  • The WHO separates odontogenic carcinomas into three groups : malignant ameloblastoma, primary intraosseous carcinoma(PIOC), and carcinomas arising from odontogenic epithelium including those arising from odontogenic cysts. In WHO criteria, primary intraosseous carcinoma is defined as a squamous cell carcinoma arising within the jaw, having no connection with the oral mucosa, and no developing from residues of odontogenic epithelium. This is a case of 52-year old man who had prolonged jaw pain and final diagnosis was primary intraosseous carcinoma(PIOC) on mandible. We obtained successful result after composite resection combined with hemimandibulectomy, RND, following reconstruction with latissmus dorsi myocutaneous flap, and postoperative radiation therapy.

수은 중독 1예 -병예 보고- (Accidentally Induced Mercury Poisoning by Charlatan -Report of a Case-)

  • Dental mercury was injected accidentally to left upper buccal mucosa of a 16-year-old girl by charlatan in order to anesthetize a tooth for extraction; thereafter,injected elemental mercury was almost removed by drug adimnistration and surgical operations.One year have passed, still, there are many scaffered small radiopaque white globular cimages in dental, maxillo-facial, and chest radiographs. Redish swollen gingiva and mobility of left upper central incisor which is adjacent to mercury injection site are oral manifestations in this mercury poisoning case. Recently, the patient complains of metallic taste, gastric discomfort and abdominal pain which are thought to be symptoms of mercury poisoning, but there seems to be no serious sequelae now.

  • 구강점막 각 부위의 지각신경의 특징은 근년 생리학적 연구의 중요한 대상이 되어왔다. 일반적으로 생리학적 기능은 연령과 더불어 감소된다는 소견이 구강점막에서도 적용된다고 하나 이에대한 형태학적 지견을 중심으로 볼 때 그러한 감소가 순전한 기능저하의 결과인지 혹은 지각신경의 해부학적 형태변화에 의한 것인지에 대해서는 분명하게 알려진 것이 없다. 본 논문은 Spraque-Dawely와 Fischer 344 두종의 숫자 20마리를 이용하여 각종을 6개월과 30개월의 연령으로 2군으로 나누어서 연령에 따른 구강점막내 신경섬유와 신경종말의 전자현미경적 특성의 변환을 관찰하고, 단위면적당 분포하고 있는 신경요소의 변화를 정량해 보았던바 다음과 같은 결과를 얻었다. 1. 구강점막에 분포하는 신경섬유와 신경종말의 전자현미경적 형태에 있어서는 젊은백서와 늙은백서 사이에 인정할 만한 질적 차이가 없었다. 2. 연령증가에 따라 구강점막의 상피내 간격에 존재하는 신경요소는 전반적으로 감소하였다. 3. 이와 같은 감소는 신경섬유보다 신경종말의 경우 더욱 현저하게 나타났다.

다양한 Laser를 이용한 Peri-Implantitis의 치료 (The treatment of peri-implantitis using various types of lasers)

  • Peri-implantitis is the inflammatory process, such as edema, bleeding, pus, of the mucosa surrounding dental implants. As the symptoms become severe, the surrounding bone is absorbed causing the implant surface to be exposed. Clinicians treat periimplantitis in various ways since a gold standard for the treatment of peri-implantitis has not been established. Various treatment methods include mechanical, chemical surface treatment and surgical excision, and recently decontamination of the implant surface using various types of lasers has been proposed. Thus, this study reviews the types of lasers and its effects that can be used for the treatment of peri-implantitis.

