Oral cancer incidence based on annual cancer statistics in Korea

  • Introduction: The objective of this research was to determine the incidence of oral cancer in Korea. Materials and Methods: The classifications of oral and maxillofacial cancer (OMFC) that we used are based on possible locations of OMFC: lip, tongue, mouth, salivary glands, tonsil, oropharynx, nasopharynx, hypopharynx, pharynx unspecified, and nose, sinuses. Results: 1) There were 2,848 OMFC cases, accounting for 1.6% of all cancers. The male to female ratio was 2.72:1. 2) The estimated crude rates (CRs) were 5.7 overall, 8.4 for males, and 3.1 for females. The age-standardized incidence rates (ASRs) were 4.6 overall, 7.3 for males and 2.3 for females. 3) The incidence of mouth cancer was highest. The mouth and salivary glands were the most frequent sites for cancer among males and females, respectively. 4) Patients who were 40 years or older accounted for 91% of OMFC cases, with the highest proportion of cases in the 60-69 year-old age group for both sexes. 5) Tongue cancer was the most prevalent OMFC overall. Nasopharyngeal cancer was highest among males, and salivary gland cancer was highest among females. 6) From 2004 to 2008, the relative 5-year survival rate of OMFC patients was 57.5%. There was a trend of increasing survival among OMFC patients during the study period. The survival rate for females (69.3%) was much higher than that for males (53.1%). Conclusion: Social and personal efforts should be required to increase the survival rates of OMFC patients and Korean national cancer management policy should establish new measures for economic and social management and support.

Outcomes of stereotactic body radiotherapy for unresectable primary or recurrent cholangiocarcinoma

  • Purpose: To report the results of stereotactic body radiotherapy (SBRT) for unresectable primary or recurrent cholangiocarcinoma. Materials and Methods: From January 2005 through August 2013, 58 patients with unresectable primary (n = 28) or recurrent (n = 30) cholangiocarcinoma treated by SBRT were retrospectively analyzed. The median prescribed dose was 45 Gy in 3 fractions (range, 15 to 60 Gy in 1-5 fractions). Patients were treated by SBRT only (n = 53) or EBRT + SBRT boost (n = 5). The median tumor volume was 40 mL (range, 5 to 1,287 mL). Results: The median follow-up duration was 10 months (range, 1 to 97 months). The 1-year, 2-year overall survival rates, and median survival were 45%, 20%, and 10 months, respectively. The median survival for primary group and recurrent group were 5 and 13 months, respectively. Local control rate at 1-year and 2-year were 85% and 72%, respectively. Disease progression-free survival rates at 1-year and 2-year were 26% and 23%, respectively. In univariate analysis, ECOG performance score (0-1 vs. 2-3), treatment volume (<50 vs. ${\geq}50mL$), and pre-SBRT CEA level (<5 vs. ${\geq}5ng/mL$) were significant in overall survival rate. In multivariate analysis, ECOG score (p = 0.037) and tumor volume (p = 0.030) were statistically significant. In the recurrent tumor group, patients with >12 months interval from surgery to recurrence showed statistically significant higher overall survival rate than those with ${\leq}12$ months (p = 0.026). Six patients (10%) experienced ${\geq}$grade 3 complications. Conclusion: SBRT can be considered as an effective local modality for unresectable primary or recurrent cholangiocarcinoma.

Survival in Fry and Juvenile Stages of Masu salmon Oncorhynchus masou : Estimates of Heritabilities and Correlations

  • A genetic analysis for survival in fry and juvenile stages of masu salmon was described. Data from two year-classes of masu salmon were analyzed to estimate the heritability for survival during the fresh water-rearing period. The overall survival for each year-class during 8 months of freshwater rearing were 17.8 and 11.6%, respectively. Whirling disease virus (WDV) was the main cause of death in all year-classes. Survival data obtained for offspring of 42 sires and 60 dams of masu salmon (two year classes of data) was analyzed. Average survival rates in the observation period ranged 2-87% for 1994; 0-98% for 1995, repectively. In both year-classes, heritabilities for survival derived from the sire components of variance were low(0.13-0.18), except one. Heritabilities derived from the dam components of variance ranged 0.14-0.61, including non-additive genetic and /or common enviromental effects. Correlations between survival in two long-term periods were all positive and medium to high in magnitude(0.345-0.918). Correlations between survival in non-succeeding periods were, in general, low and insignificant. Correlation between long-term survival and growth rate was found in masu salmon. The corresponding correlation in masu salmon was not significantly different from zero. Correlations between sire survival and body weight, length and condition factor of slaughter were not significant, but varied.

