Irreducible Acute Patellar Dislocation with Anatomical Variant: Notched Patella (해부학적 변이에 의한 정복 되지 않는 급성 슬개골 탈구: 슬개골의 내측 절흔)

  • Most acute patella dislocations can be reduced closely using a spontaneous or simple maneuver, but it may not be possible if accompanied the by anomalies, such as rotation of the patella, osteophyte ridge protruding from the femur, or anatomical variations. This case report outlines irreducible acute patella dislocation with anatomical variations, a notched patella. If the reduction of an acute patella dislocation fails, an additional radiological examination and reduction under general anesthesia may be necessary with the possibility of an anatomical variant in mind.

  • 저자등은 1996년 5월 부터 1999년 4월까지 인하대병원 치과에 악관절 질환을 주소로 내원한 958명의 환자 중 병력, 임상소견, 방사선학적 검사 결과 퇴행성 악관절 질환으로 진단된 68명에서 병록지 상에 기록된 환자의 성별, 나이, 주소, 병력, 현증상, 방사선학적 소견 및 치료방법 등에 관해 검사 후 다음과 같은 결론을 얻었다. 1. 남녀 발병 비율은 1:2로 여성에서 높았다. 2. 발병나이는 20대, 30대. 40대, 10대 순이었다. 3. 병력기간은 2년이상, 6개월이하, 1 - 2년 순이었다. 4. 전 환자에서 악관절 동통을, 44명의 환자에서 관절잡음을, 26명의 환자에서 개구제한을 호소하였다. 5. 그밖에 26명의 환자에서 관절부위에 민감성을, 5명의 환자에서 안면 비대칭을, 또한 개교합, 섬유성 악관절강직의 소견이 각각 2명의 환자에서, 양측성질환의 소견이 4명의 환자에서 관찰되었다. 6. 방사선학적 소견상, 15명 환자의 과두부에서 증식성 재성형소견(progressive remodeling)이, 51명의 환자에서 침식성 재성형소견(regressive or erosive remodeling)이 관찰되었으며, 침식성 재성형 환자 중 11명에서 과두주변부 증식성 재성형소견(peripheral remodeling or marginal lipping)이 관찰되었다. 7. 골증식체(osteophyte) 및 소성체(loose body)의 소견이 각각 2명의 환자에서 관찰되었다. 8. 약물에 의한 보존적 치료방법이 1차로 모든 환자에서 시행되었으며, 이중 12명의 환자는 교합안정장치를 병행하여 치료하였다. 9. 관절강내 스테로이드 주사요법에 의한 치료환자가 2명, 관절원판절제술 및 이개연골 이식술을 동반한 악관절성형술 환자가 각각 1명 및 2명이었다.

Effectiveness of digital subtraction radiography in detecting artificially created osteophytes and erosions in the temporomandibular joint

  • Purpose: Erosions and osteophytes are radiographic characteristics that are found in different stages of temporomandibular joint (TMJ) osteoarthritis. This study assessed the effectiveness of digital subtraction radiography (DSR) in diagnosing simulated osteophytes and erosions in the TMJ. Materials and Methods: Five intact, dry human skulls were used to assess the effectiveness of DSR in detecting osteophytes. Four cortical bone chips of varying thicknesses (0.5 mm, 1.0 mm, 1.5 mm, and 2.0 mm) were placed at the medial, central, and lateral aspects of the condyle anterior surface. Two defects of varying depth (1.0 mm and 1.5 mm) were created on the lateral, central, and medial poles of the condyles of 2 skulls to simulate erosions. Panoramic images of the condyles were acquired before and after artificially creating the changes. Digital subtraction was performed with Emago dental image archiving software. Five observers familiar with the interpretation of TMJ radiographs evaluated the images. Receiver operating characteristic (ROC) analysis was used to evaluate the diagnostic accuracy of the imaging methods. Results: The area under the ROC curve (Az) value for the overall diagnostic accuracy of DSR in detecting osteophytic changes was 0.931. The Az value for the overall diagnostic accuracy of panoramic imaging was 0.695. The accuracy of DSR in detecting erosive changes was 0.854 and 0.696 for panoramic imaging. DSR was remarkably more accurate than panoramic imaging in detecting simulated osteophytic and erosive changes. Conclusion: The accuracy of panoramic imaging in detecting degenerative changes was significantly lower than the accuracy of DSR (P<.05). DSR improved the accuracy of detection using panoramic images.

