산업위생 분야 종사자들의 사회 안전의식변화에 관한 조사 (Comparison of Risk and Safety Perceptions of Industrial Hygienist)

  • Objectives: This study was conducted to evaluate perceptions of safety and risk among Korean industrial hygienists and the change between before and after the Sewol Ferry Disaster in 2014. Two surveys with questionnaires composed of 51 questions were completed by attendees of the Korea Industrial Hygiene Association(KIHA) conference. Methods: One was conducted at the 2013 KIHA Fall Conference(N=181) and the other was from the 2014 KIHA Summer Conference(N=123). Between these two surveys was the Sewol Ferry Disaster on April 14, 2014, which was believed to seriously affect safety and risk perceptions in Korea. Results: It was revealed that industrial hygienists' awareness of safety rules strengthened after the Sewol Ferry Disaster(p<0.05). It was apparent that people over the age of 30 were more sensitive to social safety. There was no significant difference in the evaluation and attitude regarding governmental safety policy between the years of 2013 and 2014. The credibility of public organizations responsible for the disaster management system decreased. The self-evaluation of respondents' safety level also decreased. This trend shows mainly in the younger generation. It was evaluated that the overall social safety level decreased and the anxiety level increased. The score on social safety on a ±5 Likert scale was 0.68 in the 2013 survey and -0.33 in the 2014 survey(p<0.05). It was reported that the most serious threat factors for accident or disaster were 'building collapse > illegalities and corruption > side effects of radiation therapy >accidents in normal activity > occupational disease,' in order. They picked 'safety ignorance > hurry-up habits and culture > focusing on short-term benefit > easy-going attitude > insufficient safety education' for the causes of low social safety levels in 2013. In 2014, they were 'safety ignorance > easy-going attitude > focusing on short-term benefit > insufficient safety education > hurry-up habits and culture'. Conclusions: This study has some limitations because it was originally not designed to survey attitudes prior to the Sewol Ferry disaster in 2013. In addition, the survey targets are industrial hygienists who are familiar with occupational disease and injury.

한국 임금 근로자의 직장 내 폭력 경험과 근골격계 증상과의 연관성에 관한 연구 (The Relationship between Experience of Workplace Violence and Musculoskeletal Pain among Wage Workers in South Korea)

  • Objectives: We aimed to examine the association between experience of workplace violence and musculoskeletal pain among wage workers in South Korea. Methods: We analyzed a cross-sectional survey of 29,601 wage workers from the third wave Korean Working Conditions Survey in 2011. Experience of workplace violence was assessed through three questions, "Over the past 12 months, have you ever experienced: (1) physical violence, (2) bullying, or (3) sexual harassment at workplace?" Musculoskeletal pain was measured using the three questions, "Over the past 12 months, have you ever experienced: (1) low back pain, or (2) upper limb pain(i.e. shoulder, neck, and arm), or (3) lower limb pain(i.e. hip, leg, knee, and foot)?" Wage workers could answer 'Yes' or 'No' to each of the three questions. Multivariate negative binomial regression was applied to examine the association between workplace violence and musculoskeletal pain after adjusting for confounders including self-reported physical work factors. All analyses were performed using STATA/SE version 13.0. Results: Physical violence was associated with low back pain(PR: 2.16, 95% CI: 1.77, 2.65), upper(PR: 1.65, 95% CI: 1.45, 1.88) and lower limb pain(PR: 1.81, 95% CI: 1.52, 2.15) among male wage workers whereas it was related to upper(PR: 1.86, 95% CI: 1.53, 2.26) and lower limb pain(PR: 2.95, 95% CI: 2.47, 3.53) among female wage workers. Significant association was observed between sexual harassment and upper(PR: 1.25, 95% CI: 1.01, 1.56) and lower limb pain(PR: 2.41, 95% CI: 1.97, 2.93) among female wage workers whereas the association was only significant in the analysis with lower limb pain(PR: 1.87, 95% CI: 1.17, 2.97) among male wage workers. Bullying was associated only with lower limb pains among both male(PR: 1.77, 95% CI: 1.32, 2.37) and female(PR: 2.10, 95% CI: 1.69, 2.61) wage workers. Conclusions: This study found that experience of workplace violence, particularly physical violence and sexual harassment, was associated with musculoskeletal pain among Korean wage workers.