두경부에 발생한 육종의 임상적 고찰 (Clinical Analysis of Head and Neck Sarcoma)

  • Introduction : Sarcoma of the head and neck region is a very rare disease entity. This retrospective study investigated the clinical characteristics of head and neck sarcomas and analyzed its treatment methods and outcomes. Subjects and Methods : Eighty-five patients who were diagnosed as sarcomas of the head and neck region in Shinchon Severance Hospital between 1985 and 2005 were included in the study. Data concerning age, sex, symptoms, location and size of tumor, histopathologic characteristics, treatment methods, recurrence, and distant metastasis were reviewed. Result : Overall 5 year survival rate was 38% and the 5 year survival rate in the pediatric population was 60%. The 5 year survival rates for each osteosarcoma and soft tissue sarcoma cases were 42% and 37% respectively. The 5 year survival rate was significantly higher in the cases where complete surgical resection was achieved. Conclusion : In managing head and neck sarcomas, it is important to perform wide resection and to achieve complete resection.

Survival of Rectal Cancer in Yazd, Iran

  • Background: Colorectal cancer is common in Iran. However our knowledge about survival of rectal cancer in our province is low. The aim of this study is to evaluate this question. Materials and Methods: Patients with documented pathology of adenocarcinoma of the rectum and rectosigmoid junction referred to our center from September 2004 to September 2012 were enrolled in this study. Metastatic and recurrent patients were excluded. A questionnaire including clinicopathologic parameters, quality and sequence of treatment modalities was filled in for each patient. Patients treated with a combination of surgery, chemotherapy and radiation therapy were divided into standard and non-standard treatment groups, according to the sequence of treatment. Results: One hundred and nineteen patients were evaluated. Mean age was 60.8 year. The median overall survival was 62 months and five year survival was 55%. TNM staging system was not possible due to (Nx) in 21 (17.6%) patients. The others were in stage I, 20 patients (16.8%), II, 35 (29%.5) and III, 43(36.1%). According to our definition only 25 patients (21%) had been treated with standard treatment and 79% had not received it. A five year survival in patients with standard treatment was 85% and in the non-standard group it was 52%.Age, sex, stage and grade of tumor did not show any significant relation to survival. Conclusions: Our study showed a five year survival of rectal cancer in our patients was about 10% lower than the rate which is reported for developed countries. Preoperative concurrent chemoradiation significantly improved local control and even overall survival.

Perioperative Epirubicin, Oxaliplatin, and Capecitabine Chemotherapy in Locally Advanced Gastric Cancer: Safety and Feasibility in an Interim Survival Analysis

  • Purpose: Perioperative chemotherapy improves survival outcomes in locally advanced (LA) gastric cancer. Materials and Methods: We retrospectively analyzed patients with LA gastric cancer who were offered perioperative chemotherapy consisting of epirubicin, oxaliplatin, and capecitabine (EOX) from May 2013 to December 2015 at Tata Memorial Hospital in Mumbai. Results: Among the 268 consecutive patients in our study, 260 patients (97.0%) completed neoadjuvant chemotherapy, 200 patients (74.6%) underwent D2 lymphadenectomy, and 178 patients (66.4%) completed adjuvant chemotherapy. The median follow-up period was 17 months. For the entire cohort, the median overall survival (OS), 3-year OS rate, median progression-free survival (PFS), and 3-year PFS rate were 37 months, 64.4%, 31 months, and 40%, respectively. PFS and OS were significantly inferior in patients who presented with features of obstruction than in those who did not (P=0.0001). There was no difference in survival with respect to tumor histology (well to moderately differentiated vs. poorly differentiated, signet ring vs. non-signet ring histology) or location (proximal vs. distal). Survival was prolonged in patients with an early pathological T stage and a pathological node-negative status. In a multivariate analysis, postoperative pathological nodal status and gastric outlet obstruction on presentation significantly correlated with survival. Conclusions: EOX chemotherapy with curative resection and D2 lymphadenectomy is a suggested alternative to the existing perioperative regimens. The acceptable postoperative complication rate and relatively high resections, chemotherapy completion, and survival rates obtained in this study require further evaluation and validation in a clinical trial.

