피개의치를 이용한 지적장애인의 완전구강회복 (Full Mouth Rehabilitation with a Overlay Denture)

  • 본 증례보고는 구순구개열과 부정교합을 가진 지적장애인에게 피개의치를 이용하여 수복한 증례이다. 환자는 교정치료와 악교정수술이 필요한 광범위한 부정교합을 가진 상태였으나 치료시간과 비용, 환자의 관리능력을 고려하여 피개의치를 대체적인 치료방법으로 선택하였다. 구강내검사, 구강외검사 및 방사선검사를 시행하였으며 보철수복을 위해 수직교합고경을 평가하였고 진단모형에서 써베잉을 시행하였다. 치아 및 조직부 언더컷을 고려하여 치아의 외형을 재형성하였다. 구강형성이 완료된 후 최종인상을 채득하였다. 납의치를 이용하여 수직고경과 교합을 평가한 후 의치를 온성하였다. 교합기 상에 치료실재부착을 하여 교합조정을 시행하였다. 환자는 의치의 심미성과 기능에 만족하였으며 피개의치는 안정적 교합상태를 보였다. 이 환자에게 시행한 피개의치 치료는 가역적이고 상대적으로 저렴한 치료법이지만 구강위생이 나쁘면 치아우식이나 치주염이 발생할 가능성이 있기 때문에 정기적이고 전문적인 관리가 필요할 것으로 사료된다.

의치상 레진의 세포독성에 관한 연구 (CYTOTOXICITY OF DENTURE BASE RESINS)

  • The purpose of this study was to investigate the cytotoxicity and mutagenicity of denture base resins. According to manufacturer's instructions, resin specimens were made. Group 1 : heat-polymerizing acrylic resin (Luciton $199^{(R)}$) Group 2 : heat-polymerizing acrylic resin containing polyhedraloligosilsesquioxane(POSS resin) Group 3 : auto-polymerizing acrylic resin (Repair $Acrylic^{(R)}$) Group 4 : direct relining auto-polymerizing acrylic resin (Tokuso $Rebase^{(R)}$). Fresh specimens 24 hrs. and 72 hrs. soaked specimens in distil)ed water were made. Responses with metabolic assay and mutagenesis assay to eluates from resin specimens were measured. Cultures with medium alone provided controls. Cytotoxicity was assessed with agar overlay test. The results were as follows; 1. Group 4 showed higher cytotoxicity than Group 1, Group 2 and Group 3 in fresh, 24-an4 72-hour immersion caries (p<.05). Group 3 showed higher cytotoxicity than Group 2 in fresh cases and showed higher cytotoxicity than Group 1 and Group 2 in 24-and 72-hour immersion cases (p<.05) . Group 1 and Group 2 showed no significant difference. 2. All acrylic denture base resins skewed significant increase of cell activity as immersion time increased (p<.05). 3. Auto-polymerizing acrylic denture base resins skewed higher cytotoxicity than heat-polymerizing acrylic denture base resins (p<.05). 4. All acrylic denture base resins showed lower mutagenicity than controls (p<.05).

Standardizing the evaluation criteria on treatment outcomes of mandibular implant overdentures: a systematic review

