한국 성인의 치주질환 유병과 체질량지수 및 임상검사요인의 관련성 연구 국민건강영양조사 제6기 조사자료 (A study on the relationships between periodontal diseases, body mass index and clinical factors for Korean adults)

  • Purpose: This article tried to find the relations between periodontal diseases, body mass index and clinical level for Korean adults over the age of 19, utilizing data of the 6th national nutrition survey. Methods: In the collected data, 14,940 adults aged 19 years or older were included in the study to determine the relationship between the general health status of Korean adults and periodontal disease. Results: As the result, 28.6% of Korean adults had periodontal diseases, there were significant differences depending on gender, age, smoking, residential aria, education level, body mass index, HDL-cholesterol, leukocyte value and fasting blood sugar. When general factors were controlled for multiple logistic regression analysis, there was significance as BMI cross rate was 1.029(95% C1, 1.007-1.051). HDL-cholesterol cross rate was 0.989(95% C1, 0.980-0.999), leukocyte cross rate was 1.086(95% C1, 1.040-1.134) and fasting bloody sugar cross rate was 1.006(95% C1, 1.003-1.009), so there were significance. In conclusion, there was a significance relation between periodontal diseases, body mass index, HDL-cholesterol and fasting bloody sugar. Conclusion: Based on the results of this study, oral health education should be used to manage and maintain healthy oral cavity by improving periodontal disease management and awareness. In addition, it will be necessary to develop periodic oral examinations and age - specific oral health education programs, and it can be used as a basic data for public oral health plan.

어머니의 구강보건지식 및 행동과 초등학생의 구강보건행태 (Association between Oral Health Knowledge and Behavior in Mothers and Dental Health Belief of their Children)

  • This study was undertaken to verify various relations between dental health belief and related behaviors in mothers and children. Data for this study were obtained by 264 pairs of 1st and 3rd grade elementary school children and their mothers by self developed questionnaire, The questionnaires are made on the general socio-economical characteristics, the oral health knowledge and the behaviors relative to the oral health and oral health education as to how mothers implement the oral health related activities for their children, while for the children, the questionnaires have been measured relative to the oral health beliefs and health behaviors for the children. The obtained results have shown that mothers become with more ages, the usefulness to prevent children's oral diseases becomes lower, and as much as mothers have higher education level, the sensitivity relative to the oral diseases has been measured low. To the degree that the students live well in economical respect, it has been found that they placed more importance on the oral health. As much as the number of mothers tooth brushing becomes made more in a day, the tooth brushing of the children becomes more that much, and also has higher level of oral health beliefs. The number of children to see dentists has been found high, and to the extent of increasing frequency of visiting dental clinics. As the number of mothers tooth brushing education becomes made more frequent for the children. Also, as mothers put more restriction on the sugar intakes by the children, they placed higher importance on the oral health for their children. In conclusion oral health behavior in mothers' influence on dental health belief of their children, it has been reviewed necessary to activate more the mother-children joint oral health education and oral health projects that mothers and children take part together, as well as to study more in accurate and systematic approaches through more comprehensive and various subjects and elements further in the future.

당뇨병 환자의 구강질환과 구강건강관리 실태 (Oral disease and oral health care in the diabetic patients)

  • Objectives: The aim of this study is to investigate the relationship between diabetes mellitus and oral disease in the Korea adults by using the representative data of Korea National Health and Nutrition Examination Survey (KNHANES) V. Methods: Using data from a cross-sectional survey of Korea National Health and Nutrition Examination Survey V. The subjects were 5,602 adults over 19 years old. The survey data and the examination data are used for the independent variables. KNHANES included health status, nutrition survey, and oral examination. The health status was obtained by a self-reported questionnaire of the study subjects and direct interview was carried out on economic status, prevalence rate, and use of medical services. Results: The regular dental checkup of diabetes patients was significantly lower than the general population. Regular dental visits allow adults to receive early diagnosis and obtain restorative care at the first visit. The regular annual oral examination is very important to prevent and manage the chronic diseases including diabetes mellitus. Conclusions: Diabetes mellitus can be fully prevented and managed by the regular annual dental checkup. The establishment of the regular dental checkup system can prevent and manage the complication due to diabetes mellitus and other chronic diseases.

