경피적 나사못 고정술로 치료한 Maisonneuve 골절 (Maisonneuve Fractures Treated with Percutaneous Screw Fixation)

  • Purpose: To evaluate the clinical validity of percutaneous screw fixation in Maisonneuve fracture. Material and Methods: Out of 5 Maisonneuve fracture patients hospitalized in the department of orthopedic surgery of Seigang hospital from February 1995 to May 1998, 4 patients were treated with percutaneous screw fixation and 1 patient was treated with percutaneous screw fixation and repair of deltoid ligament altogether. The results were evaluated on the clinical and radiological finding. Results: In all cases, the range of motion of ankle joint was normal, the complication such as postoperative pain and post-traumatic arthritis were not found. Widening of medial clear space or talar shift were not found in the follow up X-ray and tibiofibular distance, compared with normal ankle joint was same on follow up CT. Conclusion: Percutaneous screw fixation is simple, less invasive and more effective method in the treatment of Maisonneuve fracture.

  • Management of postoperative tarsal tunnel syndrome is a perplexing problem to the surgeons. Autogenous vein wrapping graft is a effective method to decompress the tarsal tunnel. We treated 2 cases of postoperative tarsal tunnel syndrome with autogenous saphenous vein wrapping graft technique, and could get good results. Autogenous vein wrapping graft seems to be a good technique for tarsal tunnel syndrome with severe adhesion, particularly after surgery, and postoperative nerve adhesion can be prevented.

  • A posterior 3-portal arthroscopic approach with the patient in the prone position provides a novel and optimal approach for isolated subtalar arthrodesis. This approach facilitates access to the posterior talocalcaneal facet and facilitates safe access with regard to the posteromedial neuromuscular bundle. The technique involves prone positioning, establishment of two posterolateral portals and one posteromedial portal, arthroscopic posterior talocalcaneal facet debridement, percutaneous morcellized bone grafting and internal screw fixation. Preliminary results have shown high patient satisfaction, an excellent fusion rate and less postoperative morbidity than open subtalar arthrodesis.

  • Osteochondral lesion usually occurs in the elbow, knee and ankle joints. Many articles about osteochondral lesion of the talus in the ankle joint have been reported. We experienced a rare case of partially detached osteochondral lesion of the tibial plafond treated with excision and multiple drilling.

  • Occurrence of pseudoaneurysm in the foot and ankle is rare, and is usually caused by traumatic injury or by iatrogenic intervention. Iatrogenic pseudoaneurysms in the foot and ankle have been observed after rearfoot and ankle fusions, ankle arthroscopy, endoscopic and open plantar fasciotomy, tibial osteotomy with limb lengthening, midfoot amputation, and Lapidus procedure. We report on a patient who developed a pseudoaneurysm of the dorsal metatarsal artery following correction of hallux valgus. The patient underwent proximal chevron osteotomy and Akin phalangeal osteotomy. The feeding artery was ligated and the pseudoaneurysm was excised.

  • The 48 years old female who complained of painful swelling with discharge on the medial end of the left clavicle which has been lasting 3 months. We experienced arare case of tuberculosis that involved only the clavicle without other involvement. Diagnostic disturbance delayed appropriate medical therapy, leading to development of a discharging sinus with secondary infection, which further confused pathologic feature. The ability of this disease to mimic many skeletal pathologies, this has to be included in the differential diagnosis, especially at unusual sites.

  • Intraosseus ganglion cysts are uncommon, benign, juxta-articular and usually found within long bones. A 35-year-old male presented with acute right ankle pain. He denied any traumatic event. Tenderness was localized on the subtalar joint. Radiologic studies demonstrated a cystic lesion in the juxta-articular portion of the posterior facet of the calcaneus. The patient underwent excision and curettage. Postoperative recovery was uneventful. A histological examination confirmed a typical intraosseus ganglion cyst. A case of an intraosseus ganglion cyst of the calcaneus causing acute subtalar joint pain is described.

  • We presented a case of posteriorly displaced fracture of the distal clavicle in Child. The closed reduction was failed due to posteriorly displaced proximal fragment and interpositon of trapezius muscle and periosteum. We found that periosteal sleeve and coracoclaviclar ligament and acromioclavicular ligament was intact. The open reduction should be considered for the treatment of the irreducible posteriorly displaced fracture of the distal clavicle in child.
