닭의 마렉병 바이러스 강독주 실험접종에 의한 안구병변 (Ocular Lesions Induced Experimentally by Very Virulent Strain of Marek′s Disease Virus in Chickens)

  • Ocular lesions induced in 40 specific-pathogen-free Marek's disease (MD) resistant chicks by inoculation at 1 day of age with very virulent strain of Marek's disease virus (WV) were pathologically examined. Grossly,24/40 (60%) chicks had white gel-like materials in the vitreous body, whereas thickening and discoloration of iris (gray eye) were not observed. Microscopically, characteristic ocular MD lesions were observed in choroid (27/40), ciliary (30/40) and iris (23/40) in which small focal inflammatory to diffuse neoplastic Iymphoid cells were infiltrated. Five out of 40 MDV-inoculated birds revealed necrotizing Iymphomas in choroid. These lesions consisted of necrotic and degenerating Iymphoblasts accompanied by intranuclear inclusion body. There was retinal atrophy and necrosis with inclusion body detected in necrotic ganglion, inner or outer nuclear and infiltrated Iymphoblast cells. Conjunctiva showed lymphoid cell infiltration in 29/40 chicks inoculated with MDV, Vitreous body exhibited mild to severe exudation of eosinophilic proteinaceous material in 24/40 chicks. These lesions were associated with Iymphoid cell infutration, edema and fibrosis of choroid. Pecten (7/40) and optic nerve (13/40) were infiltrated usually mildly with Iymphoid cells. From these results, very virulent strain, Md/5 of MDV caused high incidence of ocular lesions in MD resistant chicks. In addition, Md/5 induced exudation of proteinaceous material into the vitreous body and fibrosis of choroid. Necrotizing ocular Iymphoma lesions in choroid is the first report in the MD literature.

Protective Effects of Verapamil against H2O2-Induced Apoptosis in Human Lens Epithelial Cells

  • Verapamil is used in the treatment of hypertension, angina pectoris, and atrial fibrillation. Recently, several studies have demonstrated that verapamil increased the optic nerve head blood flow and improved the retrobulbar circulation. All these show that verapamil is potentially useful for ophthalmic treatment. Thus, the aim of this study is to investigate whether verapamil could protect human lens epithelial cell (HLEC) from oxidative stress induced by $H_2O_2$ and the cellular mechanism underlying this protective function. The viability of HLEC was determined by the MTT assay and apoptotic cell death was analyzed by Hoechst 33258 staining. Moreover, Caspase-3 expression was detected by immunocytochemistry and flow cytometry analysis. We also detected Caspase-3 mRNA expression by reverse-transcription-polymerase chain reaction and the GSH content in cell culture. The results showed that oxidative stress produced significant cell apoptotic death and it was reduced by previous treatment with the verapamil. Verapamil was effective in reducing HLEC death mainly through reducing the expression level of apoptosis-related proteins, caspase-3, and increasing glutathione content. Therefore, it was suggested that verapamil was effective in reducing HLEC apoptosis induced by $H_2O_2$.

HiGANCNN: A Hybrid Generative Adversarial Network and Convolutional Neural Network for Glaucoma Detection

  • Glaucoma is a chronic neuropathy that affects the optic nerve which can lead to blindness. The detection and prediction of glaucoma become possible using deep neural networks. However, the detection performance relies on the availability of a large number of data. Therefore, we propose different frameworks, including a hybrid of a generative adversarial network and a convolutional neural network to automate and increase the performance of glaucoma detection. The proposed frameworks are evaluated using five public glaucoma datasets. The framework which uses a Deconvolutional Generative Adversarial Network (DCGAN) and a DenseNet pre-trained model achieves 99.6%, 99.08%, 99.4%, 98.69%, and 92.95% of classification accuracy on RIMONE, Drishti-GS, ACRIMA, ORIGA-light, and HRF datasets respectively. Based on the experimental results and evaluation, the proposed framework closely competes with the state-of-the-art methods using the five public glaucoma datasets without requiring any manually preprocessing step.

