In this study, using the plasma spray method, tensile and compression fatigue tests are performed in saline solution to examine the effect of Ti undercoat on corrosion fatigue behavior of alumina-coated specimens. The alumina-coated material using Ti in the undercoat shows better corrosion fatigue strength than the base material in the entire stress amplitude range. Fatigue cracking of UT specimens occurs in the recess formed by grit-blasting treatment and progresses toward the base metal. Subsequently, the undercoat is destroyed at a stage where the deformation of the undercoat cannot follow the crack opening displacement. The residual stress of the UT specimen has a tensile residual stress up to about 100 ㎛ below the surface of the base material; however, when the depth exceeds 100 ㎛, the residual stress becomes a compressive residual stress. In addition, the inside of the spray coating film is compressive residual stress, which contributes to improving the fatigue strength characteristics. A hardened layer due to grit-blasting treatment is formed near the surface of the UT specimen, contributing to the improvement of the fatigue strength characteristics. Since the natural potential of Ti spray coating film is slightly higher than that of the base material, it exhibits excellent corrosion resistance; however, when physiological saline intrudes, a galvanic battery is formed and the base material corrodes preferentially.
Even welds that have passed non-destructive testing in the case of brittle crack arrest steel materials will actually have very fine weld defects. Based on studies showing that these defects adversely affect the structure if subjected to a certain period of load, the following conclusions were obtained by conducting CTOD tests on welding joints of high-strength BCA materials, structures comprising the upper decks of a large container vessel. First of all, the fatigue pre-cracking in the weld metal and heat affected areas was tested and the behavior was identified. Both parts of the welding joint are allowable range for the class regulations. In addition, CTOD results showed that the CTOD value in the heat affected area was more than 0.5 times higher than in the weld metal area.
A 14-year-old girl had a midfacial trauma event caused by hitting against an opening door and experienced discomfort and swelling of the columella and upper lip. Physical examination revealed mild tenderness on light palpation without any discomfort with upper lip movement. A computed tomography scan of the maxillofacial bones with three-dimensional reconstruction showed a fracture of the anterior nasal spine with obvious leftward displacement, mild-deviation of the caudal aspect of the nasal septum, and no sign of nasal bone fracture. Open reduction and internal fixation was performed with regard to aesthetic and functional concerns, including nasal septum deviation. The postoperative course was uneventful, and healing proceeded normally without complications. Herein, we emphasize the importance of differential diagnosis of isolated anterior nasal spine fractures in patients with midfacial trauma and clinicians' strategic decision-making in treatment modalities.

In this study, mechanical, flexural post-cracking, and torsional behaviors of recycled steel fiber-reinforced concrete (RSFRC) incorporating steel fibers obtained from recycling of waste tires were investigated. Initially, three concrete mixes with different fiber contents (0, 40, and 80 kg/m3) were designed and tested in fresh and hardened states. Subsequently, the flexural post-cracking behaviors of RSFRCs were assessed by conducting three-point bending tests on notched beams. It was observed that recycled steel fibers improve the post-cracking flexural behavior in terms of energy absorption, ductility, and residual flexural strength. What's more, torsional behaviors of four RSFRC concrete beams with varying reinforcement configurations were investigated. The results indicated that RSFRCs exhibited an improved post-elastic torsional behaviors, both in terms of the torsional capacity and ductility of the beams. Additionally, numerical analyses were performed to capture the behaviors of RSFRCs in flexure and torsion. At first, inverse analyses were carried out on the results of the three-point bending tests to determine the tensile functions of RSFRC specimens. Additionally, the applicability of the obtained RSFRC tensile functions was verified by comparing the results of the conducted experiments to their numerical counterparts. Finally, it is noteworthy that, despite the scatter (i.e., non-uniqueness) in the aspect ratio of recycled steel fiber (as opposed to industrial steel fiber), their inclusion contributed to the improvement of post-cracking flexural and torsional capacities.
