  • 교합기에 거상된 모형 상에서 각 치아의 순, 협설측경사와 근원심경사를 적절하게 조절할 수 있도록 개발된 측정기구를 교정 치료의 마무리 단계에서 술자가 원하는 보다 정확하고 좋은 교합으로 유도하는 tooth Positioner의 제작을 위한 셋업 모형 제작에 이용하는 방법을 보여 주고자 한다.

양측성 구순구개열 신생아의 치료 (Treatment of the infant with bilateral cleft lip and palate)

  • The case unveils an early orthodontic intervention on 3-week old infant innately with bilateral cleft lip and palate. Presurgical Nasoalveolar Molding(PNAM) procedures were carried out for 2 months for the sake to diminish the anticipated strain of postsurgical scar by means of the retraction of protruded premaxilla and the extension of collapsed columella. The gap on the alveolar cleft decreased by 2,5 mm, and the columella manifested 1. 5 mm increase of its length, which yielded the consequent downward and backward movement of premaxilla, and expected to bring down the technical complexity of primary lip surgery. PNAM with sophisticated technical procedures at an optimal timing disclosed the passive molding of the alveolar segments and the formation of nasolabial soft tissue integuments and permitted one-time primary lip surgery.

  • The most common orthodontic methods of treating mandibular transverse deficiencies is extractions, interdental stripping, and other dento-alveolar compensation but it can not addressesd about skeletal problem This study assessed the treatment outcomes after surgically assisted rapid tooth orthodontics using the symphysis osteotomy and dentoalveolar distraction osteogenesis technique. The applications of distraction osteogenesis in mandibular widening, by symphysis osteotomy, has emerged as a definitive, predictable and better stability. The most important factors in mandibular widening is performed with simple surgical technique and devices. As a results, these techniques are very useful and effective in cases of difficult tooth movement in adult orthodontics transverse problems There were few intraoperative or postoperative complications and were not clinically significant.

  • Forces and moments delivered by a straight wire connecting two orthodontic brackets are statically indeterminate and cannot be estimated using the classical equations of static equilibrium. To identify the mechanics of such two-bracket systems, Burstone and Koenig used the principles of linear beam theory to estimate the resulting force systems. In the original publication, however, it remains unclear how the force systems were calculated because no reference or computational details on the underlying principles have been provided. Using the moment carry-over principle and the relative angulation of the brackets, a formula was derived to calculate the relative moments of the two brackets. Because of the moment equilibrium, the vertical forces that exist as a force-couple on the two brackets can also be calculated. The accuracy of the proposed approach can be validated using previously published empirical data.

  • The patient,20 year-old female, complained of anterior open bite. She had tongue thrusting habit and suffered from maxillary sinusitis since 12-years old. Cephalometric analysis revealed slightly forward relationship in maxilla, but normal in mandible. The anterior open bite was corrected through retraction and extrusion of anterior segment and mesial movement of posterior segment under multibanded system after extraction of 4-first premolars. After 2 years and 3 months, she gained ideal overbite, overjet and good interdigitation of buccal segment.

  • The movement of teeth during orthodontic treatment requires bone remodeling process in periodontal tissue. To find out the changes occuring in the cell itself, mechanical force was applied to the cultured periodontal ligament cells. Following results were obtained from measuring the changes in cyclic AMP and $PGE_2$, $^3H$-thymidine incorporation amount in time lapse after application of mechanical force. 1. When mechanical force was applied to cultured PDL cells, the amount of cAMP in cells were increased significantly after 15 min. of force application, but were decreased gradually as time lapsed. 2. When mechanical force was applied to cultured PDL cells, the amount of PGE2 were increased at 20,40,60 min. and was significantly increased at 20 min. 3. When mechanical force was applied to cultured PDL cells, the amount of $^3H$-thymidine incorporation was some increased, but was not statistically significant.

  • Treatments for restoring the function and esthetics of missing teeth include fixed bridge, partial denture, orthodontic movement, implantation and autotransplantation. However, there is no absolute indications for each techniques. Due to undevelopment of root and root resorption after autotransplantation, the success rate of autotransplantation over the past decades have been low. Recently. with the study on biological principles of the healing of periodontal ligament, the success rate of autotransplantation began to improve. We report the cases of successful autotransplantation which resulted in ideal healing of periodontal ligament, gingiva and alveolar bone.

  • Relapse following rotational movement of the tooth is a common problem in orthodontic practice. To overcome such relapse, many procedures have been advocated: prolonged retention, permanent retention, over-rotation, rotation of teeth at an early age, surgical procedures such as gingivectomy, redressement force, septotomy, et cetra. A 23-year-old woman presented with Angle's Class I malocclusion and extreme rotation of maxillary central incisors. After 15 months' active therapy, septotomy was performed on maxillary central incisors and Howley retainer was applicated for the purpose of overcoming rotational relapse. During the 1 year post-operative observation, negligible, if any, rotational relapse occurred.

인공타액하에서 교정선과 Bracket간의 마찰력에 대한 연구 (FRICTIONAL FORCES BETWEEN ORTHODONTIC WIRE AND BRACKET UNDER ARTIFICIAL SALIVA)

  • It has been suggested that the frictional force between bracket and arch wire may impede the tooth movement. The present study was aimed to compare and analyze the effect of wire size, type of ligation, and duration of ligation on the magnitude of frictional force between cobalt chromium wire and stainless steel bracket under the artificial saliva. The results were as follows: 1. Type of ligation and size of wire were the main influencing factor on the level of friction. 2. Stainless steel ligature generated higher frictional forces than elastomeric module. 3. The rectangular wire consistently exhibited more frictional force values than round wires, while there was no significant difference between frictional forces of round wires. 4. In elastic ligature, frictional force decreased with time. 5. Artificial saliva had no significant influence on the frictional force between cobalt chromium wire and bracket.

양악 수술 시 상악골 상방 이동에 따른 상기도 변화 (A study of upper airway dimensional change according to maxillary superior movement after orthognathic surgery)

  • 양악 수술 시 상악골의 상방이동을 시행하였을 경우에 상기도의 공간적 구조변화를 평가하기 위하여, 술전, 술후, 술후 6개월 후의 간격으로 두부규격방사선사진을 촬영하여 그 변화량을 분석해 보았다. 술전 교정치료를 시행하여 상악은 Le Fort I 골절단술로 상방이동 시행하였고 동시에 하악은 후방이동 시키는 수술을 시행 받은 24명을 대상으로 하였다 (평균 연령22세 1개월, 남자 9명 여자 15명). 상악골 상방이동에 대한 상기도 공간의 변화와 그에 따른 관련성을 조사한 결과, PAS (R)부위는 술후 (T1) 감소하였으나 (p<0.01) 술후 6개월 후(T2)에는 다시 증가하여 술전과 비교 시, 크기 변화를 관찰할 수 없었고, PAS (NL) 부위는 술후(T1)와 술후 6개월 후(T2)에서 유의성 있는 크기 증가를 보였다. PAS (OL)의 경우, 술후 (T1)증가를 보이다가 술후 6개월 이후(T2)에서 감소하였다. 연구개의 두께는 술후 (T1) 증가를 보이다가 6개월 이후 (T2) 처음과 같거나 약간 감소하는 것으로 나타났으며 FH-uvular 각도는 술후 6개월 이후 증가하였다. 또한 상악골 상방이동에 대한 상기도 공간의 변화를 회귀분석 시행한 결과 양악수술 시 평균 $4.40{\pm}1.14mm$의 상악골 상방이동은 양악 수술 후 상기도 공간의 변화에 큰 영향을 미치지 않는 것으로 나타났다.