부비동염에 관한 전산화단층방사선학적 연구 (Computerized Tomographic Study on the Paranasal Sinusitis)

  • Objectives : The purpose of this study is to evaluate the computed tomographic (CT) images of the paranasal sinusitis(PNS). Materials and Methods : The author examined the extent and recurring patterns of the paranasal sinusitis and some important anatomic landmarks. The author analyzed PNS images retrospectively in 500 patients who visited Chonbuk National University Hospital between January 1996 and December 1997. Results : The most frequently affected sinus was maxillary sinus (82.9%), followed by anterior ethmoid sinus(67.9%), posterior ethmoid sinus(48.9%), frontal sinus(42.0%) and sphenoid sinus(41.4%). The characteristic features of CT images of the sinusitis were sinus opacification(22.4%), mucoperiosteal thickening(34.3%), and polyposis(2.0%). Sinonasal inflammatory diseases were categorized into 5 patterns according to Babber s classification. They were 1) infundibular(13.0%), 2) ostiomeatal unit(67.4%), 3) sphenoethmoidal recess (13.0%), 4) sinonasal polyposis (9.6%) and 5) unclassifiable patterns(18.0%). The incidences of contact between sinus and optic nerve were as follows ; the incidences of contact with posterior ethmoid sinus, sphenoid sinus. both posterior sinuses were 11.4%. 66.8%, 6.3%. respectively. The incidences of contact between sphenoid sinus and maxillary nerve, vidian nerve, internal carotid artery were 74.5%. 79.2%. 45.1%. respectively. The incidences of pneumatization of the posterior ethmoid sinus were as follows ; normal 70.6% and overriding type 29.4%. The incidences of sphenoid sinus pneumatization were as follows; normal 56.9% , rudimentary 12.5%, pterygoid recess 22.7%, anterior clinoid recess 2.7%, and both pterygoid and anterior clinoid recess type 5.2%. Conclusions : The inflammatory sinonasal diseases were classified into five patterns using the CT of PNS, which was proven to be an excellent imaging modality providing detailed information about mucosal abnormality, pathologic patterns, and the proximity of the important structures to the posterior paranasal sinuses. This result will aid in the interpretation of CT of PNS functionally and systemically.

안와 림프종 환자의 방사선치료를 위한 세기조절방사선치료와 용적세기조절회전치료의 전산화 치료계획에 대한 고찰 (A comprehensive comparison of IMRT and VMAT plan quality for orbital lymphoma)

  • 목 적 : 안와 림프종(orbital lymphoma) 방사선치료를 IMRT(intensity-modulated radiotherapy)와 VMAT (volumetric modulated arc therapy)을 이용한 RapidArc로 비교 분석해 보고자 한다. 대상 및 방법 : 본원에서 치료받은 안와 림프종 환자 4명을 대상으로 IMRT, VMAT을 이용한 partial-single arc(SA)와 partial-double arc(DA)로 전산화치료계획을 수립하였다. 각 치료계획의 평가는 PTV의 CI(conform -ity Index)값과 PCI(Paddick's Conformity Index)값, HI(Homogeneity Index)값을 구하여 비교 평가하였으며 주변 정상조직의 선량평가를 시행하였다. 또한 각 치료계획의 MU(Monitor Unit)값을 비교하고 치료시간과 수정체의 선량을 측정해보았다. 결 과 : PTV의 CI값을 분석한 결과 각 0.88, 0.86, 0.92로 DA의 값이 가장 컸으며 PCI값은 1.41, 1.44, 1.33으로 DA에서 값이 가장 작게 나타났다. 또한 HI값은 1.18, 1.20, 1.15로 DA에서 가장 낮은 것을 확인할 수 있었다. 안와 림프종 주변 정상조직의 mean dose와 $V_30$, $V_20$, $V_10$, $V_5$의 값을 분석하였다. 그 결과 반대측 수정체, 눈물샘, 시신경, 안와의 선량이 SA에서 가장 높았고 IMRT에서 가장 낮게 나타났다. 평균 MU값과 치료시간은 IMRT가 SA에 비해 약 3배 정도 높게 나타났다. 결 론 : 안와 림프종 환자 IMRT 치료는 장애를 최소화하고 종양 내 선량 균등성을 높이기 위한 일반적인 치료법이다. 하지만 최근 시행하고 있는 RapidArc 치료계획에서 종양 내 선량 균등성이 IMRT 치료계획보다 우수한 결과 값을 보였고 정상조직의 선량 비교에서 큰 차이를 보이지 않았을 뿐만 아니라 MU와 치료시간 또한 약 1/3로 줄일 수 있었다. 이러한 이점은 안와 림프종 환자의 RapidArc 치료계획에 대한 지속적인 연구를 통해 치료의 효율성을 높일 수 있을 것으로 사료된다.

