Non-Mandatory Influenza Vaccination Rates among Healthcare Workers during the 2017-2018 Influenza Season: a Multicenter Study in Korea

  • Purpose: Annual influenza vaccination is the best strategy to prevent healthcare-associated influenza transmission. Influenza vaccination rates among healthcare workers (HCWs) vary by country, region, and year. We investigated the influenza vaccination rates for HCWs during the 2017-2018 influenza season in South Korea, where a non-mandatory vaccination campaign was conducted. Methods: We retrospectively investigated factors affecting the influenza vaccination rate among HCWs during the 2017-2018 influenza season in three tertiary hospitals in Goyang City, where the non-mandatory influenza vaccination program is conducted. Results: Consequently, 6,994 of 7,180 HCWs (97%) were included, and the overall vaccination rate was 85%. Nurses had the highest rate with 92%, followed by health technicians (88%), physicians (84%), and non-medical HCWs (79%, P<0.001). Vaccination rates differed, depending on the frequency of contact with patients in the non-medical HCWs (frequent contact vs. less-frequent contact; 90% vs. 73%, P<0.001). Conclusions: The influenza vaccination rate among HCWs during the 2017-2018 influenza season in Korea was 85%, which is among the highest rates compared with previously reported non-mandatory vaccination rates in other countries. The vaccination rate may vary depending on the HCW's occupational characteristics, including the extent of contact with the patient. Therefore, a multifaceted strategy is needed to increase the vaccination rate of HCWs.

A Study on the Periodic Medical Examination, and Health Care Management Programs in Industries -Kwang-Ju city and Chonnam Province- (산업장의 특성별 건강검진과 보건관리 -광주.전남지역-)

  • The purpose of this study was to explore the condition of periodic medical examination, and health care services in industries. This will offer some basic data in developing industrial nursing care requirements. To achieve this goal a self-administered questionnaire (developed by the academic affairs of community health nursing) was provided to the nurses working in 40 industries from Dec 20, 1992 to Jan 20, 1993. The statistical computer package SAS, along with t-test, and ANOVA was used to manipulate the date. The results were as follows : 1. General Characteristics: The greater part of the industries studied were manufacturing company, with over 500 employees. The shift system was used with most companies using one or three shifts, and 75.0% of them were organized with Industrial Safety and Health Committees. 2. Periodic Medical Examination : Most of the workers were receiving periodic medical examination from a designated hospital (96.7%). Of those 15.8% had a close medical examination. In the medical examination 9.4% were evaluated at 'C' and 3.8 were evaluated at 'D'. About 55.0% of those workers received the result of the periodic medical examination individually(95.0%). 3. Special Medical Examination : The rate of those who were receiving special medical examination was 98.4% and about 11.7% were evaluated at 'C' and 3.9% were evaluated at 'D'. Those who had any health problems (54.2%) were receiving follow-up care, 52.4% of them had medical treatment while working. The health care managers in the company consulted 97.1% of them who had any suspicious signs and symptoms of occupational diseases. 4. Health Care Services : The average score of health care services was 13.8 out of a possible 28 point. The score of health education and health assessment, was lower than medical diagnosis and environmental hygiene. There were no significant differences in health care services according to the size of the industries(F=.95, P=.429). The score of health care services was higher in the worker who had better health and showed significant differences (F=4.50, p=.025).

Analysis of the Work-related Accidents and Diseases in Gunsan, Buan, and Gochang Areas from 2002~2011 (군산시, 부안군 및 고창군의 2002~2011년의 산업재해 분석)

  • Objectives: This study was conducted to assess the trend of work-related accident rate in the past 10 years (2002~2011) in Gusan district, Ministry of Employment and Labor, Korea. Methods: To analyze the change of work-related accident rate in the past 10 years in Gunsan District, we analyzed the work-related accident and disease by the type of business, business scale and analyzing the victim's age and job period using workplace information management system(PKMS) in the Ministry of Employment and Labor. Results: The average rate of work-related accidents in Gunsan district was 0.96% in the past 10 years and it was a little higher than that of national average 0.75%. The rates of work-related accidents tended to decrease from 1.11% in 2002, to 1.09% in 2006, and to 0.79% in 2011. Mortality (per 10,000) rates also tended to decrease in the years 2002, 2006, and 2011 as 3.07, 2.46, and 1.53. By the type of business, the average rate of work-related accidents in the past 10 years was the highest in agriculture as 1.77%, followed by forestry farming at 1.76%. By the business scale, the average rate of work-related accidents was the highest in the group of under 5 employees as 2.05% and it was the lowest in the group of more than 300 employees as 0.50%. The ratio of work-related accidents in the 30's age group was decreased from 41.7% in 2002 to 25.2% in 2011 and the ratio for the 50's age group was increased from 28.6% in 2002 to 49.5% in 2011. Conclusions: Although the rate of work-related accident was decreased in the past 10 years, the polarization of work-related accidents rated by the business scale and age groups show a significant difference. So it is needed that the planning and implementing of work-related accidents prevention policies.

