  • Objectives : It is known that abdominal obesity increases the possibility of cardiovascular diseases, early death, and metabolic syndrome including dyslipidemia and insulin resistance. Clinical trials have been under way to verify the effectiveness of treatment for abdominal obesity. The objective of this study is to set criteria about how effective oriental medicine and acupuncture are in abdominal obesity. Methods : I manually search "www.clinicaltrial.gov", 4 Korean medical databases and 4 Korean medical journals of abdominal obesity. Search terms used were "abdominal obesity" or "visceral obesity". In order to see detail review, searching was performed from 01, 01, 2005 to 11, 31, 2009. And I classified all the searched studies into design, intervention, purpose, end point, diseases, condition and etc. Results : 1. I could search total 67 trials in "www.clinicaltrial.gov". I found 9, 10, 13, 8, 6 clinical trial from 2005 to 2009 every year. 2. Test on both gender and adult or adult and senior have been most frequent. 3. Randomization clinical trial is 51 cases that occupied 96.2%. 38 cases use control group. 4. Body mass index(BMI) and waist circunference(WC) are major criteria of abdominal obesity clincal trial. WC, BMI, abdominal fat mass, body weight are measured at end point of abdominal obesity clincal trial. 5. I could search total 86 cases in domestic study. I found 15, 23, 23, 20, 5 cases from 2005 to 2009 every year. 6. Test on female and adult have been most frequent in domestic study. 7. Randomization clincal trial is only 2 cases that occupied 2.3%. 62 cases use control group in domestic study. 8. BMI, body fat percent, WC, visceral-subcutaneous fat ratio(VSR), waist-hip ratio(WHR) are criteria of abdominal obesity clincal trial in domestic study. WC, BMI, abdominal fat mass, body weight are measured at end point of abdominal obesity clincal trial in domestic study. • Objectives: The purpose of this study was to investigate the effects of gender, age, body weight, muscle mass, fat mass, body mass index (BMI), metabolism, and compliance with medication on weight loss in obese adults. Methods: We reviewed the medical records of 178 patients who were visited to the Korean Oriental Clinic for 3~6 month and had obesity treatment using Gamitaeumjowee-tang from April 2017 to May 2017. We conducted a paired T-test, correlation coefficient and decision tree to analyze factors influencing obesity treatment. Results: The results of correlation analysis showed that initial weight (kg), initial fat mass (kg), BMI ($kg/m^2$), compliance with medication (%), Original Harris-Benedict Equation, Revised Harris-Benedict Equation and The Mifflin St Jeor Equation was significantly correlated to weight loss (kg) (P<0.001). As a result of constructing the decision tree model, it showed that over 5% weight loss of their initial weight (n=154) was related with initial BMI ($kg/m^2$), compliance with medication (%) and initial muscle mass (kg). In case of over 5 kg weight loss of their initial weight (n=131), it was related with initial BMI ($kg/m^2$), compliance with medication (%) and final BMI ($kg/m^2$). Conclusions: This study suggests that weight loss may be affected by initial factors and that initial factors can be used for obesity treatment.

  • Purpose: Sodium is essentially required for homeostasis and physiological functions, but excessive sodium consumption increases the risk of obesity and other chronic disorders. Korean studies on the sodium-obesity relationship are limited, and thus, this study was undertaken to determine the nature of the relationship between sodium intake and obesity in Korean adults. Methods: Forty-two participants were divided into 2 groups according to body mass index (BMI, non-obese BMI < 25 kg/m2, obese BMI ≥ 25 kg/m2). Dietary intakes and eating habits were analyzed using 3-day food records and a food frequency questionnaire. Anthropometric data were obtained from bioimpedance results, and fasting glucose and lipid levels were measured. Results: Mean weight, BMI, waist and hip circumferences, and body fat mass were greater in the obese group than in the non-obese group for men and women. Skeletal muscle mass and body fat mass were higher in obese women than in non-obese women. Biochemical data were no different in these two subgroups except triglycerides (TGs), which were higher in obese women. Nutrient intakes were not significantly different in obese and non-obese groups. However, obese men consumed excessive sodium, while obese women consumed slightly more than non-obese women. Obese men preferred salty foods and tended to overeat. Positive correlations were found between sodium intake and weight in men and percent body fat mass (PBFM) in women. Correlation analysis (adjusted for energy intake) of the relation between sodium intake and obesity-related factors showed sodium intake was positively correlated with PBFM and TG in women. Conclusion: This anthropometric and biochemical data analysis emphasizes the need for awareness and interventions to mitigate the health risks of elevated sodium consumption. Our findings should aid future studies on the relationship between sodium and obesity and contribute to preventing and managing this metabolic condition.