비인두암 Vertical MLC VMAT plan 유용성 평가 (Evaluating efficiency of Vertical MLC VMAT plan for naso-pharyngeal carcinoma)

  • 목 적 : 비인두암 VMAT 치료 시 콜리메이터 각도 273°와 350°를 사용한 2회전 치료계획(Vertical MLC VMAT plan, 이하 VMV plan)과 콜리메이터 각도 20°와 340°를 사용한 2회전 치료계획(Complemental MLC VMAT plan, 이하 CMV plan)과의 비교 분석을 통해 유용성을 평가해 보고자 한다. 대상 및 방법 : 본원에서 VMAT 치료를 받은 비인두암 환자 30명을 대상으로 하였다. 전산화치료계획은 Eclipse, PO, AcurosXB 알고리즘을 이용하였으며, 치료기는 Vital-beam을 사용하였다. 치료계획은 갠트리 회전반경이 각각 360°인 두 개의 회전(Arc)을 6MV를 이용하여 생성하고, 콜리메이터 각도 변화에 의한 영향을 확인하기 위해 273°와 350°로 설정한 VMV plan과 20°와 340°로 설정한 CMV plan을 수립하였다. 선량용적 히스토그램에서 확인 및 산출된 계획표적용적(PTV)과 결정장기(OAR)들의 선량지표들과 변조복잡성지수(MCSv), MU, 치료시간에 대해 기술통계 및 대응표본 t-검정을 실시하였다. 그리고 대상별로 두 치료계획의 MCSv의 차이와 각각의 평가지표들의 차이 간 상관관계가 있는지 확인하기 위해 피어슨의 상관관계 분석을 실시하였다. 결 과 : PTV 평가 지표의 경우, VMV Plan에서 PTV_67.5의 CI가 3.76% 개선되었으며, OAR의 경우 척수(-14.05%)와 뇌 줄기(-9.34%)의 선량감소 효과가 두드러지게 나타났다. 그리고 귀밑샘들(왼쪽 : -5.38%, 오른쪽 : -5.97%)과, 시각기관들(왼쪽 시신경 : -4.88%, 오른쪽 시신경 : -5.80%, 시신경교차 : -6.12%, 왼쪽 수정체 : -6.12%, 오른쪽 수정체 : -5.26%), 청각기관(왼쪽 : -11.74%, 오른쪽 : -12.31%)과 갑상샘(-2.02%)에서의 선량감소효과도 확인할 수가 있었다. 또한 MCSv가 VMV Plan에서 5.31% 높게 나왔으며, MU의 경우에 6.11% 낮은 것으로 나타났다. 그리고 두 치료계획의 MCSv의 차이는 PTV_54의 CI(r=-0.55)의 차이, PTV_48의 CI(r=-0.43)의 차이와 유의한 음(-)의 상관관계를 보였고, 척수(r=0.40), 뇌줄기(r=0.34), 양쪽 침샘들(왼쪽 : r=0.36, 오른쪽 : r=0.37)의 차이와 양(+)의 상관관계를 보였다. 이는 VMV plan의 moluation 복잡성이 CMV plan에 비해 상대적으로 높아 질 때, PTV_54, 48의 CI 개선 및 척수, 뇌줄기, 양쪽침샘들의 선량 감소 효과가 커지는 것을 의미하며, 이러한 분석결과들을 바탕으로 VMV plan의 modulation에 대한 효율성이 CMV plan 보다 크다는 것을 확인할 수 있었다. (위 모든 값에 대해 유의수준은 p<.05) 결 론 : VMV Plan은 CMV plan과 비교하여 MLC가 더 효율적으로 modulation할 수 있도록 함으로써, 치료계획의 질을 향상 시키는데 도움이 될 것이라 사료된다.

