Robust Design of Pulse Oximeter Using Dynamic Control and Motion Artifact Detection Algorithms

  • Arterial oxygen saturation ($SpO_2$) monitoring for newborns requires special attention in neonatal intensive care units (NICUs). Newborns have very low photo-plethysmogram (PPG) amplitudes and their body movements are difficult to contain. Hardware design and its associated signal processing algorithms should be robust enough so that faulty measurements can be avoided. In this study, improved designs were implemented to deal with low perfusion, motion artifact, and the influence of ambient light. Dynamic range was increased by using different LED intensities and a feedback system. To minimize the effects of motion artifact and to discard other unqualified data, four additional algorithms were used, which were based on dual-trace detection, continuity of DC level, morphology of PPG, and simultaneity check of $SpO_2$. Our $SpO_2$ system was tested with newborns with normal respiration in the NICU. Our system provided fast, real-time responses and 100% artifact detection was accomplished under 84% of $SpO_2$.

  • Transmission and reflectance are two non-invasive techniques to perform pulse oximetry. This paper presents a design of reflectance-based pulse oximetry for watch-type wearable device, in which sensor and detector are located on the same surface of the body part. The basic principle of a pulse oximeter is based on the measurement of the red and infrared (IR) light absorption. Oxygenated blood has significant differences of light absorption characteristics than deoxygenated blood under red (660 nm) and infrared (940 nm) wavelength. Infrared is absorbed more by oxygenated hemoglobin than red. So the hardware implementation is included placing of the two LEDs (red and IR) with single photo-detector in the middle on the patient's wrist to get the corresponding pulsatile signals which are used to estimate the $SpO_2$.

Seasonal Variations in Primary Productivity and Pigments of Downstream Water of the Han River (한강하류수역의 기초생산과 식물플랭크톤 색소량의 년변화)

  • This study was undertaken to assess the annual cycle of primary production and plant pigments in a downstream of the Han River. Measurements were carried out at three week intervals during April 1966 and March 1967, and ancillary data include water temperature, transparency, pH, dissolved oxygen and phytoplankton cell number. The seasonal cycle in water temperature profile shows the hihgest in the end of August with 27$^{\circ}C$, lowest in the middle of February with 0.2$^{\circ}C$. The transparency with Secchi disk reading varied from a maximum 4.0m in fall and a minimum 0.5m or less in early spring and flood season of summer. The pH of the river water varied from 6.5 to 7.3, averaged 6.91 in the surface water and 6.98 in the bottom water, showed little seasonalvariability. The dissolved oxygen in the surface water ranged from 5.93-9.64ml/L, while in the bottom water it ranged from 5.54-9.72 ml/L, and the oxygen saturation never fall below 94%. None thermal, the distribution of pH and content of oxygen, stratifications occurred. An apparent seasonal cycle of primary productivity was observed with remarkably high levels in the spring and fall, the lowest level in the winter. The range of net carbon assimilations showed 3.1-112.6 mgC/㎥/day or 15-427 mgC/㎡/day in spring, 37.0-271.2 mgC/㎥/day or 115-329 mgC/㎡/day in summer, 27.2-168.0 mgC/㎥ /day or 139-415 mgC/㎡/day in fall and 0.5-10.9 mgC/㎥/day or 5-19 mg/㎡/day in winter, respectively. Amount of chlorophyll ${\alpha}$ ranged from a minimum concentration of 0.2-0.3 mg/㎥ in the middle of February and a maximum 4.1-6.7 mg/㎥ in the middle of June. A general increase trend in chlorophyll ${\alpha}$ concentration was noted with increase of the river water temperature.

Physical Performance in Young Women with Depleted Iron Stoles (젊은 여성의 철분저장 고갈상태와 운동수행능력)

