Validity, Reliability and Efficiency of Pain Self-report Scale in Elderly with Dementia (치매노인의 통증사정에 대한 자가보고 도구의 타당도, 신뢰도 및 효율성 검증)

  • Purpose: The aim on this study was to establish the validity, reliability and efficiency of a Pain Self-Report Scale for elderly with dementia and compare these results with an observational pain rating scale. Methods: Study subjects were 136 elderly with dementia who were residents in a nursing home, geriatric hospital, or day care center. The subject's pain was measured by five self-report scales and observational scale. DS-DAT (discomfort scale-dementia of the Alzheimer's type) was used for pain behavior observational measure. Cognitive state was assessed using the MMSE (Mini-Mental State Examination). Results: Observational rating correlated moderately with self-report (r=.225~.585, p<.05) and tended to underestimate pain intensity. Test-retest reliability was high for all five self-report scales, and the correlation between these scales was very strong (r=.735~.856, p<.05). Comprehension rate of VDS (verbal descriptor scale) was 88.3%, and NRS (numeric rating scale) 69.9%, FPS (face pain scale) 66.9%, HVAS (horizontal visual analog scale) and VVAS (vertical visual analog scale) 65.4%. Conclusion: Nurses should not apply observational scales routinely in demented patients as many of these are capable of reporting their own pain. Self-report, the highest standard of pain measurement can be reliably performed in a large proportion of demented elderly.

A Review of Postural Classification Schemes for Evaluating Postural Load - Focused on the Observational Methods (작업 자세 부하 평가를 위한 자세 분류 체계의 연구 현황 - 관측법을 중심으로)

  • This study aims to review and assess the existing postural classification schemes used for evaluating postural loads in industry. The schemes can be classified into three categories: self-report, observational and instrument-based techniques depending upon how to record working postures. Of the three techniques, this study was mainly focused on the observational methods. The observational technique is most widely used in the industrial sites because it does not interfere with work, and is easy and simple to use and cost-effective without requiring the use of expensive equipment for estimating the angular deviation of a body segment from the neutral position. In spite of the usefulness and applicability, the techniques have some problems: 1) The existing observational techniques lack the consistency in the class limits of the motion categories in each body segment; 2) Most of them do not provide the post-analysis criteria needed to judge whether or not any posture is acceptable in view point of the postural load; and 3) They can not precisely evaluate the postural load for a given posture because the external loads and dynamic factors including acceleration, moment and force were not taken into consideration.

  • An appropriate research design for hypotheses and purposes leads to a good quality of research results. In this review article, we summarized the types of research methods and described the characteristics of clinical trials. Research designs are categorized into observational studies and experimental ones, depending on data collecting methods. In an observational study, there are cross-sectional, cohort and case-control studies. Parallel groups design and crossover trial studies are representative designs in a randomized controlled trial study, a kind of experimental study. Clinical researchers should understand the characteristics of clinical research designs including advantages and disadvantages and choose the suitable design according to their study purposes and the nature of collected data or subjects.

Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia (동작관찰 과제지향훈련이 만성 뇌졸중환자의 균형 및 보행능력에 미치는 효과)

  • Purpose : This study was conducted to evaluate the effects of an action-observational task oriented training on balance and gait ability of patients with chronic stroke. Method : The subjects of this study were 30 patients with chronic stroke who agreed to participate and were picked up. Participants were randomly divided into equal groups; namely, an experimental group that underwent action-observational task oriented training for at least 30 minutes/day for 6 weeks and a control group that underwent general task-oriented training. Patients' balance was assessed using the Sway Length(SL), Sway Area (SA) and Limit of Stability (LOS) test. In addition, gait ability was assessed using the 10 Meter Walking Test (10MWT). Gait time and speed taken to walk 10 metres were used to examine gait ability. Result : There were significant improvements in the subscales of the balance and gait ability test of those who participated in the action-observational task oriented training, while the control group showed no significant changes. Conclusion : Therefore, Action-observational training effectively improved the balance and gait abiltiy in patients with chronic stroke.

Body Shape Measurement Using Stereo Photo (입체사진을 위용한 인체형상계측)

  • The purpose of this study was to isolate the observational error included in plane photogrammetric data and suggest more accurate and suitable method for body shape measurement. Three experiments were carried out in this study. First of all, the variables which can be obtained from photogrammetric method were selected among the data which can be measured improperly by direct measurement or showing large deviations between the samples. Secondly, the height, the width and the depth of 50 subjects were measured by plane photogrammetry. The result showed that plane photogrammetric data contain significant observational error even for the same variables, as the angle of which photos had been taken changes. Therefore, in order to reduce the observational error and to measure the human body accurately, three-dimensional measurement, stereo photogrammetry was employed in the last experiment. As it is important to isolateonly the observational error by plane photogrammetry and to exclude the accidental error caused by movement of human body subject, body shape model(manikin) was used as subject. The result showed that the average observational error by plane photogrammetry was more than 4 cm in the height, 0.85-1.29cm in the width and 0.49cm in the depth. In conclusion, it is not adequate to use the height obtained from plane photogrammetric data as human body measurement data. And the width and the depth should be used cautiously, even though they are relatively less significant, the error still can make some difference on clothing construction.

