Correction of dental Class III with posterior open bite by simple biomechanics using an anterior C-tube miniplate

  • In the correction of dental Class III molar relationship in skeletal Class II patients, uprighting of the mandibular posterior segments without opening the mandible is an important treatment objective. In the case reported herein, a C-tube miniplate fixed to the lower labial symphysis and connected with a nickel-titanium reverse-curved archwire provided effective uprighting of the lower molars, without the need of orthodontic appliances on the mandibular anteriors. Using this approach, an appropriate magnitude of force is exerted on the molars while avoiding any negative effect on the mandibular anteriors.

Nonsurgical correction of a severe anterior deep overbite accompanied by a gummy smile and posterior scissor bite using a miniscrew-assisted straight-wire technique in an adult high-angle case

  • In the present report, we describe the successful use of miniscrews to achieve vertical control in combination with the conventional sliding MBT$^{TM}$ straight-wire technique for the treatment of a 26-year-old Chinese woman with a very high mandibular plane angle, deep overbite, retrognathic mandible with backward rotation, prognathic maxilla, and gummy smile. The patient exhibited skeletal Class II malocclusion. Orthodontic miniscrews were placed in the maxillary anterior and posterior segments to provide rigid anchorage and vertical control through intrusion of the incisors and molars. Intrusion and torque control of the maxillary incisors relieved the deep overbite and corrected the gummy smile, while intrusion of the maxillary molars aided in counterclockwise rotation of the mandibular plane, which consequently resulted in an improved facial profile. After 3.5 years of retention, we observed a stable, well-aligned dentition with ideal intercuspation and more harmonious facial contours. Thus, we were able to achieve a satisfactory occlusion, a significantly improved facial profile, and an attractive smile for this patient. The findings from this case suggest that nonsurgical correction using miniscrew anchorage is an effective approach for camouflage treatment of high-angle cases with skeletal Class II malocclusion.

피질골절단술을 이용한 상악협소증환자의 치험례 (CORTICOTOMY IN PATIENT WITH NARROW PALATAL ARCH)

  • 상악협소증으로 인하여 골격관계의 수평적인 부조화와 부정 교합을 보인 상기환자의 경우 상악확대를 위해 구개확대장치를 이용하여 치료하였으나 원하는 치료결과를 얻을 수 없었다. 방사선 평가상 안면골의 봉합부골화가 이미 끝난 것으로 보여 외과적인 피질골절단술을 시행하였다. 외과적인 피질골절단술은 상악확대시 주위안면골의 저항을 줄이기 위한 목적으로 시행하는데 좋은 결과를 얻을 수 있었다. 술후 2주경과후와 12개월 경과후 교합관계 및 확대된 상악궁의 재발상태를 평가한 결과 술후 상태로 유지되고 있었다. 전반적인 교합상태도 많이 개선되어 있었다. 그러나 피질골절단술은 사전에 악궁의 형태나 길이, 수술방법의 선택, 술후 합병증에 대한 충분한 평가를 하여야 한다. 외과적인 피질골절단술은 치료비와 치료기간을 줄일 수 있고 술후 재귀가능성을 줄일 수 있다. 이에 본과에서 피질골절단술을 이용하여 양호한 결과를 얻었기에 본 학회지에 보고하는 바이다.

비 치료목적으로 시술되고 있는 치아교정과 양악수술에 대한 일부 대학생들의 인식도에 관한 연구 (Non-therapeutic purposes in orthodontics and correction of tooth and jaw surgery for the recognition of some students on research)

  • Objectives : In this study, the treatment method for some college students, due to the misuse of non-therapeutic molding technique non-therapeutic procedure is applied in orthodontics or jaw surgery and therapeutic purposes, such as to properly recognize and to investigate whether. Methods : In this study used, 4 questions, the questionnaire general characteristics (such as gender, major, whether the treatment experience and purpose) Orthodontic and knowledge about the jaw surgery (Corrective jaw surgery hope and reason, or jaw surgery benefits, side effects, etc.) 6 items, calibration and recognition jaw surgery on seven items 17 items was constructed. Results : Of the perception of the majors of the respondents according to the presence or absence of a non-therapeutic procedure for calibration or jaw surgery the correct way, and the purpose appeared to know roughly majors and all students majoring statistically significant (p<.001). The degree of recognition for non-therapeutic procedures according to the gender of the respondents' facial aesthetic improvement, and the most common reason, you want to make a non-therapeutic procedure, correction or contrast, I've found that looks supremacist influence on the non-therapeutic procedure or roughly know the correct way, and jaw surgery for the purpose that appeared to be statistically significant (p<.05). Conclusions : Non-therapeutic procedure for the interest and knowledge to promote oral health as well as esthetic improvements to the original non-therapeutic procedure for therapeutic purposes and how to deliver education and it is very important.