Simultaneous diagnosis and resection of orofacial rhabdomyosarcoma with frozen section biopsy: a case report

  • Rhabdomyosarcoma is the most common soft tissue sarcoma in children, accounting for 4.5% of all cases of cancer in childhood. Although the head and neck are the most common sites of rhabdomyosarcoma, oral lesions are relatively rare and account for only 10% to 12% of head and neck rhabdomyosarcoma cases. This is a case report of a girl aged 2 years and 1 month who initially presented with an upper lip mass that invaded the oral mucosa, oral skin, and nostril skin, causing narrowing of the airway. Through our case, we show that rapidly growing small round cell malignancies, especially rhabdomyosarcoma, can be effectively diagnosed and treated at the same time using primary resection with intraoperative frozen section biopsy and that the time spent waiting for the results of preoperative biopsy can be saved in this way, particularly when the patient's symptoms are intensifying rapidly and require immediate operation.

  • Sturge-Weber Syndrome은 드문 선천성 질환으로 안면의 삼차신경 분포영역에 포도주양 반점(port wine nevus)을 나타내고, 녹내장 등의 안구 증상과 간질, 편측마비 등의 신경학적 증상 등을 동반한다. 또한 구강 내 증상으로 구강 점막의 편측성 혈관 증식, 치은의 혈관 증식, 치은 비대, 거대치, 편측성 거대설, 상악 또는 하악의 혈관 이상, 치아 맹출 이상 등을 나타낸다. 본 증례는 Sturge-Weber Syndrome으로 진단된 8세 남아의 구강 내 증상과 설강직증의 치료를 위해 설소대 절제술을 시행하여 양호한 결과를 보여 이를 보고하고자 한다.

저출력 레이저 조사 백서구강점막 창상부 Langerhans 세포에 미치는 영향에 관한 실험적 연구 (Experimental study on the effect of low energy laser irradiation in Langerhans cell of Lanaged rat oral mucosa)

  • The purpose or this study was to observe the histological alteration of Langerhans cells on wound healing process in applying low energy laser irradiation. For this study, 50 Spraque-Dewly rats, weighing 150Gm or more were devided into control, experimental control group(0), 47.5Hz(1), 190Hz(3), 380Hz(5), 760Hz(7), lased group. All the experimental animals were made excision wound on buccal mucosa, 2mm depth, and lased with stoma laser (904nm, semconductor type ASGaAI, Sedalac France) 47.5Hz, 380Hz, 960Hz, 3minutes one time respectively except experimental control group. After the experiment, experimental animals were sacrificed after 24hours, 48hours, 72hours on each. Taken specimens were embedden in paraffin, sectioned 6-8u in thickness. And the langerhans cell were detected using ant S-100 protein antibody, and histochemically processed with Avidin Biotin complex method. All the Langerhan cells were calculated under light microspe in 400 multiplication field and standard deviation, probability test between each group were evaluated using statistical analysis system(S.A.S)program. Following results were obtained. 1. Langerhan cells were increased in experimental control group compared to that in control group(P<0.01). 2. 24hour after experiments, Langerhans cell were decreased compare to that in control group and control experimental group 5, 1, 3. Probability test shows significance between control experimental and 5, 1, 3 group on a =0.05 range. 3. 48our after experiment, Langerhans cells were decreased compare to that on experimental control group, and probability test shows significance between control experimental and 3, 7, 5 group an a=0.05 range. 4. 72hour after experiments, Langerhans cells were decreased compare to that on experimental control group and probability test on group comparison shows significance between control experimental and 1, 5 and 1 between 3, 7 between 3, and 5, between 7, respeilively on a=0.05 range. 5. Langerhans cells number in experimental group were decreased compare to that on experimental control group in applying laser irradiation.

