Isotretinoin exposure in pregnant women in Korea

  • Objective Isotretinoin is a notorious teratogen otherwise used for the treatment of acne vulgaris. Some countries, including those in North America and the European Union, implemented the pregnancy prevention program (PPP); however, no PPP has yet been established in South Korea. So the aim of this study was to evaluate the rate of pregnant women exposed to isotretinoin among the callers of the Korean Mother Safe Counseling Center. Methods This is a prospective cohort study. We evaluated the demographic characteristics, obstetric history, and isotretinoin exposure of pregnant women based on the mother safe registry from April 2010 to July 2016. Results Among 22,374 callers, 650 (2.9%) pregnant women were exposed to isotretinoin. The mean age was $29.0{\pm}4.4$ years in the isotretinoin-exposed group and $32.0{\pm}4.2$ years in the unexposed group (P<0.001). Moreover, the incidence of pregnancies within 30 days after isotretinoin discontinuation or during isotretinoin intake was 78.9% (513/650). The median duration of isotretinoin exposure was 18 (1-4,231) days. Furthermore, from 2011 to 2015, the incidence of isotretinoin exposure was $2.9{\pm}1.2$ pregnancies per 10,000 births in South Korea. Conclusion Approximately 80% of pregnant women are exposed to isotretinoin within the recommended 30 days of contraception or during pregnancy. Therefore, the PPP has to be established in South Korea.

Split hand/foot malformation with long-bone deficiency and BHLHA9 duplication: A prenatal diagnosis report

  • Distal limb deformities are congenital malformations with phenotypic variability and high genetic heterogeneity. Split hand/foot malformation, also known as ectrodactyly, is a congenital limb malformation characterized by a defect of the central rays of the hands and/or feet. Split hand/foot malformation with long-bone deficiency (SHFLD) is a rare condition related to a 17p13.3 duplication. Recently, genomic duplications encompassing BHLHA9 have been associated with SHFLD. We report a case of SHFLD presenting with campomelia of the right femur, bilateral agenesis of fibulae, bilateral club feet, and oligosyndactyly of the hands and feet, that was associated with a 17p13.3 duplication, as determined prenatally using array comparative genomic hybridization.

한방부인과 영역에 활용되는 수기요법의 국내외 연구 동향 (Research Trends Using Manual Therapy in Obstetrics and Gynecological Diseases)

  • Objectives: The purpose of this study is to analyze the research trends using manual therapy in obstetrics and gynecological diseases. Methods: We searched for studies that performed manual therapy on diseases related to obstetrics and gynecology. All relevant papers were selected and extracted to be analyzed. Results: 12 Randomized controlled trials, 8 cases, 3 pilot studies, 3 non-comparative studies, and 1 before-after comparative study were collected. The study subjects, results, techniques of manual therapy used were analyzed. There were studies related to pregnancy-related symptoms, postpartum pain, dysmenorrhea, chronic pelvic pain, abnormal uterine bleeding, sexual dysfunction, and childbirth. Conclusions: It has been found that various manual therapies can be used for obstetrics and gynecological diseases.

CARE(CAse REport) 지침에 따른 대한한방부인과학회지의 증례보고에 대한 질 평가 (Evaluation of the Quality of the Case Reports from the Journal of Obstetrics and Gynecology of Korean Medicine Based on the CARE Guidelines)

  • Objectives: The purpose of this study is to assess the quality of case reports from the Journal of Obstetrics and Gynecology of Korean Medicine. Methods: Case reports were selected from the Obstetrics and Gynecology of Korean Medicine from January 2015 to March 2019, by utilizing Oriental Medicine Advanced Searching Integrated System (OASIS). The quality of the reports were reviewed based on the Consensus-based Clinical Case Reporting Guideline Development (CARE) guideline. Results: Total of 41 case reports were finally selected for the assessment. 69.23% of the case reports included necessary information based on the CARE guideline but the rest of the reports did not. More than 50% of the reports were missing data regarding 'Diagnostic challenges', 'Intervention adherence and tolerability', 'Adverse and unanticipated events', or 'Patient perspective or experience', and 'Informed consent'. Also, the reports did not include 'Key word', 'timeline'. Conclusions: Case reports from the Journal of Obstetrics and Gynecology of Korean Medicine have important role in women. Efforts are needed to improve the quality of the case reports as well as to develop reporting guidelines for the Journal of Obstetrics and Gynecology of Korean Medicine.