본 연구는 안면비대칭이 있는 환자의 측두하악관절원반 형태와 위치를 평가하고 악관절내장증과 안면비대칭 간의 관계를 규명하기 위하여 시행되었다. 남자 10명 및 여자 27명으로 구성된 안면비대칭 환자 37명의 74개의 TMJ MRI를 사용하였으며, 대상 집단의 연령은 12세에서 44세까지의 범위로 평균 21.4세였다. 시상 및 두정 TMJ MRI를 최대감합위 및 최대개구위에서 채득한 후 부분전방변위, 전방변위, 회전 또는 측방원반변위 등 원반변위의 종류를 기록하였으며, 원반의 모양위치, 그리고 원반의 변위 및 회전을 MRI tracing 상에서 판독하였다. 악관절내장증의 증상은 정상, 정복성 전방원반변위(ADDR), 그리고 비정복성 전방원반변위 (ADDNR) 집단으로 분류하였으며, 환자는 양측성 정상. 편측 혹은 양측 내장증 집단으로 분류하였다. 약$70\%$의 환자에서 편측성 또는 양측성 내장증을 보였으며. 통계분석 결과 ADD, 특히 회전성 ADD가 변위측에서 더 높은 빈도로 나타났으나 반대측에서는 원반의 위치가 정상인 경우가 많았다. (p<0.01). 변위측의 원반은 모양의 변형 및 하전방 변위를 유의하게 더 나타냈다. 그러나 반대쪽은 개구시 원반이 수직방향으로 과운동성이 관찰되었다. 이 연구는 안면비대칭 환자에서 ADD의 종류가 원반의 모양, 경사 변위 각도, 변위의 수직거리 및 회전각도와 연관이 있다는 것을 제시한다
본 연구는 단국대학교 치과대학 부속병원 구강내과에 내원한 측두하악장애 환자를 포함한 구강안면동통 환자들을 대상으로 치료의 결과와 예후를 평가하기 위하여, 2002년 1월부터 2004년 12월까지 3년간 (치아질환을 제외한) 구강안면동통 환자 6300명 중에서 진료기록부 상에서 경과기록을 확인할 수 있는 환자들을, 측두하악장애, 신경병성동통장애, 연조직질환 및 이 중 둘 이상을 질환을 동시에 가지고 있는 복합질환으로 나누어 치료기간, 치료유형 및 치료효과를 비교하여 다음의 결과를 얻었다. 1. 평균 진료기간은 신경병성 동통장애에서 가장 길었고 연조직질환, 복합 질환, 관절장애, 근육-관절 복합장애, 근육장애의 순이었다. 2. 사용된 치료방법을 비교하면 약물요법은 연조직질환과 신경병성 동통장애에서, 장치요법은 관절장애군에서, 물리치료는 근육장애군에서 사용빈도가 높았다. 3. 진단분류별 물리치료의 사용경향은 근육장애군은 EAST와 초단파요법의 사용빈도가 높았고, 관절장애군은 초음파 치료, 신경병성 동통 장애와 연조직 질환은 저출력 레이저 요법이 많이 사용되었다. 4. 통증에 대한 치료결과는 통증이 지속되는 경향이 신경병성 동통장애에서 가장 높았으며, 완화되거나 완전 소실되는 경향은 측두하악장애에서 높았다. 5. 측두하악장애 환자에서 근육장애, 정복성 관절원판장애, 비정복성 관절원판 장애 및 근육-관절 복합장애 세분하여 치료 후 개구량의 변화를 조사하였을 때는 진단군 간에 유의한 차이를 볼 수 없었으나 개구량 측정방법간에는 무통성 최대개구량이 치료 후 가장 증가하였고, 환자 스스로가 벌리는 능동적 최대개구량은 비정복성관절원판장애에서 가장 증가하였다. 이상의 결과로 볼 때, 구강안면통증 영역에서 현재 시행하고 있는 치료법들은 측두하악장애에서는 동통조절 및 기능개선에 좋은 결과를 보여주고 있으므로 그 예후가 좋은 반면, 연조직질환이나 신경계 질환 같은 기타의 구강안면통증은 길고 지속적인 치료를 요구하는 어려운 질환이라고 할 수 있겠다.
이 논문에서는 섬유의 혼입량에 따른 휨부재의 장기거동에 대한 영향을 평가하기 위하여 후크형 강섬유로 보강된 고강도 콘크리트의 휨 크리프 거동에 대한 평가가 이루어졌다. 실험은 150 × 150 × 600mm 크기를 갖는 섬유 혼입량(0, 0.75 및 1.5%)을 변수로 하는 6개의 휨 시험체를 대상으로 하였다. 노치를 갖는 휨 시험체에 휨 크리프 하중을 도입하기 위하여 4점 가력 휨 시험장치가 활용되었다. 휨강도의 40%인 크리프 재하하중은 레버 장치를 활용하여 도입되었고 90일 동안 도입된 하중은 로드셀에 의해 제어되었다. 크리프 하중의 도입시, 시험체 중앙부에 설치된 노치의 균열개구변위(CMOD)가 측정되었다. 크리프 시험후 각 시험체 대한 휨시험을 실시하여 각 시험체의 잔여강도를 평가하였다. 이상과 같은 실험결과로부터 섬유 혼입량은 고강도 콘크리트의 휨 크리프 거동에 주요한 영향을 끼치고 휨강도의 40% 범위내의 지속하중은 섬유보강된 고강도 콘크리트의 잔여강도에 부정적인 영향을 끼치지 않았다.