한국노인의 건강상태에 대한 조사연구 (Health Status of Elderly Persons in Korea)

  • This Study was done to design and test an instrument to measure the health status of the elderly including physical, psychologyical and social dimensions. Data collection was done from July 18 to August 17, 1990. Subjects were 412 older persons in Korea. A convenience sample was used but the place of residence was stratified into large, medium and small city and rural areas. Participants located in Sudaemun-Gu, Mapo-Gu, and Kangnam-Gu, Seoul were interviewed by brained nursing students, and those in Chungju, Jonju, Chuncheon, and Jinju by professors of nursing colleges. Rural residents were interviewed by community health practioners working in Kungsang-Buk-Do, Kyngsang- Nam - Bo, Jonla Buk -Do, and Kyung Ki- Do. The tool developed for this study was a structured questionnaire based on previous literature and then tested for reliability and validity. This tool contained 20 physical health status items, 17 mental-emotional health status items and 38 social health status items. Physical health status items clustered in to six factors such as personal hygiene, activity, home management, digestive, sexual, sensory, and climination functions. Mental-emotional health status items clustered into two factors, mental health and emotional health. Social health status items clustered into seven factors, grandparent, parent, spouse, friend, kinships, group member and religious role functions. Data analysis included percentage, average, S.D., t-test and ANOVA. The results of the analysis were as follows : 1. The tool measuring the health status of the elderly and developed for this research had a relatively high reliavility indicated by a cronbach=0.97793. 2. Average score of the subjects physical health status was 4, 054 in a 5 point likert scale, mentalemotional health status was 3.803, social health status was 2.939 and the total average was 3.521. The social status of the subjects was the lowest and the next was mental-emotional health status ; physical health status was the highest. 3. Educational background, perceived health status, the amount of pocket money were related to physical and mental-emotional health status and family structure was related mental-emotional physical and social health status. Occupation was related to physical and mental-emotional status. Area of residence was related to metal-emotional and social status. Source of living in the expeneses was related to physical and mental-emotional health status marital status to mental-emotional and social health status, and the number living in the home physical health status and religion to social health status. The following conciusions were derived from the above results ; 1. The health status of Korean elderly was relatively sound but social health status was the most vulnerable. The Social activity for Korean elderly is needed to improve social health. 2. Educational background, perceived health status and the amount of pocket money must be considered in the health assessment criteria of the elderly, Family structure, marial status, occupation, residence variables and sources of living expense must also be considered as significant. 3. A health education program based on the educational background of the elderly, and provision of an occupational socioeconomic welfare policy will be useful in order to increase social health status of Korean elderly.

건설공사 안전점검대가의 적정성에 대한 정책적 고찰 - 산업안전보건법을 중심으로 - (Policy Study on Appropriateness of Safety Check Costs in Construction Projects - Focusing on Industrial Safety and Health Act -)

  • 안전점검과 관련된 규정 중 '건설기술진흥법'과 '시설물의안전관리에관한특별법'은 건설공사 시 안전에 관한 내용의 기술과 동시에 유지관리 측면에서의 안전점검에 관한 사항을 규정하여 건설산업의 사고예방을 담당하고 있다. 실질적인 근로자의 안전 확보를 담당하고 있는 규정이면서, 고용노동부의 안전점검과 관련된 유일한 규정이라 할 수 있는 '산업안전보건법 제32조 재해예방전문지도기관의 지도기준'과 '건설업산업안전 보건관리비 계상 및 사용기준'에 따라 건설공사 착공시 관련 기관은 계약과 동시에 준공 시까지 안전점검을 받도록 하고 있다. 그러나 발주기관에 따라 관리 정도의 편차가 심하여 소규모 민간 발주 공사현장은 안전점검이 형식적으로 이루어지고 있으며, 세부규정도 없어 안전점검 대가는 지속적으로 하락하고 있는 실정이다. 본 연구에서는 제주지역에서 실질적으로 이루어지고 있는 안전점검의 현황을 조사하여 공사금액별 구분 비교하고 적정한 대가를 각각의 건설 관련 법에 근거하여 산출하고자 하였으며 나아가 세부 규정에 관한 개선방안을 제시하였다.

