Comparison of the Effects of Different Foot Positions During Body-lifting in Wheelchair on Shoulder Muscle Activities, Peak Plantar Pressure, Knee Flexion Angle, and Rating Perceived Exertion in Individuals With Spinal Cord Injury (휠체어에서 엉덩이 들기 동작 동안 발위치가 척수손상환자의 어깨 근활성도, 최대 족저압, 무릎굽힘 각도, 운동자각도에 미치는 효과 비교)

  • Background: Individuals with spinal cord injury (SCI) rely on their upper limbs for body-lifting activity (BLA). While studies have examined the electromyography (EMG) and kinematics of the shoulder joints during BLA, no studies have considered foot position during BLA. Objects: This study compared the effects of different foot positions during BLA on the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion in individuals with SCI. Methods: The study enrolled 13 mens with motor-complete paraplegic SCI, ASIA (American Spinal Injury Association) A or B. All subjects performed BLA with the feet positioned on the wheelchair footrest and on the floor independently. Surface EMG was used to collect data from the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii. The peak plantar pressure was measured using pedar-X and the knee flexion angle with Image J. Borg's rating perceived exertion scale was used to measure the physical activity intensity level. The paired t-test was used to compare the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion between the two feet positions during BLA. Results: The activity of the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii and rating perceived exertion decreased significantly and the peak plantar pressure and knee flexion angle increased significantly when performing BLA with the feet positioned on the wheelchair footrest compared with on the floor (p<.05). Conclusion: These findings suggest that individuals with SCI may perform BLA with the feet positioned on the wheelchair footrest for weight-relief lifting to decrease the shoulder muscle activities and the rating perceived exertion and to increase the peak plantar pressure and the knee flexion angle.

  • The purpose of this study is to discover the occupational identity by examining the narrative of the life of a vocational training teacher with self-esteem in programming fields. The following six types of occupational identity were found: 'a positive image of a vocational training teacher(fits oneself)', 'I feel proud of myself while doing vocational training activities.', 'a teacher who continues to develop him/herself as an expert in the subject class', 'a teacher who immerses him/herself as an expert on student change and growth', 'a teacher engaged in leading activities to create opportunities for vocational training', and 'a teacher of continuous pursuit'. This study has significance in exploring the structure of occupational identity recognition and experience of its formation of a self-esteemed vocational training teacher in programming fields, which have not been studied.

Image of dental hygienists according to information sources at online or offline: focusing on college preparatory students (온·오프라인 정보수집 경로에 따른 치과위생사 이미지: 대입 준비생을 중심으로)

  • Background: The purpose of this study was to examine the differences in the formation of dental hygienist images based on the pathways of obtaining occupational information and to establish a foundation for the correct perception and positive promotion of the dental hygienist profession. Methods: A survey was distributed to 305 college preparatory students in the metropolitan area. The questionnaire consisted of 34 items, including general characteristics(3 items), pathway-related questions(3 items), dental hygienist image-related question(2 1items), application-related questions(2 items), and admission-related questions(5 items). The images of dental hygienists based on general characteristics, perception pathways, and admission were analyzed using the Mann-Whitney U test and Kruskal-Wallis test. Results: The survey results from all participants showed that the overall image of dental hygienists was 3.75 points. Personal image scored the highest at 4.18 points, while social image was the lowest at 3.20 points(p<0.05). The overall image of dental hygienists was higher for the 'online' group (3.88) compared to the 'offline' group, and statistically significant differences were observed among groups in overall, personal, and professional images(p<0.05). The overall image of dental hygienists was higher for those who learned offline (3.87), and the only significant difference between groups was seen in the personal image. When it came to admission, the 'admitted' students gave a higher overall score (4.00) compared to 'non-admitted' students (3.64), with significance found in all areas except for social image (p<0.05). Conclusion: It is believed that effective utilization of online pathways can inform more people about the importance and expertise of dental hygienists, thereby contributing to promoting oral health and enhancing the perception of the profession. Properly valuing and educating about the role of dental hygienists through promotion and education can help improve the image of the profession.