  • 1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. Findings on Chest Low-Dose CT Images of Group Exposed to Inorganic Dusts (분진에 노출되었던 집단의 흉부 저선량 CT영상 소견)

  • The purpose of this study was to compare the findings on the chest low-dose CT (LDCT) images between the negative and positive groups for pneumoconiosis in the group exposed to inorganic dust. From May 30, 2007 to August 31, 2008, total 328 subjects were examined by a LDCT. LDCT images were read by a chest radiologist who has much experience for reading of pneumoconiosis. All subjects were classified into two groups based on digital images after consensus reading of two radiologists according to the ILO 2000 guidelines; negative group (87, 26.5%) without pneumoconiosis and positive group (241, 73.5%). Statistical analysis was performed using a SPSS 14.0. There were significant differences in age (60.9 vs. 65.0, p<0.001), and in dust expose duration (17.0 vs. 19.2, p=0.024) between two groups, but no significant difference in smoking (p=0.784). Of the 328 subjects, 13 diagnosis were extracted from 245 subjects (74.7%). Coronary artery calcification (CAC) was significantly higher in positive group than that in negative group (36.9% vs. 25.3%, p=0.049). Honeycombing showed higher frequency in positive group than in negative group (6.2% vs. 1.2%, p=0.079). Pneumoconiosis findings caused by inorganic dusts exposure showed the significant relation with CAC on LDCT images. Future studies need to prove that pneumoconiosis finding is independent risk factor for CAC using a coronary artery angiography.

Bactericidal Efficacy of Vital-Oxide®, Disinfectant Solution Against Salmonella Typhimurium and Brucella Ovis

  • $Salmonella$ spp. and $Brucella$ spp. have caused a considerable disease of farmed animals and economic loss in animal farming and food industry. In this study, the disinfection efficacy of Vital-$Oxidel^{(R)}$, a commercial disinfectant, composed to chlorine dioxide, betaine hydrochloride, and propylene glycol was evaluated against $S.$ $typhimurium$ and $Brucella$ $ovis$. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to test bacteria for 30 min at $4^{\circ}C$. Vital-$Oixdel^{(R)}$ and test bacteria were diluted with distilled water (DW), hard water (HW) or organic matter suspension (OM) according to treatment condition. On OM condition, the bactericidal activity of Vital-$Oixdel^{(R)}$ against S. typhimurium and Brucella ovis was lowered compared to that on HW condition. As Vital-$Oxidel^{(R)}$ possesses bactericidal efficacy against animal pathogenic bacteria such as $S.$ $typhimurium$ and $Brucella$ $ovis$, this disinfectant solution can be used to control the spread of bacterial diseases.

Antiviral Efficacy of Citra-kill®, Disinfectant Solution Against Avian Influenza Virus

  • Highly pathogenic avian influenza virus (HPAIV) is already panzootic in poultry and caused a considerable economic loss in poultry industry. In addition, HPAIV continues to cross species barriers to infect humans and other mammals, often with fatal outcomes. In this study, the virucidal efficacy of Citra-$Kill^{(R)}$ composed to quaternary ammonium chloride and citric acid was investigated against avian influenza H9N2 virus (AIV). A virucidal efficacy was determined with the viability of AIV contacted with the disinfectant in the allantoic membrane of chicken embryos. Citra-$Kill^{(R)}$ and AIV was reacted on the distilled water (DW), hard water (HW) or organic matter suspension (OM) condition. On DW condition, AIV was inactivated with 2,000 fold dilutions of Citra-$Kill^{(R)}$. When the antiviral effect on HW condition was evaluated, the antiviral activity of the disinfectant showed on 1,500 fold dilutions against AIV. With the investigation of the antiviral effect of the disinfectant on OM condition, AIV was inactivated on 500 fold dilutions of Citra-$Kill^{(R)}$. As Citra-$Kill^{(R)}$ possesses virucidal efficacy against AIV, the disinfectant solution can be used to limit the spread of animal viral diseases.

