Mixed Venous oxygenation saturation[SvO2 is a variable determined in part by the externally controlled factors and in part by the patient during CPB. I monitored the SvO2 and tested it as a parameter for the regulation of pump output and as a criteria for the need of inotropics after CPB. With the help of SvO2, I increased the pump flow especially during rewarming for more optimal oxygenation of cells. After CPB, the calculated cardiac index was used as an indicator for the need of inotropic support with greater accuracy and without any clinical problems. I conclude that the SvO2 is an easily checkable variable and a good indicator for optimal oxygenation at cell level, and can be used as an objective criteria for the need of postoperative inotropic support.
YBa$_2$Cu$_3$O$_{x}$ (Y123) single crystal was grown by TSMG (top seeded melt growth) method. Physical properties of Y123 single crystal were dependent on the shape and distribution of Y2BaCuOx (Y211) in it and on the oxygenation temperature and the holding time. In this work, It was investigated to an optimal oxygenation condition and a dependence of melting temperature and holding time on physical properties. The optimal oxygenation condition was found that it was heat-treated for 30 hours at 45$0^{\circ}C$, Also it was found that the critical current density and the size of Y211 were increased with the melting temperature and the holding time.e.

Acute fulminant myocarditis (AFM) occurs as an inflammatory response to an initial myocardial insult. Its rapid and deadly progression calls for prompt diagnosis with aggressive treatment measures. The demonstration of its excellent recovery potential has led to increasing use of mechanical circulatory support, especially extracorporeal membrane oxygenation (ECMO). Arrhythmias, organ failure, elevated cardiac biomarkers, and decreased ventricular function at presentation predict requirement for ECMO. In these patients, ECMO should be considered earlier as the clinical course of AFM can be unpredictable and can lead to rapid haemodynamic collapse. Key uncertainties that clinicians face when managing children with AFM such as timing of initiation of ECMO and left ventricular decompression need further investigation.
급성호흡부전증후군은 치료가 어렵고 치명율이 높다. 인공호흡기 치료에 반응하지 않는 중증의 경우 체외막 산화기(Extracorporeal membrane oxygenation, ECMO)가 적용되고 있다. 교통사고로 다발성 외상을 받은 21세 여자가 흉부 방사선상 양측폐에 심한 침윤 소견과 동맥혈가스 검사상 급성호흡부전증후군(Acute respiratory distress syndrome, ARDS)으로 판단되어 정맥동맥 체외산화기(Veno- arterial Extracorporeal Membrane Oxygenation, VA ECMO) 치료를 시작하였다. ECMO 적용 2일째 흉부 방사선상 폐 침윤 소견 악화되면서 동맥혈 산소 포화도(Systemic oxygen saturation, $SaO_2$)가 85% 미만으로 저하되었다. 이에 중추신경계와 폐의 산소화를 향상시키기 위해 ECMO의 유출로에 곁회로를 이용하여 우측 경정맥에 연결하여 정맥-정맥동맥 ECMO (Veno-venoarterial ECMO, V-VA ECMO)로 전환하였고 이후 동맥혈 산소 분압($PaO_2$) 65 mmHg, $SaO_2$ 94%로 향상되었다. V-VA ECMO로의 전환은 전신에 산소 공급을 늘리기 위해 고려해 볼 수 있는 방법으로 생각된다.
Fatal anaphylactic reactions to protamine sulfate during cardiac surgery are very rare. We report a case of catastrophic bronchial spasm due to an anaphylactic reaction to protamine. The patient was managed successfully using a bronchodilator, steroid treatment, and extracorporeal membrane oxygenation.