  • The purpose of this study is to evaluate the effects of Action-observational training program on gait and balance of patients with hemiplegia. Subjects of this study, among the patients who received the diagnosis cerebral vascular accident, for patients total of 4 people have agreed to research. Experimental group 2 people, control group 2 people, was a total of 4 people. Group-specific arbitration method, was applied to Action-observational training program(experimental group) and general Task-oriented training program(control group). Each training courses 30 minutes for 4 weeks, examined the changes in 10 Meter Walk Test(10MWT), gait speed, Berg Balance Scale(BBS) and time up and go(TUG) ability to examine a total of 4-week course effectively. The intervention were compared by measuring before and after. There were significant improvements in the subscales of the gait and balance test of those who practiced with the Action-observational training program, while the control group showed no significant changes. Therefore, Action-observational training program is effective in improvement of to improve the gait and balance in patients with hemiplegia.

A Sensitivity Study of WRF Model Simulations to Nudging Methods for A Yeongdong Heavy Snowfall Event (영동 대설 사례를 대상으로 한 WRF Simulation의 Nudging 방법에 따른 민감도 연구)

  • To investigate the influences of the observational nudging and the analysis nudging on the WRF simulation for the heavy snowfall event in Yeongdong area on 26 February 2012, the sensitivity experiments in relation to nudging effects were conducted. We initially set the magnitude of nudging coefficient of $6.0{\times}10^{-4}s^{-1}$ to apply to the analysis nudging experiments and observational experiments. To select the optimized options for the observational nudging, the radius influence experiment was carried out with radii ranging from 10 to 25 km at 5 km intervals. Among the observational nudging experiments, the experiment, which was conducted with the option of the radius influence of 15 km and that of the nudging coefficient of $6.0{\times}10^{-4}s^{-1}$ (ONG exp.), showed a best result. As giving the nudging effect only directly on D1 and D2 brought about a better result for the analysis nudging, we set the analysis nudging experiment as above (ANG exp.). We compared and analyzed the results from the control experiment, ONG experiment, and ANG experiment to reveal nudging effects. It was found that the control experiment brought about a result that it overestimated its precipitation in comparison with the observation and failed to properly simulate the time zone of rainfall concentration. When either of the two nudging (observational and analysis nudging) was applied to the data assimilation, it brought about a better result than the control experiment. Especially the observational nudging led to a meaningful result for the wind field, while the analysis nudging had the best result for the precipitation distribution among the experiments.

  • For development of teaching model for observational experiment and the major contents of science education guide books published for secondary school teacher in America, the actual survey to the secondary schools and the investigation of science education guide were carried out. The results were summarized as follows. 1. The major concepts of contents in science education books are constituted of introduction(9.67%), ability(8.94%), activity(6.61%), curriculum(11.61%), instruction planning(5.25%), instruction strategy(l7.74%), materials and facilities(1O.97%), evaluation(8.62%), society and community(l0.55%), guidelines(4.76%), practice(3.54%), and index(1.74%). 2. The constitution of multimedia teaching model for observational instruction was developed and arranged in order of six steps as observation, materialization, broad understanding, advanced study, application, and synthesis. This model was thought as the most effective for the achievement of the instructive goal for observational unit and others.

Construction of Observational Locations for Measuring Water Quality in the River Area (하천유역 수질 관측망 구성 연구)

  • The methods for constructing network of observational locations for measuring water quality in water reservoirs have been widely proposed, but they had some limitations to be applied to river areas, which lie in awkward clustering and finding representative observational locations among locations within each clustering. In this paper, a statistical approach to detect anomaly locations which were significantly different in important measurements for the water quality from the previous locations and construct observational network with them was proposed. Anomaly was detected with the sampling distribution of each primary principal component score, sum of primary PCs, or sum of residual PCs. The empirical study with the data of Nakdong Dam for guiding how to use our proposed approach and showing limitations of previous studied was described.

A case study of competing risk analysis in the presence of missing data

  • Observational data with missing or incomplete data are common in biomedical research. Multiple imputation is an effective approach to handle missing data with the ability to decrease bias while increasing statistical power and efficiency. In recent years propensity score (PS) matching has been increasingly used in observational studies to estimate treatment effect as it can reduce confounding due to measured baseline covariates. In this paper, we describe in detail approaches to competing risk analysis in the setting of incomplete observational data when using PS matching. First, we used multiple imputation to impute several missing variables simultaneously, then conducted propensity-score matching to match statin-exposed patients with those unexposed. Afterwards, we assessed the effect of statin exposure on the risk of heart failure-related hospitalizations or emergency visits by estimating both relative and absolute effects. Collectively, we provided a general methodological framework to assess treatment effect in incomplete observational data. In addition, we presented a practical approach to produce overall cumulative incidence function (CIF) based on estimates from multiple imputed and PS-matched samples.