Significance of brain magnetic resonance imaging(MRI) in the assessment of occupational manganese exposure (직업적 망간 폭로에 있어서 뇌자기공명영상의 의의)

  • Manganese is an essential element in the body. It is mainly deposited in the liver and to a lesser degree in the basal ganglia of the brain and eliminated through the bile duct. Rapid turnover of managanese in the body makes it difficult to evaluate the manganese exposure in workers, esecially in those with irregular or intermittent exposure, like welders. Therefore, conventional biomarkers, including blood and urine manganese can provide only a limited information about the long-tern or cumulative exposure to manganese. Introduction of magnetic resonance imaging (MRI) made a progress in the assessment of manganese exposure in the medical conditions related to manganese accumulation, e. g. hepatic failure and long-term total parenteral nutrition. Manganese shortens spin-lattice(T1) relaxation time on MRI due to its paramagnetic property, resulting in high signal intensity (HSI) on T1-weighted image(T1W1) of MRI. Manganese deposition in the brain, therefore, can be visualizedas an HSI in the globus pallidus, the substantia nigra, the putamen and the pituitary. clinical and epidemiologic studies regarding the MRI findings in the cases of occupational and non-occupational manganese exposure were reviewed. relationships between HSI on T1W1 of MRI and age, gender, occupational manganese exposure, and neurological dysfunction were analysed. Relationships betwen biological exposure indices and HSI on MRE werealso reviewed. Literatures were reviewed to establish the relationships between HSI, Manganese deposition in the brain, pathologic findings, and neurological dysfunction. HSI on T1W1 of MRI reflects regional manganese deposition in the brain. This relationship enables an estimation of regional manganese deposition in the brain by analysing MR signal intensity. Manganese deposition in the brain can induce a neuronal loss in the basal ganglia but functional abnormality is supposed to be related to the cumulative exposure of manganese in the brain, use of brain MRI for the assessment of exposure in a group of workers seems to be hardly rationalized, while ti can be a useful adjunct for the evaluation of manganese exposure int he cases with suspected manganese-related health problems.

The Changes in the Ultrasound Imaging of Abdominal Muscles based on the Inspiratory Muscle Strengthening Training of Low Back Pain Patients (들숨군 강화 훈련이 허리통증환자의 배근육 초음파 영상 구조 변화에 미치는 영향)

  • Purpose: The purpose of this study was to classify patients with chronic back pain according to the degree of their back pain, and to compare the pain dysfunction index with the qualitative changes in abdominal muscles. Therefore, we aimed to provide a basis for the treatment intervention method for patients with back pain. Methods: Twenty patients with chronic back pain were purposive sample to a group of 10 patients with a back pain index of 60 % or more and a group with less than 60 % of back pain, and the subjects who voluntarily participated in the study After receiving the letter, I conducted the research the dysfunction of back pain was measured by the Korean version of the Oswestry Disability Index (KODI), and the ultrasonic wave (Ultrasound MyLabOne, ESAOTE, Italy) And the white area index, and the abdominal muscle movement was used as the exercise instrument POWER breathe K5 (Hab direct, UK), which strengthens the respiratory muscles through threshold-muscle traction. Result: In this study, patients with chronic back pain were subjected to breathing exercises, which led to the decrease in back pain dysfunction. The ultrasonographic analysis of abdominal muscles revealed that both the white area index and muscle image density in the skeletal muscle and in the outer muscle of the abdomen gradually decreased over time. Conclusion: It is thought that introducing back pain patients to abdominal muscle reinforcement training is effective in improving the functions of the patients' muscles, thus increasing their quality of life.

Detection of Lung Nodule on Temporal Subtraction Images Based on Artificial Neural Network

  • The temporal subtraction technique as one of computer aided diagnosis has been introduced in medical fields to enhance the interval changes such as formation of new lesions and changes in existing abnormalities on deference image. With the temporal subtraction technique radiologists can easily detect lung nodules on visual screening. Until now, two-dimensional temporal subtraction imaging technique has been introduced for the clinical test. We have developed new temporal subtraction method to remove the subtraction artifacts which is caused by mis-registration on temporal subtraction images of lungs on MDCT images. In this paper, we propose a new computer aided diagnosis scheme for automatic enhancing the lung nodules from the temporal subtraction of thoracic MDCT images. At first, the candidates regions included nodules are detected by the multiple threshold technique in terms of the pixel value on the temporal subtraction images. Then, a rule-base method and artificial neural networks is utilized to remove the false positives of nodule candidates which is obtained temporal subtraction images. We have applied our detection of lung nodules to 30 thoracic MDCT image sets including lung nodules. With the detection method, satisfactory experimental results are obtained. Some experimental results are shown with discussion.