Analysis of prognostic factors through survival rate analysis of oral squamous cell carcinoma patients treated at the National Cancer Center: 20 years of experience

  • Objectives: This study aimed to analyze the clinicopathological prognostic factors affecting the survival of patients with oral squamous cell carcinoma (OSCC). Materials and Methods: A retrospective study was conducted on patients with OSCC who received treatment at the Oral Oncology Clinic of the National Cancer Center (NCC) from June 2001 to December 2020. The patients' sex, age, primary site, T stage, node metastasis, TNM staging, perineural invasion (PNI), lymphovascular invasion (LVI), differentiation, surgical resection margin, smoking, and drinking habits were investigated to analyze risk factors. For the univariate analysis, a Kaplan-Meier survival analysis and log-rank test were used. Additionally, for the multivariable analysis, a Cox proportional hazard model analysis was used. For both analyses, statistical significance was considered when P<0.05. Results: During the investigation period, 407 patients were received surgical treatment at the NCC. Their overall survival rate (OS) for five years was 70.7%, and the disease-free survival rate (DFS) was 60.6%. The multivariable analysis revealed that node metastasis, PNI, and differentiation were significantly associated with poor OS. For DFS, PNI and differentiation were associated with poor survival rates. Conclusion: In patients with OSCC, cervical node metastasis, PNI, and differentiation should be considered important prognostic factors for postoperative survival.

성문상부암의 치료결과 (Treatment Results for Supraglottic Cancer)

  • Purpose: In supraglottic cancer, radiation therapy is used to preserve the laryngeal function but combined surgery and radiation therapy is required in advanced stage. The authors Present the results of radiation therapy alone and combined surgery Plus Postoperative radiation therapy for supraglottic cancer. Methods and Materials: A retrospective analysis was done for 43 patients with squamous cell carcinoma of the supraglottic larynx who were treated from Feburary 1982 to December 1991, in the Department of Radiation Oncology, Korea University Hospital. Patient distribution according to the AJCC staging system was as follows: I, 3($7.0\%$); II, 7($16.3\%$); III, 17($39.5\%$); IV, 16($37.2\%$). Patients' age ranged from 30 to 72 years(median 62). Follow up durations were from 21 to 137 months(median 27). Seventeen patients($39.5\%$) were treated by radiation therapy alone with radiation doses of 6840-7380 cGy and 26 patients($60.5\%$) were treated with surgery plus postoperative irradiation with doses of 5820-6660 cGy. Results: Overall five-year survival rate for all stage was $51.8\%$, with $100\%$ for Stage I and II, $47.3\%$ for Stage III, and $29.2\%$ for Stage III. The difference of the survival rate by stage was statistically significant(p=0.0152). Five-year survival rates were $100\%$ for locally confined tumor in the supraglottic larynx, $37.5\%$ for transglottic extension, $26.7\%$ for hypopharynx extension, and only two of 5 patients with both transglottic and hypopharynx extension were alive(p=0.0033). Five-year survival rates by neck node status were as follows: $55.0\%$ for NO, $64.3\%$ for N1, $50.0\%$ for N2, and all 2 of N3 were died of disease. Overall survival rate for radiation therapy alone group was $42.8\%$, and it was $56.7\%$ for surgery plus postoperative radiation therapy group with no statistically significant difference(p=0.5215). In Stage I and II, all Patients survived. In Stage III and IV, 5-year survival rate for radiation therapy alone group was $28.5\%$ and $43.4\%$ for surgery plus postoperative irradiation group(p=0.5103). Local control rate was $58.8\%$(10/17) for radiation therapy alone group and $73.1\%$ (19/26) for surgery plus postoperative irradiation group. Three patients from surgery plus postoperative radiation therapy group developed distant metastasis in lungs. Conclusion: Treatment results of radiation therapy alone was excellent in early stage supraglottic cancer. In advanced stage, even the difference was statistically not significant, the result of postoperative radiation therapy group was superior compared with radiation therapy alone group. Since 1992, concomitant chemoradiotherapy with hyperfractionated radiotherapy is being used to improve the result of the treatment and preserve the laryngeal function in advanced stage supraglottic cancer.