Background Negative-pressure wound therapy (NPWT) is believed to accelerate wound healing by altering wound microvascular blood flow. Although many studies using laser Doppler have found that NPWT increases perfusion, recent work using other modalities has demonstrated that perfusion is reduced. The purpose of this study was to investigate the influence of NPWT on tissue oxygenation of the foot, which is the most sensitive region of the body to ischemia. Methods Transcutaneous partial pressure of oxygen ($TcpO_2$) was used to determine perfusion beneath NPWT dressings of 10 healthy feet. The sensor was placed on the tarso-metatarsal area of the foot and the NPWT dressing was placed above the sensor. $TcpO_2$ was measured until it reached a steady plateau state. The readings obtained at the suction-on period were compared with the initial baseline (pre-suction) readings. Results $TcpO_2$ decreased significantly immediately after applying NPWT, but gradually increased over time until reaching a steady plateau state. The decrease in $TcpO_2$ from baseline to the steady state was 2.9 to 13.9 mm Hg (mean, $9.3{\pm}3.6$ mm Hg; $13.5{\pm}5.8%$; P<0.01). All feet reached a plateau within 20 to 65 minutes after suction was applied. Conclusions NPWT significantly decrease tissue oxygenation of the foot by 2.9 to 13.9 mm Hg. NPWT should be used with caution on feet that do not have adequate tissue oxygenation for wound healing.
Extra-corporeal membrane oxygenation (ECMO) has the potential to rescue patients in cardiac arrest or respiratory failure. ECMO has two systems such as veno-arterial and veno-venous circulation. In cardiac arrest resulting from acute myocardial infarction, veno-arterial ECMO is mandatory for systemic circulation and oxygenation. A 75-year old female patient underwent primary coronary intervention for acute myocardial infarction. Despite successful revascularization, recurrent ventricular tachycardia and heart failure were progressing. We performed a veno-arterial ECMO through the femoral artery and vein, then the patient seemed to be stable clinically. However, laboratory studies, echocardiography, and vital signs indicated multi-organ failure and decreasing cardiac function. We found out an error that we performed veno-venous ECMO instead of veno-arterial ECMO. We added a femoral artery cannula and exchange the circuit system to veno-arterial ECMO. While the systemic circulation seemed to be recovered, the left ventricular function was decreased persistently. A hypovolemia resulting from pulmonary hemorrhage was occurred, which lead to ECMO failure. The patient died of cardiac arrest and multi-organ failure 23 hours after ECMO. Because the color of arterial and venous circuits represent the position and efficacy of ECMO, if unexpected or abnormal circuit colors are detected, prompt and aggressive evaluation for ECMO function is mandatory.
52세 여자 환자가 호흡곤란을 주소로 내원하였다. 환자는 2개월 전에 침습성 흉선종으로 광범위 흉선절제술을 받았다. 전산화단층촬영에서는 양측 폐에 수많은 소결절들이 발견되었다. 급성 호흡곤란 증후군이 악화되어 혈관외과의는 정맥-정맥 체외막산소공급(extracorporeal membrane oxygenation; 이하 ECMO)을 계획하였다. 왼쪽 대퇴동맥을 통해 경피적 삽관술을 시행하는 도중에 혈관외과의는 혈관 손상을 의심하였고 환자의 활력 징후가 불안정해졌다. 8일 전에 촬영한 복부 컴퓨터단층촬영에서 May-Thurner 증후군이 있었고, 이후 시행한 혈관조영술에서 좌측 총장골정맥의 파열이 발견되어 stent-graft를 삽입하여 출혈을 멈추었다. 8일 전 시행된 복부 전산화단층촬영을 확인해 보니 May-Thurner 증후군이 있었다. 이에 May-Thurner 증후군 환자에서 ECMO 삽관으로 인한 혈관 손상이 발생하여 스텐트 삽입술을 시행하였던 드문 증례를 보고하고자 한다.

Tracheobronchial rupture due to blunt chest trauma is a rare but life-threatening injury in the pediatric population. Computed tomography (CT) is not always reliable in the management of these patients. An additional concern is that ventilation may be disrupted during surgical repair of these injuries. This report presents the case of a 4 -year-old boy with an injury to the lower trachea and carina due to blunt force trauma that was missed on the initial CT scan. During surgery, he was administered venoarterial extracorporeal membrane oxygenation (ECMO). Although ECMO is not generally used in children, this case demonstrated that the short-term use of ECMO during pediatric surgery is safe and can prevent intraoperative desaturation.
