Successful Modified Nikaidoh Procedure (Pivot Rotation) in a Patient with Double Outlet Right Ventricle and Pulmonary Atresia: Case Report

  • Double outlet right ventricle (DORV) with pulmonary atresia (PA) is an uncommon congenital disease. Because of anatomical and physiological anomalies in the systemic and pulmonary circulation, corrective surgery may be challenging. We present the case of a patient with DORV and PA. This condition was successfully corrected using a modified Nikaidoh procedure, resulting in reduced obstruction of the left ventricular outflow tract.

  • In neurogenic equinovarus deformity, surgical intervention such as tendon transfer or osteotomy can be expected to improve symptoms. However, in rare cases of hereditary spastic paraplegia, the deformity and paralysis gradually progress. So limited operation and early post-operative rehabilitation are preferred to aggressive operation. We would like to report our clinical experience with one case of hereditary spastic paraplegia patient with reference review. A 40 year-old male, given tendon transfer of ankle and foot and tendo achilles lengthening 10 years ago, complained about aggravated spastic paraplegia which resulted in dynamic equinovarus and limited walking ability since his operation. Family history showed limited walking ability of his father with gradually progressing spastic paralysis and he was diagnosed as hereditary spastic paraplegia type I. We had performed a limited operation such as tendo achilles and tibialis posterior lengthening to induce plantigrade standing and walking with crutch. As a result, the patient was able to maintain a stabilized standing posture and walk after the operation. Hereditary spastic paraplegia presents with a progressive paralysis which limits rehabilitation after tendon transfer, and the symptoms can be aggravated. Therefore, considering potential hereditary neurogenic disorders in paients with equinovarus deformity and performing limited operative procedures seem to be important.

  • In the present emerging global economy, as the focus is shifted from manufacturing to service sector, the efficiency of service operations is crucial for the county's sustainable growth and development. As one of the fastest growing service sector industries in South Korea, the call center has become an inevitable part of customer service for most companies. Despite of the strategic importance of the call center, there has been no empirical study on the efficiency of call center service operations in the literature. The main purpose of this study is to evaluate the technical efficiency of call center services so as to not only identify the current status of call center service operations, but also suggest ways to improve service operations efficiency. For this purpose, we apply a non-parametric efficiency measurement method, DEA (Data Envelopment Analysis), to domestic call centers in order to compare their relative efficiency. Based on DEA evaluations, we provided specific directions for the inefficient call centers to possibly improve their operation efficiencies. It is expected that the measurement methods suggested in this study can be applied to various issues such as service KS certification, service operations management, and the productivity analysis of service personnel.

Evaluation of the efficacy of unipolar and bipolar spinal dorsal root ganglion radiofrequency thermocoagulation in the treatment of postherpetic neuralgia

  • Background: Different views have been proposed on the radiofrequency treatment modes and parameters of radiofrequency thermocoagulation of the spinal dorsal root ganglion for the treatment of postherpetic neuralgia (PHN). It is urgent to identify a more effective therapy for patients with PHN. Methods: Patients who underwent radiofrequency thermocoagulation therapy for PHN were retrospectively reviewed and were divided into a radiofrequency thermocoagulation (CRF) and double neddles radiofrequency thermocoagulation (DCRF). The pain scores (numerical rating scale, NRS) were evaluated at the following time points: before the operation, 1 day, 3 months, 6 months, 1 year, and 2 years after operation. The incidence of complications and the degree of pain relief were evaluated. The in vitro ovalbumin experiment was used to indicate the effects of radiofrequency thermocoagulation. Results: Compared with the preoperative NRS scores, the postoperative NRS scores decreased significantly; the NRS scores of the DCRF group was lower than that of the CRF group at all time points from 6 months to 2 years following the operation. The total effective rate of the DCRF group was significantly higher than that of the CRF group at 2 years following the operation. The incidence of numbness in the DCRF group was higher than that noted in the CRF group. The ovalbumin experiments in vitro indicated that the effects of radiofrequency thermocoagulation were optimal when the distance between the two needles was 5 mm. Conclusions: DCRF with a 5 mm spacing exhibits a longer duration and higher effective rate in the treatment of PHN and is worth promoting.

  • Condition monitoring plays a vital role since it sustains the reliable operation of industrial plant and machinery in the pursuit of economic whole life operation. In order to achieve this goal, it is needed to monitor various parameters of mechanical system such as vibration, wear, temperature, and etc., and finally to diagnosis the root causes of any possible abnormal machine condition. In this work, we constructed a rotor system where various types of functional machine failures occurred frequently in industry were induced. Characteristics of the machine failure were monitored simultaneously by the on-line measurement of vibration, wear and temperature. Result showed that these parameters responded differently to the induced functional machine failure. The availability of each parameter on effective condition monitoring was discussed in this work.