  • Osteoarthritis is a noninflammatory degenerative disease affecting the articular surfaces and is accompanied by remodeling of the underlying bone. The sympotms of osteoarthritis of the temporomandibular joint are pain in the joint and muscles of mastication, limitation. Osteoarthritis is generally accepted that several factors can contribute to the trauma, aging process, internal derangement and MPDS. Radiographic features of the osteoarthritis are seen flattening of joint, sclerosis on flattened area and osteophyte or anterior lipping. In the past, osteoarthritis was considered to self-limiting disease. Currently, synovial chondromembrane is part of the process of osteoarthritis secondary to trauma. Synovial chondromatosis is an uncommon disease of cartilaginous transformation of synovial membrane with formation of loose bodies within the joint space. The pathogenesis is more an active metaplastic than a neoplastic process. The cause of synovial chondromatosis is unknown. Although trauma and inflammation have all been cited as possible factors in tis pathogenesis. The clinical sign and symptoms are unilateral swelling of the joint region, pain in the joint area and crepitus seem to be the most reliable signs and symptoms. Radiographic evidence of loose bodies may or may not be present. This is a case report of 66 year old female with synovial chondromatosis, that is advanced disease of the osteoarthrits. We treated patient with surgical excision of lose bodies, diskectomy and synovectomy. The defected articular fossa area was reconstructed with temporalis fascia flap. The result was satisfactory.

Open Treatment of Anterior Impingement Syndrome of the Ankle in Elite Level Soccer Players (축구 선수에서 발생한 족관절 전방 충돌 증후군의 개방적 수술의 치료 결과)

  • Purpose: We assessed the results of open treatment of anterior impingement syndrome of the ankle in elite level soccer players and concomitant injuries were idenfied. Materials and Methods: We retrospectively reviewed twenty one elite level soccer players diagnosed with anterior impingement syndrome who underwent open debridement between January 1997 and January 2002. All were men and the mean age at the operation was 21 years (range 16 to 27). The mean follow-up duration was 31 months (13 to 71). Concomitant abnormalities were idenfied through physical examination, bone scan and MRI. On a preoperative lateral radiograph, patients were classified according to McDermott's stage. Anteromedial or anterolateral approach was used at the operation and osteophyte was removed with osteotome and rongeur. When chronic ankle instability was accompanying, we performed Modified Brostrom-Gould procedure and for osteochondral lesion, multiple drilling was applied. The Ogilvie-Harris scoring system was used as a clinical scale to evaluate pain, swelling, stiffness and limitation of activity. The results were scored as excellent (15 to 16 points), good (13 to 14) and otherwise unsatisfactory. The time to return to full activity including sports activity was determined. Results: Eighteen of twenty one patients had an excellent outcome. Three patients were graded unsatisfactory and two of them abandoned their career due to the persistence of residual pain. Concomitant abnormalities were found including twelve cases of chronic ankle instability, three cases of osteochondral lesion and two cases of flexor hallucis longus tendinitis. Conclusion: Open debridement was successfully applied to the elite level soccer player with anterior impingement syndrome of the ankle. Considerable coexistence of other abnormalities such as chronic ankle instability may encourage us to consider additional operative procedure.

Anatomical Characteristics and Surgical Treatments of Pincer Nail Deformity

  • Background Pincer nail deformity is a transverse overcurvature of the nail. This study aimed to define the anatomical characteristics of pincer nail deformity and to evaluate the surgical outcomes. Methods A retrospective review was conducted on 20 cases of pincer nail deformity of the great toe. Thirty subjects without pincer nail deformity or history of trauma of the feet were selected as the control group. Width and height indices were calculated, and interphalangeal angles and base widths of the distal phalanx were measured with radiography. We chose the surgical treatment methods considering perfusion-related factors such as age, diabetes mellitus, kidney disease, and peripheral vascular disease. The zigzag nail bed flap method (n=9) and the inverted T incision method (n=11) were used to repair deformities. The outcomes were evaluated 6 months after surgery. Results The interphalangeal angle was significantly greater in the preoperative patient group ($14.0^{\circ}{\pm}3.6^{\circ}$) than in the control group ($7.9^{\circ}{\pm}3.0^{\circ}$) (P<0.05). The postoperative width and height indices were very close to the measurements in the control group, and most patients were satisfied with the outcomes. Conclusions We believe that the width and height indices are useful for evaluating the deformity and outcomes of surgical treatments. We used two different surgical methods for the two patient groups with respect to the perfusion-related factors and found that the outcomes were all satisfactory. Consequently, we recommend taking into consideration the circulatory condition of the foot when deciding upon the surgical method for pincer nail deformity.