종합병원에서 조직문화와 조직유효성과의 관계 (The Relationship of Organizational Culture and Organizational Effectiveness at the General Hospital)

  • Objectives: Organizational culture has beer important in field of organizational behavior research for the past decade. Although there has been a growing interest in the organizational culture and organizational effectiveness, there is few research in health care field. This study was carried out to investigate the relationship of organizational culture and organizational effectiveness at general hospital. Methods: Data was collected by self-administrated questionares. Organizational cultures were measured by using Likert scale. A general hospital in Kyunggi-Do was selected and survey was conducted to 675 workers. Data was analyzed with computer package, PC-SPSS. Results: There were four types of organizational culture in this hospital consensual culture, developmental culture, hierarchical culture, rational culture. Many workers recognized their culture as rational culture and developmental culture. This finding showed that the hospital had both human related and task related climate. There were some differences in recognition of sub-organizational culture by occupational group, but perceived organizational culture was in accordance with sub-organizational culture in general. Multiple regression analysis and multiple logistic regression analysis were conducted to find the relationship of organizational culture and organizational effectiveness. As a result, developmental culture showed a strong relationship with organizational commitment and job-satisfaction. Conclusions: These results showed that types of organizational culture were significantly related to organizational effectiveness and understanding the existing culture is essential to develope their organizational effectiveness.

경제활동에 따른 40대 여성의 유방암 발생 위험도 (The Risk of Breast Cancer in Women in Their 40s by Economic Activity)

  • In South Korea, female individuals in their forties show a high rate of incidence, with approximately 13% of the patients being <40 years. This statistic is more than twice as high as that in Western countries. It is therefore necessary to identify the risk factors for breast cancer incidence by age and economic activity participation status. Women aged 30 to 59-whether breast cancer patients or those in the control group and having no breast cancer-were appraised from the sample cohort database. The data were analyzed using the statistical software R36.2. To identify the factors affecting breast cancer incidence, the degree of association was determined with HR and 95% CI by means of cox regression analysis. As for the socio-demographic variables, the older the individual, the higher the risk of breast cancer incidence becomes. As for the economic activity variables, those who were dependents (unemployed) and who had higher income (medium and high) were at higher risk of breast cancer incidence, which was statistically significant. The income-adjusted HR (model 1) for breast cancer development associated with the economic activity was 1.452 (95% CI, 1.19-1.77). The body mass index and alcohol intake-adjusted HR (model 2) was 1.431 (95% CI, 1.18-1.74). One needs to pay attention to policy plans regarding women's quality of life, as well as to the risk of breast cancer incidence by their economic activity. In other words, policies need to give post care, instead of focus on early detection and cancer treatment.

인구사회적 요인, 암, 일부 전신질환 등이 자살에 미치는 영향: 성별, 연령별 분석 (Effect of Sociodemographic Factors, Cancer, Psychiatric Disorder on Suicide: Gender and Age-specific Patterns)

  • Objectives : We examined the effect of sociodemographic factors, cancer, and psychiatric disorders on suicide by gender and age-specific patterns in South Korea. Methods : The study is a case-control study. Claim data was obtained from the national health insurance database and national death registration database. The number of people who committed suicide was 11,523, which was matched with a control group consisting of ten times as many people at 115,230 selected from the national health insurance and medical aids beneficiaries. The medical utilization of the case group was one year before death and that of the control group was from July 1,2003 to June 30, 2004. Four variables-address, economic status, presence of a psychiatric disease, and cancer-were used in multiple logistic regression analyses. Results : Living in cities or in rural areas showed a greater risk for suicide than living in a metropolitan city. Low economic status, the presence of a psychiatric disorder, and cancer were also statistically meaningful risk factors for suicide. The three major psychiatric diseases, schizophrenia, alcohol abuse, and bipolar disorder, were meaningful in all age groups, but the scale of the odds ratio differed by the age group. Only the psychiatric disorder variable was meaningful in the adolescent group, whereas a psychiatric disorder and economic status were meaningful for the young adult group, and all variables were meaningful for the middle-aged group. A psychiatric disorder and cancer were meaningful in the elderly group, economic status was meaningful for male subjects, and address was meaningful for female subjects. Conclusions : Factors such as living in city or rural areas, low economic status, the presence of a psychiatric disorder, and cancer were statistically meaningful risk factors in suicide. These factors also differed by age group. Therefore, policymakers should establish policies for suicide prevention that are relevant for each age group.

