청소년의 교정치료 희망여부에 따른 외모인식도와 외모 만족도 및 자아존중감 비교 (The comparison of the appearance perception, satisfaction, and self-esteem following the adolescent orthodontic treatment demand)

  • Objectives : The purpose of the study is to investigate the relationship between appearance perception, appearance satisfaction, and self-esteem in adolescents who demand the orthodontic treatment. Methods : A total sample of 403 high school students filled out the questionnaire and data were analyzed by using SPSS ver 20.0. for n (%), chi-square test, t-test, and a logistic regression. Results : Girl students (68.3%) tended to have a higher demand for orthodontic treatment than male students (31.7%). Higher appearance perception(t=6.967, p<0.001), lower appearance satisfaction(t=-5.477, p<0.001) and lower self-esteem(t=-7.333, p<0.001) were higher in group of demand for orthodontic treatment. The logistic regression analysis revealed that a gender, self-perception of their teeth, appearance perception, and self-esteem were significantly associated with orthodontic treatment demand. Conclusions : The result of this study will provide basic data for the consideration of psychosocial factors in orthodontic treatment in adolescents.

Patients' and parents' concerns and decisions about orthodontic treatment

  • Objective: Patients' and parents' expectations are important in orthodontic treatment decision making. The literature generally demonstrates the perceived benefits of orthodontic treatment, but patients' and their parents' concerns about orthodontic treatment have not been investigated comprehensively. The aim of this study was to identify patients' and parents' concerns about orthodontic treatment and compare them according to sex, age, and treatment demand level. Methods: One hundred and eighty-nine children and their parents were interviewed about concerns related to orthodontic treatment. Patients and parents were asked about orthodontic treatment decisions. Answers were recorded as "yes," "no," or "don't know." Chi-squared and Fisher's exact tests were used to compare concerns between age groups, sexes, and treatment demand levels. Kappa statistics were used to assess agreement between patients and their parents. Results: Concerns about orthodontic treatment were gathered under 10 items as follows: "feeling pain," "the appearance of braces," "being teased," "avoiding smiling," "speech problems," "dietary changes," "problems with transportation," "economic problems," "long treatment duration," and "missing school." There was no statistically significant difference in concerns between the sexes or age groups. Some concern items and treatment demand were inversely related in patients. Conclusions: The results of this study demonstrate patients' and parents' concerns about orthodontic treatment. Differences between the concerns of patients with different treatment demands imply that children might reject orthodontic treatment because of their concerns. Appropriate consultation of patients addressing their concerns may help reduce anxiety and improve the acceptance of treatment.

심미치료를 위한 전치부 부분교정 (Anterior teeth alignment for aesthetic dentistry)

  • As the demand for natural and beautiful smiles increases, the demand for anterior aesthetic treatment is increasing. Orthodontic treatment is often necessary for esthetic, healthy and natural treatment outcome. Particularly, in the case of middle-aged patients, minor tooth movement limited to anterior teeth is more effective than comprehensive orthodontic treatment which requires a long-term treatment period. Clinician who is in charge of aesthetic dentistry should have the ability to select a case that can be treated with partial orthodontic treatment and to determine the most effective treatment method. This article provides decision flowchart for case selection and choosing the best treatment modality for anterior teeth alignment.

  • In this study, in order to survey the effect on public recognition in malocclusion and orthodontic treatment by brochures made by Korean orthodontic association, sample were divided into control and experimental group and a questionnaire was inguired and the results were as follows: 1. In the experimental group, frequency of recognition for the need of orthodontic treatment was high, and this indicates dental health behavior and recognition can be inspired by adequate giving of information. 2. Between the control and the experimental group, patterns of the negative opinion was different, and especially the demand for specific orthodontic treatment can be influenced by the content of brochures. 3. The brochures used in this study can call attention to develope the orthodontic aspect for the malocclusion, hereafter more systemic and active measures for development of public dental behavior and recognition should be needed.

Part II. What drives Korean adults to seek orthodontic treatment: Factors contributing to orthodontic treatment decisions

  • Objective: This study aimed to identify the perceptions of orthodontic treatment among Korean adults and determine the factors that drive them to seek orthodontic treatment. Methods: A total of 2,321 adults aged 19-64 years were surveyed using an internet research system from a specialized research company. The participants were divided into the following groups based on their experience of and willingness to undergo orthodontic treatment: experience, acceptance, and non-acceptance groups. The characteristics of the participants were compared using analysis of variance with post-hoc analysis. Multinomial logistic regression analysis was performed in all three models with the non-acceptance group as a reference. Results: In terms of demographic characteristics, age, gender, marital status, and education had significant influences on orthodontic treatment decisions in adults in the experience and acceptance groups (p < 0.001). When all the factors were analyzed, age, marital status, past dental treatment experience, regular oral examinations, demand for orthodontic treatment, optimal treatment period, health insurance coverage, information on orthodontic treatments, perceptions regarding orthodontic treatment, and psychosocial impact of dental esthetics significantly influenced orthodontic treatment decisions in adults in the experience and acceptance groups (p < 0.001). Conclusions: These findings suggest that various factors influence orthodontic treatment decisions in adults. Individuals who seek orthodontic treatment were found to undergo more regular dental treatment and oral examination than those who did not. They also had a better perception of orthodontic treatment and more negative values for the psychosocial impact of dental esthetics.