Validity of Paramedian Tangential Approach to L5-S1 Far-Lateral Lesions

  • Objective : There are various surgical approaches to far-lateral lesions in the L5-S1 intervertebral space. Of these is the validity of a paramedian tangential approach is being investigated in this study. Methods : A retrospective study was conducted on 25 patients who had been diagnosed as having a far-lateral L5-S1 disc herniations, osteophyte, costal process hypertrophy, and had undergone a paramedian tangential approach from November 1999 through December 2003. The degree of symptoms and improvement were compared via the visual analog pain scale, before and after surgery. Results : This study included 4 males and 21 females with a mean age of $62{\pm}11.8$ years old. The average follow-up period after surgery was $8.2{\pm}2.7$ months. The visual analog pain scale taken before surgery was $6.7{\pm}1.1$ points, while the post-surgical scale was $2.4{\pm}0.9$ points showing a significant degrease [p < 005]. There were no complications that developed during surgery. Conclusion : A paramedian tangential approach is less invasive in the soft tissue than that of the median approach. This approach may effectively reduce nerve root compression and expand intervertebral foramens, and is devoid of the risk of spinal instability after surgery. The authors suppose that a paramedian tangential approach is quite an effective technique to relive compression in the far-lateral L5-S1 intervertebral space.

Anterior Cervical Microforaminotomy : A Minimally Invasive Anterolateral Approach for Spondylotic Lesions (전방 경추 미세 추간공 확대술 : 경추증에 대한 최소침습적 전측방 접근법)

  • Objective : Various surgical approaches have been implemented to fulfill the ideal goals of treatment for cervical spondylotic lesions. Conventional approaches are represented by anterior approach with or without fusion and posterior approach. The authors has applied newly developed anterior cervical microforaminotomy for these lesions on minimally invasive basis. Materials and Method : Twenty-one patients, with cervical HIVD, or stenosis, or both, underwent anterior cervical microforaminotomy between March, 1998 and April, 1999. Fifteen patients underwent unilateral decompression, and 6 bilateral decompression via unilateral foraminotomy. Operation of one level was performed in 16 patients, 2 levels in 4 patients, and 3 in 1 patient. The foraminotomy was accomplished by resecting the uncovertebral joint. Through this hole, compressed nerve root was decompressed by removing the spondylotic spur or disc fragment, and diagonal removing of posterior osteophyte from foraminotomy site to begining of contralateral nerve root made spinal cord decompression. Results : The outcome was excellent in 17 patients(81%) and good in 4 patients(19%) based on Odom's criteria. No complication was encounterd, and average post-operation hospital stay was 3.7 days. Conclusions : These results indicate that anterior cervical microforaminotomy provide adequate neural decompression, minimum postoperative discomfort and fast recovery.

A Study on the Foot and Ankle Disease of Shelter-based Homeless People (노숙인 거주 시설의 족부 족관절 실태에 대한 조사)

  • Purpose: Foot and ankle disease (FAD) is a frequent cause of morbidity among the homeless population. Various conditions, exacerbated by malnutrition, poor lifestyle habits, psychiatric disorders, physical injuries, poor hygiene, and limited access to healthcare, have been described in this population. The purpose of this study was to investigate the cognition and management status of FAD in shelter based homeless people. Materials and Methods: Fifty-two male and twenty-five female volunteer homeless individuals were recruited from two homeless shelters. Each person completed a questionnaire assessing any presence of pain, pain management, as well as foot and ankle care status. A foot and ankle surgeon examined the physical status of the individual's foot and ankle, including tenderness and instability. A radiologic evaluation was done for 18 male and 11 female homeless people who agreed to participate in this test for the existence osteophyte or joint space narrowing representing osteoarthritis and some reference angles for hallux valgus, flatfoot and cavus foot. Results: Homeless people had higher prevalence of body mass index, diabetes, and smoking than the general population. The most prevalent infectious disease was fungal infection (male 78%, female 68%), with a low compliance of management for FAD. Conclusion: Although most of shelter-based homeless people showed an acceptable foot and ankle management status, the potential risk for FAD development and exacerbation of mild FAD was high.

Anterior Cruciate Ligament Rupture in a Korean Native Cattle (한우 싸움소에서 발생한 전십자인대 파열)

  • A 9-year-old Korean native cattle was referred with chief complaint of left hind limb lameness during 2 months. He could not bear a weight on the left hind limb. On palpation, the stifle joint was swollen and mild fever was felt. In X-ray images, increased joint fluid, subchondral bone erosions, osteophyte formation along the trochlear ridge, and changes in the shape of the infrapatellar fat pad were shown. Cranial displacement of the tibia and intercondylar eminence could be seen. Based on the history, physical examination and radiographic findings, the bull was diagnosed as anterior cruciate ligament rupture. Because he could not participate in further bullfighting competition anymore, we indicated the slaughter as soon as possible.
