Factors influencing oral health behaviors in high school students (고등학생들의 구강건강행위에 영향을 미치는 요인)

  • Objectives : This study was to infectify variable the factors influencing oral health behaviors in high school students. Methods : Data were collected from 241 high school students Busan and Kyungnam Province for four weeks in July 2012. The SPSS 18.0 program was used for data analysis, which included t-test, correlation analysis, multiple logistic regression analysis. Results : Positive correlation in the order of social support(r=.381), self-efficiency(r=.309) with perceived benefit of oral health behavior and social support with total heal index(r=.298) are shown. In the contrary, the relation between social support(r=-.400), subjective oral health status(r=-.222) with impairments of oral health behavior have negative one. Experience in three or more tooth brushing has an effect on social support and subjective oral health status(p<.05). Perceived benefit(p<.001) of oral health life and subjective oral health status(p<.05) are affected by recognition of correct tooth brushing. Intake of harmful food on tooth has an effects on perceived benefit, self-efficiency, total health index and subjective oral health status(p<.05). Conclusions : In conclusion, systematic activation of oral health education in school and correct self-activity of oral health will promote oral health of youth.

Disparities in oral health according to the socioeconomic status of adults: analysis of data from the 7th Korea National Health and Nutrition Examination Survey (성인의 사회경제적 위치와 구강건강 격차: 제7기 국민건강영양조사 자료 이용)

  • Objectives: This study aimed to analyze the relationship between the socioeconomic status and oral health of adults. Methods: Data from the 7th Korea National Health and Nutrition Examination Survey (2016-2018) were analyzed, and 13,199 adults aged 19 years or older were selected as study subjects. Various oral health indicators were used to analyze the effect of socioeconomic status on oral health. Disparities in oral health according to socioeconomic status were analyzed using the complex sample chi-squared test and multiple logistic regression analysis. Results: A statistically significant difference was observed between income level, medical aid, and all oral health indicators, which indicated that the lower the income level, the lower the oral health level (p<0.001). Furthermore, all oral health indicators displayed statistically significant differences, with the exception of the prevalence of dental caries and education level. The lower the education level, the lower the oral health level (p<0.001). Therefore, the oral health level of adults presented significant differences according to different socioeconomic status indicators. Conclusions: To prevent oral health inequalities, the government and local governments need to intervene not only in the field of health care but also in the social determinants. Additionally, concerted efforts should be made to eliminate oral health disparities by improving policies and systems.

The influence of p53 mutation status on the anti-cancer effect of cisplatin in oral squamous cell carcinoma cell lines

  • Objectives: The purpose of this study was to evaluate the anti-cancer activity of cisplatin by studying its effects on cell viability and identifying the mechanisms underlying the induction of cell cycle arrest and apoptosis on oral squamous cell carcinoma (OSCC) cell lines with varying p53 mutation status. Materials and Methods: Three OSCC cell lines, YD-8 (p53 point mutation), YD-9 (p53 wild type), and YD-38 (p53 deletion) were used. To determine the cytotoxic effect of cisplatin, MTS assay was performed. The cell cycle alteration and apoptosis were analyzed using flow cytometry. Western blot analysis was used to detect the expression of cell cycle alteration- or apoptosis-related proteins as well as p53. Results: Cisplatin showed a time- and dose-dependent anti-proliferative effect in all cell lines. Cisplatin induced G2/M cell accumulation in the three cell lines after treatment with 0.5 and $1.0{\mu}g/mL$ of cisplatin for 48 hours. The proportion of annexin V-FITC-stained cells increased following treatment with cisplatin. The apoptotic proportion was lower in the YD-38 cell line than in the YD-9 or YD-8 cell lines. Also, immunoblotting analysis indicated that p53 and p21 were detected only in YD-8 and YD-9 cell lines after cisplatin treatment. Conclusion: In this study, cisplatin showed anti-cancer effects via G2/M phase arrest and apoptosis, with some difference among OSCC cell lines. The mutation status of p53 might have influenced the difference observed among cell lines. Further studies on p53 mutation status are needed to understand the biological behavior and characteristics of OSCCs and to establish appropriate treatment.

The effect of drug holiday before tooth extraction on the development of medication-related osteonecrosis of the jaw in cancer patients receiving intravenous bisphosphonates

  • Objectives: Drug holidays are suggested to reduce the formation of osteonecrosis in patients under intravenous (IV) bisphosphonates (BPs) therapy. The objectives of this study are to evaluate the incidence of medication-related osteonecrosis of the jaw (MRONJ) following tooth extraction in cancer patients using IV BP, and to assess the effect of drug holiday on the development of MRONJ. Patients and Methods: A manuel search of the patient folders of Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Hacettepe University was undertaken to identify cancer patients who used IV BPs and had at least one tooth extraction between 2012 and 2022. Patents' age, sex, systemic condition, the type of BP used, duration of BP used, number of tooth extraction, duration of drug holiday, localization of tooth extraction and incidence of MRONJ were recorded. Results: One hundred nine teeth were removed from 57 jaws in 51 patients. All tooth extractions were performed under perioperative antibiotic prophylaxis and with primary wound closure. The incidence of MRONJ was 5.3%. Stage 1 MRONJ developed in 3 patients (only one had a drug holiday). The median duration of drug holiday was 2 months. No significant difference between the patients with and without a drug holiday and MRONJ development was found (P=0.315). The mean age of patients developed MRONJ was 40.33±8.08 years. A statistically significant difference was found between age and MRONJ development (P=0.002). Conclusion: The effect of a short-term drug holiday on the development of MRONJ may be limited because BPs remain in bone tissue for a long time. Drug holidays should be applied with the approval of an oncologist with other preventive measurements.

