Folates are involved in a variety of important biosynthesis by way of donating one carbon unit. Since folate metabolism was well understood a number of antifol have been developed. Among these antifols, aminopterin was first used in the treatment of childhood leukemia. However due to its toxicity and purity problems. it was immediately replaced by another antifols. methotrexate (MTX). MTX is shown to be active against various malignancies including leukemia breast cancer, osteogenic sarcoma, and head and neck cancer. Clinically, MTX therapy is divided into 3 categories. depeding on the dose administered; low-dose is defined as doses < $80\;mg/m^2$ intermediate-dose as doses $\geqq\;80\;mg/m^2$ and < $1000\;mg/m^2$ and high-dose as doses $\geqq\;1000\;mg/m^2$. Leucovorin should be administered to minimize MTX toxicities when MTX doses are greater than $80-100\;mg/m^2$. The clinical pharmacokinetics (ADME) of MTX is discussed in this text.

Proliferative periostitis is a rare form of osteomyelitis that is characterized by new bone formation with periosteal reaction common causes of proliferative periostitis are dental caries, periodontitis, cysts, and trauma. While proliferative periostitis typically presents as a localized lesion, in this study, we describe an extensive form of proliferative periostitis involving the whole mandibular ramus and condyle. Because the radiographic findings were similar to osteogenic sarcoma, an accurate differential diagnosis was important for proper treatment.
The contents of paper are over all incidence informed from 100 case of oral cancer patients who are diagnosed histopathologically in out patient clinic, department of oral surgery, Seoul university hospital during the period of 12 months in 1979. The patients were classified for the clinical evaluation and it's assessment as like age distribution, arising regional section, and groups divided according with final histopathological diagnosis. The following are obtained results of studies
1) As regards of age distribution of oral cancer, 5th decade age group presented the highest incidence, 6th and 4th decade age groups revealed next high incidency. 2) The most common sites of oral cancer were mandible body (33%), maxillae (26%), hard palate and buccal cheek (10% each), oral floor (8%) etc.
3) According to the histopathological diagnosis, cancer of oral cavity, squamous cell carcinoma is disclosed one of the most frequently presented malignantic oral tumor (58%), and nexts are mucoepidermoid carcinoma (15%0, malignant lymphoma (10%), adenocystic carinoma (5%), osteogenic and osteolytic sarcoma were 4% each.
4) In sexual differencies of oral cancer, more prominent incidency is showed in male (68%) than in female (32%).
To study effects of cellular FLICE (FADD-like IL-$1{\beta}$-converting enzyme)-inhibitory protein (c-FLIP) inhibition by RNA interference (RNAi) on sensitivity of U2OS cells to tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL)-induced apoptosis, plasmid pSUPER-c-FLIP-siRNA was constructed and then transfected into U2OS cells. A stable transfection cell clone U2OS/pSUPER-c-FLIP-siRNA was screened from the c-FLIP-siRNA transfected cells. RT-PCR and Western blotting were applied to measure the expression of c-FLIP at the levels of mRNA and protein. The results indicated that the expression of c-FLIP was significantly suppressed by the c-FLIP-siRNA in the cloned U2OS/pSUPER-c-FLIP-siRNA as compared with the control cells of U2OS/pSUPER. The cloned cell line of U2OS/pSUPER-c-FLIP-siRNA was further examined for TRAILinduced cell death and apoptosis in the presence of a pan-antagonist of inhibitor of apoptosis proteins (IAPs) AT406, with or without 4 hrs pretreatment with rocaglamide, an inhibitor of c-FLIP biosynthesis, for 24 hrs. Cell death effects and apoptosis were measured by the methods of MTT assay with 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide and flow cytometry, respectively. The results indicated that TRAIL-induced cell death in U2OS/pSUPER-c-FLIP-siRNA was increased compared with control cells U2OS/pSUPER in the presence or absence of AT406. Flow cytometry indicated that TRAIL-induced cell death effects proceeded through cell apoptosis pathway. However, in the presence of rocaglamide, cell death or apoptotic effects of TRAIL were similar and profound in both cell lines, suggesting that the mechanism of action for both c-FLIP-siRNA and rocaglamide was identical. We conclude that the inhibition of c-FLIP by either c-FLIP-siRNA or rocaglamide can enhance the sensitivity of U2OS to TRAIL-induced apopotosis, suggesting that inhibition of c-FLIP is a good target for anti-cancer therapy.
The estimated relative incidence of primary pulmonary sarcoma to lung cancer is 0.4%. Furthermore, osteogenic sarcoma of the pulmonary artery is extremely rare. We report a case of a 63-year-old woman who visited our emergency room with the chief complaints of chest pain, dyspnea and dizziness. On echocardiography, right heart failure due to acute pulmonary artery embolism was diagnosed and we performed emergency operation. After opening the main pulmonary artery trunk, we found a mass attached to the arterial wall and massive thrombi around the mass. The mass was diagnosed as primary pulmonary artery osteosarcoma through postoperative evaluation. The patient received chemotherapy and radiotherapy. The patient is alive without specific symptoms 16 months postoperatively.
