OSTEOPETROSIS WITH MAXILLARY OSTEOMYELITIS (상악골 골수염을 수반한 골화석증)

  • The authors observed a 33-year-old male patient who had suffered from osteopetrosis with maxillary osteomyelitis. The obtained results were as follows: 1. Physical examination revealed hepatosplenomegaly and bowing legs. 2. Nearly all bones of the entire skeleton showed unusual increased radiodensity and the paranasal sinuses were markedly obliterated. 3. There were seveal old fractured sites in the femur and hip bones and subluxation of hip joint. 4. The radiodensity of both jaws were prominently increased and there were many malformed teeth and impacted teeth. 5. Bone scan revealed multiple, irregularly increased uptakes in large joints and long bones.

  • Tuberculosis is a systemic disease with a world-wide distribution, and its occurance in the oral cavity is well documented in the literature. Disease of oral cavity and jaw caused by Mycobacterium tuberculosis is very rare, so it is often difficult to diagnose tuberculosis in the oral cavity. When granulomatous and ulcerative lesion persists in the oral cavity for a long time, it may be considered a tuberculosis. When differential diagnosis is needed, the most reliable indicators of mycobacterial infection are careful clinical evaluation, skin test, acid-fast staining, biopsy and culture. We report a case of tuberculous osteomyelitis which simultaneously occurred on the maxilla and mandible in a 85 years old man that proved diagnosis difficult, but which responded very well to surgical treatment and chemotherapy.

  • We experienced a case of primary sternal tuberculosis with destroyed midsternum and bony defect. An 22-year-old female was admitted to our hospital two times for severe sternal pain and spontaneous fracture without history of trauma. On hospital admission, chest X-ray and chest CT showed destruction of midsternum and soft tissue swelling. Fine needle aspiration cytology revealed tuberculous osteomyelitis with cold abscess. And the patient was treated with usual anti-tubeculosis medication for preoperative preparation. At operation, we confirmed midsternal destruction with cold abscess and multiple sinus tracts. After removal of diseased sternal segment and cold abscess, we performed sternal reconstruction with autologus iliac bone graft. The pathologic report was compatible with tuberculous osteomyelitis and caseous necrosis The postoperative course was smooth and uneventful, and she remains well without sternal instability two months later.

  • Primary subacute pyogenic osteomyelitis, or Brodie's abscess has received much attention since its initial documentation in the literature in 1832 by Sir Benjamin Brodie. Brodie's abscess is a localized form of chronic osteomyelitis that occurs most often in the metaphyseal area of the long bones of the lower extremities of young adults, Intermittent pain of long duration is the presenting complaint, along with local tenderness over the affected area. Laboratory evaluation is unrevealing, with a normal white blood cell count and differential count. The erythrocyte sedimentation rate may also be normal. Roentgenogram shows a markedly varied appearance and an abscess may be easily mistaken for various neoplasm. The most common organism cultured from abscess is Staphylococcus species. Treatment includes curettage of the lesion and administration of antibiotics. We present a case report (with a 1-year follow-up period), demonstrating the successful surgical treatment of Brodie's abscess of the distal metaphysis of the left tibia caused by Salmonella cholerasuis in a 33-year-0ld male who had no hemoglobinopathy.

  • Previously, strains of Streptococci genera were isolated from osteomyelitis caused by the post-infection after extraction. In present study, to test the sensitivity of the Streptococci strains against seven antibiotics, penicillin G, amoxicillin, tetracycline, ciprofloxacin, cefuroxime, erythromycin, clindamycin, and vancomycin, minimum inhibitory concentration (MIC) was performed using broth dilution assay. Our data showed that the value of MIC of the Streptococci against antibiotics were different among the strains. In addition, the degree of resistance to antibiotics of Streptococci strains was mainly depended on the origin of isolation. Our results suggest that the development of the rapid and accurate method to detect the antibiotics-resistant bacteria is need to prevent the misuse or abuse of antibiotics and outbreak of antibiotics-resistant bacteria.

  • The advent and wide use of antibiotics have decreased the incidence of deep neck infection. When a deep neck infection does occur, however, it can be the cause of significant morbidity and death, resulting in airway obstruction, mediastinitis, pericarditis, epidural abscesses, and major vessel erosion. In our clinic, a patient with diffuse chronic osteomyelitis of mandible and fascial space abscess and necrotic fasciitis due to odontogenic infection at the time of first visit came. We successfully treated the patient by early diagnosis using contrast-enhanced CT and follow up dressing through the appropriate use of radiographic images.

Controlled Release of Gentamicin Sulfate from Poly(3-hydroxybu-tyrate-co-3-hydroxyvalerate) Wafers for the Treatment of Osteomyelitis

  • Biodegradable wafers were prepared with poly (hydroxybutyrate-co-hydroxyvalerate) (PHBV;5, 10, and 15 mole% for 3-hydroxyvalerate) by simple heat pressing method for the sustained release of antibiotic agent, gentamicin sulfate (GS) to investigate the possibility of the treatment for osteomyelitis. The effects of hydroxyvalerate (HV) content, thickness of wafers, various types of additives such as sodium dodecyl sulfate (SDS), microcrystalline cellulose, polyvinylpyrrolidone, and hydroxypropylcellulose (HPC), and different initial drug loading ratio on the release profile have been investigated. In vitro release studies showed that different release patterns and rates could be achieved by simply modifying factors in the preparation conditions. PHBV wafers with 3 mm thickness, 10% of GS initial loading, 15% of HV content and addition of 5% of SDS and HPC were free from initial burst and a near-zero-order sustained release was observed for over 30 days. It might be suggested that the mechanisms of G5 release may be more predominant simple dissolution and diffusion of GS than erosion of PHBV in our system.