  • Obesity, a condition in which an abnormally large amount of fat is stored in adipose tissue, causing an increase in body weight, has become a major public health concern worldwide. The purpose of this study was to optimize the process for fermented milk for the production of a functional product with an anti-obesity effect by using Lactobacillus plantarum Q180 isolated from human feces. We used a 3-factor, 3-level central composite design (CCD) combined with the response surface methodology (RSM). Concentration of skim milk powder (%, $X_1$), incubation temperature ($^{\circ}C$, $X_2$), and incubation time (h, $X_3$) were used as the independent factors, whereas pH (pH, $Y_1$), anti-lipase activity (%, $Y_2$) and anti-adipogenetic activity (%, $Y_3$) were used as the dependent factors. The optimal conditions of fermented milk for the highest anti-lipase and anti-adipogenetic activity with pH 4.4 were the 9.5% of skim milk powder, $37^{\circ}C$ of incubation temperature, 28 h of incubation time. In the fermentation condition, the predicted values of pH, anti-lipase activity and anti-adipogenetic activity were 4.47, 55.55, and 20.48%, respectively. However, the actual values of pH, anti-lipase activity and anti-adipogenetic activity were 4.50, 52.86, and 19.25%, respectively. These results demonstrate that 9.5% of skim milk powder and incubation at $37^{\circ}C$ for 28 h were the optimum conditions for producing functional fermented milk with an anti-obesity effect.

  • This study explored various issues related to the quality-of-life for obese Korean men. In-depth interviews were conducted with thirty men over BMI 25 whose age ranged from 20's to 50's. Projective techniques along with semi-structured interviews were used. The results of interviews were analyzed based on grounded theory. "Stress" was the main phenomenon resulting from the causal conditions of obesity and stigma. Stress consisted of social, psychological, physical and environmental factors. Contextual conditions mediating the relationship between causal conditions and main phenomenon were obesity level, pressure, and public appearance. Avoidance, change of personal traits, and insensitivity were the action/reactions to the main phenomenon. Various factors due to obesity disappeared and quality-of-life increased when they succeed in weight reduction; however, factors related to obesity and lowered condition of quality-of-life continued if they experienced a weight gain/loss yo-yo.

  • PURPOSE: Sarcopenic obesity (SO) is a clinical condition that combines sarcopenia and obesity. This study examined the frequency of SO in young Korean females between 20 and 29 years of age. METHODS: The study involved 1,000 participants. The height, weight, body mass index (BMI), waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, systolic and diastolic blood pressure, alcohol consumption, and smoking status were the research variables. The skeletal muscle mass index was calculated as appendicular skeletal muscle mass (ASM) divided by the BMI. The ASM was assessed using dual X-ray absorptiometry. Complex sampling analysis and multiple logistic regression were used for analysis. RESULTS: A .74(.30-1.80) frequency of SO was observed. The statistically significant risk factors in females were height, weight, BMI, waist circumference, skeletal muscle mass index, total cholesterol, systolic blood pressure, and diastolic blood pressure (p < .05). CONCLUSION: Young Korean adults with SO have a .74(.30-1.80) frequency of occurrence that is linked to specific risk factors. Hence, primary care clinicians and health care professionals should consider these factors when patients require a referral for early detection and treatment. Healthcare professionals and clinicians can identify potential SO patients by acknowledging these risk factors.

  • Obesity is a physical condition that results from excessive storage of fat in the body. The present study examined the anti-obesity effects of the selected natural medicine, Galla rhois extract (GRE) and solvent fractions on 3T3-L1 preadipocytes and in vivo studies. Here, we show that EtOAc fraction of Galla rhois inhibits the differentiation of the 3T3-L1 preadipocytes induced by differentiated medium in a dose-dependent manner. To investigate the effect of the GRE-EtOAc fraction on obesity in high fat diet-fed C57BL/6 mice, which included a normal diet (ND), high-fat diet (HFD) and HFD+GRE concentration-dependent, were fed to the mice for 6 weeks. The GRE-EtOAc fraction was inhibited the highest adipocyte differentiation in vitro, the GRE supplement significantly decreased body weight and visceral fat mass compared to the HFD group. The total cholesterol and triglyceride levels in the plasma were significantly decreased by GRE supplementation compared with those of the HFD group. Also, we aimed to determine the differentiation inhibition and the modulation of differentiation genes brought about by the Galla rhois in adipocyte. A cDNA microarray-based method was introduced for the high contents screening (HCS) of gene expressions. This technology has revolutionized gene expression studies by providing the means to measure mRNA levels in thousands of genes simultaneously in simple and complex biological samples. 13 genes were founded to be affected in their expression levels by more than 5-fold up-regulation after 4 days treatment with the EtOAc fraction from Galla rhois. Otherwise, 21 genes were founded to be affected in their expression levels by more than 5-fold down-regulation treated with the EtOAc fraction. Therefore, Galla rhois extract may be considered for use in a therapeutic agent to control obesity.