Most clinicians have taken a lot of interest in tumors arising from the bony thorax because not only of their rarity and predictable diagnosis which could be reflected as a unique radiologic shadow but also variable surgical modes for maintenance of chest wall stability encountered after en-bloc resection. By the retrospective review, we have analyzed 24 cases of primary bony thorax tumors which were experienced and surgically treated at the St. Mary`s hospital of Catholic Medical College from Jan. 1969 to Sept. 1984. The results are as follows: 1. Age incidence was evenly distributed through all decades and the male-female ratio is 15:9. 2. 16 cases out of 24 were benign tumors and the commonest one of which was fibrous dysplasia. 3. Remaining 8 cases were malignant tumors and among which osteogenic sarcoma was the commonest one. 4. The majority of tumors [22/24] were developed from the rib and the remains were from the sternum. 5. Common manifestation were palpable mass or swelling and localized tenderness. 6. Various kinds of operative procedure were underwent: single resection of rib including tumor,14 cases; multiple resection of ribs with chemotherapy or myoplasty, 2 cases; en-bloc resection of the chest wall and reconstructive procedure, 5 cases; partial resection of sternum, 1 case; bone biopsy and chemotherapy, 2 cases.
Bronchoplastic procedure has been considered as an appropriate surgery for traumatic bronchial disruption and occasionaly for primary bronchial tumors or tuberculosis because it can bring preservation of pulmonary tissue for patients without compromising the chance for cure. Nowadays bronchoplastic procedure is also applicable for the selected cases of bronchogenic carcinomas with favorable long term survival, when compared to standard pneumonectomy.Eighteen bronchoplastic procedures were performed with or without pulmonary resection at Department of Thoracic and Cardiovascular Surgery, Catholic University Medical College, between 1990 and 1994. The patients were 11 men and 7 wemen with average age of 57 years [range, 19 to 71 years . Tumor comprised 56% of the lesions, including 6 squamous cell carcinoma [33% , 2 bronchial adenoma [11% , 1 leiomyoma and 1 metastatic osteogenic sarcoma. Cicatrical stenosis secondary to endobronchial tuberculosis and traumatic disruption occurred in 6 [33% and 1 patient respectively.Applied bronchoplastic procedures were as follows ; sleeve lobectomy, 8 cases [right upper : 6, left upper : 1, right middle : 1 : bronchial segmental resection without pulmonary resection, 2 cases : sleeve bi-lobectomy, 1 cases :patch dilating bronchoplasty with or without concomitant lobectomy in 7. There was no perioperative mortality. Morbidity in 4 patients included 1 transient recurrent laryngeal nerve palsy, 1 unstability of bronchial patch resulting atelectasis of afftected lung and 2 bronchial stenosis of anastomotic site.Throughout our experiences, we feel strongly that bronchoplastic procedure is a safe and effective surgical method preserving normal pulmonary tissue below affected bronchus for the wide range of various bronchial lesion including selected cases of bronchogenic carcinoma with acceptable complication and mortality.

Osteosarcoma is the most frequently encountered primary malignant tumor of the bone. But primary osteosarcoma of the skull(POS) is rare. The author presents a case of skull neoplasm identified as osteogenic sarcoma. A twentyseven-years-old male patient was admitted because of painful swelling at left temporal and zygomatic area with impairment of extraocular movement. Chest film and long bone series showed no evidence of abnormality. Skull films revealed round irregular bony destructive area at the left pterional area. CT and MRI revealed expansile destruction of the left sphenoid bone, lateral orbital wall and temporal bone within the hemorrhagic mass lesions which showed wall enhancement. Histologic examination confirmed a rare variant of osteosarcoma of the telangiectatic type.
This is a report of 3 cases of tumors, which primarily originated from ribs and the sternum. In the first case of multiple myeloma, the patient was 67 year old male with a tumor located on the middle sternum invading the manubrium and the body of the sternum manifesting symptoms after a contusion of the anterior sternum. The sternum was entirely resected and was replaced by tantalum plate to reconstruct the defective chest wall in order to prevent the paradoxical movement during respiration. In the second case of osteogenic sarcoma, the patient was 43 year old male with a tumor located on the costochondral junction of the left 5th rib for 6 months. The left 5th rib was resected between the middle part and sternochondral junction of it including tumor and adjacent soft tissues. In the third case of chondrosarcoma, the patient was 36 year old male with a tumor located near the posterior angles of the right 7th and 8th ribs manifesting back pain on the area where the tumor was located. Resection of right lower lobe was performed since direct invasion of tumor was seen in the superior segment of right lower lobe. This was followed by the resection of both 7th and 8th ribs at the area between the costovertebral junction and the portion 10 em apart from the tumor including the tumor and intercostal soft tissues. Diagnoses of 3 cases of tumors described above were confirmed by histopathologic examination postoperatively. The postoperative courses were uneventful.
Although primary surgical excision is the treatment of choice for the vast majority of patients with nasopharyngeal angiofibroma, radiation therapy is also employed as a primary tool or in the cases of tumor recurrence or massive intracranial extension. But, especially in young patients, postradiation sequelae such as osteomyelitis, atrophic rhinitis, thyroid or bone malignancy are of concern. Postirradiation osteosarcoma is a rare, but well-recognized complication of radiotherapy, which comprise roughly 2 to 5 percent of all osteogenic sarcoma. Recently we experienced a case of postirradiation osteosarcoma of the mandible who had treated by radiation therapy for the nasopharyngeal angiofibroma 30 years ago, presently he is ongoing chemotherapy and relatively being well.