Differentiation of Dopaminergic Neurons from Mesenchymal-Like Stem Cells Derived from Human Umbilical Cord Vein

  • One of the most extensively studied populations of multipotent adult stem cells are mesenchymal stem cells (MSCs). MSCs derived from the human umbilical cord vein (HUC-MSCs) are morphologically and immunophenotypically similar to MSCs isolated from bone marrow. HUC-MSCs are multipotent stem cells, differ from hematopoietic stem cells and can be differentiated into neural cells. Since neural tissue has limited intrinsic capacity of repair after injury, the identification of alternate sources of neural stem cells has broad clinical potential. We isolated mesenchymal-like stem cells from the human umbilical cord vein, and studied transdifferentiation-promoting conditions in neural cells. Dopaminergic neuronal differentiation of HUC-MSCs was also studied. Neural differentiation was induced by adding bFGF, EGF, dimethyl sulfoxide (DMSO) and butylated hydroxyanisole (BHA) in N2 medium and N2 supplement. The immunoreactive cells for $\beta$-tubulin III, a neuron-specific marker, GFAP, an astrocyte marker, or Gal-C, an oligodendrocyte marker, were found. HUC-MSCs treated with bFGF, SHH and FGF8 were differentiated into dopaminergic neurons that were immunopositive for tyrosine hydroxylase (TH) antibody. HUC-MSCs treated with DMSO and BHA rapidly showed the morphology of multipolar neurons. Both immunocytochemistry and RT-PCR analysis indicated that the expression of a number of neural markers including NeuroD1, $\beta$-tubulin III, GFAP and nestin was markedly elevated during this acute differentiation. While the stem cell markers such as SCF, C-kit, and Stat-3 were not expressed after neural differentiation, we confirmed the differentiation of dopaminergic neurons by TH/$\beta$-tubulin III positive cells. In conclusion, HUC-MSCs can be differentiated into dopaminergic neurons and these findings suggest that HUC-MSCs are alternative cell source of therapeutic treatment for neurodegenerative diseases.

Radiation Dose and Image Quality of Low-dose Protocol in Chest CT: Comparison of Standard-dose Protocol (흉부 CT촬영에서 저선량 프로토콜의 선량과 화질: 표준선량 프로토콜과 비교)

  • The purpose of this study was to compare radiation dose and image quality between low-dose (LDP) and standard-dose protocol (SDP). LDP (120 kVp, 30 mAs, 2-mm thickness) and SDP (120 kVp, 180 mAs, 1.2-mm thickness) images obtained from 61 subjects were retrospectively evaluated at level of carina bifurcation, using multi-detector CT (Brilliance 16, Philips Medical Systems). Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated at ascending aorta and infraspinatus muscle, from CT number and back-ground noise. Radiation dose from two protocols measured at 5-point using acrylic-phantom, and CT number and noise measured at 4-point using water-phantom. All statistical analysis were performed using SPSS 19.0 program. LDP images showed significantly more noise and a significantly lower SNR and CNR than did SDP images at ascending aorta and infraspinatus muscle. Noise, SNR and CNR were significantly correlated with body mass index (p<0.001). Radiation dose, SNR and CNR from phantom were significant differences between two protocols. LDP showed a significant reduction of radiation dose with a significant change in SNR and CNR compared with SDP. Therefore, exposure dose on LDP in clinical applications needs resetting highly more considering image quality.

Prevalence of allergic and respiratory conditions among residents living near large construction sites in Sejong city: an exploratory study with mixed method approach (세종시 건설현장 인근 주민의 알레르기 및 호흡기 질환 유병 실태: 혼합연구방법론을 적용한 탐색적 연구)

  • Objectives: To clarify the association between dust from large construction sites and allergic and respiratory conditions among residents in Sejong city. Methods: We included 379 individuals older than 50 years from three areas of Sejong city; two areas near construction sites and one area for a comparison. Prevalence of self-reported allergic and respiratory conditions was compared across the three areas using logistic regression to adjust for covariates including age, gender, smoking, and educational attainment. A total of 16 individuals were interviewed to take details of the residents' experiences of construction work. Results: Those living in the nearby areas around construction sites were generally found to have the increased prevalence for all allergic and respiratory conditions. Odds Ratio(OR)s for cough and sneezing among the six symptoms and allergic rhinitis among the three diagnosed diseases reached statistical significance but only OR for cough remained significant after adjustment: adjusted OR for cough was 2.63 (95% CI, 1.07-6.46). Two relevant themes emerged from semi-structured interview were; 1) most residents from the affected area reported a considerable level of exposure to construction dust, 2) about half of them noted that construction dust influences allergic- and respiratory health. Conclusions: General pattern provides suggestive evidence that living close to construction sites may be positively associated with elevated risks of allergic and respiratory conditions. Efforts should be made to establish preventive measures and a comprehensive in-depth health impact assessment on this issue.