  • The purpose of this study was to investigate the relationship between iron deficiency without anemia and physical performance in healthy women aged 20-21 yrs. Ten subjects with normal iron stores (serum ferritin $\geq$ 12$\mu\textrm{g}$/L: iron-sufficient group) and 11 subjects with iron depletion without anemia (serum ferritin < 12 $\mu\textrm{g}$/L and serum hemoglobin > 120 g/L: iron-depleted group) were chosen from a group of 50 women and were given physical-performance tests, including determinations of maximum oxygen consumption (VO$_2$ max) and ventilatory threshold. Iron status assessment included determination of hemoglobin, hematocrit, seam ferritin, total iron-binding rapacity, serum iron and transferrin saturation values. Dietary iron intake was assessed based on seven-day food intake records written by the subjects. Physical activity level was estimated by frequency questionnaires and two-week physical activity records were compiled daily by the subjects. Blood ferritin concentration was significantly lower in the iron-depleted group than in the iron-sufficient group (p < 0.05). However, other variables showing iron status was not different between the groups. There were no significant differences in body size, body composition and physical activity levels between the groups. Daily dietary iron, total protein and animal protein intakes of the iron-sufficient group were significantly higher than those of the iron depleted group. However, no differences were found in the amount of dietary vitamin C and fiber between the groups. The values for VO$_2$max and VO$_2$max corrected with weight or fat-free mass were not different between the groups. However, the ventilatory threshold was significantly higher in the iron sufficient group than in the iron-depleted group. The lower ventilatory threshold in the iron-depleted group suggests that iron depletion without anemia could diminish aerobic physical performance in young women. In addition, a significant correlation of physical performance to serum fferritin level was shown only in the iron depleted group.

Comparison of Clinical Characteristics and Effects of Modified Jaw Thrust Maneuver During Drug-Induced Sleep Endoscopy (DISE) between Positional and Non-Positional Obstructive Sleep Apnea Patients

  • Background and Objectives : Positional OSAS is characterized by an apnea-hypopnea index (AHI) score >5, which, while sleeping in the supine position, is double that in non-supine position. This study was performed to compare the clinical characteristics of positional OSAS and non-positional OSAS patients, and the effects of the modified jaw thrust maneuver during drug-induced sleep endoscopy (DISE) between positional OSAS and non-positional OSAS patients. Materials and Methods : 68 positional OSAS patients and 19 non-positional OSAS patients were included. They all underwent full-night polysomnography and DISE. The modified jaw thrust maneuver was introduced during DISE. Airway structural changes induced by the modified jaw thrust maneuver were evaluated and documented. Results : There were no statistically significant differences in Friedman stage or tonsil grade, body mass index, Epworth sleepiness scale (ESS) score, blood pressure, AHI, or obstructive pattern between the positional and non-positional OSAS patients. However, mean arterial oxygen saturation (SaO2), lowest SaO2, and total arousal index values were more severe in the non-positional OSAS patients. After introduction of the modified jaw thrust maneuver, retrolingual level obstruction showed a tendency toward a higher rate of airway opening in positional OSAS patients than in non-positional OSAS patients. Conclusions : The effects of a mandibular advancement device (MAD) can be estimated by carrying out a modified jaw thrust maneuver during DISE. The tendency toward a higher rate of airway opening in positional OSAS patients than non-positional OSAS patients in retrolingual level obstruction after jaw thrust maneuver introduced during DISE may be clinically important for MAD.

The Changes of Cerebral Metabolic and Hemodynamic Parameters, Brain Histology, and Serum Levels of Neuron-Specific Enolase During Retrograde Cerebral Perfusion Under Pofound Hypothermic total Circulatory Arrest in Pigs (돼지에서 초저체온 순환정지 하의 역행성 뇌관류시 뇌대사, 혈류역학 지표, 뇌조직 소견 및 혈청 내 neuron-specific enolase의 변화)