1991학년도 연세대학교 학생을 대상으로 한 부정교합 빈도에 관한 연구 (A STUDY ON THE PREVALENCE OF MALOCCLUSION OF YONSEI UNIVERSITY STUDENTS IN 1991)

  • Over recent 20 years, with socioeconomic development and change of recognition of the population, more people are concerned about their health and appearance. To obtain the change of frequency of malocclusion and the demand for orthodontic treatment, with this trend, 2460 freshmen and students of Yonsei Univ. in 1991, aged from 18 to 21 were examined excluding 187 students who have history of orthodontic treatment, 86 students who are undergoing treatment, 39 students who have too much missing teeth to classify. After analize the frequency of malocclusion and the demand for orthodontic treatment, following results was obtained. 1. Sex ratio in the prevalence of malocclusion was $91.7\%/90.8\%$, male to female, so there was no sex predilection. 2. With regard to Angle's Classification, each percentage of Class I, Class II div. 1, Class II div. 2 and Class III was $61.6\%,\;11.3\%,\;1.9\%\;and\;16.7\%$. 3. Of Class I malocclusion, percentage of crowding was 53.2, this occupied the largest part of single findings and prevalent findings in combinations were also crowding - crossbite and crowding - Protrusion. 4. Of Class I malocclusion, over the portion of$95\%$, Bialveolar Protrusion have arised alone. 5. The distribution of Demands for orthodontic treatment of malocclusion were $41.3\%$ in males, and $51.3\%$ in females.

심미보철치료를 위한 교정치료의 원칙과 고려사항 (Principles and considerations of orthodontics for Prosthodontic treatment)

  • Outcome of esthetic ceramic restorations are affected by tooth size, gingival contour, occlusal relationship, etc. For this reason, demand of orthodontic treatment before esthetic ceramic restoration is increasing. If a Bolton ratio discrepancy, a problem of the maxillary incisor's vertical position, a problem of inclination of anterior teeth, a pathogenic occlusion is existed, a pre-prosthodontic orthodontics should be accomplished. These problems can be satisfactory only after the prosthetic treatment is performed after orthodontic treatment. When orthodontic treatment is given, it should be treated with the following principle. 1. Treat it in the direction of functioning occlusion. 2. Keep the patient's stable occlusal scheme. 3. Treat the teeth by considering the average tooth size and Bolton ratio. Ortho-Prostho combined treatment with optimal treatment plan can lead a patient's function, esthetics, and long-term stability.

Associations between body dysmorphic disorder (BDD) with the dental health component of the index of orthodontic treatment need (IOTN-DHC) and other BDD risk factors in orthodontic patients: A preliminary study

  • Objective: Body dysmorphic disorder (BDD) is a form of obsessive-compulsive disorder that may be negatively associated with the self-image. It might be associated with orthodontic treatment demand and outcome, and therefore is important. Thus, this study was conducted. Methods: The Yale-Brown Obsessive-Compulsive Scale modified for Body Dysmorphic Disorder (BDD-YBOCS) questionnaire was used in 699 orthodontic patients above 12 years of age (222 males, 477 females), at seven clinics in two cities (2020-2021). BDD diagnosis and severity were calculated based on the first 3 items and all 12 items of the questionnaire. The dental health component of the index of orthodontic treatment need (IOTN-DHC) was assessed by orthodontists. Multivariable and bivariable statistical analyses were performed on ordinal and dichotomized BDD diagnoses to assess potentially associated factors (IOTN-DHC, age, sex, marital status, education level, and previous orthodontic consultation) (α = 0.05). Results: IOTN-DHC scores 1-5 were seen in 13.0%, 39.9%, 29.8%, 12.4%, and 4.9% of patients. Age/sex/marital status/education were not associated with IOTN-DHC (p > 0.05). Based on 3-item questionnaire, 17.02% of patients had BDD (14.02% mild). Based on 12-item questionnaire, 2.86% had BDD. BDD was more prevalent or severer in females, married patients, patients with a previous history of orthodontic consultation, and patients with milder IOTN-DHCs (p < 0.05). Conclusions: IOTNDHC was negatively/slightly associated with BDD in orthodontic patients. Being female and married may increase BDD risk.

  • The author studied the quality and change of the effective demand for dental treatment of the cutpatients of the Wonkwang dental clinic from the March of 1982 to the August of 1983. The results were compared with the previous reports to find any changes that might come from the effect of dental insurance system. After discussing the results, the author concluded as follows: 1. The twenties were the most in the age groups, but it is desirable that age groups of mixed dentition be the most to enhance the dental health status of community. 2. The effective demand for dental treatment increased especially in the areas such as intraoral radiograph taking, amalgam filling, temporary filling, endodontic treatments, and drug application on the soft tissue, but decreased in the areas such as prosthodontic treatments, and there were no demand for the orthodontic treatment. 3. As compared with the absolute needs, the effective demand was relatively high for teeth extraction and amalgam filling, but it was generally very low. 4. So, the present extent of dental insurance system should be expanded, and the dental care delivery system should be established within the community. Finally, the economic and social status of Iri community should be enhance by political and educational plannings.

최적구조설계를 이용한 교정브라켓 성능향상에 관한 연구 (A Study on Improvement of Correction Bracket Performance Using Optimal Structural Design)

  • There has always been a demand for orthodontic treatment. Orthodontic treatment allows tooth to be arranged by flexible arch wire fixed with tooth-attached brackets. Arch wire generate constant pressure to tooth brackets which moves the teeth to proper place. When the bracket transmits force, the braced wing of the bracket may deform. Deformed tie wing will lead to lost tension of elastic ligature. Then, lacking grip between tie wing and ligature might delay the tooth movement. Furthermore, tooth brackets used for orthodontic treatment make contact with in direct oral surface and this cause feeling of irritation that comes from height of tooth braces. This study suggests an optimal teethe bracket design to make up for inconvenience by shorten the height of bracket and complement the shape of bracket to reduce strain rate using finite element analysis. As a result, new optimal design of teethe bracket indicates lower strain rate of the bracket wing and takes good effects of shorten body height in terms of convenience.