전공의들의 장시간 근무와 우울증상의 연관성에 관한 연구 (Association between Long Working Hours and Depressive Symptoms among Interns and Residents in South Korea-2014 Korea Interns & Residents Survey)

  • Objectives: This study sought to examine the association between long working hours and depressive symptoms among interns and residents in South Korea. Methods: We analyzed a cross-sectional survey of 1,661 interns and residents from the 2014 Korean Interns & Residents Survey. Total working hours during the preceding week was assessed and classified into five categories(i.e. less than 60 hours, 60-79 hours, 80-99 hours, 100-119 hours, and 120-168 hours). Depressive symptoms during the previous week were measured by using ten items from the Center for Epidemiologic Studies Depression Scale questionnaire. Multivariate negative binomial regression was applied to examine the association between long working hours and depressive symptoms after adjusting for potential confounders, including medical specialty and training year. All analyses were performed using STATA/SE version 13.0. Results: 86%(N=1,429) of interns and residents worked 60 hours or more per week. Compared to the "less than 60 hours" group, long working hours for interns and residents were significantly associated with depressive symptoms: "60-79 hours"(PR: 1.56, 95% CI: 1.05, 2.32), "80-99 hours"(PR: 2.27, 95% CI: 1.54, 3.33), "100-119 hours"(PR: 2.62, 95% CI: 1.76, 3.89), and "120-168 hours"(PR: 3.28, 95% CI: 2.21, 4.86). Conclusions: This study found that long working hours were prevalent among interns and residents in South Korea, and it was associated with depressive symptoms.

A Study of Health and Safety Management Practices of Contractors in Southern Vietnam

  • In spite of the socio-economic benefits of the construction industry, it accounts for many deaths, injuries and illnesses in many countries. In developing countries like Vietnam, the situation is even worse. Health and safety (H&S) management plays a crucial role in the efforts to improve H&S performance as it contributes to mitigating the risks of occupational injuries and illnesses. However, within the wider H&S literature in Vietnam, there is no insight into the current state of affairs of H&S management, particularly in terms of the management practices that need attention in order to improve H&S management across the construction sector. This study therefore presents the findings of an inquiry into the H&S management practices of contractors in Vietnam. The study employed a questionnaire survey which yielded 58 responses from contractors, particularly those operating in Southern Vietnam. Some commonly implemented H&S management practices by the contractors are: provision of personal protective equipment (94%), allocation of H&S supervisor on site (90%), and displaying of company H&S policy on construction sites, company website, and head/branch offices (86%). Some of the less implemented practices are: networking/engaging with other companies/institutions (e.g. government offices) about H&S issues (39%), rewarding workers for safe work behaviour (39%), reviewing and updating H&S plans (48%), and displaying regulatory H&S posters on site (45%). The findings have provided some insight into the current state of affairs of the H&S management practices of contractors in Southern Vietnam and could thus inform efforts by industry stakeholders to improve the aspects of H&S management that are lagging.

Incidence and severity of medication-related osteonecrosis of the jaw in patients with osteoporosis using data from a Korean nationwide sample cohort in 2002 to 2019: a retrospective study

  • Background: Medication-related osteonecrosis of the jaw (MRONJ) is a significant concern, particularly among patients taking bisphosphonates (BPs), denosumab, and selective estrogen receptor modulators (SERMs) for osteoporosis. Despite the known risks, large-scale cohort studies examining the incidence and severity of MRONJ are lacking. We aimed to ascertain the incidence and risk of MRONJ among these patients, whom we stratified by age groups, medication types, and duration of use. Methods: We utilized data from the National Health Insurance Service's sample cohort database, focusing on patients aged 40 years and above diagnosed with osteoporosis. The patients were divided into three groups: those prescribed BPs only, those prescribed SERMs only, and those prescribed both. Results: The overall incidence rate of MRONJ was 0.17%. A significantly higher incidence rate was observed among those taking osteoporosis medications, particularly among females with a relative risk of 4.99 (95% confidence interval, 3.21-7.74). The SERM group also had an incidence rate comparable to that of the BP group. Severity was assessed based on the invasiveness of the treatment methods, with 71.3% undergoing invasive treatment in the medication group. Conclusion: This study provides valuable insights into the incidence and severity of MRONJ among a large cohort of patients with osteoporosis. It underscores the need for comprehensive guidance on MRONJ risks across different medication groups and sets the stage for future research focusing on specific populations and treatment outcomes.