  • 구강내 궤양성 병소는 다양한 원인에 의해 발생하나 그 임상적 소견이 서로 유사하여, 병소의 진단이 쉽지 않다. 구강내 궤양성 병소를 진단할 때 임상가는 환자의 병력을 세밀하게 청취해야 하며, 전신질환과의 관련성을 고려해야 한다. 본 증례에서는 다형홍반, 천포창, 태선양 반응 및 아프타성 구내염으로 임상진단 된 급성 및 만성 구강내 궤양성 병소를 진단하고, 약물치료를 통해 증상의 호전을 보인 예를 보고자 한다.

Risk of Hemorrhage Attributed to Underlying Chronic Diseases and Uninterrupted Aspirin Therapy of Patients Undergoing Minor Oral Surgical Procedures: A Retrospective Cohort Study

  • Objectives: This study aimed to estimate the risk of bleeding following minor oral surgical procedures and uninterrupted aspirin therapy in high-risk patients or patients with existing chronic diseases compared to patients who did not use aspirin during minor oral surgery at a public hospital. Methods: This retrospective cohort study analyzed the data of 2912 patients, aged 20 years or older, who underwent 5251 minor oral surgical procedures at a district hospital in Thailand. The aspirin group was comprised of patients continuing aspirin therapy during oral surgery. The non-aspirin group (reference) included all those who did not use aspirin during surgery. Immediate and late-onset bleeding was evaluated in each procedure. The risk ratio of bleeding was estimated using a multilevel Poisson regression. Results: The overall cumulative incidence of immediate bleeding was 1.3% of total procedures. No late-onset bleeding was found. A significantly greater incidence of bleeding was found in the aspirin group (5.8% of procedures, p<0.001). After adjusting for covariates, a multilevel Poisson regression model estimated that the bleeding risk in the aspirin group was 4.5 times higher than that of the non-aspirin group (95% confidence interval, 2.0 to 10.0; p<0.001). However, all bleeding events were controlled by simple hemostatic measures. Conclusions: High-risk patients or patients with existing chronic diseases who continued aspirin therapy following minor oral surgery were at a higher risk of hemorrhage than general patients who had not used aspirin. Nonetheless, bleeding complications were not life-threatening and could be promptly managed by simple hemostatic measures. The procedures could therefore be provided with an awareness of increased bleeding risk, prepared hemostatic measures, and postoperative monitoring, without the need for discontinuing aspirin, which could lead to more serious complications.

  • 노인성 안질환과 구강질환은 비가역적 손상을 일으키기 때문에 삶의 질에 큰 영향을 미친다. 따라서 나이관련황반변성(AMD) 및 녹내장과 같은 노인성 안질환과 구강건강행태와의 관련성을 확인하고자 하였다. 제7기 2차년도(2017) 국민건강영양조사 자료를 이용하여 AMD 및 녹내장과 구강건강행태를 분석하였다. 녹내장과 AMD 유병자와 녹내장과 AMD 미보유자를 비교하기 위해 교차분석을 실시하였고, 복합로지스틱 회귀분석을 통해 AMD 및 녹내장과 구강건강행태와의 위험비를 확인하였다. 황반변성 및 녹내장의 유무에 따라 연령, 학력, 구강위생용품 사용, 발음불편 호소에서 통계적으로 유의한 차이가 있었다. 또한 녹내장 유무는 성별에서도 차이가 있었다. 구강건강행태와 노인성 안질환과의 일부 변수에서 유의한 연관성이 있었다. 구강건강행태는 AMD 및 녹내장 질환을 예방하는 데 위험 요인으로 작용할 수 있으므로 구강건강행태에 대한 인식도가 더욱 높아져야 할 것이다.