  • PURPOSE. The aim of this review was to analyze the evaluation criteria on mandibular implant overdentures through a systematic review and suggest standardized evaluation criteria. MATERIALS AND METHODS. A systematic literature search was conducted by PubMed search strategy and hand-searching of relevant journals from included studies considering inclusion and exclusion criteria. Randomized clinical trials (RCT) and clinical trial studies comparing attachment systems on mandibular implant overdentures until December, 2011 were selected. Twenty nine studies were finally selected and the data about evaluation methods were collected. RESULTS. Evaluation criteria could be classified into 4 groups (implant survival, peri-implant tissue evaluation, prosthetic evaluation, and patient satisfaction). Among 29 studies, 21 studies presented implant survival rate, while any studies reporting implant failure did not present cumulative implant survival rate. Seventeen studies evaluating peri-implant tissue status presented following items as evaluation criteria; marginal bone level (14), plaque Index (13), probing depth (8), bleeding index (8), attachment gingiva level (8), gingival index (6), amount of keratinized gingiva (1). Eighteen studies evaluating prosthetic maintenance and complication also presented following items as evaluation criteria; loose matrix (17), female detachment (15), denture fracture (15), denture relining (14), abutment fracture (14), abutment screw loosening (11), and occlusal adjustment (9). Atypical questionnaire (9), Visual analog scales (VAS) (4), and Oral Health Impact Profile (OHIP) (1) were used as the format of criteria to evaluate patients satisfaction in 14 studies. CONCLUSION. For evaluation of implant overdenture, it is necessary to include cumulative survival rate for implant evaluation. It is suggested that peri-implant tissue evaluation criteria include marginal bone level, plaque index, bleeding index, probing depth, and attached gingiva level. It is also suggested that prosthetic evaluation criteria include loose matrix, female detachment, denture fracture, denture relining, abutment fracture, abutment screw loosening, and occlusal adjustment. Finally standardized criteria like OHIP-EDENT or VAS are required for patient satisfaction.

  • To determine the vertical dimension and centric relation during the construction of implant overdentures, the record base and wax rim may need to be adjusted. The conventional method has several drawbacks, as it requires repeated tightening and loosening of the impression coping. Here, we report a useful and novel method for interocclusal records using the healing abutments in implant overdentures. Our case demonstrates that this method is easier and simpler and prevents gingival collapse.

Performance of attachments used in implant-supported overdentures: review of trends in the literature

  • The purpose of this review is to examine the performance of attachments used in implant-supported overdenture (IOD) in both clinical and in vitro settings and report the compiled findings, comparisons, and trends in the research literature. Articles published in PubMed on IOD attachment systems and performance were reviewed. Non-original articles were excluded. For each article included, the type of study, number of implants, number of attachment systems, and study outcome were recorded. Of the 283 articles found, 158 met the inclusion criteria. Ninety-four articles were clinical studies and 64 articles were in vitro studies. Studies on retention were the most common for in vitro studies, and four or more attachment systems were compared in most articles with significant differences in outcome. A clinical outcome of one attachment system was most common for clinical studies, while most studies had neutral outcomes overall. Ball attachment was the most commonly tested IOD attachment system. The trend in the literature showed that there is a large discrepancy between the study designs and outcomes between the clinical and the in vitro studies for IOD. Further clinical studies that can validate in vitro research should be encouraged to address this discrepancy between the two areas.

Number of implants for mandibular implant overdentures: a systematic review

  • PURPOSE. The aim of this systematic review is to address treatment outcomes of Mandibular implant overdentures relative to implant survival rate, maintenance and complications, and patient satisfaction. MATERIALS AND METHODS. A systematic literature search was conducted by a PubMed search strategy and hand-searching of relevant journals from included studies. Randomized Clinical Trials (RCT) and comparative clinical trial studies on mandibular implant overdentures until August, 2010 were selected. Eleven studies from 1098 studies were finally selected and data were analyzed relative to number of implants. RESULTS. Six studies presented the data of the implant survival rate which ranged from 95% to 100% for 2 and 4 implant group and from 81.8% to 96.1% for 1 and 2 implant group. One study, which statistically compared implant survival rate showed no significant differences relative to the number of implants. The most common type of prosthetic maintenance and complications were replacement or reattaching of loose clips for 2 and 4 implant group, and denture repair due to the fracture around an implant for 1 and 2 implant groups. Most studies showed no significant differences in the rate of prosthetic maintenance and complication, and patient satisfaction regardless the number of implants. CONCLUSION. The implant survival rate of mandibular overdentures is high regardless of the number of implants. Denture maintenance is likely not inflenced substantially by the number of implants and patient satisfaction is typically high again regardless os the number of implants.