Dec2 inhibits macrophage pyroptosis to promote periodontal homeostasis

  • Purpose: Macrophages play crucial roles as early responders to bacterial pathogens and promote/ or impede chronic inflammation in various tissues. Periodontal macrophage-induced pyroptosis results in physiological and pathological inflammatory responses. The transcription factor Dec2 is involved in regulating immune function and inflammatory processes. To characterize the potential unknown role of Dec2 in the innate immune system, we sought to elucidate the mechanism that may alleviate macrophage pyroptosis in periodontal inflammation. Methods: Porphyromonas gingivalis lipopolysaccharide (LPS) was used to induce pyroptosis in RAW 264.7 macrophages. Subsequently, we established an LPS-stimulated Dec2 overexpression cellular model in macrophages. Human chronic periodontitis tissues were employed to evaluate potential changes in inflammatory marker expression and pyroptosis. Finally, the effects of Dec2 deficiency on inflammation and pyroptosis were characterized in a P. gingivalis-treated experimental periodontitis Dec2-knockout mouse model. Results: Macrophages treated with LPS revealed significantly increased messenger RNA expression levels of Dec2 and interleukin (IL)-1β. Dec2 overexpression reduced IL-1β expression in macrophages treated with LPS. Overexpression of Dec2 also repressed the cleavage of gasdermin D (GSDMD), and the expression of caspase-11 was concurrently reduced in macrophages treated with LPS. Human chronic periodontitis tissues showed significantly higher gingival inflammation and pyroptosis-related protein expression than non-periodontitis tissues. In vivo, P. gingivalis-challenged mice exhibited a significant augmentation of F4/80, tumor necrosis factor-α, and IL-1β. Dec2 deficiency markedly induced GSDMD expression in the periodontal ligament of P. gingivalis-challenged mice. Conclusions: Our findings indicate that Dec2 deficiency exacerbated P. gingivalis LPS-induced periodontal inflammation and GSDMD-mediated pyroptosis. Collectively, our results present novel insights into the molecular functions of macrophage pyroptosis and document an unforeseen role of Dec2 in pyroptosis.

A simple approach to preserve keratinized mucosa around implants using a pre-fabricated implant-retained stent: a report of two cases

  • Purpose: There is no consensus regarding the relationship between the width of keratinized mucosa and the health of periimplant tissues, but clinicians prefer to provide enough keratinized mucosa around dental implants for long-term implant maintenance. An apically positioned flap during second stage implant surgery is the chosen method of widening the keratinized zone in simple procedures. However, the routine suture techniques used with this method tend to apply tension over the provisional abutments and decrease pre-existing keratinized mucosa. To overcome this shortcoming, a pre-fabricated implant-retained stent was designed to apply vertical pressure on the labial flap and stabilize it in a bucco-apical direction to create a wide keratinized mucous zone. Methods: During second stage implant surgery, an apically displaced, partial thickness flap with a lingualized incision was retracted. A pre-fabricated stent was clipped over the abutments after connecting to the provisional abutment. Vertical pressure was applied to displace the labial flap. No suture was required and the stent was removed after 10 days. Results: A clinically relevant amount of keratinized mucosa was achieved around the dental implants. Buccally displaced keratinized mucosa was firmly attached to the underlying periosteum. A slight shrinkage of the keratinized zone was noted after the healing period in one patient, but no discomfort during oral hygiene was reported. Clinically healthy gingiva with enough keratinized mucosa was achieved in both patients. Conclusions: The proposed technique is a simple and time-effective technique for preserving and providing keratinized tissue around dental implants.

구강저에 발생한 지방종의 치험례 (A LIPOMA OF THE MOUTH FLOOR : A CASE REPORT)

  • Lipoma is most common tumor that compromises 4% to 5% of all benign neoplasm, but in oral cavity it is uncommon. In oral cavity, lipoma presents painless, asymptomatic, slow growing, but sometimes it grows to larger size causing deformities, mastication and speech difficulties. While lipoma in commonly affects female patients (68-73%), oral lipoma appears more frequently in male patients. The majority of oral lipoma is seen after the age of forty (uncommon in children). Lipoma of oral cavity and maxillofacial region occurs most commonly in the parotid region, followed by the buccal mucosa, lip, tongue, palate, mouth floor, gingiva in order. A treatment of lesion is surgical excision with recurrence not expected. In this paper we present the case of a patient who has Lipoma in the mouth floor.