일반적으로 콘크리트 구조물은 보와 기둥이 서로 강결되어 있으며, 이러한 경우 강진에 의해 연결부에서 심각한 손상이 발생할 수 있다. 이를 저감시키면서 내진성능을 향상시키기 위한 다양한 연결 형태가 연구되어지고 있다. 그 한 예로 연결부에서의 회전을 허용하는 연결형식이 있으며 보나 기둥, 그리고 전단벽에 응용되고 있다. 이러한 회전형 구조요소들은 횡방향 거동시 비선형 힘-변위 관계를 나타내는데, 그 원인은 연결부의 회전으로 인한 접촉면의 깊이(contact depth)가 줄어듦과 동시에 요소의 각 단면에서의 응력이 비선형적으로 분포되는 탄성힌지 구간이 존재하기 때문이다. 이 연구에서는 축방향 하중(공칭강도의 5%와 10%)과 경계조건(양단구속 형식, 캔틸레버 형식), 세장비(L/d = 5, 7, 10) 등의 변수를 고려한 유한요소해석을 통해 회전형 기둥의 탄성힌지 구간 또는 길이를 분석하였다. 그 결과 이 세가지 변수는 탄성힌지길이 변화에는 직접적인 영향을 주지 않았으며 다만 접촉면의 깊이에 의해 지배됨을 알 수 있었다. 이 탄성힌지길이는 opening state부터 발생하기 시작하여 rocking point까지(pre-rocking 구간) 증가하였으나 그 이후(post-rocking 구간)에서는 일정한 값을 보였다. 탄성힌지길이에 대한 유한요소해석 결과를 이론적 예측식인 반무한모델(half space model)의 결과와 비교하였다.
Background: The purpose of this study is to introduce our modified disc plication technique using MITEK mini anchors and to evaluate the clinical outcome for patients with internal derangement (ID) of the temporomandibular joint (TMJ). Patients and methods: We evaluated 65 joints in 46 patients, comprised 32 women and 14 men, who first visited the Asan Medical Center from December 2012 to December 2016. The age of the patients ranged from 14 to 79 years, with a mean age of 36.6 years. The patients presented with joint problems including pain, joint noise, and mouth opening limitation (MOL). Patients who met our inclusion criteria underwent unilateral or bilateral disc repositioning surgery with our minimally invasive disc plication technique using MITEK mini anchors and No. 2-0 Ethibond® braided polyester sutures. The variables taken into account in this study were the range of maximum mouth opening (MMO), painful symptoms (evaluated with the visual analog scale, VAS), and the type of noise (click, popping, crepitus) in the TMJ. Results: Preoperative examination revealed painful symptoms in 50.7% (n = 35) of the operated joints (n = 69) and the presence of clicks in 56.5% (n = 39). Postoperative examination revealed that 4.3% (n = 3) of the operated joints had painful symptoms with lower intensity than that in the preoperative condition. Additionally, 17.4% (n = 12) had residual noise in the TMJ, among which two were clicking and the other 10 had mild crepitus. The intensity of the postoperative residual noise was significantly decreased in all cases compared to that in the preoperative condition. Among patients with MOL below 38 mm (n = 18), the mean MMO was 31.4 mm preoperatively and 44.2 mm at 6 months postoperatively, with a mean increase of 13.8 mm. A barely visible scar at the operation site was noted during the postoperative observation period, with no significant complications such as facial palsy or permanent occlusal disharmony. Conclusion: Subjective symptoms in all patients improved following the surgery. TMJ disc plication using MITEK mini anchors with our minimally invasive approach may be a feasible and effective surgical option for treating TMJ ID patients who are not responsive to conservative treatment.
The human mandible is always under the condition of loading by the various forces extorted by the attached muscles. The loading is an important condition of the stomatognathic system. This condition is composed of the direction and amount of forces of the masticatory muscles, which are controlled by the neuromuscular system, and always influenced by the movement of both opening and closing. Mandible is a strong foundation for the teeth or various prostheses, nevetheless it is a elastic body which accompanies deformation by the external forces on it. The elastic properties of the mandible is influenced by the various procedures such as conventional restorative treatments, osseointegrated implant treatments, reconstructive surgical procedures and so forth. Among the treatments the osseointegrated implant has no periodontal ligaments, which exist around the natural teeth to allow physiologic mobility in the alveolar socket. And so around the osseointegrated implant, there is almost no damping effect during the transmission of occlusal stress and displacements. If the osseointegrated implants are connected by the superstructure for the stabilization and effective distribution of occlusal stresses, the elastic properties of mandible is restricted according to the extent of 'splinting' by the superstructure and implants. To investigate the change of elastic behaviour of the mandible which has osseointegrated implant prosthesis of various numbers of implant installment and span of superstructre, a three dimensional finite element model was developed and analyzed with conditions mentioned above. The conclusions are as follows : 1. The displacements are primarily developed at the area of muscle attachment and distributed all around the mandible according to the various properties of bone. 2. The segmentation in the superstructure has few influence on the distribution of stress and displacement. 3. In the load case of ICP, the concentration of tensional stress was observed at the anterior portion of the ramus($9.22E+6N/m^2$) and at the lingual portion of the symphysis menti($8.36E+6N/m^2$). 4. In the load case of INC, the concentration of tensional stress was observed at the anterior portion of the ramus($9.90E+6N/m^2$) and the concentration of tensional stress was observed at the lingual portion of the symphysis menti($2.38E+6N/m^2$)). 5. In the load case of UTCP, the relatively high concentration of tensional stress($3.66E+7N/m^2$) was observed at the internal surface of the condylar neck.