  • Background: Retrograde cerebral perfusion(RCP) is currently used for brain protection during aorta surgery, however, for the safety of it, various data published so far are insufficient. We performed RCP using pig and investiaged various parameters of cerebral metabolism and brain injury after RCP under deep hypothermia. Material and Method: We used two experimental groups: in group I(7 pigs, 20 kg), we performed RCP for 120 minutes and in group II (5 pigs, 20 kg), we did it for 90 minutes. Nasopharyngeal temperature, jugular venous oxygen saturation, electroencephalogram were continuously monitored, and we checked the parameters of cerebral metabolism, histological changes and serum levels of neuron-specific enolose(NSE) and lactic dehydrogenase(LDH). Central venous pressure during RCP was mainained in the range of 25 to 30 mmHg. Result: Perfusion flow rates(ml/min) during RCP were 130$\pm$57.7(30 minutes), 108.6$\pm$55.2(60 minutes), 107.1$\pm$58.8(90 minutes), 98.6$\pm$58.7(120 minutes) in group I and 72$\pm$11.0(30 minutes), 72$\pm$11.0(60 minutes), 74$\pm$11.4(90 minutes) in group II. The ratios of drain flow to perfusion flow were 0.18(30 minutes), 0.19(60 minutes), 0.17(90 minutes), 0.16(120 minutes) in group I and 0.21, 0.20, 0.17 in group II. Oxygen consumptions(ml/min) during RCP were 1.80$\pm$1.37(30 minutes), 1.72$\pm$1.23(60 minutes), 1.38$\pm$0.82(90 minutes), 1.18$\pm$0.67(120 minutes) in group I and 1.56$\pm$0.28(30 minutes), 1.25$\pm$0.28(60 minutes), 1.13$\pm$0.26(90 minutes). We could observe an decreasing tendency of oxygen consumption after 90 minutes of RCP in group I. Cerebrovascular resistance(dynes.sec.cm-5) during RCP in group I incrased from 71370.9$\pm$369145.5 to 83920.9$\pm$49949.0 after the time frame of 90 minutes(p<0.05). Lactate(mg/min) appeared after 30 minutes of RCP and the levels were 0.15$\pm$0.07(30 minutes), 0.18$\pm$0.10(60 minutes), 0.19$\pm$0.19(90 minutes), 0.18$\pm$0.10(120 minutes) in group I and 0.13$\pm$0.09(30 minutes), 0.19$\pm$0.03(60 minutes), 0.29$\pm$0.11(90 minutes) in group II. Glucose utilization, exudation of carbon dioxide, differences of cerebral tissue acidosis between perfusion blood and drain blood were maintained constantly during RCP. Oxygen saturation levels(%) in drain blood during RCP were 22.9$\pm$4.4(30 minutes), 19.2$\pm$4.5(60 minutes), 17.7$\pm$2.8(90 minutes), 14.9$\pm$2.8(120 minutes) in group I and 21.3$\pm$8.6(30 minutes), 20.8$\pm$17.6(60 minutes), 21.1$\pm$12.1(90 minutes) in group II. There were no significant changes in cerebral metabolic parameters between two groups. Differences in serum levels of NSE and LDH between perfusion blood and drain blood during RCP showed no statistical significance. Serum levels of NSE and LDH after resuming of cardipulmonary bypass decreased to the level before RCP. Brain water contents were 0.73$\pm$0.03 in group I and 0.69$\pm$0.06 in group II and were higher than those of the controls(p<0.05). The light microscopic findings of cerebral neocortex, basal ganglia, hippocampus(CA1 region) and cerebellum showed no evidence of cerebral injury in two groups and there were no different electron microscopy in both groups(neocortex, basal ganglia and hippocampus), but they were thought to be reversible findings. Conclusion: Although we did not proceed this study after survival of pigs, we could perform the RCP successfully for 120 minutes with minimal cerebral metabolism and no evidence of irreversible brain damage. The results of NSE and LDH during and after RCP should be reevaluated with survival data.

Optimization of Induction Conditions for Bacillus-derived Esterase Production by High-cell Density Fermentation of Recombinant Escherichia coli (재조합 대장균의 고농도 배양과 유도조건 최적화를 통한 Bacillus 유래 esterase의 생산)

  • To increase the efficiency of esterase production by Bacillus, high cell-density culture of recombinant Escherichia coli through fed batch fermentation was tested. Cells were cultured to $OD_{600}$ of 76 (35.8 g/l DCW) with dissolved oxygen level controlled to least above 30% air saturation by supplying pure oxygen. Cells were cultured to an $OD_{600}$ of 90 (42.4 g/l DCW) with glucose feeding controlled to at least 1 g/l. However, the cells reached stationary phase at the late stage of culture, despite glucose being supplied. Cells were cultured to an $OD_{600}$ of 185 (87.3 g/l DCW) by supplying additional medium with fortified yeast extract. To increase the productivity of the recombinant protein, cell growth and esterase productivity based on induction time were evaluated. Late exponential phase induction for esterase production in fed batch fermentation resulted in maximum optical density $OD_{600}$ of 190 (89 g/l DCW) and maximum esterase activity of 1745 U/l, corresponding to a 5.8-fold enhancement in esterase production, compared to the early exponential phase induction. In this study, we established fermentation methods for achieving maximum production of Bacillus-derived esterase by optimizing IPTG induction time in high-cell density culture by supplying pure oxygen and a nitrogen source.

