This study will be able to be used as an intervention data for recovering pain threshold, blood flow, and balance in patients with knee osteoarthritis.
Purpose: The aim of this study was to apply suboccipital muscle inhibition combined with neck muscle stabilization exercise to 20~30s IT industry employees who suffer from chronic non-specific neck pain. Methods: This study was designed as single-blind and randomized controlled trial. The study participants were 20~30s IT industry employees with chonic non-specific neck pain (VAS 3/10) who were divided into an experimental group (n= 20) subjected to suboccipital muscle inhibition with neck muscle stabilization exercise, and control group (n= 20); suboccipital muscle inhibition only. The intervention was applied three times per week for eight weeks. The neck pain·pressure pain threshold·range of motion, and disability index were measured at the 1st, 8th, and 10th week at follow up, then analyzed with an analysis of variance(ANOVA) using the SPSS program. Results: The total number of study participants was 37 (experimental group 19, mean age 34.6±5.3, control group 18, mean age 35.7±4.9). The comparison and analysis of change in VAS, the pressure pain threshold, and the range of motion except the extension (p>.05) revealed a statistically significant decrease between groups over eight weeks and follow up measurement (p<.01). Regarding the within the group differences, the right side of the neck pressure pain threshold showed a statistically significant decrease over eight weeks in the control group (p<.01). The right and left lateral flexion, and the right and left rotation were statistically significant for the experimental group over eight weeks and follow up measurement, but only the left lateral flexion (p<.05) for the control group over eight weeks. The neck disability index showed a slight decrease but this was not satistically significant for the between-grop or the within-group differences (p>.05). Conclusion: The intervention of suboccipital muscle inhibition and a neck muscle stabilization exercise are more beneficial for neck pain and the range of motion than the application of suboccipital muscle inhibition alone.

Background: Parkinson's disease (PD) is a neurodegenerative disorder that is the second most common disorder after Alzheimer's disease. PD includes both "motor" and "non-motor" symptoms, one of which is pain. The aim of this study was to investigate the clinical characteristics of pain in patients with PD. Methods: This cross-sectional study included 250 patients diagnosed with PD, 70% of which had mild to moderate PD (stages 2/3 of Hoehn and Yahr scale). The average age was 67.4 years, and the average duration since PD diagnosis was 7.1 years. Relevant data collected from PD patients were obtained from their personal medical history. Results: The prevalence of pain was found to be high (82%), with most patients (79.2%) relating their pain to PD. Disease duration was correlated with the frequency of intense pain (R: 0.393; P < 0.05). PD pain is most frequently perceived as an electrical current (64%), and two pain varieties were most prevalent (2.60 ± 0.63). Our findings confirm links between pain, its evolution over time, its multi-modal character, the wide variety of symptoms of PD, and the female sex. Conclusions: Our results demonstrated that the pain felt by PD patients is mainly felt as an electrical current, which contrasts with other studies where the pain is described as burning and itching. Our classification is innovative because it is based on anatomy, whereas those of other authors were based on syndromes.

Background: Neck pain is a common problem in the general population and second only to low back pain in musculoskeletal problems. The aim of this study is to compare three different types of exercise training in patients with chronic neck pain. Methods: This study was conducted on 45 patients with neck pain. Patients were divided into 3 groups: Group 1 (conventional treatment), Group 2 (conventional treatment plus deep cervical flexor training), and Group 3 (conventional treatment plus stabilization of the neck and core region). The exercise programs were applied for four weeks, three days a week. The demographic data, pain intensity (verbal numeric pain scale), posture (Reedco's posture scale), cervical range of motion ([ROM] goniometer), and disability (Neck Disability Index [NDI]) were evaluated. Results: In all groups, a significant improvement was found in terms of pain, posture, ROM, and NDI values in all groups (P < 0.001). Between the groups, analyses showed that the pain and posture improved more in Group 3, while the ROM and NDI improved more in Group 2. Conclusions: In addition to conventional treatment, applying core stabilization exercises or deep cervical flexor muscle training to patients with neck pain may be more effective in reducing pain and disability and increasing ROM than conventional treatment alone.
Most conventional instruments measuring disability rely on total score by simply adding individual item responses, which is dependent on the items chosen to represent the underlying construct (test-dependent) and a test statistic, such as coefficient alpha for the estimate of reliability, varying from sample to sample (sample-dependent). By contrast, item response theory (IRT) method focuses on the psychometric properties of the test items instead of the instrument as a whole. By estimating probability that a respondent will select a particular rating for an item, item difficulty and person ability (or disability) can be placed on same linear continuum. These estimates are invariant regardless of the item used (test-free measurement) and the ability of sample applied (sample-free measurement). These advantages of IRT allow the creation of invariantly calibrated large item banks that precisely discriminate the disability levels of individuals. Computer adaptive testing (CAT) method often requiring a testing algorithm promise a means for administering items in a way that is both efficient and precise. This method permits selectively administering items that are closely matched to the ability level of individuals (measurement precision) and measuring the ability without the loss of precision provided by the full item bank (measurement efficiency). These measurement properties can reasonably be achieved using IRT and CAT method. This article aims to investigate comprehensive overview of the existing disability instrument for back pain and to inform physical therapists of an alternative innovative way overcoming the shortcomings of conventional disability instruments. An understanding of IRT and CAT method will equip physical therapist with skills in interpreting the measurement properties of disability instruments developed using the methods.