Mimicking Odontogenic Pain Caused by Burkitt's Lymphoma: A Case Report

  • Burkitt's lymphoma is a malignant monoclonal proliferation of early B-lymphocyte. Since Burkitt's lymphoma is a highly aggressive disease, early detection is a crucial. This disease often involves jaw and mandibular mass or swelling may also be seen, but in the early phase of Burkitt's lymphoma these symptoms cannot be observed. A rare case of Burkitt's lymphoma without any mandibular mass and the general symptoms was present. The excruciating toothache led the patient to visit the dental clinic and misdiagnosis of chronic periodontal abscess was made initially. Dentists should consider the oral manifestations of systemic disease when the multiple periodontal ligament space widening is observed and the dental treatment for mimicking odontogenic pain has no effect.

  • The purpose of this study was to investigate the characteristics of mesiodens among children in Korea. The study population consisted of 20 children whose ages ranged from 4 years to 26 years. The characteristics of mesiodens were obtained from radiographs and clinical examination results, and were confirmed by surgical intervention. Results showed that males were affected four times as frequently as female. Thirty-five percent of the patients had more than one supernumerary tooth. About 67% of the mesiodens were conical in shape and about 34% of the mesiodens were in the upward position. The age and sex distribution, number of mesiodens per patient, shape, direction, size, and effect on permanent upper incisors are also presented in this study.

  • Recent case studies raised the possibility that cutaneous and oral mucosal manifestations may be associated with the coronavirus disease 2019 (COVID-19) vaccination. A healthy 43-year-old male presented an acute aphthous stomatitis following Moderna COVID-19 vaccination. This rare case draws attention to a potential etiologic effect for oral mucosal manifestation from COVID-19 vaccination. Further investigation to shed light on prevalence and pathophysiologic association of this oral lesion and COVID-19 vaccination deserve attention.

  • Purpose: The purpose of this study was to examine the effect of tooth-brushing oral care on oral health and ventilator-associated pneumonia of patients in Intensive Care Unit (ICU). Methods: A total of 74 participants were recruited from a medical, surgical, or neurosurgical ICU at S Hospital in Seoul, Korea, from September of 2010 to January of 2011. An experimental group (n= 36) received oral care with tooth-brushing while a control group (n=38) received oral care with cotton ball and gauze. In both group, the oral care was done three times a day at least one minute for 7 days. Oral health was examined by the Modified Oral Assessment Guide before the intervention and each night. Results: There is no difference between the groups in aspect of passage of time (p = .603), interaction between time and intervention (p =.300), the performance intervention (p = .766), and the incidence of VAP (p = .486). Conclusion: The effect of oral care with tooth-brushing on oral health and VAP was not different from that of usual oral care in ICU. However, further study is necessary due to high attrition in this study.

Effect of Maintained Microorganisms against to The Phytoncide on Pr. intermedia (피톤치드 처리 후의 잔존 구강 세균이 Pr. intermedia에 미치는 영향)

  • The present study was performed to observe the effect of phytoncide on oral normal microflora and the inhibitory effect of the surviving resident oral bacteria on Pr. intermedia. In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). Surviving salivary bacteria were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against Pr. intermedia, the isolates from the phytoncide-treated saliva were cultured with Pr. intermedia. The results were as follows: 1. Among the 200 surviving resident oral bacterium, 148(74.0%) bacterium inhibit the growth of Pr. intermedia on blood agar plates. 2. The 200 surviving resident oral bacterium were 109 Streptococcus salivarius(54.5%), 25 Streptococcus sanguinis(12.5%), 15 Streptococcus mitis(7.5%). 3. Among the 148 bacteria which inhibit Pr. intermedia, Streptococcus salivarius was 85.3%(93/109), Streptococcus sanguinis was 64.0%.(16/25), Streptococcus mitis was 54.3%(8/15), Streptococcus parasanguinis was 66.7%(6/9), and Streptococcus Alactolyticus was 100%(8/8). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit Pr. intermedia. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used for preventing and ceasing the progress of periodontal disease and halitosis, and thus is expect to promote oral health.

  • Isoflurane and enflurane are currently used on orthognathic surgery in Korea. Since starting to use enflurane and isoflurane in orthognathic surgery, we have questioned their effect on liver function. Many studies have reported liver function after enflurane and isoflurane anesthesia. Although both enflurane and isoflurane are less hepatotoxic than halothane, some cases of liver dysfunction have been reported after enflurane and isoflurane anesthesia. And, we know that isoflurane is less hepatotoxic than its predecessors, enflurane. But, fulminant liver failure and necrosis were also reported after isoflurane anesthesia. The purpose of this study was to compare immediate liver function in healthy orthognathic surgical patients receiving enflurane or isoflurane anesthesia. To assess the effect of enflurane and isoflurane on liver function, we measured pre-and post-operative serum concentrations of aspartate aminotransferase(AST), and alanine aminotransferase(ALT), alkaline phosphatase(ALP), total bilirubin(Tbil).