  • 구강은 전신건강을 들여다 볼 수 있는 창문이자 많은 유해균이 들어오는 관문이기도 하다. 우리 몸에 있어서 아주 중요한 부분이다. 유전자 검사는 구강 내 세균을 검사하여 빅 데이터 AI 알고리즘 분석을 통해 우리 몸에 발생할 수 있는 전신질환까지 예측하여 예방 관리를 체계적으로 할 수 있다는 가장 큰 장점이 있다. 이에 본 논문은 연구자 가족이 직접 유전자 검사를 수행하여 결과를 도출해 보고자 한다. 본 연구는 2022년 11월 연구자 가족 4명이 서울시에 소재한 예방치과진료실 J치과의원에서 치과의사 1명, 치과위생사 1명에게 사전설명을 듣고 동의서 작성 후 구강검사와 유전자 검사를 수행하였다. 성인은 Dr.***으로 중·초등학생은 He***** 제품으로 유전자 검사를 하였다. 현재 우리나라에서 진행하고 있는 유전자 검사는 대상자가 피를 뽑지 않고 비교적 쉽게 접근할 수 있는 장점도 있지만, 시간, 비용 등 한계점도 있다. 그럼에도 불구하고 구강 내 미생물을 통해 전신질환을 예측할 수 있다는 점에서는 높이 평가할 부분이라고 생각한다.

  • The age structure has been experiencing substantial change due to the decreased birth rate as well as the increased life expectancy. Gorge Magnus, an English economist, casts warnings of population ageing which has the potential of huge socioeconomic impact human society has never experienced before. The prediction that proportion of elderly people in need of oral health care will increase substantially is a new challenge to dentists in the future. The old paradigm that the aged person is just the person who was born earlier and needs the same conventional oral health care should be shifted to the new one. Elderly people tend to express their political interest related with health care system by actively participating in the national elections. The need to sustain economic status for the extended life span makes them seek eagerly esthetic health care to maintain sound social function. Most of them are under multiple chronic diseases and take related medicines. In addition, many studies report about mental change as well as physical change among the aged people. Since the prevalence of dental diseases among the aged is higher than other chronic devastating diseases, the aged seeking oral health care will increase. The aged who has different physical and psychological status as well as chronic disease and related medicine will show unexpected response to the conventional oral health care. In addition, the impact of tooth loss is substantial physically, mentally and emotionally. Dentist should prepare different approaches for the elderly dental patient.

구강점막질환 환자의 행동패턴 연구 (Behavior patterns of patients with oral mucosal disease)

  • There are many causes of oral mucosal diseases, so accordingly, there are various treatments available. The most commonly used agents include adrenocortical hormones, antifungals, antivirals, antibacterials, and immunosuppressants. However, it must also be noted that improving oral hygiene and nutrition, and reducing stress are effective in symptom relief. Furthermore, patients with existing diseases of the oral mucosa should avoid behavior that may cause an increase in pain. Unfortunately, many patients are unaware of the activities that may lead to increased pain and therefore do not avoid these activities. The aim of this study was to investigate and analyze the behavior of patients with oral mucosal disease with regard to activities that led to increase pain. This cross-sectional study was performed on a sample of patients with oral mucosal disease selected from the Oral Medicine Clinic of the Pusan National Hospital during March to August 2013. These patients were randomly selected. From a total of 479 patients, 116 patients with mucosal disease were selected and 73 fully completed questionnaires were included in the analysis. Data were collected by using self-completed questionnaires. The results were as follows: Mean score of Question 13 (Not smoking) is $2.47{\pm}1.11$. Mean score of Question 11 (Not drinking alcohol or not using mouthwash containing alcohol) is $2.22{\pm}1.15$. The other questions resulted in scores lower than 1.5. The answers to the questions were scored according to the following assigned numerical values: not keeping = score of 0; little keeping = score of 1; often keeping = score of 2; always keeping = score of 3. In conclusion, patients with oral mucosal diseases unknowingly engage in activities that result in an increase in pain. Therefore, they need to be educated about how to behave to protect oral mucosal lesion.

  • Tuberculosis has re-emerged as serious infectious diseases, and oral tuberculosis, though uncommon, can be seen in both the primary and secondary tuberculosis. The aim of the present review is to describe the pathogenesis of tuberculosis and the characteristics of oral tuberculosis. The oral tuberculosis lesion may present as a diagnostic challenge for the clinician. In particular, the risk of tuberculosis transmission in dental practises is potentially increasing, the dentists and dental providers must exercise strict tuberculosis prevention.
