Lumbosacral Plexopathy, Complicating Rhabdomyolysis in a 57-Year-Old Man, Presented with Sudden Weakness in Both Legs

  • A 57-year-old man presented with weakness in both legs upon awakening after drinking. Magnetic resonance imaging (MRI) of the lumbar spine did not reveal any intraspinal abnormalities but MRI of the pelvis revealed lesions with abnormal intensities with heterogeneous contrast enhancement in both gluteal muscles. Serum creatine phosphokinase was markedly elevated. A diagnosis of lumbosacral plexopathy, complicating rhabdomyolysis was made. With supportive care he recovered well but mild weakness of the right ankle remained at 6 month-follow-up. Pelvic MRI is a helpful diagnostic tool in localizing rhabdomyolysis. Lumbosacral plexopathy should be included in the differential diagnosis of the such cases, presenting with sudden weakness of legs.

  • Pancoast tumor was specific lung carcinoma that has been symptoms and signs according to locations. It was located in peripheral,and involved the extrathoracic structures more than parenchyme of the lung. At 1838, Hare reported it, and at 1932 Pancoast was first described it. Prior to 1950,superior sulcus tumor was considered uniformly fatal, but at 1961 Paulson and Shaw advocated the use of preoperative irradiation therapy and followed by an extended en bloc resection. Recently we were experienced 2 cases of pancoast tumor managed with same method. One was 60-years old man that has been recommended preoperative radiation therapy with dose of 3000 cGy to 20 fractions and followed resection after 4 weeks, the other was 53-years old man that has been recommended a dose of 4000 cGy to 20 fractions and followed resection after 4 weeks. On tumor histology first case was large cell carcinoma and second case was squamous cell carcinoma. all patients was complicated atelectasis. First patient was expired with brain metastasis after 17 months, second was expired after 6 months.

  • A case of Pseudoterranova decipiens infection was found in a 43-year-old man by gastroendoscopic examination on August 20, 1996. On August 6, 1996, he visited a local clinic, complaining of epigastric pain two days after eating raw marine fishes. Although the symptoms were relieved soon, endoscopic examination was done for differential diagnosis. A white, live nematode larva was removed from the fundus of the stomach. The larva was $38.3{\times}1.0{\;}mm$ in size and had a cecum reaching to the mid-level of the ventriculus. A lot of transverse striations were regularly arranged on the cuticle of its body surface, but the boring tooth and mucron were not observed at both ends of the worm. The worm was identified as the 4th stage larva of P. decipiens.

  • Intradural lumbar disc herniation (ILDH) is rare. In this report, authors present 2 cases of ILDHs associated with severe adhesion between the dural sac and posterior longitudinal ligament. In a 40-year-old man, ILDH occurred in association with epidural adhesion due to ossification of the posterior longitudinal ligament (OPLL). In other 31-year-old man, ILDH occurred in presence of epidural adhesion due to previous spine surgery.

  • The benign cementoblastoma is a benign odontogenic tumor of cemental-forming tissue, which has an unlimited growth potential. The radiographic appearance of the lesion is pathognomonic and consists of a radiopaque mass surrounded by a thin radiolucent line. This mass is inseparable, radiologically, from the tooth root and is attached to it. Microscopic evaluation suggests that the lesion enlarges by peripheral growth, with the center of the neoplasm being more calcified and inactive than the periphery. We have observed two cases of beneign cementoblastoma{mature calcified stage) occured in the left mandibular first molar of 22-year-old man and 14-year-old man.

  • The basal cell nevus syndrome (BCNS) is an autosomal dominant disorder, characterized by basal cell carcinomas, odontogenic keratocysts and skeletal abnormalities. We experienced two cases that represented several characteristics of BCNS. Case 1: a thirty three year-old man visited CSU hospital. His radiographs showed four cystic lesions at both maxillary sinus and both mandibular angle, with bifid rib and ectopic calcification of falx cerebri. After marsupialization and enucleation, recurrent and newly developing tendency were found on his follow-up radiographs. Case 2: a seventeen year-old man had four large cystic lesions which were diagnosed as odontogenic keratocysts. He had craniofacial anomalies which included ectopic calcification and frontal bossing.

  • This research aims to study the changes In gastrointestinal function attributed to aging In human. The thresholds for recognition and detection of flavors became elevated and salivary gland acinar cells decreased in the old age. But most esophageal function remained relatively Intact. Although gastric emptying time has been slowed with aging, the total intestinal transit time did not differ. Atropic gastritis due to H. pylori in old man decreased secretion of acid and Intrinsic factor and absorbability of calcium and iron. Pancreatic secretion is droned in older persons. Prevalence of gallstones rised with age. Liver size and portal blood flow decreased significantly with age. Mucosal surface area has been reported to be slightly diminished in the aging man. Glucose transporters decreased and Insulin tolerance Increased. Absorption of aromatic amino acid is diminished with age. Dietary protein In that aging human increased fecal nitrogen excretion. Vitamin A tolerance increased. Vitamin D receptor concentration decreased and resistance to 1,25-(OH)2D3 action increased. Permeability of aging small Intestine Increased. Zinc balance dirt not differ Copper absorption appeared not to be significantly affected by age. Neurotensin secretion decreased thus slowed colonic peristaltic movements and Intestinal mucosal growth.

  • Bronchopulmonary sequestration (BPS) is a rare congenital malformation of the lower respiratory tract. Most intralobar BPSs are provided with an arterial blood via the thoracic or abdominal aorta but such a supply is rarely found in patients older than 50 years. We report a case of an intralobar BPS with a dual arterial supply from the celiac artery and thoracic aorta in a 50-year-old man presenting with a respiratory tract infection and haemoptysis. To our knowledge, this is the first case report of a BPS supplied by the celiac artery and thoracic aorta in a 50-year-old man.

  • The “Changzu”(footbinding)was an old chinese traditional custom practiced during more the thousand years and was applied to only woman. It prevents young girl’s foot from growing when she is 3 to 8 years old by big toe and forcing her to were the lotus shoe called LIANXIE. In china, the feet and shoe had special symbolic meaning other than usual simple of supporting body and protecting the foot, they are symoblic aspects of implication interal new meaning. In this study was investigate the internal meaning symbolized by Changzu from various perspectives such as religious, aesthetic, and group unconsicous aspects. In conclusion, the Changzu is a symbol that representing a dilemma which on one hand women would like to attract man physically while on other hand woman should be chaste and modest morally. And aiso it is group image representing groups unconsicous desire to preserving the artificial identity bestowed upon Chines woman by Chines man.

  • Uveitis is a comprehensive term that indicates the inflammation of not only the uvea but also its adjacent tissues, and its etiology, clinical features and prognosis are various. We conducted 2 patients with uveitis seen at Won-Kwang University Kwang- Ju Oriental Medical Hospital in 2002. We treated uveitis in a twenty-nine years old man and a thirty years old woman with the complaint of decreased vision, glaring eye, blood-shot eye, pain of eye, tears in one's eye by herbal medicine (Se-An-Myung-Mok-Tang gami) and Acupunture. Usually, uveitis have chronic phase. We had a significant results. So we report this two cases.