Deep Learning Frameworks for Cervical Mobilization Based on Website Images

  • Background: Deep learning related research works on website medical images have been actively conducted in the field of health care, however, articles related to the musculoskeletal system have been introduced insufficiently, deep learning-based studies on classifying orthopedic manual therapy images would also just be entered. Objectives: To create a deep learning model that categorizes cervical mobilization images and establish a web application to find out its clinical utility. Design: Research and development. Methods: Three types of cervical mobilization images (central posteroanterior (CPA) mobilization, unilateral posteroanterior (UPA) mobilization, and anteroposterior (AP) mobilization) were obtained using functions of 'Download All Images' and a web crawler. Unnecessary images were filtered from 'Auslogics Duplicate File Finder' to obtain the final 144 data (CPA=62, UPA=46, AP=36). Training classified into 3 classes was conducted in Teachable Machine. The next procedures, the trained model source was uploaded to the web application cloud integrated development environment (https://ide.goorm.io/) and the frame was built. The trained model was tested in three environments: Teachable Machine File Upload (TMFU), Teachable Machine Webcam (TMW), and Web Service webcam (WSW). Results: In three environments (TMFU, TMW, WSW), the accuracy of CPA mobilization images was 81-96%. The accuracy of the UPA mobilization image was 43~94%, and the accuracy deviation was greater than that of CPA. The accuracy of the AP mobilization image was 65-75%, and the deviation was not large compared to the other groups. In the three environments, the average accuracy of CPA was 92%, and the accuracy of UPA and AP was similar up to 70%. Conclusion: This study suggests that training of images of orthopedic manual therapy using machine learning open software is possible, and that web applications made using this training model can be used clinically.

  • This study involves an experiment using functional magnetic resonance imaging(fMRI) to delineate brain activation for execution functional performance. The execution function promotes a recovery major role from occupational therapy, understanding about the damage mechanism is important. When confirms the brain active area which accomplishes an execution function brain plasticity develops the cognitive therapeutic method which is effective increases usefully very, will be used.

Changes in Reaction Time during Mental Rotation of Three-Dimensional Objects for Chronic Hemiparetic Stroke (3차원 물체의 심상회전이 만성 편마비 뇌졸중 환자의 반응시간에 미치는 영향)

  • There is a cause and effect relationship in that brain injury causes impairment of mental rotation and ultimately independent functional activities. The purpose of this study was to compare the effects of mental rotation on reaction time and precision between the normal adults and chronic hemiparetic stroke patients. Thirty-one patients with chronic hemiparetic stroke and twenty normal adults participated in this study. The participants conducted 2 types of tasks for mental rotation: a comparison task using mirror images, and a rotation task using angular disparity images for 2 different 3-dimensional objects. Each of the 3 possible angled shapes ($90^{\circ}$, $180^{\circ}$, and $270^{\circ}$) appeared in each pair. The test consisted of 6 mirror-reflected image pairs and 6 angular disparity image pairs visualized during angular rotation, and 12 test periods. The subjects were judged on how accurately and rapidly they could distinguish between the mirrored and non-mirrored pairs. The study measured reaction time and precision to compare the effect of mental rotation tasks. Reaction time during all 3 angular conditions were significantly longer in the stroke patients than that in the normal adult during the comparison tasks and the rotation tasks. In addition, precision during mental rotation tasks was not significantly different between normal adults and stroke patients. Our results suggest that rehabilitation therapists should keep in mind that mental rotation is more difficult in stroke population than in normal adults.

Radiation Dose and Image Quality of Low-dose Protocol in Chest CT: Comparison of Standard-dose Protocol (흉부 CT촬영에서 저선량 프로토콜의 선량과 화질: 표준선량 프로토콜과 비교)

  • The purpose of this study was to compare radiation dose and image quality between low-dose (LDP) and standard-dose protocol (SDP). LDP (120 kVp, 30 mAs, 2-mm thickness) and SDP (120 kVp, 180 mAs, 1.2-mm thickness) images obtained from 61 subjects were retrospectively evaluated at level of carina bifurcation, using multi-detector CT (Brilliance 16, Philips Medical Systems). Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated at ascending aorta and infraspinatus muscle, from CT number and back-ground noise. Radiation dose from two protocols measured at 5-point using acrylic-phantom, and CT number and noise measured at 4-point using water-phantom. All statistical analysis were performed using SPSS 19.0 program. LDP images showed significantly more noise and a significantly lower SNR and CNR than did SDP images at ascending aorta and infraspinatus muscle. Noise, SNR and CNR were significantly correlated with body mass index (p<0.001). Radiation dose, SNR and CNR from phantom were significant differences between two protocols. LDP showed a significant reduction of radiation dose with a significant change in SNR and CNR compared with SDP. Therefore, exposure dose on LDP in clinical applications needs resetting highly more considering image quality.