당뇨병유두병증에서 유리체강내 베바시주맙 주입술 후 망막시경섬유층 두께의 장기간 변화 2예 (Two Cases of Long-Term Changes in the Retinal Nerve Fiber Layer Thickness after Intravitreal Bevacizumab for Diabetic Papillopathy)

  • 목적: 당뇨병유두병증에서 유리체강내 베바시주맙 주입술 후 망막신경섬유층 두께 변화를 장기간 측정한 2예를 보고하고자 한다. 증례요약: 36세 당뇨환자가 우안 시력저하(20/200)로 내원하였고 안저검사상 양안 시신경유두부종이 관찰되었다. 스펙트럼영역 빛간섭단층촬영기로 측정한 우안의 망막신경섬유층 두께 $278{\mu}m$로 증가 소견, 시야검사상 우안 코쪽에 국소적인 협착을 보였다. 우안 베바시주맙 주입술을 하였고 3주 후 두께는 $135{\mu}m$로 감소했으며 시력도 20/25로 호전되었으나, 좌안 시력저하(20/32)와 망막신경섬유층 두께가 $207{\mu}m$로 증가하였다. 좌안 베바시주맙 주입술을 하였고, 3주 후 두께는 $147{\mu}m$로 감소했고 시력도 20/20으로 호전되었다. 이 후 점차적으로 양안 망막신경섬유층 두께는 감소하였고, 1년 후 검사에서 우안 $87{\mu}m$, 좌안 $109{\mu}m$였다. 57세 당뇨환자가 우안 시력저하(20/200)로 내원하였고 우안의 시신경유두부종과 망막신경섬유층의 두께가 $252{\mu}m$로 증가된 소견 및 시야검사상 맹점확대를 보였다. 우안 베바시주맙 주입술 3주 후 두께는 $136{\mu}m$로 감소했고 시력은 20/70으로 호전되었으며, 3개월 후 $104{\mu}m$, 6개월에는 $83{\mu}m$로 감소했다. 결론: 본 증례에서 유리체강내 베바시주맙 주입술 후 3주내 시력의 상승과 스펙트럼영역 빛간섭단층촬영기를 통한 1년간 평균 망막신경섬유층 두께의 감소를 확인할 수 있었다.

안와부 자가지방이식술 후 시력 저하에 대한 증례보고 (Visual Disturbance following Autologous Fat Injection into Periorbital Area)

  • Purpose: Autologous fat injection into the facial area is a frequently used technique in aesthetic plastic surgery for augmentation of the soft tissue. Fat injection is a very safe procedure because of the advantage of being autologous tissue. Minimal foreign body reaction or infections are noted after fat injection. However, there may be some complications including those as severe as blindness. There have been some case reports on visual disturbances after autologous fat injection reported in the literature. Methods: A 21-year-old female patient underwent autologous fat injection into left eyebrow area to correct depression of soft tissue. Immediately after injection of autologous fat, she complained sudden visual loss on the left eye. She had come to our emergency room and ophthalmologic evaluation showed that the patient could only recognize hand motion. There was no abnormality of the optic nerve on magnetic resonance imaging. Suspecting an ischemic optic neuritis from fat embolism of the central retinal artery, the patient was treated conservatively with occular massage, antiglaucomatic agent, anti-inflammatory drugs and antibiotics. Visual field examination showed visual defect of half the lower hemisphere. Results: While maintaining antiglaucomatic agents and non steroidal anti inflammatory drugs, fundoscopic examination showed no abnormalities on the second day of admission. Visual field examination showed an improvement on the fourth day along with decreased eyeball pain. Significant improvement of vision was noted and the patient was discharged on the fifth day of admission. The patient was followed-up 2 days afterwards with improved vision and visual field defect. Conclusion: We describe an unusual case of sudden unilateral visual disturbance following autologous fat injection into periorbital area.