Analysis of Tropospheric Carbon Monoxide over East Asia

  • Carbon monoxide (CO) is one of the important trace gases because its concentration in the troposphere directly influences the concentrations of tropospheric hydroxyl (OH), which controls the lifetimes of tropospheric trace gases. CO traces the transport of global and regional pollutants from industrial activities and large scale biomass burning. The distributions of CO were analyzed using the MOPITT data for East Asia, which were compared with the ozone distributions. In general, seasonal CO variations are characterized by a peak in the spring, which decrease in the summer. The monthly average for CO shows a similar profile to that for O$_3$. This fact clearly indicates that the high concentration of CO in the spring is possibly due to one of two causes: the photochemical production of CO in the troposphere, or the transport of the CO into East Asia. The seasonal cycles for CO and O$_3$ in East Asia are extensively influenced by the seasonal exchanges of different air mass types due to the Asian monsoon. The continental air masses contain high concentrations of O$_3$ and CO, due to the higher continental background concentrations, and sometimes to the contribution from regional pollution. In summer this transport pattern is reversed, where the Pacific marine air masses that prevail over Korea bring low concentrations of CO and O$_3$, which tend to give the apparent summer minimums.

  • The Measurement of Pollution in the Troposphere (MOPITT) instrument is an eight-channel gas correlation radiometer that launched on the Earth Observing System (EOS) Terra spacecraft in 1999. Its main objectives are to measure carbon monoxide (CO) and methane (CH4) concentrations in the troposphere. This study analyzes tropospheric carbon monoxide distributions using MOPITT data and compare with ozone distributions in Northeast Asia. In general, seasonal CO variations are characterized by a peak in spring and decrease in summer. Also, this study revealed that the seasonal cycles of CO are maximum in spring and minimum in summer with average concentrations ranging from 118ppbv to 170ppbv. The monthly average of CO shows a similar profile to those of O3. This fact clearly indicates that the high concentration of CO in spring is caused by two possible causes: the photochemical CO production in the troposphere, or the transport of the CO in the northeast Asia. The CO and $O_3$ seasonal cycles in the Northeast Asia are influenced extensively by the seasonal exchange of the different types of air mass due to the Asian monsoon. The continental air masses contain high concentrations of $O_3$ and CO due to higher continental background concentrations and sometimes due to the contribution of regional pollution. Aspergillosis of Central Nervous System (중추신경계의 Aspergillosis)

  • Objectives : Aspergillosis of central nervous system(CNS) is a rare pathologic condition and it has been known to be difficult to diagnose and treat. We analyzed seven cases of central nervous system aspergillosis. The clinical characteristics, and the problems in diagnosis and treatment are discussed with review of previous literatures. Material and Methods : We reviewed the clinical records, radiological findings, and pathologic reports of 7 patients with aspergillosis which involved CNS. Results : Five patients were immunocompetent, and infection was related with previous operation in 4 of them. Two patients were immunocompromised and had no history of operation. Five patients had intracranial lesions and two had spinal lesions. Mean duration from the onset of initial symptom to pathologic diagnosis was 2.4 months. Mean duration from the previous operation to the onset of symptom was 9.3 months, and from the onset of symptom to diagnosis was 2.9 months in the patients who had histories of operation. All of them were treated with surgical procedures and intravenous and oral antifungal agents, resulting in cure in 6 cases. Mean duration of the treatment was 4.9 months. Conclusion : Because aspergillosis of CNS is a rare disease and is difficult to be differentiated from the pyogenic abscess or recurrent tumor, the pathologic diagnosis is very important for adequate treatment. Although the prognosis of aspergillosis of CNS has been known to be poor, adequate surgery for both diagnosis and treatment and antifungal chemotherapy resulted in good outcome.

Analysis of the MSC(Multi-Spectral Camera) Operational Parameters

  • The MSC is a payload on the KOMPSAT-2 satellite to perform the earth remote sensing. The instrument images the earth using a push-broom motion with a swath width of 15 km and a GSD(Ground Sample Distance) of 1 m over the entire FOV(Field Of View) at altitude 685 km. The instrument is designed to haute an on-orbit operation duty cycle of 20% over the mission lifetime of 3 years with the functions of programmable gain/offset and on-board image data compression/storage. The MSC instrument has one channel for panchromatic imaging and four channel for multi-spectral imaging covering the spectral range from 450nm to 900nm using TDI(Time Belayed Integration) CCD(Charge Coupled Device) FPA(Focal Plane Assembly). The MSC hardware consists of three subsystem, EOS(Electro Optic camera Subsystem), PMU(Payload Management Unit) and PDTS(Payload Data Transmission Subsystem) and each subsystems are currently under development and will be integrated and verified through functional and space environment tests. Final verified MSC will be delivered to spacecraft bus for AIT(Assembly, Integration and Test) and then COMSAT-2 satellite will be launched after verification process through IST(Integrated Satellite Test). In this paper, the introduction of MSC, the configuration of MSC electronics including electrical interlace and design of CEU(Camera Electronic Unit) in EOS are described. MSC Operation parameters induced from the operation concept are discussed and analyzed to find the influence of system for on-orbit operation in future.