기능교정이 만성 허리 통증 환자의 통증과 기능장애 및 건강관련 삶의 질에 미치는 영향 (Effect of Functional Adjustment Procedure on Pain, Dysfunction and, Health-related Quality of Life in Patients with Chronic Low Back Pain)

  • PURPOSE: This study was conducted to verify the effects of a functional correction of the pain of patients with chronic low back pain(CLBP), and to examine the effect of dysfunctional factors on health-related quality of life. METHODS: A preliminary survey was first conducted on 90 patients with CLBP after functional orthodontic treatment. Some revised questionnaires were also prepared. The survey was distributed for approximately eight weeks, and 215 copies were used as the final analysis data, except for questionnaires that were inadequate, error or non-response. RESULTS: Path analysis using the structural equation model of CLBP patients showed a positive correlation between all the path coefficients and the potential factors. The multidimensional relationship between pain and dysfunction after orthognathic treatment was confirmed using three subdivisions of the pain variables as independent variables and the dysfunctional variables as the dependent variables. Multiple regression analysis was performed to examine the effects of pain on the dysfunction. To identify the multidimensional relationship between dysfunction and the health-related quality of life, eight sub-factors of dysfunctional variables were set as the independent variables, and multiple regression was analyses were performed with the dependent variables of the health-related quality of life. CONCLUSION: This study examined the structural and influence relationships of the functional correction with pain, dysfunction, and health-related quality of life. The results, suggest that a functional orthodontic treatment can be used as a positive program for the health-related quality of life. In addition, this study is meaningful in that it provieds useful information for intervention such as psychosocial change of patients.

In vitro Antibacterial Effect of Orthodontic Adhesives Mixed with Silver Nanoparticles

  • Purpose: To examine the antibacterial effectiveness of silver nanoparticles (SNP) mixed with commercial orthodontic adhesives. Materials and Methods: SNP was prepared by dissolving silver perchlorate in an organic solvent and reducing it with ultraviolet radiation. SNP was then mixed with four commercial orthodontic adhesives (Light Bond, Blugloo, Transbond XT, and Fuji Ortho LC) (0.05 wt %), which were then formed into disc-shape specimens ($8.0mm{\times}3.0mm$). Commercial orthodontic adhesives containing no SNP were used as the control groups. Specimens of the four experimental and four control groups were incubated with streptococcus mutans and the medium turbidity was assessed at 3, 6, 9, 12, and 24 hours after incubation. The agar diffusion test was also performed to examine the growth inhibition zone of these groups. The data were statistically analyzed using a Wilcoxon rank sum test and t-test with a Bonferroni's correction (P<0.05). Result: The SNP containing groups had a superior antibacterial effect compared to the control groups. In the agar diffusion test, the control groups without SNP did not produce an inhibition zone, whereas the SNP containing groups showed inhibition zone of 10~13 mm. Conclusion: The incorporation of SNP into orthodontic adhesives can inhibit cariogenic bacterial growth.

  • This is to report a case of surgical and orthodontic of posterior scissor bite, deep bite and gummy smile by the segmental osteotomies. The surgical thechnics procedures used are Peterson's mandibular posterior segmental osteotomy, modified $K{\ddot{o}}le$ technic for mandibular anterior segment and Wunderer's maxillary anterior segmental osteotomy. The results are as follows : 1) Peterson's mandibular posterior segmental osteotomy could be achieved by the buccal approach with some difficulties in accessbility. 2) Upper and lower anterior segmental osteotomies were followed separately to correct the deep curve of Spee, deep bite and gummy smile in shortened period. 3) All alveolar segments were immobilized in preplanned position by the prefabricated palatal and lingual resin splint, therefore intermaxillary fixation was not necessary.

Esthetic enhancement of a traumatized anterior tooth with a combination of forced eruption and tooth alignment: a case report

  • Exposing sound structure of a subgingivally fractured tooth using orthodontic extrusion is considered to be a conservative way to re-establish biologic width without sacrificing esthetics or jeopardizing periodontal support of neighboring teeth. When a misaligned tooth is traumatically involved, a more comprehensive approach combining tooth extrusion and re-alignment may be necessary for a successful restorative outcome. This case report describes a successful esthetic management of a patient with complicated crown-root fracture on the maxillary right central incisor and pre-existing malocclusion in the maxillary anterior region. Forced eruption along with re-alignment of teeth by orthodontic movement seems to allow re-positioning of the fracture line to a favorable position and correction of crowding, providing a better esthetic result.

Correction of Angle Class II division 1 malocclusion with a mandibular protraction appliances and multiloop edgewise archwire technique

  • A Brazilian girl aged 14 years and 9 months presented with a chief complaint of protrusive teeth. She had a convex facial profile, extreme overjet, deep bite, lack of passive lip seal, acute nasolabial angle, and retrognathic mandible. Intraorally, she showed maxillary diastemas, slight mandibular incisor crowding, a small maxillary arch, 13-mm overjet, and 4-mm overbite. After the diagnosis of severe Angle Class II division 1 malocclusion, a mandibular protraction appliance was placed to correct the Class II relationships and multiloop edgewise archwires were used for finishing. Follow-up examinations revealed an improved facial profile, normal overjet and overbite, and good intercuspation. The patient was satisfied with her occlusion, smile, and facial appearance. The excellent results suggest that orthodontic camouflage by using a mandibular protraction appliance in combination with the multiloop edgewise archwire technique is an effective option for correcting Class II malocclusions in patients who refuse orthognathic surgery.