Truncus Arteriosus, Type IV -one case report- (동맥간증 제 IV형 -1예 보고-)

  • Truncus ateriosus is one of the cyanotic congenital heart disease. The incidence is relatively uncommon, as 0.4% of totoal congenital heart disease. Embryologically the defect is due to a lack of partitioning of the embryonic truncus and conus during the first few weeks of fetal life. The ventricular septal defect is invariable present. A single arterial vessel arises from the heart and supplies blood to the aorta, the lung, and the coronary arteries. In 1949, collett and Edwards classified this defect according to anatomic variation to four major types, such as type I, II, III, and IV. Type IV is defined that pulmonary arteries are absent, and the pulmonary arterial supply arises from the descending thoracic aorta. This patients often have a continuous murmur head particularly well in the interscapular area. No effective surgical treatment is available. We have experienced one case of truncus arteriosus, type IV of Collett and Edwards in the Department of Thoracic and Cardiovascular Surgery, Kyungbook National University Hospital. This patient was 10 year-old girl. The chief complaints were cyanosis and dyspnea on exertion since birth. She was admitted at this hospital on April 16, 1980. The continous machinery murmur was heard loudest at the interscapular area. The chest X-ray films revealed cardiomegaly with an increase in pulmonaryvascular markings. The pulmonary secotr was significantly concave. No filling of pulmonary arteries noticed by the right ventriculogram. There was possible biventricular hypertrophy in EKG. The echocardiogram showed that the demension of the aortic root was larger than normal and minimal increase of the left ventricular internal dimension. The cardiac catheterization data was obtained by use of the great saphenus vein approach. The systolic pressure of the right ventricular outflow tract was 80 mmHg and was similar to that of the aorta. The oxygen saturation data revealed the evidence of the left to right shunt at the level of ventricular septum. The patient was operated and the diagnosis was confirmed as trucus arteriosus, type IV. No effective surgical interventins were performed.

The Effects of Leg Elevation and Stretching Exercise on Monitored Anesthesia Care (MAC) with Dexmedetomidine (Dexmedetomidine 감시마취관리 환자의 회복 시 적용한 하지거상 및 말초운동의 효과)

  • Purpose: The purpose of this study was to examine the effect of leg raising and peripheral excercise on recovery of the patients who were applied with dexmedetomidine for their dental surgery. Methods: There were two groups, the experimental group (n=35) and control group (n=35), in this study. We checked blood pressure, pulse rate, oxygen saturation, sedation level and symptoms such as dizziness and somnolence every 30 minutes. These parameters were assessed throughout the participants' recovery room stay. Leg raising and peripheral excercise were conducted in the experimental group in the recovery room. We have conducted chi-square test, Fisher's exact test, t-test, ANOVA, and ANCOVA to compare the measured parameters in both groups. Results: The experimental group showed a significant elevation of mean arterial blood pressure, and mitigation of somnolence, sedation and dizziness compared to the control group. Conclusion: Leg raising and peripheral exercise is effective to expedite recovery in the patients who were applied with dexmedetomidine for their dental surgery.

Investigation of Seasonal Characteristics of Contaminants and Hydrochemical Factors in an Aquifer for Application of In Situ Reactive Zone Technology (원위치 반응존 공법 적용을 위한 대수층내 오염물질 및 환경영향인자의 계절 특성 평가)

  • A field investigation was conducted on an aquifer contaminated with trichloroethylene (TCE) for application of in situ reactive zone treatment using nanosized zero-valent iron (NZVI). The aquifer was an unconfined aquifer with a mean hydraulic conductivity of $5.14{\times}10^{-4}cm/sec$, which would be favorable for NZVI injection. Seasonal monitoring of TCE concentration revealed a presence of non-aqueous phase liquid form of TCE near IW (injection well). The hydrochemical data characterized the site groundwater to be a $Ca-HCO_3$ type. The average value of Langelier Saturation Index of the groundwater was -1.33, which implied that the site was favorable for corrosion of NZVI. Dissolved oxygen (DO) concentration varied between 2.5~11.5 mg/L, which indicated that DO would greatly compete with TCE as an electron acceptor. The hydrogeological and hydrochemical characterization reveals that the time around November would be appropriate for NZVI injection when water level and temperature are relatively high and DO concentration is low.