황점볼락과 조피볼락의 뇌 조직에 분포하는 neuropeptide Y성 물질 (Neuropeptide Y like Substance Distributed in the Brain Tissues of Two Rockfish Species, Sebastes oblongus and S. schlegeli)

  • 출산 전후의 황점볼락 및 조피볼락 어미의 뇌 조직에서 GtH 분비를 자극하는 신경호르몬으로 알려진 NPY를 검출하기 위해, 면역조직화학을 실시하고 뇌하수체내 GtH 분비세포의 활성을 서로 비교하였다. 두 어종 모두에서 뇌 조직중 NPY 양성반응을 나타내는 세포는 후각망울, 종뇌 및 중뇌에서, 신경섬유는 후각망울, 종뇌, 시각신경, 시상하부, 중뇌 및 시각엽에서 각각 관찰되었다. 뇌하수체내에서 NPY 양성반응을 나타내는 신경섬유는 성숙에 관계없이 두 어종 모두 앞원위부분의 AF 음성세포에 인접한 신경엽에 분포하였고, 성장 및 성숙기의 난모세포를 가진 어체에서는 앞원위부분의 신경엽과 중간원위부분의 GtH 분비세포에 인접한 신경엽에서 관찰되었다. 뇌하수체내의 GtH 분비세포는 출산전의 황점볼락 및 성숙기의 조피볼락 개체에서는 AF 염색성이 약했으나, 출산후 황점볼락 및 조피볼락의 GtH 분비세포는 출산전 및 성숙기에 비해 AF 염색성이 증가하였다. 두 어종에서 출산전 난소를 가진 개체들의 GtH 분비세포와 핵경의 크기는 출산후(황점볼락)이거나 출산 이후 휴지기의 난소를 가진 개체(조피볼락) 보다 유의하게 컸다(P<0.01).

산민달팽이 (Incilaria fruhstorferi) 눈의 미세구조 (Ultrastructure of the Eye in the Snail, Incilaria fruhstorferi)

  • After the investigation on the eye of Incilaria fruhstorieri with light and electron microscopes, the following results were obtained. The eye of Incilaria fruhstorferi comprises cornea, lens, vitreous body, retina, and optic nerve inward from the outside. Cornea is composed of squamous, cuboid, columnar and irregular cells, which appear to be light due to their low electron density. In their cytoplasms, glycogen granules, multivesicular body, and nucleus were observed. Vitreous body, located behind non-cellular transparent lens, is filled with long and short microvilli protruding from the retinal epithelia. Retinal epithelium, the organ to perceive objects, is divided into four parts; microvillar layer pigment layer, nuclear layer, and neutrophils layer, from the apical portion. Microvillar layer consists of the type-I photoreceptor cells and pigmented granule cells. In the apical portion of their cytoplasms, long microvilli (length, $19{\mu}m$) , short microvilli (length, $8{\mu}m$), and rolled microvilli grow thick in the irregular and mixed forms. Photoreceptor cells are classified into type-I and type-II, according to their structures. The type-I cell has the apical portion rising roundly like a fan and the lower part which looks like the helve of a fan. In the cytoplasm of the apical portion, there are clear vesicles, cored vesicles, ovoid mitochondria, and microfilaments, and in the cytoplasm of the lower part, photic vesicles with their diameters about 60nm aggregate densely. The type-II photoreceptor cell, located at the lower end of the type-I cells, has a very large ovoid nucleus 3nd no microvilli. In the cytoplasm of the type-II cell, the photic vesicles with sizes 60nm aggregate more densely than in the cytoplasm of the type-I cell. Pigmented cells are classified into type-A and type-B, according to their structures. The type-A is identified to be a large cell containing round granules (diameter, $0.5{\mu}m$) of very high electron density, while the type-B is identified as a small cell where the irregular granules (diameter, $0.6{\mu}m$) of a little lower electron density amalgamate. Nuclear layer ranges from the bottom of pigment layer to the top of the capsule, and contains three kinds of nuclei (nuclei of the type-II photoreceptor cell, pigmented granule cell, and accessory neuron). The capsules covering the outmost part of the eyeball are composed of collagenous fiber and three longitudinal muscle layers (the thickness of each longitudinal muscle layer, $0.4{\mu}m$) and thick circular muscle layer (thickness, $0.3{\mu}m$). Around the capsules, there is a neurophile layer consisting of neurons and nerve fibers. Each neuron has a relatively large ovoid nucleus for its cytoplasm, and in the karyosome, large lumps of keterochromatin form a wheel nucleus.