Retrospective study on factors affecting the prognosis in oral cancer patients who underwent surgical treatment only

  • Background: This study was performed to evaluate their 5-year survival rates and identify the factors affecting the prognosis of oral cancer patients who had undergone surgical treatment only. Methods: Among 130 patients who were diagnosed with malignant tumor of oral, maxillofacial, and surgical treated in the Department of Oral and Maxillofacial Surgery at Chonnam National University Hospital within a period from January 2000 to December 2010, for 11 years, 84 patients were investigated who were followed up for more than 5 years after radical surgery; oral cancer is primary and received only surgical treatment. The survival rate according to gender, age, type and site of cancer, TNM stage, cervical lymph node metastasis and its stage, recurrence or metastasis, time of recurrence and metastasis, and differentiation were investigated and analyzed. Results: Overall, 5-year survival rate in patients who received only surgical treatment was 81.2 %, and disease-specific 5-year survival rate was 83.1 %. The disease-specific 5-year survival rate based on TNM stage, metastasis of cervical lymph node, N stage, and presence of recurrence/metastasis was a significant difference (p < 0.05). The disease-specific 5-year survival rate based on sex, age, type of tumor, primary site, and differentiation was not a significant difference (p > 0.05). Conclusions: These results suggest that good survival rate can be obtained with surgical treatment only, and stage of oral cancer, cervical lymph node metastasis and stage, recurrence or metastasis, time of recurrence, and metastasis have a significant effect on survival rate in oral cancer patients.

사지와 체부에 발생한 편평상피 세포암의 치료 (Treatment of Squamous Cell Carcinoma in Extremity & Trunk)

  • 목적: 편평상피 세포암 환자의 치료 결과를 분석하여, 전반적인 생존율과 예상 가능한 예후인자들에 따른 생존율을 비교하고 분석하는데 있다. 대상 및 방법: 1999년 3월부터 2011년 2월까지 본원에서 병리학적으로 진단된 사지와 체부에 발생한 편평상피 세포암은 151예였고 수술적 치료를 시행한 환자는 51예였다. 연구는 이 중 12개월 이상 외래추적이 가능하였던 41예를 대상으로 하였다. 평균 연령은 64.4세였고, 남자와 여자는 각각 31명, 10명이었다. 수술은 광범위 절제 및 재건술(29예)과 절제연을 얻기 힘든 사지 말단부나 신경, 혈관계를 침범한 경우에 절단술(12예)을 시행하였다. 수술적 치료만 시행한 환자는 33예이고, 절제 후 항암화학치료 또는 방사선 치료를 시행한 환자는 8예가 있었다. 병기는 AJCC 분류에 따라 나누었으며 생존율은 Kaplan-Meier 법으로 계산하였고 군간의 생존율 비교는 Log-rank test를 이용하였다. 생존율과 관련된 예후 인자들로 원발 병소의 위치, 병인, 조직학적 분류, 병기, 수술방법, 추가 항암요법 여부를 조사하여 각각 생존율을 비교하였다. 결과: 평균 외래 추적은 평균 65.2개월(12-132개월)이었고 최종 추시 상 생존은 30예(73.1%)이었고 Kaplan-Meier에 의한 5년 생존율은 77%이었다. 전체 환자에서 총 3예(7.3%)의 국소 재발과 7예(17.0%)의 전이가 있었다. 국소재발 3예에서 재발시기는 평균 27개월(18-43개월)이었다. 원발 병소의 위치, 병인, 조직학적 분류, AJCC 분류에 따른 병기, 추가 항암요법에서 생존율은 통계적으로 유의한 차이가 없었으나 절단술을 시행한 환자는 광범위 절제술을 시행한 환자보다 생존율이 낮았다. 결론: 41예의 편평상피 세포암의 치료결과를 분석한 결과 5년 생존율이 77%이었고 예후에 영향을 미칠 수 있는 여러 인자 중에 수술 방법만이 통계적으로 유의하였다.
